Abstract

Sea storm vulnerability in the coastal area of northern Marche between the mouths of Foglia and Metauro rivers has been analysed. The method used had already been applied by other authors in some areas of the northern Adriatic region and utilizes a series of different variables, in particular: present and recent morphodynamic parameters of beach and shoreface, bathymetric characteristics and pressure of use at beaches. Data were processed using GIS software. Results point to high potential vulnerability, often mitigated by coastal protection systems that are highly developed in the area. Moreover, considering the amount of exposure of different coastal sectors, it was possible to obtain risk scenarios that, in some cases, result to be high. This emphasizes the importance of extending this type of investigation to larger areas in order to get indications for long-term coastal area management, also in the context of relative sea level rise.

Abstract

Aims Patients with acute coronary syndromes (ACSs) who are managed without coronary revascularization represent a mixed and understudied population that seems to receive suboptimal pharmacological treatment. Methods and results We assessed patterns of antithrombotic therapies employed during the hospitalization and in-hospital clinical events of medically managed patients with ACS enrolled in the prospective, multicentre, nationwide EYESHOT (EmploYEd antithrombotic therapies in patients with acute coronary Syndromes HOspitalized in iTalian cardiac care units) registry. Among the 2585 consecutive ACS patients enrolled in EYESHOT, 783 (30.3%) did not receive any revascularization during hospital admission. Of these, 478 (61.0%) underwent coronary angiography (CA), whereas 305 (39.0%) did not. The median GRACE and CRUSADE risk scores were significantly higher among patients who did not undergo CA compared with those who did (180 vs. 145, P < 0.0001 and 50 vs. 33, P < 0.0001, respectively). Antithrombotic therapies employed during hospitalization significantly differ between patients who received CA and those who did not with unfractioned heparin and novel P2Y12 inhibitors more frequently used in the first group, and low-molecular-weight heparins and clopidogrel in the latter group. During the index hospitalization, patients who did not receive CA presented a higher incidence of ischaemic cerebrovascular events and of mortality compared with those who underwent CA (1.6 vs. 0.2%, P = 0.04 and 7.9 vs. 2.7%, P = 0.0009, respectively). Conclusion Almost one-third of ACS patients are managed without revascularization during the index hospitalization. In this population, a lower use of recommended antiplatelet therapy and worse clinical outcome were observed in those who did not undergo CA when compared with those who did. Abstract

Aims In the last decades, several new therapies have emerged for the treatment of acute coronary syndromes (ACS). We sought to describe real-world patterns of use of antithrombotic treatments in the catheterization laboratory for ACS patients undergoing percutaneous coronary interventions (PCI). Methods EmploYEd antithrombotic therapies in patients with acute coronary Syndromes HOspitalized in iTalian cardiac care units was a nationwide, prospective registry aimed to evaluate antithrombotic strategies employed in ACS patients in Italy. Results Over a 3-week period, a total of 2585 consecutive ACS patients have been enrolled in 203 cardiac care units across Italy. Among these patients, 1755 underwent PCI (923 with ST-elevation myocardial infarction and 832 with non-ST-elevation ACS). In the catheterization laboratory, unfractioned heparin was the most used antithrombotic drug in both ST-elevation myocardial infarction (64.7%) and non-ST-elevation ACS (77.5%) undergoing PCI and, as aspirin, bivalirudin and glycoprotein IIb/IIIa inhibitors (GPIs) more frequently employed before or during PCI compared with the postprocedural period. Any crossover of heparin therapy occurred in 36.0% of cases, whereas switching from one P2Y12 inhibitor to another occurred in 3.7% of patients. Multivariable analysis yielded several independent predictors of GPIs and of bivalirudin use in the catheterization laboratory, mainly related to clinical presentation, PCI complexity and presence of complications during the procedure. Conclusion In our contemporary, nationwide, all-comers cohort of ACS patients undergoing PCI, antithrombotic therapies were commonly initiated before the catheterization laboratory. In the periprocedural period, the most frequently employed drugs were unfractioned heparin, leading to a high rate of crossover, followed by GPIs and bivalirudin, mainly used during complex PCI. Abstract

