Groningen gas field, which is being exploited for more than five decades, has been experiencing shallow and small magnitude earthquakes that cause limited structural damage to the building inventory in the region. These earthquakes are recorded in a relatively small area with multiple recording networks, which constitute a dense strong ground motion sensor grid, providing valuable insight into the characteristics of the motions produced by some 2500+ small faults at 3 km depth. The particularities of the Groningen soil, as well as the high seismic vulnerability of the structural inventory, render the Groningen earthquake problem complex, although the magnitudes experienced so far did not exceed 3.6ML. This paper is an attempt to compare the Groningen earthquake records to records from other induced seismicity and natural seismic events with similar characteristics in terms of magnitude, epicentral distance and depth, for identifying systematic differences in terms of component-to-component variability. In order to achieve that, a total of 1831 recording suites (i.e. couples of horizontal components) are used. 201 of these are Groningen records, 1112 are from other induced seismicity events to geothermal, waste water injection and hydraulic fracturing activities, while 517 are from natural earthquakes. The high polarity of the Groningen records, that is previously reported in the literature and represented as component-to-component variability in risk models, is the main focus of this paper. The component-to-component variability constitutes an important step when implementing ground motion models (GMMs) in risk assessment studies. In this study, in agreement with previous research, the component-to-component variances show that the Groningen induced seismicity events present stronger polarity than the other records used for comparison. The other induced seismicity recordings also show high component-to-component variances as compared to the natural events records, but the main difference is that their variances start decreasing in medium-to-long (i.e. above 0.6 s) periods while the variances steadily increase in the case of Groningen records. Furthermore, it is also observed that the component-to-component variances increase considerably when the rotated-to-max-PGA angle, explained in the paper, is used for defining the two horizontal components instead of using components as-recorded or rotated perpendicular to the station back azimuth. A modification to the component-to-component variance model of Groningen GMM v5 has also been proposed in this study for taking into account the orientation of the structural plan directions in respect to the ground motion component directions.

Background. Most studies on multiple health risk behaviors among adolescents have cross-sectionally studied a limited number of health behaviors or determinants. Purpose. To examine the prevalence, longitudinal patterns and predictors of individual and multiple health risk behaviors among adolescents.Methods. Eight health risk behaviors (no regular consumption of fruit, vegetables or breakfast, overweight or obesity, physical inactivity, smoking, alcohol use and cannabis use) were assessed in a prospective population study (second and third wave). Participants were assessed in three waves between ages 10 and 17 (20012008; n = 2230). Multiple linear regression was used to assess the influence of gender, self-control, parental health risk behaviors, parental monitoring and socioeconomic factors on the number of health risk behaviors adjusted for preceding multiple health risk behaviors (analysis: 2013-2014).Results. Rates of >5 health risk behaviors were high: 3.6% at age 13.5 and 10.2% at age 16. Smoking at age 13.5 was frequently associated with health risk behaviors at age 16. No regular consumption of fruit, vegetables and breakfast, overweight or obesity, physical inactivity and smoking predicted the co-occurrence of health risk behaviors at follow-up. Significant predictors of the development of multiple health risk behaviors were adolescents' levels of self-control, socioeconomic status and maternal smoking.Conclusions. Multiple health risk behaviors are common among adolescents. Individual and social factors predict changes in multiple health risk behaviors, showing that prevention targeting multiple risk behaviors is needed. Background\ud\udProphylaxis for gastrointestinal stress ulceration is frequently given to patients in the intensive care unit (ICU), but its risks and benefits are unclear.\udMethods\ud\udIn this European, multicenter, parallel-group, blinded trial, we randomly assigned adults who had been admitted to the ICU for an acute condition (i.e., an unplanned admission) and who were at risk for gastrointestinal bleeding to receive 40 mg of intravenous pantoprazole (a proton-pump inhibitor) or placebo daily during the ICU stay. The primary outcome was death by 90 days after randomization.\udResults\ud\udA total of 3298 patients were enrolled; 1645 were randomly assigned to the pantoprazole group and 1653 to the placebo group. Data on the primary outcome were available for 3282 patients (99.5%). At 90 days, 510 patients (31.1%) in the pantoprazole group and 499 (30.4%) in the placebo group had died (relative risk, 1.02; 95% confidence interval [CI], 0.91 to 1.13; P=0.76). During the ICU stay, at least one clinically important event (a composite of clinically important gastrointestinal bleeding, pneumonia, Clostridium difficile infection, or myocardial ischemia) had occurred in 21.9% of patients assigned to pantoprazole and 22.6% of those assigned to placebo (relative risk, 0.96; 95% CI, 0.83 to 1.11). In the pantoprazole group, 2.5% of patients had clinically important gastrointestinal bleeding, as compared with 4.2% in the placebo group. The number of patients with infections or serious adverse reactions and the percentage of days alive without life support within 90 days were similar in the two groups.\udConclusions\ud\udAmong adult patients in the ICU who were at risk for gastrointestinal bleeding, mortality at 90 days and the number of clinically important events were similar in those assigned to pantoprazole and those assigned to placebo. Background The face is a frequent site of burn, but prevalence rates vary and reports are often limited to one healthcare setting. We examined the incidence of facial burns in the Netherlands in Emergency Departments (ED), hospitals and burn centres. Additionally, we identified which patient, injury and burn-related characteristics were predictors of facial burns, facial surgery and facial reconstruction in burn centres. Methods A retrospective, observational study was conducted including data from the Dutch Injury Surveillance System, the National Hospital Discharge Register and burn centres in a 5-year period (2003–2007). Results Facial burn incidences per 100,000 were 15.1 for ED visits, 1.3 for hospital admissions and 1.4 for burn centre admissions. A total of 2691 patients were admitted to Dutch burn centres; 47.5% ( n =1277) had facial burns of which 20.5% received primary facial surgery and 5.3% received facial reconstruction in follow-up. Predictors of facial burns and facial surgery were identified. Predictors of facial reconstructive surgery were burns to the neck (ventral), fire/flame burns and number of facial surgeries in the acute phase of the burn. Conclusions One in five patients with facial burns admitted to a Dutch burn centre received primary facial surgery and 1 in 20 received facial reconstructive surgery within a follow-up of minimum 2 years.

