Abstract: Background Motor imagery (MI) training is often used to improve physical practice (PP), and the functional equivalence between imagined and practiced movements is widely considered essential for positive training outcomes. Objective We previously showed that a 5-week MI training program improved tenodesis grasp in individuals with C6–C7 quadriplegia. Here we investigated whether functional equivalence changed during the course of this training program. Methods In this descriptive pilot study, we retrospectively analyzed data for 6 individuals with C6–C7 quadriplegia (spinal cord injured [SCI]) and 6 healthy age-matched controls who trained for 5 weeks in visual and kinesthetic motor imagery or visualization of geometric shapes (controls). Before training, we assessed MI ability by using the Kinesthetic and Visual Imagery Questionnaire (KVIQ). We analyzed functional equivalence by vividness measured on a visual analog scale (0–100) and MI/PP time ratios computed from imagined and physically practiced movement durations measured during MI training. These analyses were re-run considering that half of the participants with quadriplegia were good imagers and the other half were poor imagers based on KVIQ scores. To investigate generalization of training effects, we analyzed MI/PP ratios for an untrained pointing task before (3 baseline measures), immediately after, and 2 months after training. Results During MI training, imagery vividness increased significantly. Only the good imagers evolved toward temporal equivalence during training. Good imagers were also the only participants who showed changes in temporal equivalence on the untrained pointing task. Conclusion This is the first study reporting improvement in functional equivalence during an MI training program that improved tenodesis grasp in individuals with C6–C7 quadriplegia. We recommend that clinical MI programs focus primarily on vividness and suggest that feedback about movement duration could potentially improve temporal equivalence, which could in turn lead to further improvement in PP.

Abstract: International audience; PURPOSE: Lack of physical activity (PA), weight gain, and overweight have been associated with increased risk of recurrence and mortality after breast cancer diagnosis. We evaluated the feasibility of implementing an individualized exercise program and nutritional counseling during adjuvant treatment of localized invasive breast cancer. METHODS: Sixty-one patients eligible for adjuvant chemotherapy were randomized 2:1 to receive a 6-month program of weekly aerobic exercises associated with nutritional counseling (n = 41) or usual care with nutritional counseling (n = 20, one withdrawal). The primary endpoints were the proportion of patients compliant with two weekly supervised sessions and their overall adherence (i.e., proportion of supervised and unsupervised sessions completed versus planned sessions). RESULTS: Ten percent of patients in the intervention group were compliant with the two weekly supervised sessions for 6 months, but the overall median adherence rate was 85% of supervised and non-supervised sessions completed. Non-adherence was mainly due to intrinsic reasons (medical, organizational, psychological barriers). Adherence was positively associated with education and baseline PA level and inversely associated with baseline weight and tumor grade. No statistically significant benefits were observed in the intervention group, even if overall PA level and body composition improved and anthropometrics were maintained over time (p \textless 0.05). CONCLUSIONS: Overall, there was good adherence with the 6-month exercise program during adjuvant treatment for breast cancer, despite poor compliance to twice-weekly supervised sessions. This study highlights the need for flexible exercise modalities and innovative experimental design to reach patients who would most adhere and benefit from intervention. Abstract: KEY POINTS: During exercise skeletal muscles use the energy buffer phosphocreatine. The post‐exercise recovery of phosphocreatine is a measure of the oxidative capacity of muscles and is traditionally assessed by (31)P magnetic resonance spectroscopy of a large tissue region, assuming homogeneous energy metabolism. To test this assumption, we collected spatially resolved spectra along the length of human tibialis anterior using a home‐built array of (31)P detection coils, and observed a striking gradient in the recovery rate of phosphocreatine, decreasing along the proximo‐distal axis of the muscle. A similar gradient along this muscle was observed in signal changes recorded by (1)H muscle functional MRI. These findings identify intra‐muscular variation in the physiology of muscles in action and highlight the importance of localized sampling for any methodology investigating oxidative metabolism of this, and potentially other muscles. ABSTRACT: The rate of phosphocreatine (PCr) recovery (k (PCr)) after exercise, characterizing muscle oxidative capacity, is traditionally assessed with unlocalized (31)P magnetic resonance spectroscopy (MRS) using a single surface coil. However, because of intramuscular variation in fibre type and oxygen supply, k (PCr) may be non‐uniform within muscles. We tested this along the length of the tibialis anterior (TA) muscle in 10 male volunteers. For this purpose, we employed a 3T MR system with a (31)P/(1)H volume transmit coil combined with a home‐built (31)P phased‐array receive probe, consisting of five coil elements covering the TA muscle length. Mono‐exponential k (PCr) was determined for all coil elements after 40 s of submaximal isometric dorsiflexion (SUBMAX) and incremental exercise to exhaustion (EXH). Abstract: Grâce au rassemblement de la communauté de Lyon et de St Etienne, associant chercheurs, cliniciens, étudiants et entreprises, cette première conférence internationale visera à explorer comment et dans quelle mesure l’exercice peut être bénéfique pour les principales cellules de l’appareil locomoteur (muscle, tendon, os, cartilage, vaisseau). Cette journée offrira la possibilité de mettre en avant à la fois des travaux de recherche fondamentale réalisés à un niveau préclinique et des investigations conduites chez le volontaire sain ou sur des cohortes de patients. Des approches cellulaires, moléculaires et fonctionnelles seront mises en avant au cours de cette conférence. This age-related decline in muscle mass is accompanied by a loss of strength and a decline of physical performance, named Dynapenia. Both events decrease the autonomy and the quality of life of the individuals aﬀecting about 40-50% of people over the age of 80. Nonetheless, inter-individual diﬀerences in prevalence of sarcopenia/dynapenia exist, as some remain ﬁt and strong, whereas other become frail and weak when they get old. Until now, no study has examined the inter-individual variations of muscle tissue and its biomarkers. At the ﬁber level, age-related variations in skeletal muscle mass induce typological and capillarization modiﬁcations. Furthermore, the loss of muscle mass with aging could be associated with serious metabolic consequences or accumulation of intramyocellular lipid droplets. Immunohistochemical studies were performed with muscle biopsies from 30 healthy elderly men, aged 80 ±0.5 years selected from the PROgnostic indicator of cardiovascular and cerebrovascular events (PROOF) cohort, classiﬁed into three groups. On the basis of appendicular mass variation between two DEXA at mean interval of seven years, some people lose more muscle mass, named ”Lost”, others remain ”Stable”, and others ”Gain” muscle mass. The loss of skeletal muscle mass was associated with a reduction in Type-I ﬁbers surface area (-24.6%), accompanied by a proportional loss of capillaries number around each ﬁber-type (CAF) and capillary-to-ﬁber perimeter exchange index (CFPE) (-15%, -10% respectively), compared to ”Stable” and ”Gain” groups. Also subjects from the ”Lost” group exhibited signiﬁcant accumulation of intramyocellular lipid droplets in Type-I ﬁbers compared to the ”Gain” (+23%). Lastly, this decline in muscle mass induced a remodeling of the extracellular matrix with an increase in the endomysium area (+12.2% vs Gain).If usually, it is recognized that chronological aging mainly aﬀects Type-II motor units, our results suggest that biological aging is characterized by impairment of Type-I muscle ﬁbers, their microvascular environment and oxidative metabolism for elderly men on their eighties.

Abstract: The origin of the sense of effort has been debated for several decades and there is still no consensus among researchers regarding the underlying neural mechanisms. Some advocate that effort perception mainly arises from an efference copy originating within the brain while others believe that it is predominantly carried by muscle afferent signals. To move the debate forward, we here tested the hypothesis that there is not one but several senses of effort which depend on the way it is evaluated. For this purpose, we used two different psychophysical tests designed to test effort perception in elbow flexors. One was a bilateral isometric force-matching task in which subjects were asked to direct similar amounts of effort toward their two arms, while the other consisted of a unilateral voluntary isometric contraction in which subjects had to rate their perceived effort using a Borg scale. Throughout two distinct experiments, effort perception was evaluated before and following different tendon vibration protocols intended to differentially desensitize muscle spindles and Golgi tendon organs, and to affect the gain between the central effort and muscle contraction intensity. By putting all the results together, we found that spindle afferents played divergent roles across tasks. Namely, while they only acted as modulators of motor pathway excitability during the bilateral task, they clearly intervened as the predominant psychobiological signal of effort perception during the unilateral task. Therefore, the sensory origin of the sense of effort is not central or peripheral. Rather, it is context-dependent.

