52. Continuity or Change? : Improved Understanding of Attitudes Towards Female Genital Cutting after Migration from Somalia to Sweden

Do people’s attitudes towards female genital cutting (FGC) change after they migrate from a country where the practice is common, to one where it is not? Alongside increased levels of migration, this question is increasingly being raised. This thesis aimed to expand the understanding about attitudes towards FGC held by Somali men and women in Sweden, and thereby to identify potential factors that impede or facilitate the cessation of FGC. Cross-sectional questionnaire data were collected in four Swedish municipalities to assess attitudes to FGC. To further explore perceptions of FGC, as well as the circumcision of boys, semi-structured interviews and focus group discussions were conducted. Data were collected in 2015. The findings identified an overall widespread opposition to forms of FGC that cause anatomical change. A majority (78%) expressed an opposition to the continuation of all forms of FGC, with the odds of supporting FGC decreasing with increased years of residency in Sweden. An identified 18% reported a support for the continuation of pricking (FGC type IV). A support of pricking was linked with perceiving it as acceptable according to Islam, not a violation of children’s rights, and not causing long-term health complications. Pricking was not defined as a form of FGC by 32%. Most men described a preference to marry an uncircumcised woman (76%) or one who had had pricking (16%). How the individuals perceived the support of FGC in the Swedish Somali community corresponded well with their own approval of the practice. While there seemed to be a continuity regarding the Swedish Somalis’ core values of being a good Muslim, not inflicting harm, and upholding respectability, re-evaluation of how these are applied when it comes to circumcision of girls and boys was identified. This resulted in FGC being viewed as a practice that could be abandoned or adapted. Paradoxically, based on the same core values, the circumcision of boys was continuously perceived as an unquestionable required practice. Altogether, these results suggest that a shift in convention towards no FGC is taking place. However, the identified lack of consensus on practices regarded as FGC needs further attention.

Background: Pricking, classified as female genital cutting (FGC) type IV by the World Health Organization, is an under-researched area gaining momentum among diaspora communities. Our aim was to explore factors associated with being supportive of pricking among Somalis in Sweden. Methods: In a cross-sectional design, attitudes and knowledge regarding FGC, and measures of socioeconomic status, acculturation, and social capital, were assessed by a 49-item questionnaire in four municipalities in Sweden. Data were collected in 2015 from 648 Somali men and women, ≥ 18 years old, of which 113 supported the continuation of pricking. Logistic regression was used for the analysis. Results: Those more likely to support the continuation of pricking were older, originally from rural areas, and newly arrived in Sweden. Further, those who reported that they thought pricking was: acceptable, according to their religion (aOR: 10.59, 95% CI: 5.44–20.62); not a violation of children’s rights (aOR: 2.86, 95% CI: 1.46–5.61); and did not cause long-term health complications (aOR: 5.52, 95% CI: 2.25–13.52) had higher odds of supporting pricking. Religion was strongly associated with the support of pricking among both genders. However, for men, children’s rights and the definition of pricking as FGC or not were important aspects in how they viewed pricking, while, for women, health complications and respectability were important. Conclusions: Values known to be associated with FGC in general are also related to pricking. Hence, there seems to be a change in what types of FGC are supported rather than in their perceived values.

Objectives: To present the primary outcomes from a baseline study on attitudes towards female genital cutting (FGC) after migration. Design: Baseline data from a planned cluster randomised, controlled trial. Face-to-face interviews were used to collect questionnaire data in 2015. Based on our hypothesis that established Somalis could be used as facilitators of change among those newly arrived, data were stratified into years of residency in Sweden. Setting: Sweden. Participants: 372 Somali men and women, 206 newly arrived (0–4 years), 166 established (>4 years). Primary outcome measures: Whether FGC is acceptable, preferred for daughter and should continue, specified on anatomical extent. Results: The support for anatomical change of girls and women’s genitals ranged from 0% to 2% among established and from 4% to 8% among newly arrived. Among those supporting no anatomical change, 75%–83% among established and 53%–67% among newly arrived opposed all forms of FGC, with the remaining supporting pricking of the skin with no removal of tissue. Among newly arrived, 37% stated that pricking was acceptable, 39% said they wanted their daughter to be pricked and 26% reported they wanted pricking to continue being practised. Those who had lived in Sweden ≤ 2 years had highest odds of supporting FGC; thereafter, the opposition towards FGC increased over time after migration. Conclusion: A majority of Somali immigrants, including those newly arrived, opposed all forms of FGC with increased opposition over time after migration. The majority of proponents of FGC supported pricking. We argue that it would have been unethical to proceed with the intervention as it, with this baseline, would have been difficult to detect a change in attitudes given that a majority opposed all forms of FGC together with the evidence that a strong attitude change is already happening. BACKGROUND: Telenursing is an expanding part of healthcare, staffed with registered nurses whose work environment is typical of a call centre. Work-related stress has been shown to be a major problem in nurses' work environments and of importance to the outcome of care, patient safety, nurse job satisfaction and burnout. Today, however, we have a limited understanding of and knowledge about the work environment for telenurses. The aim of the present study is to explore and reach consensus on perceived important obstacles and prerequisites in telenurses' work environment. METHODS: A modified Delphi design, using qualitative as well as quantitative data sequentially through three phases, was taken. Data were initially collected via semi-structured interviews (Phase I) and later using a web survey (Phase II-III) between March 2015 and March 2016. RESULTS: The findings present a consensus view of telenurses' experiences of important obstacles and prerequisites in their work environment. Central to the findings are the aspects of telenurses having a demanding work, cognitive fatigue and having no opportunity for recovery during the work shift was ranked as important obstacles. Highly ranked prerequisites for managing were being able to focus on one caller at a time, working in a calm and pleasant environment and having technical support 24/7. CONCLUSIONS: Managers need to enable telenurses to experience control in their work, provided with possibilities to control their work and to recover during work; shortening work time could improve their work environment. Limited possibilities to perform work might contribute to feelings of stress and inability to perform work.