Dit rapport gaat over populatie attributieve risico's (PAR's) die de bijdrage van verschillende risicofactoren (leefstijlfactoren, zoals roken, alcoholconsumptie, lichamelijke activiteit, en voeding, alsmede biologische factoren, zoals totaal serum cholesterol, HDL-cholesterol, bloeddruk, glucosetoleratie, en lichaamsgewicht naar lengte) aan mortaliteit ten gevolge van een aantal ziekten (coronaire hartziekten, beroerte, CARA, diabetes, en sommige vormen van kanker) kwantificeren. Relatieve risico's en prevalenties van risicofactoren zijn nodig om PAR's te berekenen. Relatieve risico's zijn ontleend aan de internationale literatuur en prevalenties van risicofactoren (voornamelijk in de leeftijdsklasse van 20-59 jaar) zijn ontleend aan het MONITORING project 1987-1991. In hoofdstuk twee worden belangrijke aannamen en problemen voor het berekenen van PAR's en PAR-achtige maten uiteengezet. Hoofdstuk drie bevat een beschrijving van PAR berekeningen voor een aantal determinanten, later samengevat in hoofdstuk vier vanuit het perspectief van zowel ziekten als determinanten. PAR's voor individuele determinanten varieerden van 10 tot 90%. Vanuit volksgezondheidsperspectief levert ingrijpen in een determinant met een matige tot grote invloed op een of vaak voorkomende ziekten, meer winst op dan interventie op een determinant met een grote invloed op een betrekkelijk zeldzame ziekte. Het roken van sigaretten blijkt het grootste gezondheidsverlies te bewerkstelligen: de prevalentie van roken is in Nederland hoog (40% van de mannen en vrouwen van 20-59 jaar). Het roken van sigaretten beinvloedt de mortaliteit van een aantal indicatoren (longkanker, keelkanker, slokdarmkanker, coronaire hartziekten, beroerte, en CARA). Coronaire hartziekten heeft het grootste aantal determinanten waarvoor PAR's konden worden berekend. De algemene conclusie is dat er voor genoemde gezondheidsproblemen in Nederland, althans theoretisch, nog een aanzienlijke gezondheidswinst is te boeken. Aangezien een groot gedeelte van de risicofactoren bestaat uit leefstijlfactoren, zijn er belangrijke individuele en collectieve keuzeproblemen met een verdere verbetering van de volksgezondheid gemoeid. De PAR's berekend voor individuele determinanten zijn een vereenvoudiging van de werkelijkheid, omdat interacties tussen determinanten niet in de beschouwing zijn opgenomen. Wiskundige modellen worden thans ontworpen om deze interacties wel in de beschouwing te betrekken. Ook zal meer aandacht worden besteed aan gezondheidswinst te behalen op hoge leeftijd.

