The 11th edition of the International Classification of Disease has included Complex Post-Traumatic Stress Disorder (cPTSD) as a clinical diagnosis separate to Post-Traumatic Stress Disorder (PTSD). Research has shown that cPTSD differs significantly from PTSD on core symptoms relating to the individual’s sense of self; which has driven research investigating treatment approaches to address these specific features of cPTSD. Evidence for effective treatments for cPTSD delivered within in-person settings has been synthesised, however, authors have noted significant access barriers to such specialised treatments. Digital-based intervention (DBI) approaches may overcome such access barriers and have shown effectiveness in treating non-complex PTSD across several systematic reviews and meta-analyses. The current protocol however, proposes a methodology for conducting the first systematic review of research investigating the use of DBIs for treating cPTSD, as there has not yet been a comprehensive synthesis of this research. The review seeks to understand (1) the treatment approaches employed in DBIs for addressing symptoms of cPTSD, (2) the symptoms of cPTSD that are targeted by DBIs, (3) the evidence for the acceptability and effectiveness of DBIs in treating symptoms of cPTSD, and (4) measures taken to ensure the safe and confidential delivery of DBIs for cPTSD. A systematic search of Scopus, PsychINFO, and EBSCOhost is proposed, using search terms targeting ‘cPTSD’ and ‘DBIs’. Literature will be screened against eligibility criteria by two independent reviewers and included studies will be assessed for quality using the Quality Assessment Tool for Quantitative Studies. Data will be extracted by the same two reviewers, then analysed and synthesised in narrative form by a single reviewer. The findings from the systematic review will provide an understanding of the state of current evidence and inform directions for direct future research aiming to develop effective DBIs for cPTSD.

Diatoms are of great interest for large-scale cultivation due to their high lipid content. The ability to grow over a wide range of salinities is also of great advantage. We studied the effect of temperature and salinity on the growth, lipids and fatty acid profiles of a newly isolated halophilic diatom Amphora sp. MUR 258. Amphora sp. MUR 258 is unusual in that it grows over a wide range of temperatures (24–35 °C) and salinities (7–12% (w/v) NaCl). The highest specific growth rate (SGR; 0.607 ± 0.017 day−1) was achieved at 7% NaCl at 35 °C, and the lowest SGR (0.433 ± 0.087 day−1) was obtained at 9% NaCl at 25 °C. The cells contained more lipids in the exponential phase, except when grown at 12% NaCl where the lipid content was higher in the stationary phase. The alga achieved its highest lipid content (57.69 ± 2.039% ash-free dry weight (AFDW) when grown at 7% NaCl at 25 °C and the lowest (34.43 ± 3.955% AFDW) obtained at 12% NaCl at 35 °C. The highest biomass productivity (0.171 ± 0.017 gAFDW L−1 day−1) and the lipid productivity (0.062 ± 0.017 gAFDW L−1 day−1) were achieved when the Amphora were grown at 9% NaCl at 35 °C and 7% at 25 °C, respectively. Irrespective of the growth conditions, the predominant fatty acids of Amphora sp. MUR 258 were palmitic acid (C16:0), stearic acid (C18:0), palmitoleic acid (C16:1) and oleic acid (C18:1), as well as low quantities of eicosapentaenoic acid (C20:5).