Abstract: This study was designed to explore the effects of intrafusal thixotropy, a property affecting muscle spindle sensitivity, on the sense of force. For this purpose, psychophysical measurements of force perception were performed using an isometric force matching paradigm of elbow flexors consisting of matching different force magnitudes (5, 10 and 20% of subjects' maximal voluntary force). We investigated participants' capacity to match these forces after their indicator arm had undergone voluntary isometric conditioning contractions known to alter spindle thixotropy, i.e., contractions performed at long ('hold long') or short muscle lengths ('hold short'). In parallel, their reference arm was conditioned at the intermediate muscle length ('hold-test') at which the matchings were performed. The thixotropy hypothesis predicts that estimation errors should only be observed at low force levels (up to 10% of the maximal voluntary force) with overestimation of the forces produced following 'hold short' conditioning and underestimation following 'hold long' conditioning. We found the complete opposite, especially following 'hold-short' conditioning where subjects underestimated the force they generated with similar relative error magnitudes across force levels. In a second experiment, we tested the hypothesis that estimation errors depended on the degree of afferent-induced facilitation using the Kohnstamm phenomenon as a probe of motor pathway excitability. Because the stronger post-effects were observed following 'hold-short' conditioning, it appears that the conditioning-induced excitation of spindle afferents leads to force misjudgments by introducing a decoupling between the central effort and the cortical motor outputs.

Abstract: This study aimed (1) to describe and compare scapular kinematics between three groups of swimmers of different levels and a group of non-swimmers, and (2) to assess whether swimming practice alters the asymmetries in scapular kinematics between the dominant and non-dominant sides, both during unilateral arm raising and lowering in the scapular plane. Scapular kinematics were assessed bilaterally during arm raising and lowering in the scapular plane using an electromagnetic system in 42 healthy males, which were split into four groups: control (n = 11), adolescent elite swimmers (n = 11), adult elite swimmers (n = 10), and club-level adult swimmers (n = 10). One-Way ANOVA SPM(t) on two repeated measures showed that the three groups of swimmers had more protracted shoulder between 30° and 90° of arm raising and lowering (p .001). The three groups of swimmers presented no bilateral difference in scapular upward rotation, while the dominant scapula was more upwardly rotated than the non-dominant one between 74° and 104° of arm elevation in the control group (p .001). The scapula of adult elite swimmers was more internally rotated between 67° and 116° of humeral elevation during arm raising, and between 81° and 54° during arm lowering in comparison to the other swimming and control groups (p ≤ .02), who presented similar scapular positioning in internal rotation. In conclusion, the findings of the study pointed out that swimming practice generated protracted shoulders and removed bilateral differences in scapular upward rotation during scaption, while accumulation of swimming practice at elite level enlarged scapular internal rotation.

Methodological considerations on the relationship between the 1,500-m rowing ergometer performance and vertical jump in national-level adolescent rowers. J Strength Cond Res 33(11): 3000-3007, 2019-The purpose of this study was to investigate whether 3 different approaches for evaluating squat jump performance were correlated with rowing ergometer performance in elite adolescent rowers. Fourteen young male competitive rowers (15.3 ± 0.6 years), who took part in the French rowing national championships, performed a 1,500-m all-out rowing ergometer performance (P1500) and a squat jump (SJ) test. The performance in SJ was determined by calculating the jump height (HSJ in cm), a jump index (ISJ = HSJ·body mass·gravity, in J), and the mean power output (PSJ in W) from the Samozino et al.'s method. Furthermore, allometric modeling procedures were used to consider the importance of body mass (BM) in the relationships between P1500 and jump scores. P1500 was significantly correlated with HSJ (r = 0.29, p ≤ 0.05), ISJ (r = 0.72, p < 0.0001), and PSJ (r = 0.86, p < 0.0001). Furthermore, BM explained at least 96% of the relationships between SJ and rowing performances. However, the similarity between both allometric exponents for PSJ and P1500 (1.15 and 1.04, respectively) indicates that BM could influence jump and rowing ergometer performances at the same rate, and that PSJ could be the best correlate of P1500. Therefore, the calculation of power seems to be more relevant than HSJ and ISJ to (a) evaluate jump performance and (b) infer the capacity of adolescent rowers to perform 1,500-m all-out rowing ergometer performance, irrespective of their body mass. This could help coaches to improve their training program and potentially identify talented young rowers.

Abstract: Modern football is characterized by very high-intensity intermittent efforts. During a match, players perform technical and physical tasks in relation to their specific positions on the field. A high-level football match induces heart-rate variations, energetic storage lowering, muscular damage and oxidative stress increase and immune status alteration. These physiological variations are accompanied by modifications of fatigue perceived, muscle soreness, wellness, sleep quality, psychological stress and overall mood. All these incidences can be measured, quantified and analyzed, in direct relation to contextual factors like game location, time of the day, playing system, …, and congested period of matches (e.g. two to three matches per week). The present thesis aims to report all the ways to monitor match load and fatigue and aims to analyze the influence of playing matches during congested periods on physical activity and on physiological post-match kinetics, over high-level football players; Le football moderne est caractérisé par des efforts intermittents de très haute intensité. Pendant un match, les joueurs réalisent des performances, qu'elles soient physiques ou techniques, en lien direct avec la spécificité de leur poste de jeu, leur rôle tactique et leur positionnement sur le terrain. Un match de football de haut niveau induit des variations de fréquence cardiaque, une baisse de réserves énergétiques, une augmentation des dommages musculaires, du stress oxydatif et une affectation du statut immunitaire. Incidences physiologiques auxquelles se rajoutent des modifications de perception de la fatigue, des douleurs musculaires, du bien-être, de la qualité du sommeil, du stress psychologique et de l'humeur. Toutes ces incidences se mesurent, se quantifient et s'analysent en lien direct avec des facteurs contextuels comme le lieu du match, le moment de la journée, le système de jeu, …, et les périodes d'enchainement de match (e.g. deux à trois matchs par semaine) qui peuvent avoir une influence significative. Abstract: International audience; Ensuring glenohumeral stability during repetitive lifting tasks is a key factor to reduce the risk of shoulder injuries. Nevertheless, the literature reveals some lack concerning the assessment of the muscles that ensure glenohumeral stability during specific lifting tasks. Therefore, the purpose of this study was to assess the stabilization function of shoulder muscles during a lifting task. Kinematics and muscle electromyograms (n = 9) were recorded from 13 healthy adults during a bi-manual lifting task performed from the hip to the shoulder level. A generic upper-limb OpenSim model was implemented to simulate gleno-humeral stability and instability by performing static optimizations with and without gleno-humeral stability constraints. This procedure enabled to compute the level of shoulder muscle activity and forces in the two conditions. Abstract: Muscular dystrophies are characterized by weakness and wasting of skeletal muscle tissues. Several drugs targeting the myostatin pathway have been used in clinical trials to increase muscle mass and function but most showed limited efficacy. Here we show that the expression of components of the myostatin signaling pathway is downregulated in muscle wasting or atrophying diseases, with a decrease of myostatin and activin receptor, and an increase of the myostatin antagonist, follistatin. We also provide in vivo evidence in the congenital myotubular myopathy mouse model (knock-out for the myotubularin coding gene Mtm1) that a down-regulated myostatin pathway can be reactivated by correcting the underlying gene defect. Our data may explain the poor clinical efficacy of anti-myostatin approaches in several of the clinical studies and the apparent contradictory results in mice regarding the efficacy of anti-myostatin approaches and may inform patient selection and stratification for future trials., Drugs targeting myostatin reverse muscle wasting in animal models, but have limited efficacy in patients. The authors show that the myostatin pathway is downregulated in patients, possibly explaining the poor outcome of anti-myostatin approaches, and that it can be reactivated by correcting disease-causing mutations in mice.