Het Regenboog-project is een samenwerkingsverband tussen het RIVM, CBS en GGD Nederland en GGD'en. Het doel van het Regenboogproject is een beeld te krijgen van de gezondheidssituatie van de Nederlandse bevolking op het gebied van chronische en infectieziekten. Een aselecte steekproef wordt getrokken door het CBS. Bij deze personen wordt thuis de gezondheidsenquete afgenomen. Vervolgens wordt gevraagd om deel te nemen aan een aanvullend lichamelijk onderzoek op de GGD. Hierop reageert 56,7% positief. Het onderzoek op de GGD bestaat uit het invullen van een vragenlijst over infectieziekten, meting van bloeddruk, lengte, gewicht en middel-heupomtrek. Tot slot wordt er een gewrichtsfunctietest en 4 buisjes bloed afgenomen. Van de geinterviewde personen komt uiteindelijk 25,7 % op de GGD voor het lichamelijk onderzoek. De prevalentie van hypertensie is voor mannen 34% en vrouwen 30%. De prevalentie van overgewicht neemt toe met de leeftijd. De schatting van overgewicht blijkt sterk te verschillen tussen gerapporteerde en gemeten lengte en gewicht. Met name bij vrouwen is dit verschil duidelijk aanwezig. De onderzochte groep blijkt een bruikbare afspiegeling van de Nederlandse bevolking.

Bloedverwantschap tussen ouders (consanguiniteit) is een van de risicofactoren die de kans op aangeboren aandoeningen verhogen. Andere risicofactoren zijn bijvoorbeeld moederschap op oudere leeftijd, diabetes bij de moeder of erfelijke aandoeningen in de familie. Kinderen van consanguine ouders hebben vooral een verhoogde kans op zeer zeldzame autosomaal recessieve aandoeningen. Deze groep zeldzame aandoeningen maakt een zeer klein deel uit van het totale aantal aangeboren aandoeningen. Een (beperkt) deel van deze aandoeningen leidt tot sterfte, het risico hierop is het grootst in het eerste levensjaar. In absolute aantallen is het aandeel van met consanguiniteit samenhangende aandoeningen in de perinatale en zuigelingensterfte zeer gering. Over de ziektelast door autosomaal recessieve aandoeningen, wanneer geen sprake is van sterfte, zijn geen goede cijfers beschikbaar. Er zijn geen goede representatieve cijfers over het voorkomen van consanguiniteit in Nederland. Uit incidentele en soms indirecte gegevens valt af te leiden dat huwelijken tussen verwanten in Nederland waarschijnlijk het meest frequent voorkomen in de Turkse en Marokkaanse bevolkingsgroepen en in kleine religieuze gemeenschappen. Hoewel de absolute aantallen klein zijn, hebben de betreffende aandoeningen een grote impact op de getroffen families. De aandacht voor tijdige opsporing en advisering over genetische risico's, waaronder consanguiniteit en het risico binnen specifieke bevolkingsgroepen op minder zeldzame autosomaal recessieve aandoeningen (zoals erfelijke bloedarmoede), neemt toe. Idealiter zou ieder aanstaand ouderpaar, bij voorkeur vssr een zwangerschap, zich moeten kunnen laten informeren over een eventueel verhoogde kans op een kind met een aangeboren aandoening en over mogelijkheden voor preventie.

