Background: We aimed to investigate the extent of the agreement on practices around brain death and postmortem organ donation. Methods: Investigators from 67 Collaborative European NeuroTrauma Effectiveness Research in Traumatic Brain Injury (CENTER-TBI) study centers completed several questionnaires (response rate: 99%). Results: Regarding practices around brain death, we found agreement on the clinical evaluation (prerequisites and neurological assessment) for brain death determination (BDD) in 100% of the centers. However, ancillary tests were required for BDD in 64% of the centers. BDD for nondonor patients was deemed mandatory in 18% of the centers before withdrawing life-sustaining measures (LSM). Also, practices around postmortem organ donation varied. Organ donation after circulatory arrest was forbidden in 45% of the centers. When withdrawal of LSM was contemplated, in 67% of centers the patients with a ventricular drain in situ had this removed, either sometimes or all of the time. Conclusions: This study showed both agreement and some regional differences regarding practices around brain death and postmortem organ donation. We hope our results help quantify and understand potential differences, and provide impetus for current dialogs toward further harmonization of practices around brain death and postmortem organ donation.

Abstract

Gem-quality (cabochon) ruby-bearing occurrences (here termed PAR-1 and PAR-5) located near Paranesti, north eastern Greece have been systematically studied for the first time in this paper. Tectonically, the occurrences are located within the Nestos Shear Zone (NSZ). The NSZ separates two distinct geological units. The Rhodope Terrane is a heterogeneous unit of gneisses, mafic, ultramafic, and meta-sedimentary rocks in the hanging wall. The footwall Pangaion-Pirin Complex consists of marbles and acid gneisses of a Mesozoic carbonate platform on pre-Mesozoic continental basement. In this paper, a range of petrographic and geochemical techniques were used to determine (1) any similarities and differences to other mafic-ultramafic hosted ruby deposits worldwide; (2) distinctive geochemical fingerprints for Paranesti; and (3) the likely P-T conditions of formation. Detailed petrographic and whole-rock analyses utilizing ICP-MS, XRF, and XRD have found the Paranesti corundum to be of a mafic/ultramafic protolith with approximately 40 wt.% SiO2, 16 wt.% Mg, 11000 ppm Cr, and 440 ppm Ni. EMPA major element analysis determined the mineral inclusions within the corundum grains to be picotite and hercynite spinels. Pargasite is the dominant amphibole within the corundum-bearing amphibole schist host. The surrounding non-corundum bearing chlorite schist mainly comprises clinochlore. Petrographic examination of the mineral assemblages within the corundum-bearing schists revealed strong fracturing and alignment (parallel to the main regional foliation) of the corundum grains and margarite reaction rims around the corundum. The surrounding non-corundum amphibolites also contain anorthite, along with relict sillimanite, kyanite, and chlorite/muscovite/epidote overprinting. Detailed LA-ICP-MS trace element analysis of the color range of corundum from the two occurrences showed the corundum to be mainly of metamorphic origin, though pale rubies from PAR-5 suggest some metasomatic influence. The corundum displays distinctive geochemical locality signatures, with a combination of high Cr (average 2300 ppm with 15% sample points on core positions >5000 ppm and maximum 8600 ppm); high Si (average 1400 ppm with 40% over 1500 ppm and maximum 2500 ppm), low Mg (average 30 ppm), and very low V, Ti, and Ga. Abstract

Barrett’s esophagus (BE) with high-grade dysplasia (HGD) defines a group of individuals at high risk of progression to esophageal adenocarcinoma (EA). Fluorescence in situ hybridization (FISH) has been shown to be useful for the detection of dysplasia and EA in endoscopic brushing specimens from BE patients. The aim of this study was to determine whether FISH in combination with histological findings would further identify more rapid progressors to EA. This is a retrospective cohort study of high-risk patients, having a history of biopsy-confirmed HGD without EA, with an endoscopic brushing specimen analyzed by FISH while undergoing endoscopic surveillance and treatment between April 2003 and October 2010. Brushing specimens were assessed by FISH probes targeting 8q24 (MYC), 9p21 (CDKN2A), 17q12 (ERBB2), and 20q13 (ZNF217) and evaluated for the presence of polysomy, defined as multiple chromosomal gains (displaying ≥ 3 signals for ≥ 2 probes). Specimens containing ≥ 4 cells exhibiting polysomy were considered polysomic. HGD was confirmed by at least two experienced gastrointestinal pathologists. Of 245 patients in this study, 93 (38.0%) had a polysomic FISH result and 152 (62.0%) had a non-polysomic FISH result. Median follow-up was 3.6 years (interquartile range [IQR] 2–5 years). Patients with a polysomic FISH result had a significantly higher risk of developing EA within 2 years (14.2%) compared with patients with a non-polysomic FISH result (1.4%, P < 0.001). These findings suggest that a polysomic FISH result in BE patients with simultaneous HGD identifies patients at a higher risk for developing EA compared with those with non-polysomy.