Abstract: In stroke patients, loss of skeletal muscle mass leads to prolonged weakness and less efficient rehabilitation. We previously showed that expression of myostatin, a master negative regulator of skeletal muscle mass, was strongly increased in skeletal muscle in a mouse model of stroke. We therefore tested the hypothesis that myostatin inhibition would improve recovery of skeletal muscle mass and function after cerebral ischemia. Cerebral ischemia (45 minutes) was induced by intraluminal right middle cerebral artery occlusion (MCAO). Swiss male mice were randomly assigned to Sham-operated mice (n = 10), MCAO mice receiving the vehicle (n = 15) and MCAO mice receiving an anti-myostatin PINTA745 (n = 12; subcutaneous injection of 7.5 mg.kg−1 PINTA745 immediately after surgery, 3, 7 and 10 days after MCAO). PINTA745 reduced body weight loss and improved body weight recovery after cerebral ischemia, as well as muscle strength and motor function. PINTA745 also increased muscle weight recovery 15 days after cerebral ischemia. Mechanistically, the better recovery of skeletal muscle mass in PINTA745-MCAO mice involved an increased expression of genes encoding myofibrillar proteins. Therefore, an anti-myostatin strategy can improve skeletal muscle recovery after cerebral ischemia and may thus represent an interesting strategy to combat skeletal muscle loss and weakness in stroke patients.

Abstract: Despite their unprecedented success in relapsed/refractory (R/R) large B-cell lymphoma (LBCL), anti-CD19 CAR T cells are associated with significant toxicity, and more than half of patients relapse. As monocytes emerged as key players in CAR therapy, we sought to evaluate the evolution of HLA-DR expression on monocytes (mHLA-DR) before and after commercial anti-CD19 CAR T-cell infusion in a large cohort (n = 103) of patients with R/R LBCL and its association with adverse events and treatment response. Cy-Flu-based lymphodepletion (LD) upregulated mHLA-DR in 79% of the cases, whereas in 2l% of cases (15 patients), the mHLA-DR level decreased after LD, and this decrease was associated with poorer outcome. Abstract: International audience; Hydroxyurea (HU) is commonly used as a treatment for patients with sickle cell disease (SCD) to enhance fetal hemoglobin production. This increased production is expected to reduce anemia (which depresses oxygen transport) and abnormal Hb content alleviating clinical symptoms such as vaso-occlusive crisis and acute chest syndrome. The effects of HU on skeletal muscle bioenergetics in vivo are still unknown. Due to the beneficial effects of HU upon oxygen delivery, improved skeletal muscle energetics and function in response to a HU treatment have been hypothesized. Muscle energetics and function were analyzed during a standardized rest-exercise-recovery protocol, using 31P-magnetic resonance spectroscopy in Townes SCD mice. Measurements were performed in three groups of mice: one group of 2-mo-old mice (SCD2m, n = 8), another one of 4-mo-old mice (SCD4m, n = 8), and a last group of 4-mo-old mice that have been treated from 2 mo of age with HU at 50 mg/kg/day (SCD4m-HU, n = 8). As compared with SCD2m mice, SCD4m mice were heavier and displayed a lower acidosis. As lower specific forces were developed by SCD4m compared with SCD2m, greater force-normalized phosphocreatine consumption and oxidative and nonoxidative costs of contraction were also reported. HU-treated mice (SCD4m-HU) displayed a significantly higher specific force production as compared with untreated mice (SCD4m), whereas muscle energetics was unchanged. Overall, our results support a beneficial effect of HU on muscle function.

Abstract: International audience; On-chip study of blood flow has emerged as a powerful tool to assess the contribution of each component of blood to its overall function. Blood has indeed many functions, from gas and nutrient transport to immune response and thermal regulation. Red blood cells play a central role therein, in particular through their specific mechanical properties, that directly influence pressure regulation, oxygen perfusion, or platelet and white cells segregation towards endothelial walls. As the bloom of in-vitro studies has led to the apparition of various storage and sample preparation protocols, we address the question of the robustness of the results involving cell mechanical behavior against this diversity. The effects of three conservation media (EDTA, citrate and glucose-albumin-sodium-phosphate) and storage time on the red blood cell mechanical behavior are assessed under different flow conditions: cell deformability by ektacytometry, shape recovery of cells flowing out of a microfluidic constriction, and cell flipping dynamics under shear flow. The impact of buffer solutions (phosphate-buffered saline and density-matched suspension using iodixanol/Optiprep) are also studied by investigating individual cell flipping dynamics, relative viscosity of cell suspensions and cell structuration under Poiseuille flow. Our results reveal that storing blood samples up to seven days after withdrawal and suspending them in adequate density-matched buffer solutions has in most experiments a moderate effect on the overall mechanical response, with a possible rapid evolution in the first three days after sample collection. SIGNIFICANCE Blood is in intimate contact with all organs in the body, supplying oxygen, nutrients and drugs while removing waste. It carries cells involved in immune response, wound repair and tumor dissemination. Blood is easily collected, revealing the presence of disease through biomarker analysis. It is storable and transfusable. Thus, blood is the subject of many in-vitro studies for research and medical purposes. Guidelines associated to sample preparation or storage conditions have been established, but these may affect its mechanical behavior. In this collaborative study, we provide new guidelines to minimize the impact of specific experimental requirements (e.g. density matching, blood freshness) by focusing on the single or collective motion of red blood cells in a large range of flow conditions.

Abstract: Background: Scientific evidence and public health reports keep highlighting the continuous and alarming worldwide progression of physical inactivity and sedentary behaviors in children and adolescents. The present paper summarizes findings from the 2022 French Report Card (RC) on physical activity for children and youth and compares them to the 2016, 2018, and 2020 RCs. Methods: The 2022 edition of the French RC follows the standardized methodology established by the Active Healthy Kids Global Matrix. Ten physical activity indicators have been evaluated and graded based on the best available evidence coming from national surveys, peer-reviewed literature, government and nongovernment reports, and online information. The evaluation was also performed in children and adolescents with disabilities. Indicators were graded from A (high level of evidence) to F (very low level of evidence) or INC for incomplete. Results: The evaluated indicators received the following grades: overall physical activity: D−; organized sport participation and physical activity: C; active play: F; active transportation: C; sedentary behaviors: D−; family and peers: D; physical fitness: C; school: C−; community and the built environment: F; government: B. Conclusions: While this 2022 French RC shows progression for 7 out of the 10 indicators considered, it also underlines the continuous need for actions at the local, regional, and national levels to develop better surveillance systems and favor a long-term improvement of youth movement behaviors.

Abstract: Objective To investigate the effects of focal muscle fatigue induced by electromyostimulation (EMS) on Anticipatory Postural Adjustments (APAs) during arm flexions performed at maximal velocity. Methods Fifteen healthy subjects performed self-paced arm flexions at maximal velocity before and after the completion of fatiguing electromyostimulation programs involving the medial and anterior deltoids and aiming to degrade movement peak acceleration. APA timing and magnitude were measured using surface electromyography. Results Following muscle fatigue, despite a lower mechanical disturbance evidenced by significant decreased peak accelerations (−12%, p p >.11 for all analyses). Conclusion The fatigue signals evoked by externally-generated contractions seem to be gated by the Central Nervous System and result in postural strategy changes which aim to increase the postural safety margin. Significance EMS is widely used in rehabilitation and training programs for its neuromuscular function-related benefits. However and from a motor control viewpoint, the present results show that the use of EMS can lead to acute inaccuracies in predictive motor control. We propose that clinicians should investigate the chronic and global effects of EMS on motor control.