AIM: To validate the Montreal classification system for Crohn's disease (CD) and ulcerative colitis (UC) within the Netherlands.METHODS: A selection of 20 de-identified medical records with an appropriate representation of the inflammatory bowel disease (IBD) sub phenotypes were scored by 30 observers with different professions (gastroenterologist specialist in IBD, gastroenterologist in training and IBD-nurses) and experience level with IBD patient care. Patients were classified according to the Montreal classification. In addition, participants were asked to score extra-intestinal manifestations (EIM) and disease severity in CD based on their clinical judgment. The inter-observer agreement was calculated by percentages of correct answers (answers identical to the "expert evaluation") and Fleiss-kappa (kappa). Kappa cutoffs: 0.8 excellent.RESULTS: The inter-observer agreement was excellent for diagnosis (kappa = 0.96), perianal disease (kappa = 0.92) and disease location in CD (kappa = 0.82) and good for age of onset (kappa = 0.67), upper gastrointestinal disease ( kappa = 0.62), disease behaviour in CD (kappa = 0.79) and disease extent in UC (kappa = 0.65). Disease severity in UC was scored poor (kappa = 0.23). The additional items resulted in a good inter-observer agreement for EIM (kappa = 0.68) and a moderate agreement for disease severity in CD ( kappa = 0.44). Percentages of correct answers over all Montreal items give a good reflection of the inter-observer agreement (> 80%), except for disease severity (48%-74%). IBD-nurses were significantly worse in scoring upper gastrointestinal disease in CD compared to gastroenterologists (P = 0.008) and gastroenterologists in training (P = 0.040). Observers with less than 10 years of experience were significantly better at scoring UC severity than observers with 10-20 years (P = 0.003) and more than 20 years (P = 0.003) of experience with IBD patient care. Observers with 10-20 years of experience with IBD patient care were significantly better at scoring upper gastrointestinal disease in CD than observers with less than 10 years (P = 0.007) and more than 20 years (P = 0.007) of experience with IBD patient care.CONCLUSION: We found a good to excellent inter-observer agreement for all Montreal items except for disease severity in UC (poor). Context. Parallaxes for 331 classical Cepheids, 31 Type II Cepheids, and 364 RR Lyrae stars in common between Gaia and the HIPPARCOS and Tycho-2 catalogues are published in Gaia Data Release 1 (DR1) as part of the Tycho-Gaia Astrometric Solution (TGAS). Aims. In order to test these first parallax measurements of the primary standard candles of the cosmological distance ladder, which involve astrometry collected by Gaia during the initial 14 months of science operation, we compared them with literature estimates and derived new period-luminosity (PL), period-Wesenheit (PW) relations for classical and Type II Cepheids and infrared PL, PL-metallicity (PLZ), and optical luminosity-metallicity (MV-[Fe/H]) relations for the RR Lyrae stars, with zero points based on TGAS. Methods. Classical Cepheids were carefully selected in order to discard known or suspected binary systems. The final sample comprises 102 fundamental mode pulsators with periods ranging from 1.68 to 51.66 days (of which 33 with sigma(omega)/omega < 0 : 5). The Type II Cepheids include a total of 26 W Virginis and BL Herculis stars spanning the period range from 1.16 to 30.00 days (of which only 7 with sigma(omega)/omega 0 : 5). The RR Lyrae stars include 200 sources with pulsation period ranging from 0.27 to 0.80 days (of which 112 with sigma(omega)/omega < 0 : 5). The new relations were computed using multi- band (V; I; J; K-s) photometry and spectroscopic metal abundances available in the literature, and by applying three alternative approaches: (i) linear least-squares fitting of the absolute magnitudes inferred from direct transformation of the TGAS parallaxes; (ii) adopting astrometry-based luminosities; and (iii) using a Bayesian fitting approach. The last two methods work in parallax space where parallaxes are used directly, thus maintaining symmetrical errors and allowing negative parallaxes to be used. The TGAS-based PL; PW; PLZ, and MV [Fe/H] relations are discussed by comparing the distance to the Large Magellanic Cloud provided by different types of pulsating stars and alternative fitting methods. Results. Good agreement is found from direct comparison of the parallaxes of RR Lyrae stars for which both TGAS and HST measurements are available. Similarly, very good agreement is found between the TGAS values and the parallaxes inferred from the absolute magnitudes of Cepheids and RR Lyrae stars analysed with the Baade-Wesselink method. TGAS values also compare favourably with the parallaxes inferred by theoretical model fitting of the multi-band light curves for two of the three classical Cepheids and one RR Lyrae star, which were analysed with this technique in our samples. The K-band PL relations show the significant improvement of the TGAS parallaxes for Cepheids and RR Lyrae stars with respect to the HIPPARCOS measurements. This is particularly true for the RR Lyrae stars for which improvement in quality and statistics is impressive. Conclusions. TGAS parallaxes bring a significant added value to the previous HIPPARCOS estimates. The relations presented in this paper represent the first Gaia-calibrated relations and form a work-in-progress milestone report in the wait for Gaia-only parallaxes of which a first solution will become available with Gaia Data Release 2 (DR2) in 2018. High resolution IUE spectra of SN 1993J in M81 show the existence of several clouds on the line of sight. These include local galactic gas at 0 km/s, gas in M81 at -130 km/s, a cloud in the halo in of the Milky Way near -45 km/s, and an intergalactic cloud near +140 km/s. Arguments for each of these locations are given. There is little gas in M81 in front of SN 1993J and no detectable CIV at or near -130 km/s. The gas of the halo of the Milky Way shows a large amount of CIV and SiIV. The intergalactic cloud shows a large velocity width extending from +100 km/s to +200 km/s and is very prominent in the absorption lines of AlIII, SiIV, and CIV. It may be related to energetic events in the M81 group galaxies.

