The foot is an uncommon location for chondrosarcoma. The presentation, diagnosis, pathological findings, surgical treatment and follow-up of three patients with chondrosarcoma in this rare location are presented. Though nonspecific, MR imaging findings were of aid in the diagnosis and treatment planning of these patients. If the diagnosis of this tumor is rapidly made, a tumor excision instead of limb amputation may be sufficient treatment at surgery.

54. Vertebral dimensions:influence of X-ray technique and patient size on measurements

- Abstract
In this study a new reference value, "corrected vertebral dimension," is presented for vertebral height measurements. Of 68 females (age 18-88 years; mean 44.2 years) and 40 males (age 16-81 years; mean 55 years) the projected vertebral dimensions (T4-L5) were measured on lateral radiographs. In addition to this, the vertebra-to-film distances (VFD) were measured and a fixed focus-to-film distance (FFD) was used during the study. Corrected dimensions of the thoracic and lumbar vertebrae (T4-L5) were calculated using the FFD and VFD. These corrected dimensions were then used to recalculate projected vertebral dimensions at different focus-to-film distances. The applied geometric corrections were verified in a phantom study representing an in vitro situation. The results indicate that studies using different X-ray techniques for making lateral radiographs of the spine can become comparable when using corrected vertebral dimensions.

- Abstract
Om het benedenstroomse gebied van de Mary River te Australite beschermen tegen verdere zoutindringing, zijn er door "Australian Marine and Offshore Group", een Australisch ingenieursbureau, verschillende oplossingen aangedragen die deze zoutindringing reduceren of zelfs compleet tegen gaan. Vervolgens zijn de gevolgen van de gekozen oplossingen bekeken door "International Institute for Infrastructural, Hydraulic and Environmental Engineering" (IHE DELFT), tegenwoordig UNESCO-IHE. Zij zijn tot een alternatieve oplossing gekomen wat de bouw van twee dammen inhield, een aantal kilometers stroomopwaarts vanaf de mondingen van de Mary River. Deze dammen zullen worden gebouwd met behulp van zakken gevuld met klei die ter plaatse wordt gewonnen. Dit rapport gaat vooral in op de vraag of het mogelijk is zakken te fabriceren die kunnen worden gevuld met klei en ook kunnen fungeren als bouwstenen van dammen. In eerste instantie is bepaald aan welke eisen de geokunststof moet voldoen om te kunnen fungeren als materiaal om zakken van te stikken zonder dat deze later, nadat ze zijn gevuld met klei, kunnen uitspoelen. Hieruit blijkt, uit de vele mogelijkheden die er zijn wat geokunststoffen betreft, dat er zeker een materiaal is te vinden dat kan voldoen aan de te stellen eisen. Een van de belastingen op een dam opgebouwd uit kleizakken is de stroming in een watergang. Om te kunnen bepalen wat de maximale stroomsnelheid is waarbij de zakken nog net blijven liggen, is in eerste instantie onderzocht wat de wrijvingsweerstand van de zakken op elkaar is. Dit is in het Laboratorium voor Vloeistofmechanica bepaald aan de hand van eenvoudige doch toereikende proeven. Nadat deze eigenschap van het geotextiel is bepaald, is aan de hand van de door Izbash opgestelde formules berekend bij welke stroomsnelheid de zakken kunnen bezwijken. - Abstract
Background: The incremental value of CAC over traditional risk factors to predict coronary vasodilator dysfunction and inherent myocardial blood flow (MBF) impairment is only scarcely documented (MBF). The aim of this study was therefore to evaluate the relationship between CAC content, hyperemic MBF, and coronary flow reserve (CFR) in patients undergoing hybrid (15)O-water PET/CT imaging.Methods: We evaluated 173 (mean age 56 ± 10, 78 men) patients with a low to intermediate likelihood for coronary artery disease (CAD), without a documented history of CAD, undergoing vasodilator stress (15)O-water PET/CT and CAC scoring. Obstructive coronary artery disease was excluded by means of invasive (n = 44) or CT-based coronary angiography (n = 129).Results: 91 of 173 patients (52%) had a CAC score of zero. Of those with CAC, the CAC score was 0.1-99.9, 100-399.9, and ≥400 in 31%, 12%, and 5% of patients, respectively. Global CAC score showed significant inverse correlation with hyperemic MBF (r = -0.32, P < .001). With increasing CAC score, there was a decline in hyperemic MBF on a per-patient basis [3.70, 3.30, 2.68, and 2.53 mL · min(-1) · g(-1), with total CAC score of 0, 0.1-99.9, 100-399.9, and ≥400, respectively (P < .001)]. CFR showed a stepwise decline with increasing levels of CAC (3.70, 3.32, 2.94, and 2.93, P < .05). Multivariate analysis, including age, BMI, and CAD risk factors, revealed that only age, male gender, BMI, and hypercholesterolemia were associated with reduced stress perfusion. Furthermore, only diabetes and age were independently associated with CFR.Conclusion: In patients without significant obstructive CAD, a greater CAC burden is associated with a decreased hyperemic MBF and CFR. However, this association disappeared after adjustment for traditional CAD risk