Abstract: The effects of exercise on decision-making performance have been studied using a wide variety of cognitive tasks and exercise interventions. Although the current literature supports a beneficial influence of acute exercise on cognitive performance, the mechanisms underlying this phenomenon have not yet been elucidated. We review studies that used single-pulse transcranial magnetic stimulation (TMS) to probe the excitability of motor structures during whole-body exercise and present a framework to account for the facilitating effects of acute exercise on motor processes. Recent results suggest that, even in the absence of fatigue, the increase in corticospinal excitability classically reported during submaximal and exhausting exercises may be accompanied by a reduction in intracortical inhibition. We propose that reduced intracortical inhibition elicits an adaptive central mechanism that counteracts the progressive reduction in muscle responsiveness caused by peripheral fatigue. Such a reduction would render the motor cortex more sensitive to upstream influences, thus causing increased corticospinal excitability. Furthermore, reduction of intracortical inhibition may account for the more efficient descending drive and for the improvement of reaction time performance during exercise. The adaptive modulation in intracortical inhibition could be implemented through a general increase in reticular activation that would further account for enhanced sensory sensitivity.

74. Endurance training reduces exercise-induced acidosis and improves muscle function in a mouse model of sickle cell disease

Abstract: Sickle cell disease (SCD) mice (Townes model of SCD) presented exacerbated exercise-induced acidosis and fatigability as compared to control animals. We hypothesize that endurance training could represent a valuable approach to reverse these muscle defects. Endurance-trained HbAA (HbAA-END, n=10), HbAS (HbAS-END, n=11) and HbSS (HbSS-END, n=8) mice were compared to their sedentary counterparts (10 HbAA-SED, 10 HbAS-SED and 9 HbSS-SED mice) during two rest - exercise - recovery protocols during which muscle energetics and function were measured. In vitro analyses of some proteins involved in muscle energetics, pH regulation and oxidative stress were also performed. Exercise-induced acidosis was lower in HbSS-END mice as compared to their sedentary counterparts during both moderate (p0.001) and intense (p0.1) protocols. The total force production measured during both protocols was higher in trained mice compared to sedentary animals. In vitro analyses revealed that enolase/citrate synthase ratio was reduced in HbSS-END (p0.001) and HbAS-END (p0.01) mice compared to their sedentary counterparts. In addition, malondialdehyde concentration was reduced in trained mice (p0.05). In conclusion, endurance training would reverse the more pronounced exercise-induced acidosis, reduce oxidative stress and ameliorate some of the muscle function parameters in SCD mice.

J Strength Cond Res 31(9): 2388-2397, 2017-The aim of this study was to investigate the effects of 2 soccer-training seasons on physical fitness and hormone concentrations in elite youth soccer players. Twenty male elite soccer players (SP, age 14.5 ± 0.4 years) and 20 male control subjects (CS, age 14.3 ± 0.3 years) participated in the study. Anthropometric measurements, aerobic (Yo-Yo Intermittent Recovery Test Level 1 [YYIRT1]) and anaerobic soccer relevant performances (jump and sprint tests), blood testosterone (T), cortisol (C), sex hormone binding globulin (SHBG), and T/C ratio were assessed 5 times (from T0 to T4) during 2 competitive seasons. Significant differences from basal values (Δ) of T, SHBG, and C between SP and CS were observed (p < 0.01). Additionally, T and T/C ratio changes were positively correlated with physical performance (p ≤ 0.05). In conclusion, as expected, higher T concentration and greater power performance were observed in the soccer players group compared with controls. Our findings also show that the T concentrations and power performance outcomes co-vary positively over the 2 soccer seasons in soccer players.

Abstract: Skeletal muscle shows high plasticity in response to external demand. Moreover, adult skeletal muscle is capable of complete regeneration after injury, due to the properties of muscle stem cells (MuSCs), the satellite cells, which follow a tightly regulated myogenic program to generate both new myofibers and new MuSCs for further needs. Although reactive oxygen species (ROS) and reactive nitrogen species (RNS) have long been associated with skeletal muscle physiology, their implication in the cell and molecular processes at work during muscle regeneration is more recent. This review focuses on redox regulation during skeletal muscle regeneration. An overview of the basics of ROS/RNS and antioxidant chemistry and biology occurring in skeletal muscle is first provided. Then, the comprehensive knowledge on redox regulation of MuSCs and their surrounding cell partners (macrophages, endothelial cells) during skeletal muscle regeneration is presented in normal muscle and in specific physiological (exercise-induced muscle damage, aging) and pathological (muscular dystrophies) contexts. Recent advances in the comprehension of these processes has led to the development of therapeutic assays using antioxidant supplementation, which result in inconsistent efficiency, underlying the need for new tools that are aimed at precisely deciphering and targeting ROS networks. This review should provide an overall insight of the redox regulation of skeletal muscle regeneration while highlighting the limits of the use of nonspecific antioxidants to improve muscle function. Abstract: We investigated whether and how the movement initiation condition (IC) encountered during the early movements performed following focal muscle fatigue affects the postural control of discrete ballistic movements. For this purpose, subjects performed shoulder flexions in a standing posture at maximal velocity under two movement IC, i.e., in self-paced conditions and submitted to a Stroop-like task in which participants had to trigger fast shoulder flexions at the presentation of incongruent colors. Shoulder flexion kinematics, surface muscle activity of focal and postural muscles as well as center-of-pressure kinematics were recorded. The initial IC and the order in which subjects were submitted to these two conditions were varied within two separate experimental sessions. IC schedule was repeated before and after fatigue protocols involving shoulder flexors. The aim of this fatigue procedure was to affect acceleration-generating capacities of focal muscles. In such conditions, the postural muscle activity preceding and accompanying movement execution is expected to decrease. Following fatigue, when subjects initially moved in self-paced conditions, postural muscle activity decreased and scaled to the lower focal peak acceleration. This postural strategy then transferred to the Stroop-like task. In contrast, when subjects initially moved submitted to the Stroop-like task, postural muscle activity did not decrease and this transferred to self-paced movements. Regarding the center-of-pressure peak velocity, which is indicative of the efficiency of the postural actions generated in stabilizing posture, no difference appeared between the two sessions post-fatigue. This highlights an optimization of the postural actions when subjects first moved in self-paced conditions, smaller postural muscle activation levels resulting in similar postural consequences. In conclusion, the level of neuromuscular activity associated with the postural control is affected and can be optimized by the initial movement IC experienced post-fatigue. Beyond the fundamental contributions arising from these results, we point out potential applications for trainers and sports instructors.

Abstract: International audience; The present study investigated cerebral and muscle hemoglobin oxygen saturation (tissue oxygen index, TOI) in children with sickle cell anemia (SS), sickle cell hemoglobin C disease (SC) and healthy children (AA). TOI was measured by near-infrared spectroscopy (NIRS) and spectral analysis of the TOI variability was used to assess flowmotion and vasomotion. Arterial oxyhemoglobin saturation (SpO2), hemorheological and hematological parameters were also measured in SS and SC children. Both TOI were lower in SS compared to both AA and SC children, with SC exhibiting lower values than AA children. Cerebral vasomotion expressed in absolute values was enhanced in SS compared to AA and SC children. Muscle vasomotion did not differ between the three groups. Hematocrit, SpO2 and red blood cell deformability were positively associated with cerebral TOI in SS children. We demonstrated that 1) cerebral and muscle TOI were markedly decreased in SS children while the decrease of TOI was milder in SC children, 2) cerebral TOI level was associated with several biological markers in SS children only and 3) cerebral vasomotion was enhanced in SS, possibly to counterbalance the effects of chronic cerebral hypoxia.

