Abstract

Background. A large outbreak of hepatitis A affected individuals in several Australian states in 2009, resulting in a 2-fold increase in cases reported to state health departments compared with 2008. Two peaks of infection occurred (April-May and September-November), with surveillance data suggesting locally acquired infections from a widely distributed food product. Methods. Two case-control studies were completed. Intensive product trace-back and food sampling was undertaken. Genotyping was conducted on virus isolates from patient serum and food samples. Control measures included prophylaxis for close contacts, public health warnings, an order by the chief health officer under the Victorian Food Act 1984, and trade-level recalls on implicated batches of semidried tomatoes. Results. A multijurisdictional case-control study in April-May found an association between illness and consumption of semidried tomatoes (odds ratio [OR], 3.0; 95% CI 1.4-6.7). A second case-control study conducted in Victoria in October-November also implicated semidried tomatoes as being associated with illness (OR, 10.3; 95% CI, 4.7-22.7). Hepatitis A RNA was detected in 22 samples of semidried tomatoes. Hepatitis A virus genotype IB was identified in 144 of 153 (94%) patients tested from 2009, and partial sequence analysis showed complete identity with an isolate found in a sample of semidried tomatoes. Conclusions. The results of both case-control studies and food testing implicated the novel vehicle of semidried tomatoes as the cause of this hepatitis A outbreak. The outbreak was extensive and sustained despite public health interventions, the design and implementation of which were complicated by limitations in food testing capability and complex supply chains. Abstract

Background: Pyogenic flexor tenosynovitis is a closed space infection involving the digital flexor tendon sheaths of the upper extremity that can cause considerable morbidity. The purpose of the present report is to describe the various risk factors leading to poor outcomes and to recommend a clinical classification system for this condition. Methods: We studied seventy-five patients with pyogenic flexor tenosynovitis over a six-year period. The amputation rate and total active motion were used as outcomes measures. The clinical factors influencing outcomes were identified and analyzed. Results: The five risk factors associated with poor outcomes were (1) an age of more than forty-three years, (2) the presence of diabetes mellitus, peripheral vascular disease, or renal failure, (3) the presence of subcutaneous purulence, (4) digital ischemia, and (5) polymicrobial infection. On the basis of the clinical findings and outcomes, three distinct groups of patients could be identified, each with a progressively worse outcome. Patients in Group I had no subcutaneous purulence or digital ischemia; these patients had the best prognosis, with no amputations and a mean 80% return of total active motion. Patients in Group II demonstrated the presence of subcutaneous purulence but no ischemic changes; these patients had an amputation rate of 8% and a mean 72% recovery of total active motion. Patients in Group Ill had both extensive subcutaneous purulence and ischemic changes; these patients had the worst prognosis, with an amputation rate of 59% and a mean 49% return of total active motion. Conclusions: We propose a three-tier clinical classification system that can aid in prognosis and guidance in the treatment of pyogenic flexor tenosynovitis of the upper extremity. 84. Assessment of Hypercholesterolemia Control in a Managed Care Organization

Abstract

To determine the extent of achievement of goal low‐density lipoprotein cholesterol (LDL) as defined by National Cholesterol Education Program—Adult Treatment Panel II (NCEP‐ATP II) and American Diabetes Association (ADA) 2000 guidelines, we conducted a retrospective study by integrating data from medical, laboratory, and pharmacy claims databases. Subjects were selected from a 232,000‐member staff‐model managed care organization consisting of 19 clinics in the Minneapolis—St. Paul, Minnesota, metropolitan area. A total of 124,971 members aged 18 years and older, who had been continuously enrolled from July 1, 1996–June 6, 1998, were included. Outcome measures were the extent of achievement of goal LDL as defined by NCEP‐ATP II and the use of antihyperlipidemic drugs for patients with and without diabetes at various levels of risk for coronary heart disease (CHD). Of 124,971 subjects, 6538 had a history of CHD, 1523 of whom met their LDL goal. Of the population with CHD who did not achieve goal, 1141 (43%) missed by over 30 mg/dl; 621 (54%) of these patients were not receiving drug therapy. A total of 17,267 had no history of CHD but had two or more risk factors; 3298 of these achieved their LDL goal. Of those who did not achieve goal, 1136 (35%) missed by over 30 mg/dl; 897 (79%) of these were not receiving drug therapy. A total of 6586 had a history of diabetes; 1004 and 2340 reached an LDL of 100 mg/dl or lower and less than 130 mg/dl, respectively. Of those with diabetes who had an LDL greater than 100 mg/dl, 1276 (49%) missed their goal by over 30 mg/dl; 898 (70%) of these were not receiving drug therapy. Inadequate use of pharmacologic agents plays a significant role in failure to achieve goal LDL for patients with CHD, without CHD, and with diabetes. Analysis of the data based on the new ADA guidelines for LDL demonstrates the need for continued vigilance. Finally, the successful merging of medical, laboratory, and pharmacy claims databases provides a benchmark for other institutions.