De zwangerschapsuitkomsten bij allochtone vrouwen en hun kinderen zijn over het algemeen minder gunstig dan bij zwangere vrouwen en hun kinderen van autochtone afkomst. Allochtone kinderen hebben gemiddeld een lager geboortegewicht. Vroeggeboorte en aangeboren aandoeningen komen bij hen vaker voor. Ook de sterfte rond de geboorte is minder gunstig onder allochtone dan onder autochtone vrouwen en kinderen. Dit blijkt uit verschillende onderzoeken in Nederland naar gezondheidsverschillen tussen autochtone en allochtone zwangere vrouwen en hun kinderen en de onderliggende oorzaken. In deze studie wordt een overzicht gegeven van afgeronde en lopende onderzoeken naar verschillen in zwangerschapsuitkomsten, in risicofactoren, in zorggebruik en in het gebruik van preventieve maatregelen. De sterfteverschillen zijn deels te verklaren door verschillen in risicofactoren. Niet westers allochtone vrouwen zijn minder bekend met preventieve maatregelen zoals extra foliumzuurgebruik, melden zich later voor verloskundige zorg en maken minder gebruik van kraamzorg. Hierdoor missen zij kansen om tijdig geonformeerd te worden over preventieve maatregelen om de gezondheid van hun kind of zichzelf zo optimaal mogelijk te maken. De beschreven onderzoeken bieden aangrijpingspunten voor maatregelen ter verbetering van de zwangerschapsuitkomsten bij met name niet westers allochtone vrouwen en kinderen. Soms is nader onderzoek nodig om te verklaren waarom die verschillen bestaan en om aan te geven welke (cultuur)specifiekere begeleiding en preventieve maatregelen nodig zijn. Voor het terugdringen van perinatale gezondheidsverschillen tussen zwangeren in Nederland is integraal gezondheidsbeleid nodig. Een deel van de verschillen in sterfte rond de geboorte wordt namelijk veroorzaakt door achterliggende factoren die grotendeels buiten het bereik van de (volks)gezondheidszorg vallen, zoals het niet beheersen van de Nederlandse taal of sociaaleconomische verschillen.

Design Prospective cohort study setting level one trauma center in the Netherlands participants adult trauma patients transported to the Emergency Department on a backboard, with extrication collar and headblocks and admitted to the hospital for treatment or evaluation of their injuries. Methods Between January and December 2013, 254 trauma patients were included. The following dependent variables were collected: Age, Skin color and Body Mass Index, and Time in Emergency Department, Injury Severity Score, Mean Arterial Pressure, hemoglobin level, Glasgow Coma Score, and admission ward after Emergency Department. Results Pressure ulcer development during admission was associated with a higher age (p 0.00, OR 1.05) and a lower Glasgow Coma Scale score (p 0.00, OR 1.21) and higher Injury Severity Scores (p 0.03, OR 1.05). Extra nutrition decreases the probability of PU development during admission (p 0.04, OR 0.20). Pressure ulcer development within the first 48 h of admission was positively associated with a higher age (p 0.01, OR 1.03) and a lower Glasgow Coma Scale score (p 0.01, OR 1.16). The proportion of patients admitted to the Intensive Care Unit and Medium Care Unit was higher in patients with pressure ulcers. Conclusions The pressure ulcer risk during admission is high in patients with an increased age, lower Glasgow Coma Scale and higher Injury Severity Score in the Emergency Department. Pressure ulcer risk should be assessed in the Emergency Department to apply preventive interventions in time.

Bovine tuberculosis (bTB) is a zoonotic disease, and remains a cause of concern for livestock, wildlife and human health, especially in Ethiopia. It is a contagious disease, so close contact between animals or sharing of feed between infected and non-infected animals are major risk factors for transmission. Thus, improving the understanding of the factors that promote contact between hosts (i.e., livestock animals but also wild ruminants) is critical for limiting bTB transmission in pastoral, multi-host communities. I found that the older the age of the cattle and the lower the body condition, the higher the chance of a positive bTB test result at the individual animal level. Moreover, at herd level, herd size, contact with wildlife, and the interaction of herd size and contact with wildlife were identified as significant risk factors for bTB prevalence in cattle in Ethiopia. Further to what is already known from the past studies, I found that the probability of contact with wildlife was positively influenced by herd size, through herd movement. As larger herds moved more and grazed in larger areas, the probability of grazing in an area with wildlife and contact with either infected cattle or infected wildlife hosts increased; this also increased the chances for bTB infection. I detected a possible ‘dilution effect’ in bTB, where a higher evenness of mammal species reduced the probability of bTB occurrence. This dilution effect might be caused by encounter reduction. Because the encounter rate is proportional to the distribution of the host species; evenness would then capture the probability of encounter between pathogens and each host species. Thus, species evenness can be an appropriate measure of biodiversity to explain disease risk. I also showed that bTB prevalence was positively associated with the invasion of the plant Prosopis (Prosopis juliflora), maybe due to the loss in host species evenness and the increase in cattle movement as a consequence of the loss of palatable grasses in Prosopis-infested areas. Moreover, social contacts between herd owners are also important, as I found that herds with a greater number of edges in a (social) network had more connections in the livestock transfer network, increasing the probability of becoming infected with bTB. Thus, cultural components like large herd size and social contacts are at odds with the global One Health rationale to reduce bTB.

