Contains fulltext : 220851.pdf (Publisher’s version ) (Open Access) OBJECTIVES: Contradictory results have been reported regarding the association between polycystic ovary syndrome (PCOS) and cardiovascular disease (CVD). We assessed the cardiometabolic phenotype and prevalence of CVD in middle-aged women with PCOS, compared with age-matched controls from the general population, and estimated 10-year CVD risk and cardiovascular health score. DESIGN: A cross-sectional study. PARTICIPANTS: 200 women aged >45 with PCOS, and 200 age-matched controls. MEASUREMENTS: Anthropometrics, insulin, lipid levels, prevalence of metabolic syndrome and type II diabetes. Ten-year Framingham risk score and the cardiovascular health score were calculated, and carotid intima-media thickness (cIMT) was measured. RESULTS: Mean age was 50.5 years (SD = 5.5) in women with PCOS and 51.0 years (SD = 5.2) in controls. Increased waist circumference, body mass index and hypertension were more often observed in women with PCOS (P < .001). In women with PCOS, the prevalence of type II diabetes and metabolic syndrome was not significantly increased and lipid levels were not different from controls. cIMT was lower in women with PCOS (P < .001). Calculated cardiovascular health and 10-year CVD risk were similar in women with PCOS and controls. CONCLUSIONS: Middle-aged women with PCOS exhibit only a moderately unfavourable cardiometabolic profile compared to age-matched controls, even though they present with an increased BMI and waist circumference. Furthermore, we found no evidence for increased (10-year) CVD risk or more severe atherosclerosis compared with controls from the general population. Long-term follow-up of women with PCOS is necessary to provide a definitive answer concerning long-term risk for CVD.

In this article, we describe the process of establishing agreed international pregnancy research priorities to address the global issues of unhealthy lifestyles and rising maternal obesity. We focus specifically on the prevention of maternal obesity to improve related clinical pregnancy and long-term complications. A team of multidisciplinary, international experts in preconception and pregnancy health, including consumers, were invited to form the Health in Preconception, Pregnancy and Postpartum (HiPPP) Global Alliance. As an initial activity, a priority setting process was completed to generate pregnancy research priorities in this field. Research, practice and policy gaps were identified and enhanced through expert and consumer consultation, followed by a modified Delphi process and Nominal Group Technique, including an international workshop. Research priorities identified included optimising: (1) healthy diet and nutrition; (2) gestational weight management; (3) screening for and managing pregnancy complications and pre-existing conditions; (4) physical activity; (5) mental health; and (6) postpartum (including intrapartum) care. Given extensive past research in many of these areas, research priorities here recognised the need to advance pregnancy research towards pragmatic implementation research. This work has set the agenda for large-scale, collaborative, multidisciplinary, implementation research to address the major public health and clinical issue of maternal obesity prevention.Copyright © 2020 by the authors. Abstract

This article discusses the use of virtual reality (VR) and artificial intelligence (AI) to measure placental volume (PV) during the first trimester of pregnancy. Currently, PV is measured manually using Virtual Organ Computer-aided AnaLysis (VOCAL) software, which is time-consuming and prone to variability. The study found that both VR and AI measurements of PV were comparable to VOCAL measurements, with AI being closer to VR measurements. AI measurements were automated and therefore more favorable, as they eliminated variability and saved time. Ultimately, these novel methods of measuring PV can contribute to earlier and better screening, diagnostics, and treatment of placenta-related pregnancy complications. [Extracted from the article]

Abstract

This article, titled "Maternal lifestyle risk factors and human embryonic morphologic development: the Generation R Next study," explores the association between lifestyle choices during the periconception period and embryonic development. The study, conducted on a population-based periconception cohort, used 3D ultrasound scans and the Carnegie staging system to evaluate embryonic development. The results showed that inadequate folic acid supplement use was associated with delayed embryonic development, while smoking, alcohol consumption, caffeine use, and physical activity did not show a significant association. Abstract

This article discusses the development of an artificial intelligence (AI) algorithm for the automated detection of congenital brain anomalies in the first trimester of pregnancy using three-dimensional (3D) ultrasound images. The algorithm was trained using a dataset of 404 normally developing embryos and 8 embryos with brain anomalies. The algorithm achieved a 90% accuracy in detecting abnormal ultrasound datasets, with a precision of 44% and a recall of 100%. The next step is to evaluate if the abnormal features identified by the algorithm correspond to actual brain anomalies. The researchers plan to extend the algorithm to include a broader age range and all anatomical structures for automated congenital anomaly screening in the first trimester.
