51. Glial Fibrillary Acidic Protein (GFAP) Astrocytopathy: An Emerging Cause of Meningoencephalomyelitis in Children and Adolescents.

Abstract

Introduction: We describe 5 children with GFAP astrocytopathy with the goal of further characterizing this rare form of meningoencephalomyelitis. Methods: Retrospective chart review of patients diagnosed with GFAP astrocytopathy between 2019 and 2021. Results: Patients were 8-17 years old, and all were male. Fever, headache, and vomiting were common presenting symptoms, and weakness, tremor, and ataxia were common initial examination findings. Initial magnetic resonance imaging (MRI) showed spinal cord abnormalities in 2 patients and leptomeningeal enhancement in 1. Most patients had cerebral spinal fluid pleocytosis, and all screened negative for malignancy. Three patients progressed to coma, and all were treated with immunosuppressant therapy. By discharge, all patients had improved over their clinical nadir, although none had returned to baseline. Discussion: GFAP astrocytopathy is a recently recognized cause of meningoencephalomyelitis in children. 55. Negative and Positive Factors Associated with the Well-Being of Lesbian, Gay, Bisexual, Transgender, Queer, and Questioning (LGBTQ) Youth

Abstract

Factors associated with the well-being of lesbian, gay, bisexual, transgender, queer, and questioning (LGBTQ) youth were qualitatively examined to better understand how these factors are experienced from the youths' perspectives. Largely recruited from LGBTQ youth groups, 68 youth participated in focus groups (n = 63) or individual interviews (n = 5). The sample included 50% male, 47% female, and 3% transgender participants. Researchers used a consensual methods approach to identify negative and positive factors across 8 domains. Negative factors were associated with families, schools, religious institutions, and community or neighborhood; positive factors were associated with the youth's own identity development, peer networks, and involvement in the LGBTQ community. These findings suggest a pervasiveness of negative experiences in multiple contexts, and the importance of fostering a positive LGBTQ identity and supportive peer/community networks. Efforts should work towards reducing and eliminating the prejudicial sentiments often present in the institutions and situations that LGBTQ youth encounter.

70. Multisite Randomized Trial of Behavioral Interventions for Women with Co-Occurring PTSD and Substance Use Disorders

Abstract

The authors compared the effectiveness of the Seeking Safety group, cognitive-behavioral treatment for substance use disorder and posttraumatic stress disorder (PTSD), to an active comparison health education group (Women's Health Education [WHE]) within the National Institute on Drug Abuse's Clinical Trials Network. The authors randomized 353 women to receive 12 sessions of Seeking Safety (M = 6.2 sessions) or WHE (M = 6.0 sessions) with follow-up assessment 1 week and 3, 6, and 12 months posttreatment. Primary outcomes were the Clinician Administered PTSD Scale (CAPS), the PTSD Symptom Scale-Self Report (PSS-SR), and a substance use inventory (self-reported abstinence and percentage of days of use over 7 days). Intention-to-treat analysis showed large, clinically significant reductions in CAPS and PSS-SR symptoms (d = 1.94 and 1.12, respectively) but no reliable difference between conditions. Substance use outcomes were not significantly different over time between the two treatments and at follow-up showed no significant change from baseline. Study results do not favor Seeking Safety over WHE as an adjunct to substance use disorder treatment for women with PTSD and reflect considerable opportunity to improve clinical outcomes in community-based treatments for these co-occurring conditions. 71. Michigan State University Extension Educators' Perceptions of the Use of Digital Technology in Their Work