Full text beschikbaar met HU-account in Boomportaal Onderzoek naar de oorzaken van antisociaal gedrag is van oudsher gericht op de invloed van sociale en psychologische factoren, maar in toenemende mate worden ook biologische factoren onderzocht. Het is lang niet altijd duidelijk met welk type factor we te maken hebben. Dat is bijvoorbeeld te zien aan de verschillende redenen waar een kind van een tienermoeder een verhoogd risico op de ontwikkeling van antisociaal gedrag heeft. Sociaal gezien is de kans groot dat een tienermoeder haar kind opvoedt onder ongunstige omstandigheden: ze woont in een slechte buurt, is laagopgeleid, werkeloos en verslaafd. Vanuit psychologische perspectief is het belangrijk om te vermelden dat meisjes die veel antisociaal gedrag vertonen en een lage intelligentie hebben, relatief vaker als tiener zwanger raken dan brave, intelligente meisjes. Baby's van tienermeisjes lopen meer risico op mishandeling en verwaarlozing en voor de geboorte kampen zij met de biologische gevolgen van middelengebruik of ervaren stress van de moeder. Daarnaast is sprake van genetische invloed, bijvoorbeeld op persoonlijkheidskenmerken als impulsiviteit en agressiviteit, van zowel de moeder als de vader, want antisociale meisjes krijgen doorgaans een relatie met antisociale jongens. Een sociale factor als drugsgebruik kan tijdens de zwangerschap biologische gevolgen hebben voor de baby, met een negatieve uitwerking op de intelligentie-ontwikkeling (psychologische factor), wat de kans verhoogt dat het kind later laagopgeleid zal blijven(sociale factor)

Het is lang niet altijd duidelijk met welk type factor we te maken hebben. Dat is bijvoorbeeld te zien aan de verschillende redenen waar een kind van een tienermoeder een verhoogd risico op de ontwikkeling van antisociaal gedrag heeft. Sociaal gezien is de kans groot dat een tienermoeder haar kind opvoedt onder ongunstige omstandigheden: ze woont in een slechte buurt, is laagopgeleid, werkeloos en verslaafd. Vanuit psychologische perspectief is het belangrijk om te vermelden dat meisjes die veel antisociaal gedrag vertonen en een lage intelligentie hebben, relatief vaker als tiener zwanger raken dan brave, intelligente meisjes. Baby's van tienermeisjes lopen meer risico op mishandeling en verwaarlozing en voor de geboorte kampen zij met de biologische gevolgen van middelengebruik of ervaren stress van de moeder. Daarnaast is sprake van genetische invloed, bijvoorbeeld op persoonlijkheidskenmerken als impulsiviteit en agressiviteit, van zowel de moeder als de vader, want antisociale meisjes krijgen doorgaans een relatie met antisociale jongens. Een sociale factor als drugsgebruik kan tijdens de zwangerschap biologische gevolgen hebben voor de baby, met een negatieve uitwerking op de intelligentie-ontwikkeling (psychologische factor), wat de kans verhoogt dat het kind later laagopgeleid zal blijven(sociale factor)

Er is iets goed mis met de honingbij: ’s winters legt een derde van de volken het loodje. Geleidelijk aan krijgt de wetenschap meer inzicht in mogelijke oorzaken van deze bijensterfte, maar één boosdoener is (nog) niet aan te wijzen.

