A propósito de una observación clínica que realiza una revisión sobre el síndrome de Plummer-Vinson, se estudian sus componentes c'líñícos, hematológicos, radiológicos y endoscópicos. El análisis de los conceptos patogénicos muestra la variedad de opiniones en el mecanismo de produ;::ción de esta curiosa afección, destacándose el valor de las perturbaciones hematológicas como elementos prínceps. La anemia hipocrómica microcítica, probablemente debida a perturbaciones en el metabolismo férrico y a la avitaminosis parecen ser los principales responsables de la situación. El diagnóstico se funda en el conocimiento del síndrome y en la pesquisa de sus componentes. El tratamiento exige la reposición de los elementos en déficit y una vigilancia permanente del paciente, dado el curso crónico de la enfermedad y su tendencia a rem1s10nes y exacerbaciones. Debe considerarse la posibilidad de la transformación neoplásica, lo que exige un control periódico de los pacientes portadores de este original síndrome.

The hyperarid climate of the central Sahara precludes permanent agriculture, although occasional temporary ponds, or etaghas, as a result of rain-fed flooding of wadi beds in the Tadrart Acacus Mountains of the Libyan Sahara allow the pastoral Kel Tadrart Tuareg to cultivate cereals. Geoarchaeological and archaeological data, along with radiocarbon dating and evidence from rock art, however, suggest a much greater antiquity for the exploitation of these etaghas. The authors propose that the present-day cultivation of etaghas mirrors attempts at flood-recession or rainfed cultivation by late prehistoric Pastoral Neolithic groups, who first exploited residual water resources to supplement their pastoral subsistence practices. (1) Background: Patients with mild cognitive impairment (MCI) often present impairment in executive functions (EFs). This study aimed to investigate the effect of high-frequency repetitive transcranial magnetic stimulation (rTMS) on EFs in patients with MCI. (2) Methods: A prospective trial was conducted on 11 patients with MCI. Participants underwent 25 min of 20 Hz rTMS for ten days on the right temporo-parietal junction (RTPJ) and medial prefrontal cortex (MPFC). Before (T0) and after rTMS treatment (T1), global cognitive profile and EFs were investigated using the Montreal cognitive assessment (MoCA), trial making test (TMT) A and B, and frontal assessment battery (FAB). Depression symptoms were assessed using the geriatric depression scale (GDS). Statistical analysis included Wilcoxon signed-rank test. (3) Results: After treatment, patients showed a significant improvement in the MoCA EFs subtask (T0 vs. T1, p = 0.015) and TMT-B (T0 vs. T1, p = 0.028). Five MCI patients with EF impairment showed full recovery of these deficits. No significant changes in the GDS were observed. (4) Conclusions: rTMS stimulation over the TPJ and MPFC induced significant short-term improvements in EFs in MCI patients. These findings suggest that the TPJ and MPFC may be involved in the attention-executive skills to redirect attention toward behaviorally relevant stimuli.

The positioning and construction of castles in ancient times responded not only to strategic opportunity, but also to the issue of geomorphological risk. We investigated castles and strongholds built in the era of the Great Countess Matilda of Canossa in part of the northern Apennines (Italy), in order to study the relationship between their positioning and the distribution of geomorphological and geological hazards. We observe how the location of castles follows clear patterns of avoidance of potential hazards: castles are kept far from the main fault systems and stream networks, and are mainly at a safe distance from landslide- and badlands-susceptible terrains. The knowledge of Medieval communities on landscape hazards was sufficiently advanced to minimise risks, while maintaining the strategic value of fortifications.

Hypertrophic cardiomyopathy (HCM) is the most common genetic cardiomyopathy. The molecular mechanisms determining HCM phenotypes are incompletely understood. Myocardial biopsies were obtained from a group of patients with obstructive HCM (n = 23) selected for surgical myectomy and from 9 unused donor hearts (controls). A subset of tissue-abundant myectomy samples from HCM (n = 10) and controls (n = 6) was submitted to laser-capture microdissection to isolate cardiomyocytes. We investigated the relationship among clinical phenotype, cardiac myosin proteins (MyHC6, MyHC7, and MyHC7b) measured by optimized label-free mass spectrometry, the relative genes (MYH7, MYH7B and MYLC2), and the MyomiR network (myosin-encoded microRNA (miRs) and long-noncoding RNAs (Mhrt)) measured using RNA sequencing and RT-qPCR. MyHC6 was lower in HCM vs. controls, whilst MyHC7, MyHC7b, and MyLC2 were comparable. MYH7, MYH7B, and MYLC2 were higher in HCM whilst MYH6, miR-208a, miR-208b, miR-499 were comparable in HCM and controls. These results are compatible with defective transcription by active genes in HCM. Mhrt and two miR-499-target genes, SOX6 and PTBP3, were upregulated in HCM. The presence of HCM-associated mutations correlated with PTBP3 in myectomies and with SOX6 in cardiomyocytes. Additionally, iPSC-derived cardiomyocytes, transiently transfected with either miR-208a or miR-499, demonstrated a time-dependent relationship between MyomiRs and myosin genes. The transfection end-stage pattern was at least in part similar to findings in HCM myectomies. These data support uncoupling between myosin protein/genes and a modulatory role for the myosin/MyomiR network in the HCM myocardium, possibly contributing to phenotypic diversity and providing putative therapeutic targets.

