Objective. Prior studies of full-marathon participants have demonstrated a higher incidence of hyponatraemia in runners with completion times of 4 hours or more. Our primary aim was to determine if slower pace is associated with increased prevalence of hyponatraemia. Secondly, we evaluated the prevalence of hyponatraemia in full-marathoners v. half-marathoners. Methods. This observational, cross-sectional study comprised consenting runners in the 26.2 With Donna, The National Marathon to Finish Breast Cancer, in Jacksonville Beach, Florida, February 2008. On race day, participants completed a questionnaire, provided finger-stick blood samples, and were weighed both pre- and post-race.Results. A significant negative association was found between pace and post-race sodium level (p<0.001). A negative correlation was found between finishing time and post-race sodium level (p<0.001). The prevalence of post-race hyponatraemia was 4% (4/106) among half-marathoners and 13% (12/89) among full-marathoners (P=0.02). An inverse correlation was found between sodium change and weight change, significant in full-marathoners (r=-0.55, p<0.001) but not half-marathoners (r=-0.23, p=0.042).Conclusions. Slower race pace and longer finishing times were associated with lower post-race sodium levels. Full-marathoners had a significantly higher prevalence of hyponatraemia. The development of hyponatraemia was associated with weight gain. Our data indicate that the relationship between post-race sodium concentration and pace differs according to the distance of the event. We can extrapolate from this data that longer race distance with increased availability of fluid stations combined with a slower pace may increase the risk of developing exercise-induced hyponatraemia. The article provides information on severe traumatic brain injury (TBI) and its clinical management. Critical determinants of outcome are mentioned including the prevention of hypoxia and hypotension, and the immediate diagnosis and evacuation of an expanding intracranial hematoma. The guidelines of the Brain Trauma Foundation for managing severe TBI are discussed. Details are provided on additional therapies not included in the guidelines.

Background. Only a few large cities have published their out-of-hospital cardiac arrest (OHCA) survival statistics using the Utstein style reporting method. To date, to the best of our knowledge there has been no published OHCA survival data for a state. Objective. To describe the process, benefits, and challenges of establishing a statewide OHCA database and educational network. Methods. Arizona's Bureau of Emergency Medical Services and Trauma System initiated a statewide, prospective, observational cohort review of all OHCA victims on whom resuscitation was attempted in the field. Emergency medical services (EMS) first care reports, voluntarily submitted by 35 departments in Arizona, were analyzed. We chronicled the development of our data-collection process along with how we obtained patient outcomes and delivered feedback to field providers. Entry data included time intervals and nodal events conforming to the Utstein style template. Results. In data collected between January 1, 2005, and April 1, 2006, there were 1,484 OHCAs reported, of which 1,104 were of presumed cardiac etiology occurring prior to EMS arrival. The OHCA incidence was approximately 0.44 per 1,000 population per year. In our database, bystander CPR provided an odds ratio of 3.0 for survival (95% confidence interval 1.3, 6.7). Outcomes for 1,076 patients were obtained. Thirty-seven (3.4%) of the 1,076 cardiac arrest victims survived to hospital discharge. Twenty-seven (8.6%) of the 331 ventricular fibrillation cardiac arrest victims survived to hospital discharge. Conclusion. It is feasible for a public health agency to implement a voluntary, statewide data-collection system and educational network to determine and improve survival from OHCA. [ABSTRACT FROM AUTHOR]
