The Hans algorithm categorizes the diffuse large B-cell lymphoma (DLBCL) into two major subtypes: the germinal center B-cell-like (GCB) DLBCL and the non-GCB DLBCL. This classification is based on three immunohistochemical markers: CD10, BCL6, and MUM1. The non-GCB subtype is associated with lower overall survival (OS) and progression-free survival (PFS) rates compared to the GCB. DLBCL without positive staining for these three markers (CD10–, BCL6–, MUM1–), also called a triple negative or TN, are classified as the non-GCB subtype. However, they show different clinical characteristics and better prognosis than others assigned to the same cell-of-origin group. Herein, we report a case of a TN non-GCB DLBCL with a complete response after R-CHOP therapy. Together with previous reports of TN non-GCB DLBCLs, our case might depreciate the prognostic value of the Hans algorithm, which was already controversial in the literature, especially in the chemoimmunotherapy era.

Resume Le prurigo nodulaire est une dermatose prurigineuse chronique, responsable d’un retentissement majeur sur la qualite de vie des patients. Le traitement est difficile et souvent decevant. Une comorbidite psychiatrique est souvent retrouvee chez les patients atteints de prurigo nodulaire. Notre objectif etait de proposer une prise en charge psychotherapeutique chez des patients atteints de prurigo nodulaire refractaire. Au cours du travail, les patients ont pu verbaliser et se decharger d’une souffrance psychique importante. Le soulagement des tensions psychiques s’est traduit sur le plan clinique par la resolution du prurit et la regression des lesions de prurigo nodulaire. Ces elements confirment l’importance d’une evaluation et d’un accompagnement psychologiques dans la prise en charge d’un patient souffrant d’un prurigo nodulaire, en plus des therapeutiques classiques.

Background: Subungual exostosis is an unusual benign nail tumor, mostly located on the big toe. It generally affects young people and manifests as uncomfort during footwear. Method: A monocentric retrospective study was conducted at the outpatient consultation for nail disorders at the Department of Dermatology of the University of Casablanca, Casablanca, Morocco, between April 2006 and October 2019. Results: We diagnosed subungual exostosis in 48 patients, including 25 men and 23 women, with an average age of 20 years. The subungual exostosis was located on the hallux in 34 cases, the second toe in 10 cases, and the index finger in 2 cases. Nail trauma was found in 27 patients. The tumor was entirely excised in all the patients. Conclusion: Large series of subungual exostosis are reported by orthopedic surgeons. However, our outpatient consultation for nail disorders allowed the diagnosis and appropriate management of subungual exostosis.

Resume Le syndrome de Wells secondaire a une piqure d’abeille est peu frequent. Nous en rapportons un cas, survenu chez un nourrisson de 18 mois ayant presente un œdeme de la main gauche surmonte de lesions bulleuses apres une piqure d’abeille. Le patient a ete traite initialement par des antibiotiques, mais sans amelioration. Un traitement par corticotherapie systemique a ete instaure avec une bonne evolution. Le diagnostic de la cellulite de Wells a ete retenu devant l’aspect clinique, les donnees biologiques et histopathologiques.

Background: Cutaneous cryptococcosis occurs in 10–15% of patients with disseminated cryptococcosis. It typically presents as papulonodular molluscum-like lesions, but it can also produce a wide variety of lesions. Cryptococcal infection of the nail unit has never been reported. Case Report: A 28-year-old woman with a history of HIV with disseminated cryptococcosis in complete remission was referred to evaluate a subungual swelling of the right middle finger. Examination revealed an ulcero-burgeoning nodule over the right middle finger’s subungual area with onycholysis, eschar, and erosion. An excisional biopsy was performed. Histopathological analysis demonstrated multiple histiocytic granulomas centered by encapsulated yeast cells. Culture grew Cryptococcus neoformans var. neoformans. After 9 months of follow-up, there was no recurrence of the lesion. Discussion: It is the first reported case of nail involvement in the course of cutaneous cryptococcosis. Definitive diagnosis required pathology and culture. Cryptococcal infection of the nail unit was recalcitrant to systemic therapy while the remaining infection cleared. Our case report suggests that surgical excision associated with systemic therapy is the best treatment approach for subungual cryptococcosis. Recognition of rare manifestations of cutaneous cryptococcosis, such as ours, is essential because HIV cases increase continuously.

