Abstract

People with methamphetamine use disorder (MUD) struggle to shift their behaviour from methamphetamine‐orientated habits to goal‐oriented choices. The model‐based/model‐free framework is well suited to understand this difficulty by unpacking the computational mechanisms that support experienced‐based (model‐free) and goal‐directed (model‐based) choices. We aimed to examine whether 1) participants with MUD differed from controls on behavioural proxies and/or computational mechanisms of model‐based/model‐free choices; 2) model‐based/model‐free decision‐making correlated with MUD symptoms; and 3) model‐based/model‐free deficits improved over six weeks in the group with MUD. Participants with MUD and controls with similar age, IQ and socioeconomic status completed the Two‐Step Task at treatment commencement (MUD n = 30, Controls n = 31) and six weeks later (MUD n = 23, Controls n = 26). We examined behavioural proxies of model‐based/model‐free decisions using mixed logistic regression, and their underlying mechanisms using computational modelling. At a behavioural level, participants with MUD were more likely to switch their choices following rewarded actions, although this pattern improved at follow up. At a computational level, groups were similar in their use of model‐based mechanisms, but participants with MUD were less likely to apply model‐free mechanisms and less likely to repeat rewarded actions. We did not find evidence that individual differences in model‐based or model‐free parameters were associated with greater severity of methamphetamine dependence, nor did we find that group differences in computational parameters changed between baseline and follow‐up assessment. Decision‐making challenges in people with MUD are likely related to difficulties in pursuing choices previously associated with positive outcomes. [ABSTRACT FROM AUTHOR]

Abstract

Objective: Borderline Personality Disorder (BPD) diagnosis comprises several constellations of trait, neurocognitive, and psychosocial alterations. Dimensional models of psychopathology provide new opportunities to parse heterogeneity and create a stronger interface between individual characterization and psychosocial outcomes. However, dimensional models have focused on either traits or neurocognitive features, lacking integration to capture the multifaceted nature of BPD. Method: We assessed 100 participants with BPD using a combination of tools stemming from trait (Alternative Model for Personality Disorders) and neurocognitive models (Research Domain Criteria; RDoC) to examine if trait‐derived subgroups display distinctive social‐processing and psychosocial profiles. We used two complementary analytical approaches: person‐centered (k‐means clustering) and construct‐based (multiple factor analysis). Results: Our person‐centered approach identified four subgroups with separable internalizing, detached, externalizing, and low psychopathology trait profiles. These profiles revealed distinctive patterns of affiliation, emotion recognition and mentalization performance in RDoC tasks, and psychosocial measures of quality of life and social connectedness. RDoC‐based measures showed close construct proximity with negative affectivity, disinhibition, and antagonism trait domains, relative to the detachment domain, which had close proximity with self‐knowledge. Conclusions: Altogether, findings support consilience between trait‐based and neurobiological frameworks and suggest that trait models are useful to parse BPD heterogeneity leading to unique social functioning profiles. Abstract

BACKGROUND AND AIMS: Substance use disorders (SUD) are associated with cognitive deficits that are not always addressed in current treatments, and this hampers recovery. Cognitive training and remediation interventions are well suited to fill the gap for managing cognitive deficits in SUD. We aimed to reach consensus on recommendations for developing and applying these interventions.DESIGN: Delphi approach with two sequential phases: survey development and iterative surveying of experts.SETTING: Online study.PARTICIPANTS: During survey development, we engaged a group of 15 experts from a working group of the International Society of Addiction Medicine (Steering Committee). During the surveying process, we engaged a larger pool of experts (n=54) identified via recommendations from the Steering Committee and a systematic review.MEASUREMENTS: Survey with 67 items covering four key areas of intervention development: targets, intervention approaches, active ingredients, and modes of delivery.FINDINGS: Across two iterative rounds (98% retention rate), the experts reached a consensus on 50 items including: (i) implicit biases, positive affect, arousal, executive functions, and social processing as key targets of interventions; (ii) cognitive bias modification, contingency management, emotion regulation training, and cognitive remediation as preferred approaches; (iii) practice, feedback, difficulty-titration, bias-modification, goal setting, strategy learning, and meta-awareness as active ingredients; and (iv) both addiction treatment workforce and specialized neuropsychologists facilitating delivery, together with novel digital-based delivery modalities.CONCLUSIONS: Expert recommendations on cognitive training and remediation for substance use disorders highlight the relevance of targeting implicit biases, reward, emotion regulation, and higher-order cognitive skills via well-validated intervention approaches qualified with mec