SammanfattningHaninge‐ och Tyresö kommuner har i samarbete med KTH genomfört ett projekt –Ekopiloterna ‐ med tio familjer och sju kommunala enheter. Projektet kompletterar andraliknande projekt i Sverige genom att även inkludera verksamheter/enheter inom kommunerna.Projektet, som pågick under ett år, syftade till att minska utsläppen av växthusgaser och övrigmiljöpåverkan. Resultaten visar tydligt att det går att göra miljö‐ och klimatframsteg utan attman upplever en försämring av livskvalitet eller innehåll i verksamheten.Ekopiloterna i projektet har upplevt att de själva fått stor vinning av att vara miljö‐ ochklimatsmarta. Ekopilotfamiljerna minskade sina totala utsläpp av klimatgaser med 16 % – ellercirka 20 ton ‐ vilket ungefär motsvarar utsläppen från sju genomsnittssvenskars bilresor per år.Den största minskningen skedde inom kategorierna shopping och återvinning därEkopilotfamiljerna totalt minskade sina utsläpp med 49 %.Ekopilotåret var indelat i sju olika teman: Resor, Konsumtion/Avfall/Återvinning, Semester,Mat, El & Energi, Vatten och Egen Utmaning. I slutet av året genomförde Ekopiloterna enranking av hur svårt det är att vara miljö‐ och klimatsmart inom respektive tema. Bådefamiljerna och enheterna ansåg att ”Avfall/återvinning” och ”Mat” var de områden som varlättast att förändra. Det svåraste området var, enligt familjerna, ”Semestern”, där man inteville göra avkall på spännande resmål eller på besök hos släkt och vänner långt bort. Det nästsvåraste området för familjerna var ”Resor” där vardagspusslet ofta inte höll för de mermiljövänliga, men mer tidskrävande, alternativen. Enheterna rankade ”Konsumtion” som denallra svåraste, då man på en arbetsplats inte alltid kan påverka vad som köps in.Ekopilotprojektet har jobbat med en framgångsrik kombination av Kunskap – Uppgifter –Coachning – Uppföljning. Inspiratörer har varit föreläsare som är experter inom respektiveämnesområde. Målet har varit att inspirera till att göra mer miljö‐ och klimatriktiga val undermottot; ”Det är bättre att göra 80 % rätt än 100 % fel.” Projektet har resulterat i en rad godaexempel som nu kan spridas i kommunerna för att inspirera andra. Ekopilotprojektet har ocksåidentifierat ett antal hinder för miljö‐ och klimatsmarta val. Enheterna upplevde att deschablonavtal för el och värme som tillämpas hämmade personalens motivation att varaenergismarta. Likaså upplevdes upphandlingsavtalen ibland hindrande för att köpa tillexempelbegagnad utrustning. I anslutning till varje tema har Ekopiloterna också gett tips till politikerhur de kan underlätta för medborgarna att bli mer miljö‐ och klimatsmarta. Tips finns även tillvänner och kollegor hur man kan leva mer miljö‐ och klimatsmart.Samtliga deltagare har varit nöjda, eller mycket nöjda, med projektet och de vill se någon typav fortsättning och uppföljning. Projektet rekommenderar att kommunerna går vidare medEkopiloterna och tar hand om och följer upp den entusiasm som har väckts. För enheternarekommenderas att starta nya Ekopilotgrupper enligt samma koncept. Potentialen för att göramiljö‐ och klimatförbättringar är stor, pengar kan på sikt sparas och – inte minst – ärkommunen en viktig förebild för medborgarna. Om kommunen inte visar att de arbetar aktivtmed frågorna, minskar engagemanget bland de som bor där. Projektet rekommenderar ocksåatt kommunen använder den kunskap de har fått in via familjerna att man visar upp ochanvänder Ekopilotfamiljerna i kommunikationen och att man bjuder in till återträffar ochföreläsningar.Till sist kan det vara värt att påpeka att trots att familjerna mätbart minskat sina utsläpp ochatt både enheter och familjer gjort många goda insatser under året, är de uppnådda nivåernafortfarande långt ifrån de nivåer som krävs för en globalt hållbar livsstil. Detta är en gigantiskutmaning som kräver politiska beslut. Men projektet visar att med kunskap och inspiration kanman komma en bra bit på vägen – och samtidigt få nöjda medborgare på köpet.