Abstract: Increased oxidative stress, defined as an imbalance between prooxidants and antioxidants, resulting in molecular damage and disruption of redox signaling, is associated with numerous pathophysiological processes and known to exacerbate chronic diseases. Prolonged systemic hypoxia, induced either by exposure to terrestrial altitude or a reduction in ambient O2 availability is known to elicit oxidative stress and thereby alter redox balance in healthy humans. The redox balance modulation is also highly dependent on the level of physical activity. For example, both high-intensity exercise and inactivity, representing the two ends of the physical activity spectrum, are known to promote oxidative stress. Numerous to-date studies indicate that hypoxia and exercise can exert additive influence upon redox balance alterations. However, recent evidence suggests that moderate physical activity can attenuate altitude/hypoxia-induced oxidative stress during long-term hypoxic exposure. The purpose of this review is to summarize recent findings on hypoxia-related oxidative stress modulation by different activity levels during prolonged hypoxic exposures and examine the potential mechanisms underlying the observed redox balance changes. The paper also explores the applicability of moderate activity as a strategy for attenuating hypoxia-related oxidative stress. Moreover, the potential of such moderate intensity activities used to counteract inactivity-related oxidative stress, often encountered in pathological, elderly and obese populations is also discussed. Finally, future research directions for investigating interactive effects of altitude/hypoxia and exercise on oxidative stress are proposed.

Abstract: Context: Playing tennis requires unilateral and intensive movement of the upper limb, which may lead to functional adaptations of the shoulder and an increased injury risk. Identifying which athletes will be future elite tennis players starts at 5 to 6 years of age. Therefore, highly skilled players practice intensively in their childhood. However, whether these functional changes occur during the prepubertal years has not been established. Objectives: To assess changes in glenohumeral-joint–rotation range of motion and strength of the shoulder-complex muscles in prepubertal elite tennis players. Design: Cross-sectional study. Setting: Tennis training sports facilities. Patients or Other Participants: Sixty-seven male tennis players (age range = 7–13 years) selected by a regional tennis center of excellence were divided into 3 biological age groups relative to their predicted age at peak height velocity: greater than 4 (n = 26; age = 8.7 ± 0.7 years, height = 132.4 ± 12.9 cm, mass = 27.8 ± 3.8 kg), 3 to 4 (n = 21; age = 10.3 ± 0.6 years, height = 144.9 ± 5.7 cm, mass = 34.7 ± 4.0 kg), and 2 (n = 20; age = 12.8 ± 1.4 years, height = 158.5 ± 8.7 cm, mass = 43.0 ± 8.2 kg) years before their age at peak height velocity. Main Outcome Measures(s): We measured the internal- and external-rotation ranges of motion of the glenohumeral joint using a goniometer and calculated the total arc of motion. Maximal isometric strength of 8 shoulder muscles was measured using a handheld dynamometer. Strength values were normalized to body weight and used to calculate 4 agonist-to-antagonist strength ratios. Results: The total arc of motion of the glenohumeral joint decreased gradually with biological age (P ≤ .01) due to the decrease in internal-rotation range of motion (P < .001). Absolute strength increased gradually with biological age (P < .001), but the relative strengths and ratios remained similar. Conclusions: Functional adaptations of the shoulder seen in adolescent and adult tennis players were observed in healthy prepubertal players. This knowledge could help clinicians and coaches more effectively monitor shoulder adaptations to tennis practice during the prepubertal years.

Abstract: International audience; 12 The primary role of the shoulder joint in tennis forehand drive is at the expense of the 13 loadings undergone by this joint. Nevertheless, few studies investigated glenohumeral (GH) 14 contact forces during forehand drives. The aim of this study was to investigate GH 15 compressive and shearing forces during the flat and topspin forehand drives in advanced 16 tennis players. 3D kinematics of flat and topspin forehand drives of 11 advanced tennis 17 players were recorded. The Delft Shoulder and Elbow musculoskeletal model was 18 implemented to assess the magnitude and orientation of GH contact forces during the 19 forehand drives. The results showed no differences in magnitude and orientation of GH 20 contact forces between the flat and topspin forehand drives. The estimated maximal GH 21 contact force during the forward swing phase was 3573 ± 1383 N, which was on average 1.25 22 time greater than during the follow-through phase, and 5.8 times greater than during the 23 backswing phase. Regardless the phase of the forehand drive, GH contact forces pointed 24 toward the anterior-superior part of the glenoid therefore standing for shearing forces. 25 Knowledge of GH contact forces during real sport tasks performed at high velocity may 26 improve the understanding of various sport-specific adaptations and causative factors for 27 shoulder problems. 28 2 29

Abstract: We express our sincere gratitude to Marie-Anne Cousseau who established the public-private partnership between WittyFit and the University Hospital of Clermont-Ferrand,France.This study is integrally funded by WittyFit,Asniere-sur-Seine; International audience; BACKGROUND: Morbidity before retirement has a huge cost, burdening both public health and workplace finances. Multiple factors increase morbidity such as stress at work, sedentary behavior or low physical activity, and poor nutrition practices. Nowadays, the digital world offers infinite opportunities to interact with workers. The WittyFit software was designed to understand holistic issues of workers by promoting individualized behavior changes at the workplace.OBJECTIVE: The shorter term feasibility objective is to demonstrate that effective use of WittyFit will increase well-being and improve health-related behaviors. The mid-term objective is to demonstrate that WittyFit improves economic data of the companies such as productivity and benefits. The ultimate objective is to increase life expectancy of workers.METHODS: This is an exploratory interventional cohort study in an ecological situation. Three groups of participants will be purposefully sampled: employees, middle managers, and executive managers. Four levels of engagement are planned for employees: commencing with baseline health profiling from validated questionnaires; individualized feedback based on evidence-based medicine; support for behavioral change; and formal evaluation of changes in knowledge, practices, and health outcomes over time. Middle managers will also receive anonymous feedback on problems encountered by employees, and executive top managers will have indicators by division, location, department, age, seniority, gender and occupational position. Managers will be able to introduce specific initiatives in the workplace. WittyFit is based on two databases: behavioral data (WittyFit) and medical data (WittyFit Research). Statistical analyses will incorporate morbidity and well-being data. When a worker leaves a workplace, the company documents one of three major explanations: retirement, relocation to another company, or premature death. Therefore, WittyFit will have the ability to include mortality as an outcome. WittyFit will evolve with the waves of connected objects further increasing its data accuracy. Ethical approval was obtained from the ethics committee of the University Hospital of Clermont-Ferrand, France. RESULTS: WittyFit recruitment and enrollment started in January 2016. First publications are expected to be available at the beginning of 2017. CONCLUSIONS: The name WittyFit came from Witty and Fitness. The concept of WittyFit reflects the concept of health from the World Health Organization: being spiritually and physically healthy. WittyFit is a health-monitoring, health-promoting tool that may improve the health of workers and health of companies. WittyFit will evolve with the waves of connected objects further increasing its data accuracy with objective measures. WittyFit may constitute a powerful epidemiological database. Finally, the WittyFit concept may extend healthy living into the general population

Abstract: Display Omitted Smartphone accelerometers can be used for the classification of physical activities.The PredEE function discriminates stillness from light-intensity activities.Time devoted to still, light, moderate and intense activities is estimated.Energy expenditure from categories in free-living conditions is estimated. The proliferation of smartphones is creating new opportunities to monitor and interact with human subjects in free-living conditions since smartphones are familiar to large segments of the population and facilitate data collection, transmission and analysis. From accelerometry data collected by smartphones, the present work aims to estimate time spent in different activity categories and the energy expenditure in free-living conditions. Our research encompasses the definition of an energy-saving function (PredEE) considering four physical categories of activities (still, light, moderate and vigorous), their duration and metabolic cost (MET). To create an efficient discrimination function, the method consists of classifying accelerometry-transformed signals into categories and of associating each category with corresponding Metabolic Equivalent Tasks. The performance of the PredEE function was compared with two previously published functions (f(,d)aedes,f(,d)nrjsi), and with two dedicated sensors (Armband and Actiheart) in free-living conditions over a 12-h monitoring period using 30 volunteers. Compared to the two previous functions, PredEE was the only one able to provide estimations of time spent in each activity category. In relative value, all the activity categories were evaluated similarly to those given by Armband. Compared to Actiheart, the function underestimated still activities by 10.1% and overestimated light- and moderate-intensity activities by 7.9% and 4.2%, respectively. The total energy expenditure error produced by PredEE compared to Armband was lower than those given by the two previous functions (5.7% vs. 14.1% and 17.0%). PredEE provides the user with an accurate physical activity feedback which should help self-monitoring in free-living conditions.