Abstract

This research study examined Michigan State University Extension educators' perceptions of the use of digital technology in their work. It used a mixed method of research which included a mailed survey and interviews of selected respondents. A census survey using Dillman's Total Design method was sent to 290 field staff of Michigan State University Extension. Of these, 272 completed and returned the survey instrument for a 94% rate of return. Semi structured interviews were conducted with 15 of the respondents to provide in-depth qualitative data to enrich the understanding of the issues for the researcher. The mailed survey instrument was examined for validity by a panel of experts and pilot tested on scale items to assess reliability. The mailed survey included questions on access to technology both at work and at home, preparation for the use of technology, actual use of technology, usefulness and ease of use, confidence and comfort in use and general and technical support for the use of technology. Low, medium and high total use respondent were compared and analyzed. Results show that although Extension Educators consider themselves to be well prepared to use technology and said it was highly useful to them in their work, most use of technology was limited to e-mail, word processing, file attachments and cell phones. Only a small minority use web technology, wikis or had published educational materials on a website or the MSUE portal. Staff sometimes furnished their own digital technology tools if they thought they were highly useful. Barriers to use of newer technologies were sited as lack of access, lack of support, lack of time to learn new technologies. Low users sometimes said they would only use technology if it was required and they preferred one-on-one tutoring to learn how to use technology. Low users recognized that they were themselves a barrier to the use of technology. Medium users said clientele preferred face-to-face education and would not use technology. They often viewed technology as "somebody else's problem." High users were the only group to use web based digital technology and they were able to integrate the three spheres of Mishra and Koehler's TPACK model of technology use; expertise in technology, pedagogy and content. High users were more apt to be self taught, client oriented and to have a grasp of the affordances of various technology applications. They preferred advanced classes on web page design, as well as photo and video editing and production. Recommendations were to provide local and regional training which includes practical ways to use technology to enhance programming, identify regional sources of support, integrate technology use into the MSUC culture and encourage the use of technology by highlighting creative solutions to use and providing opportunities for playful use. Better access must be provided and technology support should be easily accessible. Further research recommendations include case studies of individual counties, case studies of high users, research on difference by programming area and the development of documented technology solution to programming needs which could be accessed by educators looking for ideas. 72. The Cerebellar Mutism Syndrome and Its Relation to Cerebellar Cognitive Function and the Cerebellar Cognitive Affective Disorder

Abstract

The postoperative cerebellar mutism syndrome (CMS), consisting of diminished speech output, hypotonia, ataxia, and emotional lability, occurs after surgery in up to 25% of patients with medulloblastoma and occasionally after removal of other posterior fossa tumors. Although the mutism is transient, speech rarely normalizes and the syndrome is associated with long-term adverse neurological, cognitive, and psychological sequelae. The clinical, neuroradiographic, and neuropsychological findings associated with CMS as well as possible mechanisms of injury are reviewed. Theories about the pathophysiology of CMS have evolved along with our understanding of the cerebellum as an important structure in the distributive neurocircuitry underlying complex speech, cognition, and behavior. CMS shares many similarities with the cerebellar cognitive affective syndrome, more commonly described in adults and consisting of disturbances of executive function, visuospatial skills, nonmotor language, and affect regulation. Future directions include more thorough neuropsychological characterization, functional and diffusion tensor imaging studies, and investigations into the underlying differences that may make some patients more vulnerable to CMS.

73. Replicating a Teen HIV/STD Preventive Intervention in a Multicultural City

Abstract

Although there are now several adolescent HIV and STD preventive interventions of demonstrated efficacy in the literature, little is understood about the portability of these interventions. This study replicated Stanton's Focus on Kids intervention, developed for inner city African American adolescents, in a different population, transferring it to a multicultural city. Despite careful replication of the original study's procedures, youth in the preventive intervention condition of the replication study did not improve in attitudes, perceived norms, self-efficacy, or intentions toward sexual initiation, condom use, or abstinence compared with a carefully matched control condition. We discuss several possible reasons for this failure to replicate, concluding that the most likely reason is the lower rates of sexual activity among youth in the replication city.

Abstract

A framework for approaching an issue-oriented American history course through the small group technique (grouping of 5 to 7 students) is provided in this paper. Student grouping in a cooperative style enhances affective and cognitive development and, further, allows for evaluation of each member's and group's contributions. Provided are rationale; educational objectives; suggestions for small group implementation, including information on the methods of cooperative grouping and evaluation; twenty most often asked questions about the cooperative group process with an answer to each question; training for roles in the cooperative group with responsibilities enumerated for the key roles of moderator, evaluator, assistant moderator, and teacher; and instructions for small group teams. Also included are forms to be used in small group evaluation along with a practice session rating sheet, student rating sheet for a tournament, and a guide sheet for rating. (SJM)

95. Race Differences in Antisocial Behaviors and Attitudes and Early Initiation of Substance Use.

Abstract

Findings from fifth grade African-American, white, and Asian American students revealed that antisocial behavior and attitudes were stronger predictors of substance initiation for Asian Americans than for other groups. For whites, self-reported and teacher-rated behavior were significantly related to substance initiation. For African Americans, self-reported antisocial behavior, and for Asian Americans self-reported delinquent behavior and attitudes, predicted substance initiation. (Author/NB)