Introduction and importance - Angiolymphoid hyperplasia with eosinophilia (ALHE) is a rare benign microvascular proliferation. It is clinically characterized by intradermal or subcutaneous papules and/or nodules. Treatment consists mainly of surgery or laser therapy, with a high recurrence rate. Spontaneous regression is rare but possible. Case presentation A 72-year-old man presented for an erythematous plaque on his scalp. Physical examination demonstrated a large angiomatous indurated subcutaneous plaque. The lesion was not pulsatile. The doppler ultrasound revealed no underlying vascular malformation. The biopsy revealed an ALHE. No further treatment was carried out. During the follow-up, the lesion gradually subsided. After one-year follow-up, the lesion had regressed with no sign of recurrence. Clinical discussion There are different treatment modalities for AHLE management, including surgical excision, cryotherapy and lasers. However, none of these modalities has provided consistent results and recurrence after treatment is common.). Rare instances of spontaneous regression have been reported. Conclusion Given the rarity of cases of spontaneous resolution, ALHE may be considered as a chronic condition, especially since recurrences are frequent. Emerging reports show that coronavirus disease 2019 (COVID-19) may lead to autoimmune and autoinflammatory diseases. However, COVID-19 triggered systemic lupus erythematosus (SLE) has never been reported to our knowledge. COVID-19 also has associated cutaneous manifestations. Here we present a case of SLE with antiphospholipid antibody syndrome in a previously healthy patient with COVID-19, who subsequently developped a varicella-like exanthem on the trunk. The disease resulted in death of the patient. The pathophysiological mechanisms resulting in overlapping disorders in our patient remain unknown, adding to the growing mystery of this virus and raising questions about the nature of its link with cutaneous, autoimmune, and autoinflammatory manifestations. Sharing the images of this case may benefit physicians dealing with similar patients during this pandemic.

Introduction Devant un lymphome B primitif cutane a grandes cellules (LBPC-GC), l’algorithme de Hans se fonde sur 3 marqueurs (CD10, Bcl6, et MUM1), et permet la distinction entre lymphome B a centre germinatif (GCB) et a centre non germinatif (non-GCB). Cette distinction a une valeur diagnostique et pronostique. Les LBPC-GC dits « triple negatif » presentent une negativite de ces 3 marqueurs. Ils sont rares et appartiennent au groupe non-GCB selon l’algorithme de Hans. Ils s’en distinguent toutefois par des caracteristiques cliniques differentes et un meilleur pronostic. Nous rapportons un cas de lymphome B primitif cutane a grandes cellules presentant un profil « triple negatif » ayant necessite le recours a une chimiotherapie. Observation Un homme de 51 ans presentait depuis fevrier 2018 un nodule du haut du dos, augmentant progressivement de taille et evoluant vers une ulceration centrale avec issue de pus, dans un contexte d’asthenie, d’amaigrissement de 8 kg et de sueurs nocturnes. L’examen trouvait une tumeur du haut du dos, lobulee, a base induree, mesurant 10 cm de grand axe, a surface richement vascularisee et siege de 2 ulcerations a fond fibrineux. L’examen histologique montrait une proliferation lymphocytaire avec des cellules de grande taille disposees en nodules et nappes diffuses, siege d’atypies cytonucleaires. L’etude immunophenotypique montrait une positivite des marqueurs de phenotype B (CD20 + ), une negativite du bcl-6, du bcl-2, du CD10 et de Mum 1. Le myelogramme et la biposie osteomedullaire n’objectivaient pas de signe d’infiltration lymphomateuse. L’image histologique, le profil phenotypique, ainsi que l’absence d’expression du bcl-2 et d’atteinte medullaire nous orientaient vers un LBPC-GC. Le patient beneficiait d’un protocole rituximab-polychimiotherapie. L’evolution etait marquee par la regression complete de la lesion ; le recul est de 6 mois. Discussion Notre patient presente un LBPC-GC « triple negatif », appartenant au groupe non-GCB selon l’algorithme de Hans, et ayant montre une reponse complete au traitement. Le LBPC-GC « triple negatif » reste une entite mal definie, et il n’existe a notre connaissance aucune etude ayant caracterise avec precision le comportement clinique et biologique de ce type de lymphome. De fait, bien qu’appartenant au groupe non-GCB, le LBPC-GC « triple negatif », presente des caracteristiques cliniques et un pronostic similaires au groupe GCB. L’existence d’une telle entite semble remettre en cause la valeur pronostique de l’algorithme de Hans, deja controverse dans la litterature. Conclusion Compte tenu de l’heterogeneite clinique et biologique des LBPC-GC, l’identification de nouveaux biomarqueurs est necessaire afin de mieux predire leur pronostic et guider leur prise en charge therapeutique.