The optimal biogas system design model (OBSDM) described in this paper is intended to be used as a decision-making tool to increase awareness of the potential of biogas technology for different applications in Sub-Saharan Africa (SSA). The decision-making tool identifies the most suitable biodigester design based on user defined inputs, including energy and fertiliser requirements; feedstock (type, amount, and rate of supply); water supply; land use (area, soil type, ground water level); climate (temperature and rainfall); construction materials available locally; and the priorities (based on sustainability criteria) of the intended biogas user. The output of the model provides a recommended design with estimates of the expected costs, energy and fertiliser production, and links to contact biodigester suppliers. In order to test the model, data from household surveys conducted in rural regions of Kenya and Cameroon were used as inputs to the model. An innovative fixed dome biodigester design, which uses stabilised soil blocks instead of bricks, was identified as optimal for both Kenyan and Cameroonian rural households. The expected performance of the optimal biogas system design from the model output was consistent with survey data on existing biogas systems in the region.

Potentially decision-relevant stimuli have been proposed to undergo immediate semantic processing. The current study investigated whether information regarding the general desirability ('Wanting') of visually presented 'everyday' objects was rapidly and automatically processed. Participants completed a foreground task while their electroencephalogram (EEG) was recorded, and task-irrelevant images were presented in the background. Following this, participants rated the images with regards to Wanting and the potentially related attributes of Relevance, Familiarity, Aesthetic Pleasantness and Time Reference. Multivariate pattern classification was used to predict the ratings from patterns of EEG data. Prediction of Wanting and Relevance was possible between 100 and 150ms following stimulus presentation. The other dimensions could not be predicted. Wanting and Relevance ratings were highly correlated and displayed similar feature weight maps. The current results suggest that the general desirability and subjective relevance of everyday objects is rapidly and automatically processed for a wide range of visual stimuli.

- Abstract
Context. At about 1000 days after the launch of Gaia we present the first Gaia data release, Gaia DR1, consisting of astrometry and photometry for over 1 billion sources brighter than magnitude 20.7. Aims. A summary of Gaia DR1 is presented along with illustrations of the scientific quality of the data, followed by a discussion of the limitations due to the preliminary nature of this release. Methods. The raw data collected by Gaia during the first 14 months of the mission have been processed by the Gaia Data Processing and Analysis Consortium (DPAC) and turned into an astrometric and photometric catalogue. Results. Gaia DR1 consists of three components: a primary astrometric data set which contains the positions, parallaxes, and mean proper motions for about 2 million of the brightest stars in common with the HIPPARCOS and Tycho-2 catalogues - a realisation of the Tycho-Gaia Astrometric Solution (TGAS) - and a secondary astrometric data set containing the positions for an additional 1.1 billion sources. The second component is the photometric data set, consisting of mean G-band magnitudes for all sources. The G-band light curves and the characteristics of similar to 3000 Cepheid and RR Lyrae stars, observed at high cadence around the south ecliptic pole, form the third component. For the primary astrometric data set the typical uncertainty is about 0.3 mas for the positions and parallaxes, and about 1 mas yr(-1) for the proper motions. A systematic component of similar to 0.3 mas should be added to the parallax uncertainties. For the subset of similar to 94 000 HIPPARCOS stars in the primary data set, the proper motions are much more precise at about 0.06 mas yr(-1). For the secondary astrometric data set, the typical uncertainty of the positions is similar to 10 mas. The median uncertainties on the mean G-band magnitudes range from the mmag level to similar to 0.03 mag over the magnitude range 5 to 20.7. Conclusions. - Abstract
Gaia is a cornerstone mission in the science programme of the European Space Agency (ESA). The spacecraft construction was approved in 2006, following a study in which the original interferometric concept was changed to a direct-imaging approach. Both the spacecraft and the payload were built by European industry. The involvement of the scientific community focusses on data processing for which the international Gaia Data Processing and Analysis Consortium (DPAC) was selected in 2007. Gaia was launched on 19 December 2013 and arrived at its operating point, the second Lagrange point of the Sun-Earth-Moon system, a few weeks later. The commissioning of the spacecraft and payload was completed on 19 July 2014. The nominal five-year mission started with four weeks of special, ecliptic-pole scanning and subsequently transferred into full-sky scanning mode. We recall the scientific goals of Gaia and give a description of the as-built spacecraft that is currently (mid-2016) being operated to achieve these goals. We pay special attention to the payload module, the performance of which is closely related to the scientific performance of the mission. We provide a summary of the commissioning activities and findings, followed by a description of the routine operational mode. We summarise scientific performance estimates on the basis of in-orbit operations. Several intermediate Gaia data releases are planned and the data can be retrieved from the Gaia Archive, which is available through the Gaia home page.

