Abstract

Aims To characterize patient radiation doses from nuclear myocardial perfusion imaging (MPI) and the use of radiation-optimizing 'best practices' worldwide, and to evaluate the relationship between laboratory use of best practices and patient radiation dose. Methods and results We conducted an observational cross-sectional study of protocols used for all 7911 MPI studies performed in 308 nuclear cardiology laboratories in 65 countries for a single week in March-April 2013. Eight 'best practices' relating to radiation exposure were identified a priori by an expert committee, and a radiation-related quality index (QI) devised indicating the number of best practices used by a laboratory. Patient radiation effective dose (ED) ranged between 0.8 and 35.6 mSv (median 10.0 mSv). Abstract

In this study, the residual strain was evaluated for human saphenous vein. Rings of the vein from four donors (two male and two female; age 62±5 years) were radially cut to obtain the opening angle (α) of the tissue. It was found that the average opening angle (α) 45°±18° (mean±SD). Then, the intraluminal distribution of circumferential stress was computed for one donor in order to verify the uniform stress hypothesis (opening angle is homogenizing the stress distribution across the wall thickness). The results suggests that α obtained from experiments is close to the value of opening angle which homogenizes the stress distribution across the wall thickness determined from simulations.

Abstract

PURPOSE: Nuclear cardiology is widely used to diagnose coronary artery disease and to guide patient management, but data on current practices, radiation dose-related best practices, and radiation doses are scarce. To address these issues, the IAEA conducted a worldwide study of nuclear cardiology practice. We present the European subanalysis. METHODS: In March 2013, the IAEA invited laboratories across the world to document all SPECT and PET studies performed in one week. The data included age, gender, weight, radiopharmaceuticals, injected activities, camera type, positioning, hardware and software. Radiation effective dose was calculated for each patient. A quality score was defined for each laboratory as the number followed of eight predefined best practices with a bearing on radiation exposure (range of quality score 0 - 8). The participating European countries were assigned to regions (North, East, South, and West). Comparisons were performed between the four European regions and between Europe and the rest-of-the-world (RoW). RESULTS: Data on 2,381 European patients undergoing nuclear cardiology procedures in 102 laboratories in 27 countries were collected. A cardiac SPECT study was performed in 97.9 % of the patients, and a PET study in 2.1 %. The average effective dose of SPECT was 8.0 ± 3.4 mSv (RoW 11.4 ± 4.3 mSv; P < 0.001) and of PET was 2.6 ± 1.5 mSv (RoW 3.8 ± 2.5 mSv; P < 0.001). The mean effective doses of SPECT and PET differed between European regions (P < 0.001 and P = 0.002, respectively). The mean quality score was 6.2 ± 1.2, which was higher than the RoW score (5.0 ± 1.1; P < 0.001). Adherence to best practices did not differ significantly among the European regions (range 6 to 6.4; P = 0.73). Of the best practices, stress-only imaging and weight-adjusted dosing were the least commonly used. Abstract

Objectives: In Europe, the services provided for the investigation and management of obstructive sleep apnoea (OSA) varies from country to country. The aim of this questionnaire-based study was to investigate the current status of diagnostic pathways and therapeutic approaches applied in the treatment of OSA in Europe, qualification requirements of physicians involved in diagnosis and treatment of OSA, and reimbursement of these services. Methods: Two questionnaires were sent to 39 physicians in 22 countries in Europe. In order to standardize the responses, the questionnaire was accompanied by an example. Results: Sleep centers from 21 countries (38 physicians) participated. A broad consistency among countries with respect to the following was found: pathways included referral to sleep physicians/sleep laboratories, necessity for objective diagnosis (primarily by polysomnography), use of polygraphic methods, analysis of polysomnography (PSG), indications for positive airway pressure (PAP) therapy, application of standard continuous PAP (CPAP) therapy (100% with an CPAP/APAP ratio of 2.24:1), and the need (90.5%) and management of follow-up. Differences were apparent in reimbursement of the diagnostic procedures and follow-up, in the procedures for PAP titration from home APAP titration with portable sleep apnea monitoring (38.1%) up to hospital monitoring with PSG and APAP (85.7%), and in the qualification requirements of sleep physicians. Conclusions: Management of OSA in different European countries is similar except for reimbursement rules, qualification of sleep specialists and procedures for titration of the CPAP treatment. A European network (such as the one accomplished by the European Cooperation in Science and Technology [COST] B26 Action) could be helpful for implementing these findings into health-service research in order to standardize management in a cost effective perspective. © 2010 Elsevier B.V.

