Abstract

The incidence of nasopharyngeal carcinoma (NPC; cancer of the nose and throat) is very high among the population of southern China. The prognosis for patients who present with NPC is usually poor because most people are not examined until the cancer has reached an advanced stage. The reason most people do not seek help earlier appears to be, at least partially, related to the fact that early symptoms resemble those of many minor diseases. This study examined various patient characteristics and their relation to the stage of NPC at which medical care was sought. Data on 437 patients with newly diagnosed cases of NPC were analyzed. The patients were seen at one hospital in Hong Kong between July 1984 and December 1988. All patients had been born in southern China and were residents of Hong Kong. The average patient age was 47.3 years. Advancing stage of disease at presentation correlated with increasing age at presentation. The average age of patients presenting with stage I disease was 40.8 years, and for those presenting with stage V disease the average age was 54.1 years. The duration of the complaint or symptoms that led to presentation was less than three months for 56 percent of the patients and less than six months for 80 percent. Patients with stage V disease waited significantly longer from the time symptoms first appeared before seeking help than did patients with less severe disease. The most common symptoms included neck masses in 57.6 percent of the patients, deafness in 34.4 percent, bloody discharge from the nose in 46.3 percent, and postnasal drip in 30.8 percent of patients. Seventy percent of the patients presented with stages III, IV or V NPC. It is suggested that because of the high rate of NPC in the study population and the often benign symptoms during the early stages of the disease, regular mass screening of this population for nasopharyngeal cancer would be useful. 59. Frontal inverted papillomas: A 25-year study.

Abstract

Objectives/hypothesis: This study analyzes the treatment outcomes of frontal inverted papillomas (FIPs) in an attempt to provide guidelines for surgery selection.Study Design: Retrospective case series.Methods: The treatment results of 29 FIPs classified into five categories were retrospectively analyzed. The five categories are F1, tumor prolapsed into frontal sinus, tumor origin outside frontal sinus; F2, tumor origin inside frontal sinus, medial to the plane of lamina papyracea; F3, tumor origin inside frontal sinus, lateral to the plane of lamina papyracea; F4, bilateral; and F5, extrasinonasal.Results: Of the 11 F1 cases, 73% had Draf I and 27% had Draf IIA procedures. There was one (9%) frontal recurrence and one (9%) frontal stenosis. Of the 10 F2 cases, 10% had Draf I, 40% had Draf IIA, 40% had Draf IIB, and 10% had Draf III surgery with a trephination. One patient (10%) had a frontal recurrence. Of the five F3 cases, 40% had Draf IIA surgery, 20% had external frontoethmoidectomy, and 40% had external frontal sinusotomy. The recurrence rate was 60%, and frontal stenosis rate was 60%. The two F4 cases had external frontal sinusotomies and Draf III surgery with no frontal recurrence or stenosis. The patient with the F5 had a frontal recurrence after Draf IIA surgery and external frontoethmoidectomy.Conclusions: Draf I or IIA surgery is adequate for most F1 tumors, and Draf II surgery is adequate for most F2 tumors. F3 and F4 tumors can be managed initially by Draf III surgery with external frontal sinusotomy added when required. Abstract

Objectives/hypothesis: This study aimed to evaluate the effects of neuromuscular electrical stimulation (NMES) on vocal functions in patients with nasopharyngeal carcinoma following radiation therapy.Study Design: Prospective, randomized controlled trial.Methods: One hundred forty newly treated NPC patients were recruited and randomized into NMES or traditional swallowing exercise (TE) group. Participants received intensive NMES or traditional swallowing therapy and were followed up until 12 months postrandomization. Fifty-seven participants completed the treatment and all of the follow-up assessments. The Voice Handicap Index-30 (VHI-30) was used to measure the vocal functions of the participants.Results: The NMES group showed no significant changes to their vocal functions, whereas the TE group showed a short-term deterioration of voice functions at the 6-month follow-up. VHI-30 scores returned to the baseline level for both groups at the 12-month follow-up.Conclusions: NMES is shown to provide a short-term benefit on vocal functions for NPC patients following radiation therapy.Level Of Evidence: 1b Laryngoscope, 127:1119-1124, 2017. Abstract

This article focuses on experiences of teenage cochlear implantees in Hong Kng, China. A retrospective analysis of all implantees in a single centre in Hong Kong who were between the ages of 10 and 18 at the time of implant surgery from 1995 to 2002 was performed. The results of speech perception tests were analysed and discussed. Sixteen children aged 10 to 18 were included in the analysis. Open-set recognition was achievable in adolescent implantees especially for those who are 15 or below.

Abstract

In Corporate Real Estate Management in de ziekenhuiszorg is onderzocht hoe de beschikbare kennis over Corporate Real Estate Management (CREM) wordt toegepast op de ontwikkeling en het beheer van ziekenhuisvastgoed en welke verbeteringen hierin mogelijk zijn, zodat zo goed mogelijk kan worden ingespeeld op de trends en ontwikkelingen in de gezondheidszorg. Het doel was om een middel voor het ziekenhuisvastgoed te ontwikkelen, waarmee een ziekenhuis kan worden gepositioneerd op de mate van toepassing van Corporate Real Estate Management (CREM). Daarnaast werd hiermee duidelijk welke stappen door een ziekenhuis moeten worden ondernomen, zodat ingespeeld kan worden op de trends en ontwikkelingen in de ziekenhuissector. Uit onderzoek naar de trends en ontwikkelingen in de ziekenhuiszorg komen twee gevolgen voor vastgoed naar voren: prikkels om efficiënter en effectiever met vastgoed om te gaan en een verandering naar de vraag van vastgoed en de functie ervan. Om in te kunnen spelen op deze gevolgen voor het ziekenhuisvastgoed is de CREM thermometer ontworpen, waarmee een ziekenhuis kan worden gepositioneerd op de mate van toepassing van CREM en waarmee bepaald kan worden welke stappen moeten worden ondernomen om te komen tot het stadium waar men zich in 2010 zou moeten bevinden. De thermometer is getoetst bij drie algemene ziekenhuizen in Nederland en kan hiermee als aanzet worden gezien voor een gevalideerde CREM thermometer voor ziekenhuisvastgoed., Architecture