factors. Objective: We investigated the feasibility of serial dynamic contrast-enhanced computed tomography (DCE-CT) in patients with advanced/metastatic non-small cell lung cancer (NSCLC) receiving anti-angiogenic (sorafenib) and anti-EGFR (erlotinib) treatment, and correlated tumour blood flow (BF) with treatment outcome.Methods: DCE-CTs were performed at baseline and 3 and 6 weeks after starting treatment. Tumour BF, calculated with the maximum slope method, and percentage change were measured in 23 patients (14 male; median age 59 years). Tumour BF was compared at baseline and weeks 3 and 6; the relation with RECIST/Crabb response and progression-free survival (PFS) was assessed.Results: Mean tumour perfusion decreased from 39.2 ml/100 g/min at baseline to 15.1 ml/100 g/min at week 3 (p < 0.001) and 9.4 ml/100 g/min at week 6 (p < 0.001). Tumour perfusion was lower in RECIST and Crabb responders versus non-responders at week 3 (4.2 versus 17.7 ml/100 g/min, p = 0.03) and week 6 (0 versus 13.4 ml/100 g/min, p = 0.04). Patients with a decrease larger than the median at week 6 tended to have a longer PFS (7.1 versus 5.7 months, p = 0.06).Conclusion: Serial DCE-CTs are feasible in patients with NSCLC and demonstrated a significant decrease in tumour BF following sorafenib/erlotinib therapy. Objective: Abdominal wall endometriosis (AWE) is defined as endometrial tissue that is superficial to the peritoneum. AWE is often difficult to diagnose, mimicking a broad spectrum of diseases. The aim of this study was to describe the appearance of AWE on magnetic resonance (MR) imaging.Methods: We present ten patients with AWE (12 lesions) in which MR imaging was used for diagnosis. MR imaging included T2-weighted imaging and T1-weighted imaging with fat suppression. To assess the value of diffusion-weighted imaging (DWI) in endometriosis, four patients underwent additional DWI. The apparent diffusion coefficient (ADC) was calculated using b values of 50, 400, 800 and 1,200 s/mm(2).Results: In most cases, the lesion was located ventral or dorsal to the aponeurosis of the rectus oblique muscle (n = 6) or in the rectus abdominis (n = 5). MR of AWE lesions showed isointense or slightly hyperintense signal compared with muscle on T2-weighted images and showed isointense or slightly hyperintense signal compared with muscle on T1-weighted images with foci of high signal intensity, indicative of haemorrhage. The mean ADC value of AWE was 0.93 x 10(-3)/mm(2)/s.Conclusion: MR imaging seems to be useful in determining the location and depth of infiltration in surrounding tissue preoperatively. Purpose: The purpose of this study was to derive parameters that predict which high-energy blunt trauma patients should undergo computed tomography (CT) for detection of chest injury.Methods: This observational study prospectively included consecutive patients (>or=16 years old) who underwent multidetector CT of the chest after a high-energy mechanism of blunt trauma in one trauma centre.Results: We included 1,047 patients (median age, 37; 70% male), of whom 508 had chest injuries identified by CT. Using logistic regression, we identified nine predictors of chest injury presence on CT (age >or=55 years, abnormal chest physical examination, altered sensorium, abnormal thoracic spine physical examination, abnormal chest conventional radiography (CR), abnormal thoracic spine CR, abnormal pelvic CR or abdominal ultrasound, base excess <-3 mmol/l and haemoglobin <6 mmol/l). Of 855 patients with >or=1 positive predictors, 484 had injury on CT (95% of all 508 patients with injury). Of all 192 patients with no positive predictor, 24 (13%) had chest injury, of whom 4 (2%) had injuries that were considered clinically relevant.Conclusion: Omission of CT in patients without any positive predictor could reduce imaging frequency by 18%, while most clinically relevant chest injuries remain adequately detected. - Abstract
The purpose of this study was to evaluate the feasibility of a total-liver-volume perfusion CT (CTP) technique for the detection and characterization of liver metastases. Twenty patients underwent helical CT of the total liver volume before and 11 times after intravenous contrast-material injection. To decrease distortion artifacts, all phases were co-registered using 3-D image fusion before creating blood-flow maps. Lesion-based sensitivity and specificity for liver metastases of first the conventional four phases (unenhanced, arterial, portal venous, and equilibrium) and later all 12 phases including blood-flow maps were determined as compared to intraoperative ultrasound and surgical exploration. Arterial and portal venous perfusion was calculated for normal-appearing and metastatic liver tissue. Total-liver-volume perfusion values were comparable to studies using single-level CTP. Compared to four-phase CT, total -liver-volume CTP increased sensitivity to 89.2 from 78.4% (P=0.046) and specificity to 82.6 from 78.3% (P=0.074). Total -liver-volume CTP is a noninvasive, quantitative, and feasible technique. Preliminary results suggest an improved detection of liver metastases for CTP compared to four-phase CT.