Dit artikel is een samenvatting van het rapport "Laag pathogene aviaire influenza virus iInfecties op pluimveebedrijven in Nederland" (CVI 2012). Dit rapport is geschreven naar aanleiding van vragen van het toenmalige Ministerie van Economische zaken, Landbouw en Innovatie. De vragen die werden gesteld zijn: - hebben pluimveebedrijven met vrije uitloop een grotere kans op introductie van LPAI virus infecties?; - is de kans op introductie gerelateerd aan wilde vogels?; - is er een periode in het jaar waarin het risico op infectie groter is? - kunnen er factoren geidentificeerd worden die de kans op introductie verminderen?

BACKGROUND: Unhealthy diets and inactivity are still common among patients with cardiovascular diseases. This study evaluates the effects of the telephonic lifestyle intervention 'Hartcoach' on risk factors and self-management in patients with recent coronary events.DESIGN: This was a randomised trial in five Dutch hospitals.METHODS: Patients (18-80 years), less than eight weeks after hospitalisation for acute myocardial infarction or (un)stable angina pectoris were randomised to the Hartcoach-group, who received telephonic coaching every four weeks for a period of six months (in addition to usual care), and a control group receiving usual care only. Simple random allocation was used (without relation to prior assignment). Measurements were taken by research nurses blinded for group allocation. Differences after six months of participation were compared using linear or logistic regression models with treatment-group and baseline score for the outcome under analysis as covariates, resulting in adjusted mean change (b).RESULTS: Altogether 374 patients were randomised (173 Hartcoach + usual care, 201 usual care only). Follow-up was obtained in 331 patients who still participated after six months. Hartcoach had significant favourable effects on body mass index (BMI) (b = -0.32; 95% CI:(-0.63- -0.003)), waist circumference (b = -1.71; 95% CI:(-2.73- -0.70)), physical activity (b = 15.08 (score); 95% CI:(0.13, 30.04)) daily intake of vegetables (b = 13.41; 95% CI:(1.10-25.71)), self-management (b = 0.11; 95% CI:(0.00-0.23)) and anxiety (b = -0.65; 95% CI:(-1.25- -0.06)). Hartcoach slightly increased the total number of risk scores on target (b = 0.45; 95% CI:(0.17-0.73)).CONCLUSIONS: Hartcoach has modest impact on BMI, waist circumference, physical activity, intake of vegetables, self-management and anxiety. Therefore, it may be a useful maintenance programme in addition to usual care, to support patients with recent coronary events to improve self-management and reduce risk factors.

As found in research on chronic pain, different psychophysiological processes may also play a role in chronic itch, of which central sensitization, conditioned modulation, and attentional processes have been studied most frequently. This study aimed to explore psychophysiological processes of chronic post-burn itch by comparing 15 patients with long-term itch due to burn injury with 15 matched healthy controls. Exploratory results indicated tendencies for higher itch sensitivity in patients than in controls, for mechanical stimuli and histamine, but not for electrical stimulation. Results further suggest that the efficacy of itch modulation by an itch- or pain-conditioning stimulus or directing attention towards itch stimuli do not differ between these patients and controls. Further elucidation of the processes underlying post-burn itch may improve the early identification and treatment of burn patients developing chronic itch.

Background: Paratonia is a progressive motor problem that is observed in individuals with dementia and is not a well-known phenomenon. This study explores the development and risk factors of paratonia in moderate stage dementia patients.Methods: A multi-center, longitudinal, one-year follow-up cohort study was performed. Patients with an established diagnosis of dementia, with a score of 6 or lower on the Global Deterioration Scale (GDS) were included. The participants were assessed using the Paratonia Assessment Instrument (PAI), the Timed Up and GO test, the Qualidem, the Global Deterioration Scale (Reisberg et al., 1982) and the Mini-mental State Examination. Information about each patient's diagnosis of dementia, comorbidities and use of medication were obtained from the participant's medical file. The PAI was assessed every three months, the other variables at baseline and after 12 months. Cross-tabulation χ2 and logistic regression tests were used for the statistical analyses.Results: Baseline measures were assessed in the 204 participants – 111 (54%) female and 93 (46%) male, with a mean age of 79.8 years (56–97). Seventy-one patients (34.8%) were diagnosed with paratonia at baseline, and 51 patients developed paratonia over one year. The highest hazard ratio (3.1) for developing paratonia within one year was observed in the vascular dementia group. The logistic regression analysis revealed that the presence of diabetes mellitus (OR = 10.7) was significantly related to the development of paratonia (Wald χ2 p-value < 0.01).Conclusions: Diabetes mellitus and likely vascular damage are risk factors for the development of paratonia.