Abstract

Hoffmann's isolobal theory was largely credited for bridging the gap between inorganic complexes and carbon-based compounds. Recent advances have extended its applications to organotransition metal monomers and cluster molecules. Isolobal mapping is not only applicable among molecules and molecular fragments, but also significant in comparing organometallic reactions. Such advancement is likely to influence current trends in mechanistic chemistry. Through a concise review of some of the latest and more representative results, this article aims to highlight and illustrate the versatility and applicability of the theory. Coupled with the Polyhedral Skeletal Electron Pair theory, the isolobality principle provides a useful theoretical underpinning for some synthetic and structural characteristics of many organotransition metal compounds.

When synthase D was converted into synthase I in a liver extract, it progressed through a synthase form with activity characteristics that could not be explained by a mixture of the original synthase D and the final product, synthase I. This form was distinguished by an affinity for UDP-glucose, in the absence of glucose 6-phosphate, which was intermediate between those of the two known forms.

Abstract

This research dissertation aims to understand the contemporary social issues and challenges that are endemic to Singapore and to address these issues and challenges so as to achieve and sustain the strategic competitiveness in Singapore. The pursuit of high Gross Domestic Product (GDP) growth aggressively by the Singapore government has resulted in welcoming foreign direct investments, technology and talents to exploit economic growth and address the greying population. Over time, this obsession has brewed a lethal concoction of contemporary social issues and challenges of elevated cost of living, widening income gap, reduced productivity and inflation. Mounting displeasure, job complacency and increasing social disquiet amongst the residents lead to unmotivated employees. This resulted in the firms hiring of foreign talent with lower remuneration which will replace the local native citizens. These burning issues have escalated into income disparity and social tensions which will threaten the political stability. With the rising operating costs, the firms do not upgrade their business operations or re-train their employees, leading to declining productivity. The critical findings are essential to understand and derive solutions to achieve and sustain the strategic competitive advantages and economic growth in Singapore. As the contemporary social issues and challenges are alarming, there is an urgency to undertake this research topic. To facilitate this management project, important references that had been cited and applied include Michael Porter’s competitive advantages, Porter’s Diamond models, SWOT Analysis, PEST analysis and Resource based view framework. Other sources consulted include newspapers, online journals, books and government websites to further appreciate the factual and empirical information from statistics and public opinions to support this research.

100. A nursing home outbreak of Clostridium perfringensassociated with pureed food

Abstract

Objective: To investigate and ascertain the source of a nursing home outbreak of gastroenteritis in Melbourne in 1997. Method: A retrospective cohort study. We obtained fecal and food samples and environmental swabs, and reviewed food handling. Results: There were 25 cases, and 21 of these had their meals pureed (liquidised). The relative risk for eating pureed food and becoming ill was 5.8 (95% CI 2.2–15.4). Clostridium perfringensand its enterotoxin was detected in nine fecal samples. Samples of pureed food tested positive for coliforms. Conclusions: This outbreak was caused by C. perfringenscontaminating pureed food. Food liquidising provides opportunities for re‐contamination of cooked food through the use of contaminated equipment and deficiencies in food handling practices. Pureed foods should be reheated to 70°C after liquidising to inactivate pathogens. Implications: Health care facilities need to take specific precautions to manage the special hazards involved in preparing pureed food.
