Abstract

Background: The aim of this study was to identify which factors were associated with a risk of hospital readmission within 3 months after discharge of a sample of elderly patients admitted to internal medicine and geriatric wards. Methods: Of the 1178 patients aged 65 years or more and discharged from one of the 66 wards of the 'Registry Politerapie SIMI (REPOSI)' during 2010, 766 were followed up by phone interview 3 months after discharge and were included in this analysis. Univariate and multivariate logistic regression models were used to evaluate the association of several variables with rehospitalization within 3 months from discharge. Results: Nineteen percent of patients were readmitted at least once within 3 months after discharge. By univariate analysis in-hospital clinical adverse events (AEs), a previous hospital admission, number of diagnoses and drugs, comorbidity and severity index (according to Cumulative Illness Rating Scale-CIRS), vascular and liver diseases with a level of impairment at discharge of 3 or more at CIRS were significantly associated with risk of readmission. Multivariate logistic regression analysis showed that only AEs during hospitalization, previous hospital admission, and vascular and liver diseases were significantly associated with the likelihood of readmission. Conclusions: The results demonstrate the need for increased medical attention towards elderly patients discharged from hospital with characteristics such as AEs during the hospitalization, previous admission, vascular and liver diseases. © 2012 European Federation of Internal Medicine. 67. Clinical and coronary haemodynamic determinants of persistent chest pain in patients with non-obstructive coronary artery disease-A pilot study.

Abstract

Introduction: A third of patients undergoing elective angiography have chest pain without obstructive coronary artery disease (NoCAD), with many having persistent symptoms. The objective of the study is to identify the clinical and coronary haemodynamic determinants of persistent chest pain at 1 month in NoCAD patients. Method(s): Patients with NoCAD underwent invasive coronary haemodynamic testing including: (1) Microvascular function assessment using coronary flow reserve (CFR) and hyperaemic microvascular resistance (hMR) measured by a combined pressure/Doppler wire at peak hyperaemia induced by intravenous adenosine infusion (140mcg/kg/min), (2) Coronary endothelial function assessment with low dose intracoronary acetylcholine (IC-ACh) infusions (0.18mcg/min & 1.8mcg/min over 2 min respectively), and (3) Spasm provocation testing by IC-ACh boluses (25, 50 and 100mcg). Health status (symptoms, physical limitations and quality of life) was assessed at baseline and 1 month. Result(s): Of the 40 recruited patients (77% female, 54+/-11 years), 30 (75%) continued to experience chest pain. Univariate analysis revealed frequent chest pain prior to angiography (93% vs 50% P=0.004), prior unstable angina admission (73% vs 33% P=0.014) and microvascular dysfunction defined as hMR >1.9 (67% vs 30% P= 0.042) were determinants of persistent symptoms at 1 month.Onmultivariate logistic regression analysis, only abnormal hMR was an independent determinant of persistent chest pain (odd ratio, 4.83; 95% confidence interval, 0.91-25.71; P=0.06). Conclusion(s): Two-thirds of patients with NoCAD have persistent chest pain one month following elective angiography. An abnormal hMR is an independent predictor of this outcome and therapies targeting the microvascular dysfunction may alleviate the symptoms.

Abstract

Background: As chronicity represents one of the major challenges in the healthcare of aging populations, the understanding of how chronic diseases distribute and co-occur in this part of the population is needed. Objectives: The aims of this study were to evaluate and compare patterns of diseases identified with cluster analysis in two samples of hospitalized elderly. Methods: Data were obtained from the multicenter ‘Registry Politerapie SIMI (REPOSI)' that included people aged 65 or older hospitalized in internal medicine and geriatric wards in Italy during 2008 and 2010. The study sample from the first wave included 1,411 subjects enrolled in 38 hospitals wards, whereas the second wave included 1,380 subjects in 66 wards located in different regions of Italy. To analyze patterns of multimorbidity, a cluster analysis was performed including the same diseases (19 chronic conditions with a prevalence >5%) collected at hospital discharge during the two waves of the registry. Results: Eight clusters of diseases were identified in the first wave of the REPOSI registry and six in the second wave. Several diseases were included in similar clusters in the two waves, such as malignancy and liver cirrhosis; anemia, gastric and intestinal diseases; diabetes and coronary heart disease; chronic obstructive pulmonary disease and prostate hypertrophy. Conclusion: These findings strengthened the idea of an association other than by chance of diseases in the elderly population.

Abstract

The present study aims to evaluate the potential genotoxic and associated factors among coal miners, divided by degree of exposure. Blood and buccal smears were collected from 158 workers, who actively participate in different activities in coal mining, and 48 individuals living in the same city but do not have participation in coal mining activities (control group). The workers were divided into three different groups, according to the level of contact with coal extraction. A questionnaire intended to identify factors associated with DNA damage was performed in participants. The results regarding oral mucosa micronucleus test showed a significant difference (p < 0.001) of the worker groups 1 and 2 in relation to the control group, where the group 1 has a higher degree of exposure to coal than group 2. For the lymphocyte micronucleus test and comet assay, there was no significant difference between the exposed groups and control group. There is an association between the outcome and the fact of living in the municipality of the mining company and the exposure to radiation in the last 12 months. Besides, the multivariate analysis showed an association of the tail moment with radiation exposure in the last 12 months. Thus, the findings of this study reveal genotoxicity in oral mucosa cells of workers exposed to coal and that workers with higher degree of contact with coal have a more pronounced response. Abstract

Abstract Background Improvements in patient-reported health-related quality of life (HRQoL) are important goals of cardiac rehabilitation (CR). In patients undergoing coronary angiography for angina and with documented coronary artery disease (CAD), the present study compared HRQoL over 6 months in CR participants and non-participants. Clinical predictors of CR participants were also assessed. Methods A total of 221 consecutive patients undergoing angiography for angina with documented CAD and who were eligible for a CR program were recruited. CR participants were enrolled in a six-week Phase II outpatient CR course (31%, n = 68) within 2 months following angiography and the non-participants were included as a control. At baseline (angiography), one and six months post angiography, clinical and HRQoL data were obtained including the Short Form-36 (SF-36) and the Seattle Angina Questionnaire (SAQ). The response rate for the HRQoL assessment was 68% (n = 150). Cross sectional comparisons were age-adjusted and performed using logistic or linear regression as appropriate. Longitudinal changes in HRQoL were assessed using least squares regression. Finally, a multiple logistic regression was fitted with CR participant as the final outcome. Results At angiography, the CR non-participants were older, and age-adjusted analyses revealed poorer physical (angina limitation: 54 ± 25 versus 64 ± 22, p Conclusion Following angiography, CAD patients reported improvements in both generic and disease-specific HRQoL, however CR participation did not influence this outcome. This may be explained by biases in CR enrollment, whereby acute patients, who may be less limited in HRQoL compared to stable, chronic patients, are targeted for CR participation. Further investigation is required so CR programs maximize the quality of life benefits to all potential CR patients.