Abstract

Arteries in situ are subjected to the pretension developed upon the growth period. The magnitude of the pretension was shown to be age-dependent. Detailed statistics are, however, rare. This study was designed to expand our knowledge of the prestrain sustained by arteries during the lifespan. Age-related distribution of the longitudinal prestrain in the male abdominal aorta obtained within 93 regular autopsies is shown (age = 41.6±15.8 years; prestretch =1.174±0.099). Data indicate that the prestrain decreases nonlinearly during the aging. Bilinear regression function revealed the breakpoint in the prestrain–age dependency at age about 40. The comparison of this result with studies which document the nonlinearity in the pulse wave velocity–age dependency indicates that steep increase of the velocity is preceded by the loss of the pretension. It suggests that the pretension could play a compensation role within artery stiffening.

Abstract

Ein gleichmäßiges Raumklima mit geringen Schwankungen von Temperatur und relativer Feuchte trägt zur Behaglichkeit und Gesundheit der Bewohner bei. Es ist seit langem bekannt, dass die thermische Masse der Gebäudehülle starken Temperaturveränderungen - z.B. hervorgerufen durch Sonneneinstrahlung - entgegenwirkt. Weniger bekannt ist die Tatsache, dass es auch so etwas wie eine "hygrische Masse" gibt, die starken Feuchteschwankungen entgegenwirkt. Mit hygrischer Masse ist hier die Wasserdampfspeicherfähigkeit der raumumschließenden Flächen gemeint, die in der Lage ist, Feuchteschwankungen im Raum zu puffern. Dies wäre vor allem dann von Vorteil, wenn in einem Raum die Feuchteproduktion (z.B. durch menschliche Aktivitäten) und die Feuchteabfuhr (z.B. durch Lüftung) zeitversetzt stattfinden. Zu diesem Thema wurden am VTT in Finnland rechnerische Untersuchungen durchgeführt, die gezeigt haben, dass Bekleidungen aus Holz einen positiven Einfluss auf die Raumluftfeuchte von Schlafräumen haben, die nur tagsüber gelüftet werden. Zur Überprüfung der Interpretation dieser Berechnungen und zur parxisgerechten Quantifizierung der Feuchtepufferwirkung unterschiedlicher Bekleidungen basierend auf Holz und Holzwerkstoffen bzw. Zellulosefasern unter definierten Randbedingungen wurden am Fraunhofer-Institut für Bauphysik (IBP) in Holzkirchen vergleichende Freilandversuche geplant, deren Durchführung und Ergebnisse in diesem Buch beschrieben werden.

Abstract

Suppression of radon background is one of main tasks in ultra-low background experiments. The most promising technique for suppression of radon is its adsorption on charcoal. Within the frame of the NEMO-3 experiment, radon trapping facility (RTF) was installed in Modane underground laboratory in 2004. Based on long-term experience with this facility a new compact transportable anti-radon facility was constructed in cooperation among IEAP CTU, SÚRO and ATEKO company. The device provides 20m3/h of purified air (air radon activity at the output ~10mBq/m3). The basic features and preliminary results of anti-radon device testing are presented. [ABSTRACT FROM AUTHOR]

Abstract

It has previously been shown that human granulocyte-macrophage colony-stimulating factor (GM-CSF) can be fused to a truncated diphtheria toxin (DT) to produce a recombinant fusion toxin that kills GM-CSF receptor-bearing cells. We now report that DT388-GM-CSF induces apoptosis and inhibition of colony formation in semisolid medium in receptor positive cells, and that the induction of apoptosis correlates with GM-CSF-receptor occupancy at low ligand concentrations. Also, the induction of apoptosis correlates with the inhibition of protein synthesis and is inversely related to the amount of intracellular antiapoptotic proteins (Bcl2 and Bc1XL). Nine myeloid leukemia cells lines and four nonmyeloid leukemia cell lines were incubated with 0.7 nmol/L of 125I-GM-CSF in the presence or absence of excess cold GM-CSF and bound label measured. High affinity receptor numbers varied from 0 to 291 molecules per cell. Cells were incubated with varying concentrations of recombinant fusion toxin for 48 hours and incorporation of 3H-leucine (protein synthesis), segmentation of nuclei after DAPI staining (apoptosis), and colony formation in 0.2% agarose (clonogenicity) were measured. DT388-GM-CSF at 4 x 10(-9) mol/L inhibited colony formation 1.5 to 3.0 logs for receptor positive cell lines. Protein synthesis and apoptosis IC50s varied among cell lines from greater than 4 x 10(-9) mol/L to 3 x 10(-13) mol/L. GM-CSF-receptor occupancy at 0.7 nmol/L GM-CSF-ligand concentration correlated with the protein synthesis IC50. Similarly, the protein synthesis inhibition and apoptosis induction correlated well, except in cells overexpressing Bcl2 and BclXL, in which 25- to 150-fold inhibition of apoptosis was observed. We conclude that DT388-GM-CSF can kill acute myeloid leukemia blasts but that apoptotic sensitivities will depend on the presence of at least 100 high affinity GM-CSF receptors/cell and the absence of overexpressed antiapoptotic proteins.