International audience; Background and PurposeThe tachykinin NK2 receptor antagonist ibodutant is under Phase III clinical investigation to treat female patients with irritable bowel syndrome. The aim of this study was to investigate the NK2 receptor-related gender specificity in a model of colitis. Experimental ApproachColitis was induced by rectal instillation of 2,4,6-trinitrobenzenesulfonic acid (TNBS, 0.5mL, 30mgmL(-1) in 30% ethanol) in female and male guinea pigs. Electromyographic recording of the responses to colorectal distension (CRD) was made 3days later. Ibodutant (0.33 , 0.65, 1.9 and 6.5mgkg(-1)) was given s.c., 30min before CRD. Release of neurokinin A and substance P from isolated mucosal and smooth muscle tissues following treatment with KCl (80mM) or capsaicin (10M) was measured by EIA. Plasma pharmacokinetics of ibodutant following a single s.c. administration (0.73 or 2.1mgkg(-1)) were measured over 24h. Key ResultsIbodutant did not affect abdominal contractions in control animals. After TNBS-induced colitis, ibodutant prevented the increased visceral hypersensitivity to CRD in females, at lower doses than in males. Ibodutant pharmacokinetics did not differ between females and males. Tachykinins release was greater in smooth muscle than in mucosal samples. Capsaicin-stimulated release of tachykinins from inflamed mucosal samples from females was significantly lower than in males. Conclusions and ImplicationsIbodutant prevented abdominal nociception in a model of visceral hypersensitivity in guinea pigs with a greater efficacy in females than in males. Our results highlight a gender-related difference in colonic visceral hypersensitivity and mucosal nerve activation.

Abstract

L’azione e le conseguenze derivanti dalla diffusa presenza di emissioni metanifere spontanee nei fondali centro-nord adriatici ha ormai assunto una valenza ed un risalto che non è più possibile sottovalutare e trascurare. Questo non solo sotto il profilo strettamente “ambientale” (sia fisico-geologico che bio-ecologico che geochimico) ma anche in relazione al sempre maggior sviluppo assunto dalle attività antropiche, di vario livello e tipologia (dalla posa e controllo di condotte, alla pesca e maricoltura, alle attività di esplorazione e sfruttamento di idrocarburi) che ormai tendono ad insistervi. Nuovi dati scaturiti da rilievi multibeam, chirp e campionature condotti negli ultimi anni in diversi settori del centro-nord adriatico, integrati con il riesame critico e la riorganizzazione di quanto rilevabile in letteratura, hanno permesso di giungere ad alcune interessanti precisazioni e valutazioni per quanto riguarda: a) presenza, distribuzione ed evoluzione di morfostrutture e strutture sedimentarie come pockmarks e mud volcanoes che sono ascrivibili alle emissioni spontanee; b) origine e sviluppo di depositi carbonatici di diversa tipologia e loro localizzazione in relazione alle morfostrutture. La reciproca interazione tra i processi di formazione delle strutture e la precipitazione di carbonati è infatti molto spesso evidente. Di particolare interesse e novità risultano i recenti studi effettuati su numerosi campioni di carbonati provenienti da diversi siti del nord e centro Adriatico. Le caratteristiche isotopiche e composizionali e la modalità di formazione di tali concrezioni suggeriscono una notevole diversificazione di processi, innescati dalla presenza delle emissioni di gas e delle loro relazioni con i depositi silicoclastici e biogenici.

Abstract

A stratigraphic revision of the Paludi Formation, cropping out in Northern Calabria on the north-eastern side of Sila Massif, has confirmed that it has been deposited between two main tectonic phases. Actually the formation is involved only by top to the north-east thrusts, different from those separating the crystalline and ophiolitic nappes of Northern Calabria, and rests unconformably on the Sila Unit, the uppermost nappe of the tectonic stack. In the frame of the interpretation of Northern Calabria as a fragment of the Alps, the Paludi Formation play the role of a deposit post-dating the eo-alpine tectonic phases and pre-dating the Apenninic orogenic transport. Therefore the Aquitanian age of the formation, obtained by nannofloras analyses, can be considered the minimum age to the Apulia-verging orogenic transport, witnessed by top to north-east thrusts. It fairly corresponds to the age referred to the Lucanian ocean closure.