Syftet med studien var att undersöka behovet av stöd hos högstadieelever med hörselnedsättning som går integrerat i den ordinarie grundskolan. Jag har använt mig utav en kvantitativ enkätmetod samt en kvalitativ intervjumetod, där jag har genomfört tre telefonintervjuer innan utformningen av enkäten påbörjades. Resultatet av studien grundar sig på 18 enkätsvar, av 54 utskickade, samt tre telefonintervjuer med personer som har en hörselnedsättning och tidigare har gått integrerat i den ordinarie grundskolan. Resultatet visar att eleverna har ansträngande skoldagar till följd av att det krävs full koncentration för att höra vad klasskamraterna säger. För att de ska ha en chans att följa med i samtalen i klassrummen krävs en bra ljudmiljö, det är fyra elever som har uppgett att de inte kan följa med i samtalen. Med tanke på att många av eleverna går i stora klasser, vissa klasser har fler än 30 elever, kan jag förstå att drygt hälften av respondenterna har uppgett att de har ett behov av att få studera i ett enskilt rum för att få lugn och ro. De respondenter och informanter som har svarat att de brukar lämna klassrummet för till exempel träffa en hörselpedagog, har upplevt detta som positivt. En central fråga i denna studie är om eleverna har behov av stöd och vad stödet bestod av. Det är endast fyra elever som har uppgett att de är i behov av stöd. Fem elever har svarat att de får stöd, till exempel i form av hörselpedagogiskt stöd och specialpedagogiskt stöd.

67. Causes of death among undocumented migrants in Sweden, 1997-2010

Background: Undocumented migrants are one of the most vulnerable groups in Swedish society, where they generally suffer from poor health and limited health care access. Due to their irregular status, such migrants are an under-researched group and are not included in the country’s Cause of Death Register (CDR). Objective: To determine the causes of death among undocumented migrants in Sweden and to ascertain whether there are patterns in causes of death that differ between residents and undocumented migrants. Design: This is a cross-sectional study of death certificates issued from 1997 to 2010 but never included in the CDR from which we established our study sample of undocumented migrants. As age adjustments could not be performed due to lack of data, comparisons between residents and undocumented migrants were made at specific age intervals, based on the study sample’s mean age at death±a half standard deviation. Results: Out of 7,925 individuals surveyed, 860 were classified as likely to have been undocumented migrants. External causes (49.8%) were the most frequent cause of death, followed by circulatory system diseases, and then neoplasms. Undocumented migrants had a statistically significant increased risk of dying from external causes (odds ratio [OR] 3.57, 95% confidence interval [CI]: 2.83–4.52) and circulatory system diseases (OR 2.20, 95% CI: 1.73–2.82) compared to residents, and a lower risk of dying from neoplasms (OR 0.07, 95% CI: 0.04–0.14). Conclusions: We believe our study is the first to determine national figures on causes of death of undocumented migrants. We found inequity in health as substantial differences in causes of death between undocumented migrants and residents were seen. Legal ambiguities regarding health care provision must be addressed if equity in health is to be achieved in a country otherwise known for its universal health coverage.

Universitetsservice US-AB är ett tryckeri som specialiserat sig på akademiska trycksaker såsom avhandlingar, visitkort samt studentlitteratur.Sedan en tid tillbaka har företaget arbetat mot ISO 9001 standarden. De dokument som styr standarden har hittills förvarats i pärmar på de lokala kontoren. Detta har visat sig vara ineffektivt då det är lätt hänt att pärmarna bara blir stående och de anställda på grund av det inte hålls uppdaterade.Syftet med detta examensarbete är att göra dessa dokument mer användarvänliga genom programmet AutoFlowChart från Netforms. Arbetet resulterade i en HTML-sida som Universitetsservice US-AB kan använda i sitt intranät, vilket gör att medarbetarna får snabb och lätt åtkomst till alla dokument. Detta medför även att när dokumentationen uppdateras på intranätet får samtliga fyra kontor direkt åtkomst till uppdatering.Dokumenten skulle även utformas så att de blir mer lättlästa och får ett trevligare utseende. En fördjupning gjordes därförinom kvalitetsstyrning för att få en förståelse över hur dokumenten bör hanteras och utformas.

