Abstract

Limited evidence exists on the long‐term effects of early feeding practices on child growth and development. We examined the relationships between infant feeding practices and child height and development at ages 2 and 6–7 years. We studied 885 mother–child dyads from a randomized controlled trial of preconception supplementation in Vietnam. Early initiation of breastfeeding (EIBF), exclusive breastfeeding (EBF), breastfeeding (BF) duration and minimum dietary diversity (MDD) were assessed using World Health Organization (WHO) guidelines. Child development was assessed by the Bayley Scales of Infant Development‐III at 2 years and the Wechsler Intelligence Scale for Children® – IV at 6–7 years. Child height‐for‐age z‐score (HAZ) was calculated from child height and age. Multivariable regression and structural equation models were used in analyses that controlled for confounding. EIBF and EBF at 6 months occurred in 52% and 62% of children, respectively. Mean breastfeeding duration was 18 months and 83% achieved MDD at 1 year. EIBF was associated with motor (β = 0.13, 95% confidence interval [CI]: 0.00, 0.28) and cognitive development at 2 years (β = 0.12, 95% CI: −0.01, 0.26), which in turn were positively associated with cognitive development at 6–7 years. EBF was directly associated with development at 6–7 years (β = 0.21, 95% CI:0.08, 0.34) whereas motor and cognitive development at 2 years explained 41%–75% of the relationship between EIBF and development at 6–7 years. HAZ at 2 years also mediated 70% of the association between MDD at 1 year and HAZ at 6–7 years. BF duration was not associated with child development and HAZ. Early infant feeding practices, especially EIBF and EBF, have important long‐term implications for optimizing child linear growth and cognition as they begin school. Key messages: Optimal breastfeeding practices during the first few months of life were positively associated with cognitive development in early childhood and school‐aged children.While exclusive breastfeeding was directly associated with child cognition at 6–7 years, cognitive development at 2 years mediated the association of early initiation of breastfeeding on cognition at 6–7 years. More research is needed to replicate these findings, explore mechanisms and evaluate the causal relationship.Child linear growth in early childhood mediated the association of minimum dietary diversity at 1 year on child height‐for‐age z‐score at 6–7 years. Abstract

The increasing acuity of mental health needs amongst youth coupled with a dwindling and lesser trained workforce (e.g., bachelor level and dependently licensed, early career providers) means that those with less experience and training in the delivery of evidence-based treatments (EBTs) are more likely to provide services to youth with the highest level of needs. EBTs for common pediatric mental health conditions are well documented in the literature. However, standardized and consistent implementation of EBTs do not exist at a systems level. Psychologists have expertise in the theory and clinical application of EBTs, developed through years of coursework and clinical rotations supervised by licensed doctoral level psychologists. Therefore, it is fruitful for psychologists to take leadership roles to ensure that the workforce is appropriately prepared to deliver and maintain the fidelity of EBTs. This paper will provide an overview of the six levels of mental health care delivered across our system (Levels 0–5), as well as outline five core components of an effective training and case consultation model designed to scale and spread EBTs across the system. These five core components include: (1) content experts as leaders, (2) institutional commitment, (3) intentionality in hiring and onboarding providers receptive to the model, (4) data driven approach, and (5) ongoing training and case consultation that includes modeling, role-playing and/or direct observation. Application of the model to multidisciplinary teams and sustainability at each level of care is addressed, as well as plans for further spread in scale and scope of this model. Integrated behavioral health approaches vary in scope and components of the models can be selected and implemented to meet the needs of each practice and the patient population served.

Abstract

Cancer cells with defects in DNA repair are highly susceptible to DNA-damaging agents, but delivery of therapeutic agents into cell nuclei can be challenging. A subset of lupus autoantibodies is associated with nucleolytic activity, and some of these antibodies are capable of nuclear penetration. We hypothesized that such antibodies might have potential as therapeutic agents targeted towards DNA repair-deficient malignancies. We identified the lupus autoantibody 5C6 as a cell-penetrating nucleolytic antibody and found that 5C6 has a differential effect on a matched pair of BRCA2-proficient and deficient DLD1 colon cancer cells. 5C6 selectively induced γH2AX in, and suppressed the growth of, the BRCA2-deficient cells. These findings demonstrate the potential utility of 5C6 in targeted therapy for DNA repair-deficient malignancies and strengthen the rationale for studies of additional lupus autoantibodies in order to identify the best candidates for development as therapeutic agents. In addition, the toxic effect of 5C6 on BRCA2-deficient cells provides further support for the hypothesis that some lupus autoantibodies contribute to the lower risk of specific cancers associated with systemic lupus erythematosus.