Biogas technology has been recognised as a suitable technology for improving energy access, waste management, and sanitation in Sub-Saharan Africa (SSA). Uptake of the technology in the region has been sporadic and currently dissemination programmes are limited to five countries. The main barriers hindering larger dissemination include high installation costs, inadequate user training, insufficient servicing, and inappropriate designs. Poor design choices, mainly due to overlooking the user energy needs and local conditions, contribute to the short lifespan of many installed biogas systems. This research aims to address the gap in appropriate designs of biogas systems through the development of a model that identifies optimal designs for particular applications in SSA based on user defined priorities of sustainability criteria. TOPSIS, a multi-criteria decision making method is used in the model to identify the optimal system based on the priority rating of the criteria and the designs that are feasible according to the main parameters. The model was applied to a typical Kenyan household where a 9 m3 modified CAMARTEC solid state digester was identified as the optimal biogas system.

This paper aims to provide a broad review and assessment of the feedstocks and applicable biogas technologies that are feasible in Sub-Saharan Africa (SSA). Biodigesters and feedstocks available in SSA were identified according to scale and application – household, community, institutional, and commercial. Aside from livestock manure, suitable feedstocks for household, community, and institutional biodigesters include crop residues, night soil/domestic sewage, and the organic fraction of municipal solid waste (OFMSW). Significant untapped feedstocks exist from SSA agro-processing and food production industries. Biodigesters available in SSA for household, community, and institutional installations include variations of fixed dome, plug flow, and floating cover digesters. Commercial digester designs applicable to the region include continuously stirred tank reactors and fixed film digesters. The key factors that need to be considered in selecting suitable biodigester designs for specific applications include: feedstock availability, water supply, energy demand, local materials and labour, and the level of commitment to operate and maintain the biodigester effectively.

Background There is an ongoing discussion on the rising CS rate worldwide. Suboptimal guideline adherence may be an important contributor to this rise. Before improvement of care can be established, optimal CS care in different settings has to be defined. This study aimed to develop and measure quality indicators to determine guideline adherence and identify target groups for improvement of care with direct effect on caesarean section (CS) rates. Method Eighteen obstetricians and midwives participated in an expert panel for systematic CS quality indicator development according to the RAND-modified Delphi method. A multi-center study was performed and medical charts of 1024 women with a CS and a stratified and weighted randomly selected group of 1036 women with a vaginal delivery were analysed. Quality indicator frequency and adherence were scored in 2060 women with a CS or vaginal delivery. Results The expert panel developed 16 indicators on planned CS and 11 indicators on unplanned CS. Indicator adherence was calculated, defined as the number of women in a specific obstetrical situation in which care was performed as recommended in both planned and unplanned CS settings. The most frequently occurring obstetrical situations with low indicator adherence were: 1) suspected fetal distress (frequency 17%, adherence 46%), 2) nonprogressive labour (frequency 12%, CS performed too early in over 75%), 3) continuous support during labour (frequency 88%, adherence 37%) and 4) previous CS (frequency 12%), with adequate counselling in 15%. Conclusions We identified four concrete target groups for improvement of obstetrical care, which can be used as a starting point to reduce CS rates worldwide.
