Abstract

Study Question: What is the global variability in recurrent implantation failure (RIF) definition, investigation and therapy, currently offered to patients undergoing IVF?Sumamry Answer: Definitions, diagnostic investigations and treatments offered to RIF patients differ widely amongst assisted reproduction healthcare professionals and clinical guidelines on RIF are urgently needed.What Is Known Already: RIF affects around 10% of patients undergoing IVF worldwide. There is no consensus on the definition of RIF, its diagnostic investigations or the therapeutic options, which leads to inconsistencies in clinical practice.Study Design, Size, Duration: A cross-sectional study of clinicians and embryologists was conducted between May and June 2020. The survey included 43 questions aimed at understanding participants' background and their current practice with regards to defining, investigating and managing RIF. The questions were designed by the European Society of Human Reproduction and Embryology (ESHRE) Special Interest Group (SIG) on implantation and early pregnancy following three consensus meetings.Participants/materials, Setting, Methods: A total of 8579 ESHRE members from 6916 IVF centers were invited to participate using two global email calls based on their pre-specified interest in implantation and early pregnancy. SurveyMonkey and SPSS were used for data collection and analysis, respectively. Furthermore, differences were reported in the answers of European and non-European professionals, as well as between public and private settings and among clinicians clustered according to the average number of RIF patients treated per year.Main Results and the Role Of Chance: The final data set included 735 clinicians and 300 embryologist or IVF-biologists. The majority defines RIF based on the number of failed embryo transfers (ETs) with the most common threshold adopted being three ETs both fresh and frozen. More than two-thirds take lifestyle factors into account, mainly drugs, smoking and BMI. The highest consensus on which diagnostic investigations should be performed was reached for anatomical malformations and gynecological aspects focusing on hydrosalpinx, Asherman's syndrome, endometrial thickness and endometriosis. Concerning treatment of RIF patients, the highest consensus was reached for preconceptional therapies, including BMI adjustment, smoking and endometritis followed by therapies during IVF procedures.Limitations, Reasons For Caution: The response rate was relatively low, but comparable to other surveys.Wider Implications Of the Findings: A consensus on definition, diagnosis and treatment of RIF would help to reduce costly, time-consuming and poorly validated approaches.Study Funding/competing Interest(s): No external funding was used. Abstract

Досліджуване питання: якою є оптимальна тактика лікування жінок з ендометріозом, виходячи з найкращих наявних доказів, представлених у літературних джерелах?Резюмуюча відповідь: за допомогою структурованої методології Інструкції для розробки гайдлайнів ESHRE, були сформульовані 83 рекомендації, які відповіли на 22 ключових запитання щодо оптимальної тактики ведення жінок з ендометріозом.Що вже було відомо: упродовж багатьох років орієнтиром для клінічної допомоги при ендометріозі було керівництво ESHRE з діагностики та лікування ендометріозу (2005), але це керівництво потребувало оновлення.Дизайн дослідження, розмір, тривалість: керівництво було розроблене групою фахівців у даній області з використанням методології Інструкції для розробки гайдлайнів ESHRE, зокрема шляхом ретельного систематичного пошуку літературних даних, оцінки якості включених даних за період до січня 2012 року і висновки з усіх рекомендацій.Основні результати і роль випадку: керівництво надає 83 рекомендації щодо діагностики ендометріозу і лікуванню пов’язаних з ендометріозом болю і безпліддя, та менеджменту жінок, у яких хвороба виявлена випадково (без болю або безпліддя), з профілактики рецидивів захворювання та/або больових симптомів, лікуванню симптомів менопаузи у хворих з анамнезом ендометріозу і про можливий зв’язок ендометріозу та злоякісних новоутворень.Обмеження: ми визначили кілька областей допомоги жінкам з ендометріозом, для яких не достатньо надійних доказів. Ці області були оцінені як належна клінічна практика (GPP), що грунтується на експертних думках учасників групи з розробки керівництва.Широкі наслідки висновків: 32 із 83 рекомендацій з менеджменту жінок з ендометріозом не засновані на доказах високого рівня, і тому були віднесені до GPP, робоча група сформулювала дослідницькі рекомендації для майбутніх досліджень з метою підвищення рівня доказів.Фінансування дослідження/конкуруючий інтерес (и): керівництво розроблене та фінансується ESHRE. These areas were addressed by formulating good practice points (GPP), based on the expert opinion of the guideline group members.Wider implications of the findings: Since 32 out of the 83 recommendations for the management of women with endometriosis could not be based on high level evidence and therefore were GPP, the guideline group formulated research recommendations to guide future research with the aim of increasing the body of evidence.Study funding/competing interest(s): The guideline was developed and funded by ESHRE, covering expenses associated with the guideline meetings, with the literature searches and with the implementation of the guideline. 93. Risk assessment of paracetamol-induced liver toxicity based on human in vitro data

Abstract

Currently risk assessment is based on animal experiments with limited success. The aim of this study was to explore the feasibility to replace the use of animals in risk assessment for drug-induced liver injury, by hazard identification and kinetic modeling based on human in vitro data for metabolism and toxicity. Paracetamol was used as model compound. Human hepatocytes were used to elucidate the toxic mechanisms by transcriptomics and metabolite formation. Time- and dose-dependent toxicant exposure of the human liver was modeled with a mechanism-based physiology-based toxicokinetic (PBTK) model (Simcyp®) and time and concentration-dependent metabolite production and exposure of the hepatocytes to the toxic reactive metabolite was modeled with an in vitro biokinetic model using experimentally derived and literature data on metabolism. Plasma concentration profiles were adequately modeled and the effect of individual variability in metabolism was assessed. Phase 2 metabolism as observed in human hepatocytes was well predicted by the biokinetic model. However the rate of phase 1 metabolism was over predicted in both models. Pathway analysis learned that oxidative phosphorylation, drug metabolism and immune response were consistently regulated, suggesting that adverse effects of paracetamol are related to oxidative stress and immune dysfunction. The results indicate the feasibility of in vitro-based risk assessment of xenobiotics for the human population without the use of animals taking into account the individual variation in drug metabolism. For future refinement more data on metabolizing enzyme and transporter protein abundance is required.