Technological variability within East African Middle Stone Age (MSA) lithic assemblages is considered a reflection of regional or local identities. These identities are a possible outcome of different social dynamics in human populations or interaction with the environment. Here we analysed the lithic assemblage from GOT 1-S site, in the Gotera area (Southern Ethiopia) in order to integrate fresh data from the region into the broader discussion on the technological variability of the MSA of Eastern Africa. To reach this goal, we considered lithic data from a surface dispersion, through the combination of different methods. Technological and statistical analyses performed on lithic assemblage suggest the existence of two distinct reduction sequences related to different raw materials: quartz and basalt. The technological analysis shows a more opportunistic reduction strategy on basalt artefacts, while the quartz lithic assemblage exhibits a more accurate preparation of core striking platforms and more predetermined products. The results of technological analyses on flakes, tools and cores were elaborated using Principal Component Analysis and Cluster Analysis. The two analyses allowed to confirm the hypothesis of two reduction strategies according to different raw material selection and managing. Furthermore, the categorical attributes from cores and flakes were processed by means of Correspondence Analysis, highlighting the technological differences linked to the different raw materials exploited. Moreover, the combined results from the technological and statistical analyses proved the validity of this integrated methodology to analyse a lithic collection from a surface context.

Contact (allergic) dermatitis is a skin disorder related to natural exposure to various allergens. Systemic contact dermatitis (SCD) describes a cutaneous eruption in response to systemic exposure to an allergen. The exact pathologic mechanism remains uncertain. Herein we describe a 36-year-old woman with symmetric systemic allergic contact dermatitis, unresponsive to conventional treatment, associated with dental alloy-contact hypersensitivity. We did skin patch testing and the blood lymphocyte transformation test (LTT) from the dental allergen series to assess contact allergy to restorative dental materials. On patch testing, positive allergic contact dermatitis reactions to metals occurred (nickel, potassium dichromate, and gold). Nickel hypersensitivity was confirmed by LTT, which also revealed silver-amalgam sensitization. Our case report highlights the need to consider adverse reactions to base-metal dental alloys in the differential diagnosis of cases of systemic allergic contact dermatitis.

Rain-fed cultivation in drylands--especially in arid and hyper-arid areas--is often considered to play a minor role in human subsistence. Drawing upon the results of ethnoarchaeological research in North Africa, this paper reviews non-irrigated agricultural practices in the absence of anthropogenic water-harvesting structures, and presents a proposal for how such practices can be identified in the drylands of the past. An improved understanding of the long-term development of rain fed cultivation augments our knowledge of past land-use strategies and can inform future models of sustainable agriculture in some of the world's driest regions. Funerary landscapes are eminent results of the relationship between environments and superstructural human behavior, spanning over wide territories and growing over centuries. The comprehension of such cultural palimpsests needs substantial research efforts in the field of human ecology. The funerary landscape of the semi-arid region of Kassala (Eastern Sudan) represents a solid example. Therein, geoarchaeological surveys and the creation of a desk-based dataset of thousands of diachronic funerary monuments (from early tumuli up to modern Beja people islamic tombs) were achieved by means of fieldwork and remote sensing over an area of ∼4100 km2. The wealth of generated information was employed to decipher the spatial arrangement of sites and monuments using Point Pattern Analysis. The enormous number of monuments and their spatial distribution are here successfully explained using, for the first time in archaeology, the Neyman-Scott Cluster Process, hitherto designed for cosmology. Our study highlights the existence of a built funerary landscape with galaxy-like aggregations of monuments driven by multiple layers of societal behavior. We suggest that the distribution of monuments was controlled by a synthesis of opportunistic geological constraints and cultural superstructure, conditioned by the social memory of the Beja people who have inhabited the region for two thousand years and still cherish the ancient tombs as their own kin's.

The Noceto 'Vasca Votiva' (votive tank), discovered in excavations on a terrace at the southern edge of the Po Plain, northern Italy, is a unique well-preserved wooden (primarily oak) structure dated to the advanced through late Middle Bronze Age (~1600-1300 BCE). This complex monument, comprising two super-imposed tanks, is generally linked with an important but uncertain ritual role involving water. The context provides extraordinary preservation of both wooden, other organic, and cultural finds. The key question until now, hindering further interpretation of this remarkable structure, has been the precise date of the tanks. Initial work pointed to use of the two tanks over about a century. Using dendrochronology and radiocarbon 'wiggle-matching' we report near-absolute construction dates for both of the tanks. The lower (older) tank is dated ~1444±4 BCE and the upper (more recent) tank is dated 12 years later at ~1432±4 BCE. This dating of the construction of the Noceto tanks in the 3rd quarter of the 15th century BCE further enables us to reassess the overall period of activity of this wooden complex and its association with a major period of societal change in the Bronze Age of northern Italy starting in the later 15th century BCE.

Herpes simplex virus 1 (HSV-1) infects skin and mucosal epithelial cells and then travels along axons to establish latency in the neurones of sensory ganglia. Although viral gene expression is restricted during latency, the latency-associated transcript (LAT) locus encodes many RNAs, including a 2 kb intron known as the hallmark of HSV-1 latency. Here, we studied HSV-1 infection and the role of the LAT locus in human skin xenografts in vivo and in cultured explants. We sequenced the genomes of our stock of HSV-1 strain 17syn+ and seven derived viruses and found nonsynonymous mutations in many viral proteins that had no impact on skin infection. In contrast, deletions in the LAT locus severely impaired HSV-1 replication and lesion formation in skin. However, skin replication was not affected by impaired intron splicing. Moreover, although the LAT locus has been implicated in regulating gene expression in neurones, we observed only small changes in transcript levels that were unrelated to the growth defect in skin, suggesting that its functions in skin may be different from those in neurones. Thus, although the LAT locus was previously thought to be dispensable for lytic infection, we show that it is a determinant of HSV-1 virulence during lytic infection of human skin.