Abstract: Physical inactivity and sedentary behaviors are on the rise worldwide and contribute to the current overweight and obesity scourge. The loss of healthy life style benchmarks and the lack of the need to move make it necessary to provide feedback about physical and sedentary activities in order to promote active ways of life. The aim of this study was to develop a specific function adapted to overweight and obese people to identify four physical activity (PA) categories and to estimate the associated total energy expenditure (TEE). This function used accelerometry data collected from a smartphone to evaluate activity intensity and length, and TEE. The performance of the proposed function was estimated according to two references (Armband® and FitmatePro®) under controlled conditions (CC) for a 1.5-h scenario, and to the Armband® device in free-living conditions (FLC) over a 12-h monitoring period. The experiments were carried out with overweight and obese volunteers: 13 in CC and 27 in FLC. The evaluation differences in time spent in each category were lower than 7% in CC and 6% in FLC, in comparison to the Armband® and FitmatePro® references. The TEE mean gap in absolute value between the function and the two references was 9.3% and 11.5% in CC, and 8.5% according to Armband® in FLC.

Abstract: Background Ankle sprain is the most common sport-related injury and eccentric weakness of ankle evertors is regarded as a significant muscular deficit related to chronic ankle instability. However, the eccentric performance of the evertors is rarely assessed by clinicians because procedures used for research purposes (i.e. isokinetic tests) are not easily applicable in daily practice. Methods The present study assessed the ability of two different testing procedures to distinguish between groups of 12 healthy subjects or 12 patients suffering from chronic ankle instability. On the one hand, the strength of evertors was assessed with a gold standard isokinetic procedure. On the other hand, we assessed the ability of the subjects to control ankle inversion during weight bearing (functional standing test). Findings Data showed no significant difference between groups for isokinetic peak torque values normalized to body weight. Conversely, the functional test revealed a significantly impaired ability to control ankle inversion during weight bearing in subjects with chronic ankle instability. Interpretation This suggests that this easy-to-apply functional test is better suited compared to isokinetic testing procedures to assess weakness of evertors in patients suffering from chronic ankle instability. Moreover, this test may also be used to objectively monitor improvements during rehabilitation or progression in prevention protocols.

Abstract: International audience; KEY POINTS: Vascular brain lesions and atherosclerosis are two similar conditions that are characterized by increased inflammation and oxidative stress. Non-invasive imaging in a murine model of atherosclerosis showed vascular brain damage and peripheral inflammation. In this study, exercise training reduced magnetic resonance imaging-detected abnormalities, insulin resistance and markers of oxidative stress and inflammation in old ApoE-/- mice. Our results demonstrate the protective effect of exercise on neurovascular damage in the ageing brain of ApoE-/- mice. ABSTRACT: Vascular brain lesions, present in advanced atherosclerosis, share pathological hallmarks with peripheral vascular lesions, such as increased inflammation and oxidative stress. Physical activity reduces these peripheral risk factors, but its cerebrovascular effect is less documented, especially by non-invasive imaging. Through a combination of in vivo and post-mortem techniques, we aimed to characterize vascular brain damage in old ApoE-/- mice fed a high-cholesterol (HC) diet with dietary controlled intake. We then sought to determine the beneficial effects of exercise training on oxidative stress and inflammation in the brain as a treatment option in an ageing atherosclerosis mouse model. Using in vivo magnetic resonance imaging (MRI) and biological markers of oxidative stress and inflammation, we evaluated the occurrence of vascular abnormalities in the brain of HC-diet fed ApoE-/- mice \textgreater70 weeks old, its association with local and systemic oxidative stress and inflammation, and whether both can be modulated by exercise. Exercise training significantly reduced both MRI-detected abnormalities (present in 71% of untrained vs. 14% of trained mice) and oxidative stress (lipid peroxidation, 9.1 +/- 1.4 vs. 5.2 +/- 0.9 mumol mg-1 ; P \textless 0.01) and inflammation (interleukin-1beta, 226.8 +/- 27.1 vs. 182.5 +/- 21.5 pg mg-1 ; P \textless 0.05) in the brain, and the mortality rate. Exercise also decreased peripheral insulin resistance, oxidative stress and inflammation, but significant associations were seen only within brain markers. Highly localized vascular brain damage is a frequent finding in this ageing atherosclerosis model, and exercise is able to reduce this outcome and improve lifespan. In vivo MRI evaluated both the neurovascular damage and the protective effect of exercise.

Abstract: International audience; Mass spectrometry imaging (MSI) is a powerful tool to visualize the spatial distribution of molecules on a tissue section. The main limitation of MALDI-MSI of proteins is the lack of direct identification. Therefore, this study focuses on a MSI~LC-MS/MS-LF workflow to link the results from MALDI-MSI with potential peak identification and label-free quantitation, using only one tissue section. At first, we studied the impact of matrix deposition and laser ablation on protein extraction from the tissue section. Then, we did a back-correlation of the m/z of the proteins detected by MALDI-MSI to those identified by label-free quantitation. This allowed us to compare the label-free quantitation of proteins obtained in LC-MS/MS with the peak intensities observed in MALDI-MSI. We managed to link identification to nine peaks observed by MALDI-MSI. The results showed that the MSI~LC-MS/MS-LF workflow (i) allowed us to study a representative muscle proteome compared to a classical bottom-up workflow; and (ii) was sparsely impacted by matrix deposition and laser ablation. This workflow, performed as a proof-of-concept, suggests that a single tissue section can be used to perform MALDI-MSI and protein extraction, identification, and relative quantitation.

Abstract: Muscle fatigue modifies the gain between motor command magnitude and the mechanical muscular response. In other words, post-fatigue, central drives to the muscles must increase to maintain a particular submaximum mechanical output. In this study, we tested the hypothesis that this modified gain can be predicted by the central nervous system (CNS) during discrete ballistic movements. In two separate experiments, subjects were required to perform shoulder flexions in standing and sitting positions at submaximum target peak accelerations. They were assisted with visual feedback informing them on their performance after each trial. Shoulder flexions were performed before and after fatiguing protocols of the focal muscles. Acceleration signals, focal and postural muscle electromyograms (EMGs) were recorded. The results demonstrated that participants were able to reach with precision the target acceleration during the first movements post-fatigue at the cost of significant increase in focal motor command magnitude. Decreased variance of peak accelerations associated with increased focal command variability was observed post-fatigue. During the standing experiment, postural muscle EMGs revealed that anticipatory postural adjustments (APAs) scaled to focal movement acceleration post-fatigue. All these results support that fatigue effects are taken into account during movement planning. Indeed, given that no feedback could enable participants to adjust acceleration during movement, this capacity to anticipate fatigue effects is the exclusive result of feedforward processes. To account for this prediction capacity, we discuss the role of fatigue-related modifications in sensory inputs from the working muscles.

Abstract: International audience; Aim: Advanced atherosclerosis increases inflammation and stroke risk in the cerebral vasculature. Exercise is known to improve cardio-metabolic profiles when associated with a caloric restriction, but it remains debated whether it is still beneficial without the dietary control. The aim of this study was to determine both the peripheral and central effects of exercise training combined with a cholesterol-rich diet given ad libitum in old ApoE-/- mice. Methods: Forty-five-weeks old obese ApoE-/- mice fed with a high cholesterol diet ad libitum were divided into Exercise-trained (EX; running wheel free access) and Sedentary (SED) groups. Insulin tolerance and brain imaging were performed before and after the twelve-weeks training. Tissue insulin resistance, oxidative stress, and inflammation markers in plasma, aorta, and brain were then assessed. Results: In EX ApoE-/- mice, no beneficial effect of exercise was observed on weight, abdominal fat, metabolic parameters, oxidative stress, or inflammation compared to SED. Despite the regular exercise training in ApoE-/- EX mice (mean of 12.5 km/week during 12 weeks), brain inflammation imaging score was significantly associated with increased blood brain barrier (BBB) leakage evaluated by imaging follow-up (r2 = 0.87; p = 0.049) with a faster evolution compared to SED ApoE-/-mice. Conclusion: We conclude that in a context of high cardio-metabolic risk, exercise does not provide any protective effect in old ApoE-/- animals under high cholesterol diet given ad libitum. Peripheral (insulin sensitivity and oxidative/inflammatory status) but also central features (BBB preservation and protection against inflammation) did not show any benefits of exercise. Indeed, there was a fast induction of irreversible brain damage that was more pronounced in exercise-trained ApoE-/- mice.