At a risk management level, other factors, such as cultural, social, or economic issues, are often more important than scientific advice in determining policy. There is a need for transparency at an international level as trade barriers are gradually being removed and these other factors arc becoming more apparent. Therefore it is important that all the factors implicated in the food safety policy-making process are recognized in a standard framework.

Committed by the Dutch poultry sector research is carried out concerning acting perspective for prevention of AI-introduction on poultry farms, based on existing knowledge. The findings are shown in two parts: part I with practical advices for poultry farmers, and part II with its underpinning with a summary of existing knowledge of risk factors, preventive measures, early detection and possibilities for promoting desired (preventive) behaviour. In Nederland zijn tot en met 2006 tweeëntachtig gevallen van BSE geconstateerd bij i n Nederland geboren runderen. In dit artikel wordt een update gegeven over de BSE-situatie in Nederland. De klinische symptomen waaraan BSE is te herkennen worden samengevat. Uit de Nederlandse gegevens blijkt dat een passieve surveillance waarbij dierhouders en dierenartsen klinisch verdachte dieren moeten aanmelden, een systeem met lage gevoeligheid is. Tevens wordt er ingegaan op de epidemiologie en risicofactoren van BSE. Alle BSE-gevallen in Nederland kunnen worden verklaard door versleping van gecontamineerd diermeel. Op basis van het verloop van de epidemie en alle achtergronden die bekend zijn geworden, is te verwachten dat het aantal BSE-gevallen in Nederland en elders in Europa verder zal afnemen

The disease-specific-TACQOL-asthma questionnaire measures health-status and appraisal of health-status. The TACQOL-asthma evaluates the personal feelings about problems in the domains, 'complaints, situations, emotions, treatment and medication'. The TACQOL-asthma can be used alone or in combination with the generic TACQOL. Our objective was to study the psychometric properties of the TACQOL-asthma-questionnaire. Responses of 298 parents and children with asthma (age eight to 16 years) in four paediatric practices in the northern part of the Netherlands were studied. The factor-analysis and item-domain correlation analysis show a moderate to strong correlation between the different items and their hypothesised domains. For all items, the correlation of the separate item with the hypothesized domain is stronger than with any other domain. The internal consistency (Cronbach's alpha) of the domains is moderate to good. Concurrent correlation with the Paediatric-Asthma-Quality-of-Life-Questionnaire- (PAQLQ) was significant. Effect sizes of differences between asthma-severity classes in TACQOL-asthma and PAQLQ-scores were similar and of clinical importance. This study validates the TACQOL-asthma as a new disease-specific questionnaire. The TACQOL-asthma ensures a measurement of health status as well as appraisal of health problems. The TACQOL-asthma has good reliability and validity properties to serve as an evaluative and discriminate disease-specific health-related-quality-of-life questionnaire. © 2006 Edward Arnold (Publishers) Ltd.

Potential risk factors for clinical signs of post-weaning multisystemic wasting syndrome (PMWS) and porcine dermatitis and nephropathy syndrome (PDNS) in pigs in the Netherlands were investigated in a matched case-control study using a questionnaire (personal interview). Eighty-two pig farmers were questioned about management, hygiene, husbandry systems, disease history, and preventive health care. Organic poultry production in the Netherlands is developing. Although consumers assume organic products to be safer and healthier, there are aspects of organic animal husbandry, like access to an outdoor run, that can result in increased risks of food safety problems. The aim of this study was to compare housing and management of organic and conventional broiler farms in the Netherlands and to study the occurrence of Salmonella and Campylobacter infections on the former. Large differences were found between the two farming systems with respect to mixed or single farming, manure storage, drinking-water system, ventilation, access to an outdoor run, and pest control. From the 31 organic flocks sampled for Salmonella and Campylobacter in 2003, 13% were positive for Salmonella and 35% for Campylobacter. Results for the summer period are missing due to an outbreak of avian influenza, so the actual number of flocks infected with Campylobacter can be expected to be even higher. Campylobacter appears to be the main risk on organic broiler farms, so that it would be interesting to study specific risk factors of infection with this pathogen on these farms.
