Abstract

Background. Accurate preoperative diagnosis of tumors of the ampulla of Vater is difficult because ampullary biopsies have a high false-negative rate. Recently, it has been suggested that p53 mutations in tumors of the ampulla of Vater are associated with the transformation of adenomas and low grade carcinomas to high grade carcinomas. The purpose of this study was to determine the extent of p53 protein accumulation in tumors of the ampulla of Vater, and to determine whether p53 accumulation can be detected in false-negative biopsies. Methods. Using a monoclonal anti-p53 antibody, sections of 4 normal ampullas, 5 adenomas, 17 carcinomas, and 9 initial biopsies of 9 of the tumors of the ampulla of Vater that had no morphologic evidence of carcinoma were immunostained. Results. None of the 4 normal ampullas (0%), 2 of 5 adenomas (40%), and 16 of 17 carcinomas (94%) were positive for p53. This p53 positivity was present through all stages of ampullary carcinoma. Of the nine initial biopsies negative for carcinoma, seven were positive for p53 and, of these, six (86%) were found to be carcinomas upon resection. Conclusions. 1) The molecular events leading to p53 accumulation in tumors of the ampulla of Vater occur early in the neoplastic process. 2) Tumors of the ampulla of Vater with biopsies negative for malignancy but positive for p53 are very likely to be carcinomas. Abstract

Inflammatory bowel disease (IBD) patients in apparent clinical remission who present with irritable bowel syndrome (IBS)-like symptoms pose a diagnostic and therapeutic dilemma that is called post-IBD IBS. When associated with a diarrheal IBS presentation, this clinical syndrome is known as post-IBD IBS-D. We review and describe the literature regarding the clinical overlap of IBD and IBS. We discuss prevalent theories regarding the pathophysiology of post-IBD IBS-D and whether this presentation represents coincident inherent IBS-D, IBS-D triggered by IBD, or an even more subtle level of IBD activity that is unrecognized by available laboratory modalities. We also discuss observations that post-IBD IBS-D patients harbor significantly increased colon mucosal eosinophils and appear to respond to a GI-hypoallergenic diet and budesonide therapy. The symptoms overlap between IBD and IBS complicates diagnosis and subsequent management of patients with post-IBD IBS-D. In addition to current theories regarding the pathophysiology of this condition such as alterations in mucosal inflammation, the microbiota, mucosal permeability, and gut–brain interactions. This new avenue of eosinophilic colopathy and therapy directed toward food-derived immune response in patients with post-IBD IBS-D deserves additional investigation. [ABSTRACT FROM AUTHOR]

95. The value of the preoperative mucosal biopsy in the diagnosis of colorectal mucinous adenocarcinoma

Abstract

Background. As a rule, mucinous colorectal adenocarcinomas tend to be at a more advanced stage at the time of discovery than their nonmucinous counterparts. This study assesses the potential value of the preoperative biopsy in the diagnosis of such mucinous adenocarcinomas. Methods. The preoperative biopsy specimens and the corresponding resection specimens of 189 patients with colorectal carcinomas were examined and compared by conventional light microscopic study. The stage of the tumor, using the modified Dukes classification of Astler and Coller, was correlated with the percentage of mucinous component (MC) in the resection specimens. The MC in the preoperative biopsy was assessed by the presence of: (1) malignant-appearing glands disrupted by the presence of abundant extruded intraluminal mucin; (2) pools of mucin in the connective stroma of the adenocarcinoma; and (3) superficial pools of mucin containing ribbons or clusters of neoplastic epithelial cells. Results. The presence of a significant (more than 25%) MC in the resection specimen correlated well with an advanced stage of the tumor; 82% of tumors with more than 25% MC were at the B2 or higher stage, compared with 64% of tumors with less than 25% MC (P < 0.05). Finding MC in the preoperative biopsy correlated well with a similar finding in the resection specimen and with a B2 or higher stage of the tumor in such specimens; 83% of MC-positive biopsy specimens exhibited more than 25% MC in the corresponding resection specimen, whereas only 10% of MC-negative biopsy specimens were associated with a surgical specimen containing more than 25% MC (P < 0.001). Similarly, 83% of such MC-positive biopsy specimens revealed a carcinoma at the B2 or higher stage upon resection, compared with 63% of the MC-negative biopsy specimens (P < 0.02). Conclusions. Colorectal adenocarcinomas showing MC in the preoperative biopsy are significantly more likely to reveal a high mucin content and to be at an advanced stage at resection. Thus, such preoperative findings should be recorded and made available on a prospective basis to the treating physicians.

100. The significance of 'indefinite for dysplasia' grading in Barrett metaplasia

Abstract

Context.--For a confident diagnosis of dysplasia in Barrett metaplasia, the epithelial atypia should also involve the surface epithelium. However, pathologists are often faced with biopsies where the crypts show dysplasia, but the surface epithelium is either uninvolved or unevaluable. We previously grouped these cases with indefinite for dysplasia (IND). Objective.--To determine the clinical significance of IND grading in Barrett metaplasia. Design.--All biopsies from 276 prospectively followed patients with Barrett metaplasia, who did not have high-grade dysplasia (HGD) or esophageal adenocarcinoma (EAC) on initial biopsy, were graded as negative for dysplasia, IND, low-grade dysplasia (LGD), HGD, and EAC. Biopsies with multifocal IND or LGD were graded as INDM or LGDM, respectively. Results.--Only 3 of 193 patients (2%) with an initial diagnosis of negative for dysplasia and only 1 of 48 patients (2%) diagnosed with IND progressed to HGD or EAC. By contrast, 1 of 7 patients (14%) with INDM, 2 of 21 (10%) with LGD, and 1 of 7 (14%) with LGDM progressed to HGD or EAC. There was no significant difference in progression rate between patients with an initial diagnosis of negative for dysplasia and those diagnosed IND nor were there significant differences among patients with initial diagnoses of INDM, LGD, or LGDM. Kaplan-Meier analysis showed that patients with INDM, LGD, or LGDM on initial biopsy (group 1) were more likely to progress to HGD or EAC than were those patients who were diagnosed negative for dysplasia or IND (group 2; log-rank test, P < .001). Conclusions.--Multifocal IND in an esophageal biopsy from a patient with Barrett metaplasia has the same clinical implication as LGD., Histopathologic diagnosis of dysplasia is the cornerstone of current surveillance programs aimed at detecting esophageal adenocarcinoma (EAC) at an early stage in patients with Barrett metaplasia.