Abstract

Introduction: The strength of evidence underpinning care and treatment recommendations in traumatic brain injury (TBI) is low. Comparative effectiveness research (CER) has been proposed as a framework to provide evidence for optimal care for TBI patients. The first step in CER is to map the existing variation. The aim of current study is to quantify variation in general structural and process characteristics among centers participating in the Collaborative European NeuroTrauma Effectiveness Research in Traumatic Brain Injury (CENTER-TBI) study. Methods: We designed a set of 11 provider profiling questionnaires with 321 questions about various aspects of TBI care, chosen based on literature and expert opinion. After pilot testing, questionnaires were disseminated to 71 centers from 20 countries participating in the CENTER-TBI study. Reliability of questionnaires was estimated by calculating a concordance rate among 5% duplicate questions. Results: All 71 centers completed the questionnaires. Median concordance rate among duplicate questions was 0.85. The majority of centers were academic hospitals (n = 65, 92%), designated as a level I trauma center (n = 48, 68%) and situated in an urban location (n = 70, 99%). The availability of facilities for neuro-trauma care varied across centers; e.g. 40 (57%) had a dedicated neuro-intensive care unit (ICU), 36 (51%) had an in-hospital rehabilitation unit and the organization of the ICU was closed in 64% (n = 45) of the centers. In addition, we found wide variation in processes of care, such as the ICU admission policy and intracranial pressure monitoring policy among centers. Conclusion: Even among high-volume, specialized neurotrauma centers there is substantial variation in structures and processes of TBI care. This variation provides an opportunity to study effectiveness of specific aspects of TBI care and to identify best practices with CER approaches.

Abstract

In this paper, methods to analyze the flow during semiconductor chip encapsulation have been developed. A numerical method is used for the flow analysis in the chip cavity. In this study, for accurate analysis of flow in the chip cavity, models for the cross flow through the leadframe openings have been developed. The models have been verified by comparing with two experiments. In the first experiment, clear polymer and transparent mold have been used for the visualization of flow in a cavity with a leadframe. In the next experiment, actual epoxy molding compound together with an industrial encapsulation process have been used to observe the melt-front advancement shapes. Abstract

In this paper, we present the development of a new micro fiber optical sensor probe with side viewing capability for early diagnosis of cancers in upper gastrointestinal tract. The probe consists of a oblique incidence source and a collection array of microfabricated curved SU-8 waveguides for collecting spatially resolved diffuse reflectance from tissue surface. The probe has been used to measure ex-vivo esophagus specimens to differentiate cancerous tissues from benign ones.

Abstract

Barrett's esophagus ( BE) with high-grade dysplasia ( HGD) defines a group of individuals at high risk of progression to esophageal adenocarcinoma ( EA). Fluorescence in situ hybridization ( FISH) has been shown to be useful for the detection of dysplasia and EA in endoscopic brushing specimens from BE patients. The aim of this study was to determine whether FISH in combination with histological findings would further identify more rapid progressors to EA. This is a retrospective cohort study of high-risk patients, having a history of biopsy-confirmed HGD without EA, with an endoscopic brushing specimen analyzed by FISH while undergoing endoscopic surveillance and treatment between April 2003 and October 2010. Brushing specimens were assessed by FISH probes targeting 8q24 ( MYC), 9p21 ( CDKN2A), 17q12 ( ERBB2), and 20q13 ( ZNF217) and evaluated for the presence of polysomy, defined as multiple chromosomal gains (displaying ≥ 3 signals for ≥ 2 probes). Specimens containing ≥ 4 cells exhibiting polysomy were considered polysomic. HGD was confirmed by at least two experienced gastrointestinal pathologists. Of 245 patients in this study, 93 (38.0%) had a polysomic FISH result and 152 (62.0%) had a non-polysomic FISH result. Median follow-up was 3.6 years (interquartile range [ IQR] 2-5 years). Patients with a polysomic FISH result had a significantly higher risk of developing EA within 2 years (14.2%) compared with patients with a non-polysomic FISH result (1.4%, P < 0.001). These findings suggest that a polysomic FISH result in BE patients with simultaneous HGD identifies patients at a higher risk for developing EA compared with those with non-polysomy.