Abstract: International audience; Purpose: Sleep deprivation (SD) is characterized by reduced cognitive capabilities and endurance exercise performance and increased perceived exertion (RPE) during exercise. The combined effects of SD and exercise-induced changes in neuromuscular function and cognition are unknown. This study aimed to determine whether central fatigue is greater with SD, and if so, whether this corresponds to diminished cognitive and physical responses. Methods: Twelve active males performed two 2-d conditions (SD and control (CO)). On day 1, subjects performed baseline cognitive and neuromuscular testing. After one night of SD or normal sleep, subjects repeated day 1 testing and then performed 40-min submaximal cycling and a cycling test to task failure. Neuromuscular and cognitive functions were evaluated during the cycling protocol and at task failure. Results: After SD, exercise time to task failure was shorter (1137 +/- 253 vs 1236 +/- 282 s, P = 0.013) and RPE during 40 min submaximal cycling was greater (P = 0.009) than that in CO. Maximal peripheral voluntary activation decreased by 7% (P = 0.003) and cortical voluntary activation tended to decrease by 5% (P = 0.059) with exercise. No other differences in neuromuscular function or cognitive control were observed between conditions. After SD, mean reaction time was 8% longer (P = 0.011) and cognitive response omission rate before cycling was higher (P < 0.05) than that in CO. Acute submaximal exercise counteracted cognitive performance deterioration in SD. Conclusions: One night of complete SD resulted in decreased time to task failure and cognitive performance and higher RPE compared with the control condition. The lack of difference in neuromuscular function between CO and SD indicates that decreased SD exercise performance was probably not caused by increased muscular or central fatigue.

Abstract: AB Introduction: Prolonged and strenuous exercise (PSE) induces transient left ventricular (LV) dysfunction. Although a consensus exists regarding the decrease in diastolic function, the existence of a decrease in systolic function by a PSE remains controversial, probably due to the transient tachycardia and changes in loading conditions observed upon the completion of exercise. Therefore, the objective was to evaluate LV systolic function before and after a PSE using two-dimensional speckle tracking echocardiography not only at rest but also during incremental tests to adjust heart rates (HR). Methods and Results: Sixteen healthy young men (23 +/- 3 yr old) performed a 3-h period of intensity-controlled upright cycling. LV strain (S), systolic strain rate (SR), rotation, and systolic rotational rate were evaluated by two-dimensional speckle tracking echocardiography before and after a 3-h period of PSE at rest and during incremental tests. Posttest evaluation was performed once the HR had returned to the pretest value. Under resting conditions, parameters of systolic function were either unchanged or increased after the PSE. However, during the incremental test, all LV systolic SR and apical rotational rates were decreased after PSE (radial SR at workload 3 (W3): 2.21 +/- 0.12.s-1 vs 1.87 +/- 0.10.s-1, P < 0.01 and apical rotational rate at W3: 128 +/- 28 deg.s-1 vs 105 +/- 26 deg.s-1, P < 0.05). Regression analyses between LV systolic SR and HR showed lower y-intercepts without differences in slopes, suggesting a decrease of both global and regional systolic functions irrespective of HR after the PSE. Conclusion: Our findings based on LV S and SR data during incremental tests demonstrate that the 3-h period of PSE induces LV systolic dysfunction

Abstract: International audience; Objectives: The aim of the present study was to investigate the level of cardiorespiratory fitness and neuromuscular function of ICU survivors after COVID-19 and to examine whether these outcomes are related to ICU stay/mechanical ventilation duration.Design: Prospective nonrandomized study.Setting: Patients hospitalized in ICU for COVID-19 infection.Patients: Sixty patients hospitalized in ICU (mean duration: 31.9 ± 18.2 d) were recruited 4-8 weeks post discharge from ICU.Interventions: None.Measurements and main results: Patients visited the laboratory on two separate occasions. The first visit was dedicated to quality of life questionnaire, cardiopulmonary exercise testing, whereas measurements of the knee extensors neuromuscular function were performed in the second visit. Maximal oxygen uptake (V o2 max) was 18.3 ± 4.5 mL·min -1 ·kg -1 , representing 49% ± 12% of predicted value, and was significantly correlated with ICU stay/mechanical ventilation (MV) duration ( R = -0.337 to -0.446; p < 0.01 to 0.001), as were maximal voluntary contraction and electrically evoked peak twitch. V o2 max (either predicted or in mL· min -1 ·kg -1 ) was also significantly correlated with key indices of pulmonary function such as predicted forced vital capacity or predicted forced expiratory volume in 1 second ( R = 0.430-0.465; p ≤ 0.001) and neuromuscular function. Both cardiorespiratory fitness and neuromuscular function were correlated with self-reported physical functioning and general health status.Conclusions: V o2 max was on average only slightly above the 18 mL·min -1 ·kg -1 , that is, the cut-off value known to induce difficulty in performing daily tasks. Overall, although low physical capacities at admission in ICU COVID-19 patients cannot be ruled out to explain the association between V o2 max or neuromuscular function and ICU stay/MV duration, altered cardiorespiratory fitness and neuromuscular function observed in the present study may not be specific to COVID-19 disease but seem applicable to all ICU/MV patients of similar duration.

