56. Extension of CART using multiple splits under order restrictions

Abstract

CART was introduced by Breiman et al. (1984) as a classification tool. It divides the whole sample recursively in two subpopulations by finding the best possible split with respect to a optimisation criterion. This method, restricted up to date to binary splits, is extended in this paper for allowing also multiple splits. The main problem with this extension is related to the optimal number of splits and the location of the corresponding cutpoints. In order to reduce the computational effort and enhance parsimony, the reduced isotonic regression was used in order to solve this problem. The extended CART method was tested in a simulation study and was compared with the classical approach in an epidemiological study. In both studies the extended CART turned out to be a useful and reliable alternative.

Abstract

Die Wirkungsevaluation wird je nach Milieu idealistisch verklärt oder als Teufelszeug verdammt. Dieser Beitrag verfolgt das Ziel die Debatte um die Wirkungsevaluation zu versachlichen. Dazu wird ein vierstufiges Vorgehen gewählt. Zuerst erfolgt eine begriffliche Klärung, um die Wirkungsevaluation gegenüber anderen Formen der Evaluation abzugrenzen und um auf die Besonderheiten der Wirkungsevaluation hinzuweisen. Nicht zuletzt sollen in diesem ersten Schritt auch einige verbreitete evaluationsfachliche Missverständnisse offengelegt werden, die einen fundierten Diskurs über Zulässigkeit, Realisierbarkeit und Nützlichkeit von Wirkungsevaluationen behindern.

96. Dimension reduction in human functioning and disability outcomes research: graphical models versus principal components analysis.

Abstract

Purpose. The complexity of rehabilitation outcomes in terms of functioning and disability leads to the need for dimension reduction in relation to specific research or clinical problems. Several statistical procedures are at hand. This article examines whether and to what extent principal component analysis (PCA) and graphical models differ in the dimension reduction of data based on the International Classification of Functioning, Disability and Health (ICF). Methods. Using a data set of 1048 persons with spinal cord injury from 14 different countries as a case in point, this article compares the solutions in dimension reduction generated with a graphical model based on least average shrinkage selection operator (LASSO) regression on one hand and with a PCA on the other. Results. Some factors extracted with the PCA properly match the clusters found with the graphical model, while in others less commonality can be found. In general, overlap ranges from 9 to 100% with 75% on average. Many of the discovered clusters or factors, i.e. dimensions, are compatible with the ICF structure, particularly in activity and participation. Conclusion. Functioning is a multidimensional, complex and dynamic outcome. We recommend being careful with dimension reduction based on statistical procedures alone. Theoretical considerations and clinical significance should always guide statistics. Stability of emergent dimensions that are compatible with theoretical or clinical considerations is a most important point. At least two different statistical procedures for dimension reduction, for instance PCA and LASSO regression, should be applied to conservatively select those dimensions that stay stable with both procedures. 98. The effect of transdermal scopolamine on the incidence and severity of postoperative nausea and vomiting in a group of high-risk patients given prophylactic intravenous ondansetron.

Abstract

Specific risk factors place patients at greater risk for postoperative nausea and vomiting (PONV). Routinely, these patients are treated prophylactically with intravenous (IV) ondansetron or transdermal (TD) scopolamine. No study has examined what effect using a combination of these prophylactic treatments would have on the incidence of PONV in a group of high-risk patients. A total of 56 patients at high risk for PONV were treated prophylactically with IV ondansetron and randomized to receive a TD scopolamine patch or placebo. Demographics, incidence, and severity of PONV and side effects and antiemetic requirements were measured. Nausea was measured using a 0 to 10 verbal numeric rating scale. Descriptive and inferential statistics were used for analysis. No difference in demographics or the incidence of side effects was noted between groups. Patients in the scopolamine group had a lower incidence of PONV (P = .043), longer time to first reported nausea (P = .044), longer time to first episode of emesis (P = .031), and decreased supplemental antiemetic requirements (P = .016) compared with the placebo group. Based on this study, we recommend using a combination of TD scopolamine and IV ondansetron to prevent PONV in patients identified as high risk for PONV. [ABSTRACT FROM AUTHOR]