Abstract

Bradykinin, through its B(2) receptor, is involved in inflammatory processes related to arthropathies. In carrageenan and lipopolysaccharide (LPS)-induced arthritis in rat, the anti-inflammatory activity of MEN16132, a potent and selective kinin B(2) receptor antagonist, was compared with that of steroidal and nonsteroidal anti-inflammatory drugs. The interaction between MEN16132 and dexamethasone was also investigated.Drugs, alone or in combination, were injected into the knee joint 30min before intra-articular administration of carrageenan or LPS, in pentobarbital anaesthetized rats. Effects on incapacitation, oedema, neutrophil recruitment and kallikrein system activation, in the knee joint, were assessed.MEN16132 and dexamethasone (10-300µg per knee) dose-dependently reduced carrageenan-induced joint pain, oedema and neutrophil infiltration, reaching a maximal inhibition of about 50%. Dexketoprofen exerted a similar analgesic activity, whereas it did not affect the other inflammatory responses. MEN16132 showed a partial inhibition of LPS-induced joint pain, whereas dexamethasone produced a full analgesic effect. Combination of MEN16132 and dexamethasone showed a strong synergistic interaction in inhibiting both carrageenan and LPS-induced knee joint inflammation. Dexamethasone did not prevent the contact activation of prekallikrein by carrageenan and the subsequent release of kallikreins and bradykinin in the synovium.Steroids and kinin B(2) receptor antagonists appear to relieve arthritic symptoms induced by carrageenan or LPS and act synergistically to inhibit joint inflammation. This could have interesting therapeutic implications, possibly opening the way for combination therapies in the control of inflammatory arthropathies.

Abstract

The cost of implementing real-time monitoring and control of industrial processes is a significant barrier for many companies. Acoustic techniques provide complementary information to optical spectroscopic sensors and have a number of advantages: they are relatively inexpensive, can be applied non-invasively, are non-destructive, multi-point measurements are possible, opaque samples can be analysed in containers that are made from opaque materials (e.g. steel or concrete) and the analysis can be conducted in real-time. In this paper a new theoretical model is proposed which describes the transport of particles in a stirred reactor, their collision with the reactor walls, the subsequent vibrations which are then transmitted through the vessel walls, and their detection by an ultrasonic transducer. The particle-wall impact is modelled using Hertz-Zener impact theory. Experimental data is then used in conjunction with this (forward) model to form an inverse problem for the particle size distribution using a least squares cost function. Application of an integral smoothing operator to the power spectra greatly enhances the accuracy and robustness of the approach. One advantage of this new approach is that since it operates in the frequency domain, it can cope with the industrially relevant case of many particle-wall collisions. The technique will be illustrated using data from a set of controlled experiments. In the first instance a set of simplified experiments involving single particles being dropped in air onto a substrate are utilised. The second set of experiments involves particles in a carrier fluid being stirred in a reactor vessel. In each case the approach is able to successfully recover the associated particle size.

Dear Editor, I Sarcoptes scabiei i is an astigmatid mite of the genus I Sarcoptes i with high morphological variability in relation to its host.[1] In humans, I S. scabiei i var. I hominis i causes common and crusted scabies.[1] The latter typically occurs in patients with immunological, neurological and metabolic disorders.[2] We report a nosocomial outbreak of crusted scabies as a result of I S. scabiei i var. I canis i in immunosuppressed patients. Moreover, 1 month before our observation, when the patient's dog was already completely healed, the patient's wife and two sons presented recurrent, self-limiting and itchy erythemato-papular lesions without burrows, compatible with the diagnosis of pseudo-scabies.