Abstract: International audience; Introduction:Motor imagery (MI) can be defined as a “dynamic state during which one simulates an action mentally without any body movement” [1]. The aim of MI is to optimise learning (e.g., in athletic training) or re-learning (e.g., in motor rehabilitation after stroke) by mastering the technique of new motor skills, but also through attentional focus [2] thanks to brain plasticity mechanisms. Indeed, similarities exist between MI and motor execution with regards to the solicitation of certain brain networks and regions, including premotor, parietal, and somatosensory regions [3]. Current BCI protocols targeting MI consist in positively reinforcing the maximum modulation of sensorimotor rhythms (SMRs) from baseline levels. This suggests that we consider that the growing expertise in the MI task will be associated with a higher desynchronisation of neurons in the sensorimotor cortices [4]. Yet, experiments investigating the neural efficiency hypothesis have shown that experts happen to have a reduced modulation of neural activity in comparison to novices [5], which can be attributed to a more efficient resource distribution. This efficiency would take form of reinforced temporal and spatial stability during MI tasks [6,7]. Thus, our questions are as follows: Q1. Does expertise modify the brain patterns associated with a MI task? Q2. If so, are those modifications elicited exclusively during MI of mastered movements? Or do these modifications reflect the acquisition of a generic skill (whatever the imagined movement)? Q3. Is maximising the percentage of desynchronisation a relevant objective? If not, what metrics of performance should be used in order to optimise the training of users/patients to self-regulate their brain activity? In order to investigate those different questions, we will recruit athletes who can be considered as an expert population in MI because of their frequent mental training use (to prevent overtraining or during rehabilitation but also as warm-up routines and rehearsal technique). Material and Methods:We will recruit 48 participants who will be divided into three groups: “basketball experts” (G1exp), “dance experts” (G2exp) and “novices” (G3) [16 participants per group, 8 men, 8 women]. All participants will perform 20 MI trials lasting 10s each for all four of the following movements: a simple reaching action (T1 – for which all participants are experts), a complex novel drawing task (T2 – for which none are expert), a basketball free throw (T3 – expertise of G1exp only) and a short pre-defined dancing choreography (T4 – expertise of G2exp only). EEG activity will be recorded during trials. This paradigm will enable us to compare MI-related brain patterns between experts and novices (G1exp/G2exp vs. G3) (Q1). It will also enable us to assess the extent to which the potential modifications of brain patterns due to expertise are specific to expert movements or generic (for G1exp and G2exp: T3 vs. T4; for all, control: T1 vs. T2) (Q2). From those results, we will investigate which performance metrics seem the most relevant to use in MI-based BCI/NF paradigms (Q3).Discussion:This study will enable us to acquire new knowledge regarding the neural efficiency hypothesis. If our results comfort this hypothesis, it will be important to identify, implement and evaluate new metrics of performance to guide BCI/NF users during their training (i.e., instead of ERD%). Following the neural efficiency hypothesis, we expect experts to show brain patterns that are more spatially and temporally stable than those of novices [6,7]. In other terms, in experts, we expect the modulations of brain activity during MI to be circumscribed to sensorimotor cortices (provided that they perform kinaesthetic MI) and to be highly stable across trials in terms of location and frequency. In addition, we hypothesise that those modulations will represent a general skill. In other terms, we expect the same patterns to be elicited for experts when doing MI of a mastered technique from their discipline, but also when doing MI of a novel movement of similar nature (in our case a different physical activity). Indeed, we believe that a transfer of neural efficiency exists. However, this phenomenon might only happen to a certain extent and not be identifiable for MI of a complex novel task or of a simple everyday life action. Abstract: International audience; Introduction: Originally developed for astronauts, lower body positive pressure treadmills (LBPPTs) are increasingly being used in sports and clinical settings because they allow for unweighted running. However, the neuromuscular adjustments to unweighted running remain understudied. They would be limited for certain lower limb muscles and interindividually variable. This study investigated whether this might be related to familiarization and/or trait anxiety. Methods: Forty healthy male runners were divided into two equal groups with contrasting levels of trait anxiety (high, ANX+, n = 20 vs. low, ANX−, n = 20). They completed two 9-min runs on a LBPPT. Each included three consecutive 3-min conditions performed at 100%, 60% (unweighted running), and 100% body weight. Normal ground reaction force and electromyographic activity of 11 ipsilateral lower limb muscles were analyzed for the last 30 s of each condition in both runs. Results: Unweighted running showed muscle- and stretch-shortening cycle phase-dependent neuromuscular adjustments that were repeatable across both runs. Importantly, hamstring (BF, biceps femoris; STSM, semitendinosus/semimembranosus) muscle activity increased during the braking (BF: +44 ± 18%, p < 0.001) and push-off (BF: +49 ± 12% and STSM: +123 ± 14%, p < 0.001 for both) phases, and even more so for ANX+ than for ANX−. Abstract: International audience; Motor imagery (MI) is widely used in sports, in particular for motor learning and anxiety management [1]. Indeed, athletes consistently report using MI for rehearsing technique, maintaining arousal or enhancing self-confidence [2]. However, optimal gains are limited by the fact that they receive no feedback and cannot assess their performance. This can have detrimental effects on their motivation to diligently practice MI. Therefore, using Neurofeedback (NF) appears to be an adapted solution, allowing athletes to know if they are correctly performing the task. This addition also makes it possible to objectify performance and quantify progression throughout the sessions. In fact, three recent meta-analysis testify that NF improves the ability to self-regulate brain activity and sport performance [3, 4, 5]. The number of papers using MI-NF protocols has increased in the past few years. However, their efficiency has been shown to depend on various factors such as expertise in the imagined task [6], personality traits [7], MI ability [8] or NF acceptability [9]. Some of these factors can be assessed using validated questionnaires [10,11,12] but items are generally focused on questioning the current use of MI or thoughts on NF. An issue, as we know that practice can sometimes differ from real needs and expectations. To our knowledge, no previous work has investigated sports field actors’ preferences with the aim of designing specific NF tools, perfectly fitted to their profile, goals and expectations. Therefore, based on pre-existing questionnaires, we will build and broadcast an online questionnaire for athletes where personality, MI use, NF acceptability and NF preferences will be assessed. This method will enable us to personalise MI-NF trainings to fit each athlete’s needs. Abstract: Objectives To study the responsiveness of a combined power Doppler and greyscale ultrasound (PDUS) score for assessing synovitis in biologic-naive patients with rheumatoid arthritis (RA) starting abatacept plus methotrexate (MTX). Methods In this open-label, multicentre, single-arm study, patients with RA (MTX inadequate responders) received intravenous abatacept (∼10 mg/kg) plus MTX for 24 weeks. A composite PDUS synovitis score, developed by the Outcome Measures in Rheumatology–European League Against Rheumatism (OMERACT–EULAR)-Ultrasound Task Force, was used to evaluate individual joints. The maximal score of each joint was added into a Global OMERACT–EULAR Synovitis Score (GLOESS) for bilateral metacarpophalangeal joints (MCPs) 2–5 (primary objective). The value of GLOESS containing other joint sets was explored, along with clinical efficacy. Results Eighty-nine patients completed the 24-week treatment period. The earliest PDUS sign of improvement in synovitis was at week 1 (mean change in GLOESS (MCPs 2–5): −0.7 (95% CIs −1.2 to −0.1)), with continuous improvement to week 24. Early improvement was observed in the component scores (power Doppler signal at week 1, synovial hyperplasia at week 2, joint effusion at week 4). Comparable changes were observed for 22 paired joints and minimal joint subsets. Mean Disease Activity Score 28 (C reactive protein) was significantly reduced from weeks 1 to 24, reaching clinical meaningful improvement (change ≥1.2) at week 8. Conclusions In this first international prospective study, the composite PDUS score is responsive to abatacept. GLOESS demonstrated the rapid onset of action of abatacept, regardless of the number of joints examined. Ultrasound is an objective tool to monitor patients with RA under treatment. Abstract: External ankle supports, such as ankle braces, may improve postural stability by stimulating cutaneous receptors. It remains unknown whether these supports have an effect on the posture central regulation. The aim of this study was to determine the effect of wearing a soft ankle brace on soleus H-reflex amplitude and presynaptic inhibition during standing. Sixteen subjects stood on a rigid floor with their eyes opened, either barefoot or wearing a soft ankle brace. H-reflex amplitude was measured on the soleus muscle by stimulating the tibial nerve electrically. Modulation of presynaptic inhibition was assessed by conditioning the H-reflex with fibular nerve (D1 inhibition) and femoral nerve (heteronymous facilitation) electrical stimulations. The unconditioned H-reflex amplitude was significantly greater when wearing the ankle brace than barefoot, whereas D1 and HF conditioned soleus H-reflex did not differ significantly between bracing conditions. These results suggest that the ankle brace increased the soleus motoneuron excitability without altering presynaptic mechanisms, potentially because of increased cutaneous mechanoreceptors afferent signals provided by the soft ankle brace.

Abstract: International audience; Objective: To assess the level of scientific evidence and the place in the rehabilitation framework of isokinetic muscle strengthening (IMS) for knee osteoarthritis (OA). Methods: A systematic review of the English literature in MEDLINE via PubMed, the Cochrane Library, and PEDro databases for only randomized comparative trials. Data that were sufficiently homogeneous underwent comprehensive meta-analysis. Methodological assessment was done by using the CLEAR scale for non-pharmacologic trials. Results: We identified articles for 9 trials (696 patients). All trials were of low to moderate quality. Tolerance of IMS was considered good. Improvement in muscle strength was better with an IMS program than no treatment or an isometric exercise but did not differ with an aerobic program. We found an important effect for pain (standardized mean difference 1.218 [95% CI 0.899-1.54], P < 0.001) and functional Lequesne index (1.61 [0.40-2.81], P = 0.009) and a moderate effect for the Western Ontario and McMaster Universities Osteoarthritis Index subscore C for disability (0.58 [0.04-1.11], P = 0.03). Conclusions: IMS is an effective way to propose dynamic muscle strengthening for knee OA rehabilitation and has a significant effect on pain and disability. Because of the weak methodology and the great heterogeneity of studies, particularly in IMS protocol and outcome measures, insufficient data are available to provide guidelines about efficacy and strategy. Future clinical trials are needed, but more attention should be paid to the methods of such studies to clarify the role of IMS in the therapeutic armamentarium of knee OA. (