- Abstract
Telephone advice nursing includes triage, advice, referral, information and coordination. The aim of the study was to explore what telephone nurses base their assessments on. We conducted 14 interviews with seven telephone nurses at a health-care call centre in Sweden. Two authentic calls per nurse were used in stimulated recall interviews, where the nurses commented on the basis for their assessments. A qualitative manifest content analysis was employed. Three major categories emerged in the analysis: care-seeker, e.g. 'symptomatic sounds', nurse, e.g. 'nurse's own experience', and organization, e.g. 'health-care accessibility'. The findings show that the telephone advice nurses' bases for assessments appear to be very broad. They include both verbally and non-verbally communicated information, and care-seeker-, nurse- and certain organization-related factors influence the assessments. We found that an individualistic view of the care-seeker seems to dominate the assessments in non-urgent calls to a health-care call centre.

- Abstract
At a health care call center, care-seekers can consult telephone nurses 24 hours a day. Telephone advice nursing (TAN) includes triage, advice, support, education, referral, information and coordination. The overall aim of the thesis was to explore the core of TAN from different perspectives, the careseeker/patient, emergency medical dispatching and the telephone nurse. The first study described the telephone nurses' context of TAN at a health care call center. Calls from one week (n=2866) were registered and 203 callers were asked to answer a questionnaire, out of which 144 responded (71%). Altogether 20 different reasons for calling were found and in 47% of the calls were made on behalf of someone else. 49% received self-care advice and 85% stated in the questionnaire that they followed the nurses' advice. 95% stated that they received the help from the call center as they anticipated. To obtain a deeper understanding of how the callers perceived their contact with the call center, comments in two open-ended questions in the questionnaire were analyzed with qualitative content analysis in the second study. The comments were categorized in satisfactory and unsatisfactory and practical and emotional aspects. The findings highlighted the importance of receiving appropriate advice, being treated in a kindly manor and receiving a feeling of security through the consultation. In the third study an analysis of the complaints in emergency medical dispatching (EMD) filed with the Medical Responsibility Board, the Swedish National Board of Health and Welfare and Patient Advisory Committees revealed 54 cases between 1991 and 2000. The complaints were analyzed with focus on factors and circumstances that influenced misjudgments and in 19 out of the 21 complaints; the telephone assessments were made on the basis of second-hand information. - Abstract
In every society where non-therapeutic female circumcision (FC) occurs, so too does non-therapeutic male circumcision (MC). In the past few decades, the norm in Euro-American societies has been to distinguish between the practices: FC is banned, while MC is condoned or encouraged. We explored Somalis' post-migration perceptions of FC and MC, while considering that they once lived in a society where both practices were widely accepted and now live in a society where there is a legal ban on FC alongside acceptance of MC. Eighteen individual interviews and seven focus group discussions were conducted with Somali men and women in three Swedish cities. There seemed to be a continuity of values across male and female forms of genital cutting concerning being a good Muslim, not inflicting harm and upholding respectability. Following migration, however, a renegotiation of how these values relate to MC and FC resulted in a conceptual split between the two: MC was perceived as an unquestionably required practice, but FC was viewed as a practice that can be adapted or abandoned. In a new cultural context after migration, perceptions of ideal male and female genitals, and what kinds of inscriptions on the body are desired, seem to have changed.

There are 20 SOS Alarm emergency dispatch centres in Sweden, and from these centres approximately one million ambulances are dispatched each year. The aim of the study was to explore factors and circumstances that contributed to misjudgements or filed complaints in emergency medical dispatching filed at the Patient Advisory Committees (n=24), the Swedish National Board of Health and Welfare (n=20) and the Medical Responsibility Board (n=10). This is a retrospective study of all decisions on complaints made between 1991 and 2000, and the data were analysed in two phases. In the first phase, categories of cause were revealed, and in the second phase, factors and circumstances were identified that related to misjudgements or filed complaints. The findings show that in nearly half (23/54) of the complaints, the assessments were stated as been made on the basis of second-hand consultations, e.g. the SOS operators had not spoken directly to the ill person. In 19 out of 21 complaints dealing with the decision-making process, the assessments were made on second-hand information. Second-hand information may constitute an aggravating circumstance in assessing the urgency of the care needed on the telephone. The findings suggest that the call receivers should be more cautious when undertaking assessments on the basis of information provided by a third person.
