57. Tuberkulose der Glandula Parotis mit simultanem Warthin Tumor

Abstract

Rationale: Ein TBC-Befall der Gl Parotis ist eine Rarität in Mitteleuropa. Histologisch und molekularbiologisch lässt sich ein Befall exakt darstellen und von einer Neoplasie abgrenzen. Ein Doppelbefund stellt dabei eine besondere diagnostische Herausforderung dar. Fallbericht: Eine 78-jährige Patientin stellte sich mit eine schmerzlosen langsam entstandenen Schwellung der linken Parotis vor. Klinisch und sonografisch konnte ein kugeliger, verschieblicher Tumor von knapp 3 cm Durchmesser dargestellt werden. Zytologisch ergab sich der Hinweis auf ein pleomorphes Adenom und nach Durchführung eines MRTs wurde ein OP-Termin festgelegt. Auffallend war, dass nach der FNPE sich eine Wundheilungsstörung über dem Knoten entwickelte. Anamnestisch war die Patientin vor 70 Jahren wegen einer Halsweichteil-Tbc operiert worden und befand sich jahrelang in Tbc-Überwachung. Der Tumor wurde in ITN über eine laterocaudale Parotidektomie mit Facialismonitoring und Facialisdarstellung mit einer Hautinsel entfernt. Histomorphologisch fand sich ein Zystadenolymphom und eine floride Tuberculose mit verkäsenden epitheloidzelligen Granulomen. Der Nachweis säurefester Stäbchen gelang mittels Zeel-Neelsen Färbung, ebenso die PCR-Analyse des Gewebes mit dem positiven Ergebnis von Genom von Mycobacterium tuberculosis. Nach erfolgter Wundheilung wurde eine 3-monatige tuberculostatische Therapie angeschlossen mit insgesamt unauffälliger Wundheilung. Schlussfolgerung: Es wird der seltene Fall einer Parotis-Raumforderung mit TBC-Befall und Warthin Tumor dargestellt als Bestätigung der klinischen Notwendigkeit Tumoren der Parotis konsequent zu behandeln und entsprechend histomorphologisch und ggf. molekularbiologisch zu analysieren. Es ist zu erwarten, dass im Rahmen der demografischen Entwicklung immer wieder auch ungewöhnliche Reaktivierungen einer TBC gefunden werden. Abstract

The chromophobe renal cell carcinoma is a distinct type of renal cancer presumably derived from the intercalated cell of the collecting duct system and exhibiting a better prognosis than other types of renal cell carcinoma. Chromophobe carcinomas can be separated from other types of renal cell carcinoma by their characteristic cytomorphology, ultrastructural appearance, cytoskeletal architecture, and cytogenetic aberrations. As no permanent cell line of the chromophobe tumor type has previously been described, we are the first to report on the successful establishment and characterization of two divergent permanent cell lines, ie, chrompho-A and chrompho-B, derived from the same chromophobe renal cell carcinoma. With immunocytochemistry, two-dimensional gel electrophoresis, and Western blot, chrompho-A and chrompho-B exclusively exhibited cytokeratins (Nos. 7, 8, 18, and 19) but not vimentin. Ultrastructural studies revealed numerous cytoplasmic microvesicles as well as coated vesicles that are known to be characteristic features of the intercalated cell. Chrompho-B cells exhibited a shorter mean population doubling time (tD = 43 hours) than chrompho-A cells (tD = 51 hours). Both cell lines failed to produce tumors in nude mice with the subrenal capsule assay. Cytogenetic analyses revealed hyperdiploid chromosome numbers in both cell lines with telomeric associations as well as numeric aberrations known from chromophobe renal cell carcinomas in vivo.

Abstract

Four new permanent cell lines (RCC-A, -B,-C, and -D) derived from different human renal cell carcinomas of the clear cell type were established in tissue culture. The cell lines displayed characteristic differences in cell size and shape, which allowed individual identification by phase contrast microscopy. Ultrastructurally, the cell lines exhibited varying amounts of cytoplasmatic glycogen and lipid. Immunohistochemistry revealed co-expression of vimentin and cytokeratin in all cell lines. The mean population doubling time ranged from 27 h (RCC-A) to 104 h (RCC-D). RCC-B and -C cells produced slowly growing tumours after heterotransplantation into nude mice, whereas RCC-A and RCC-D cells were non-tumorigenic. The modal chromosome number was either near-diploid (RCC-A, -B, and -C) or near triploid (RCC-D). Clonal abnormalities affecting the short arm of chromosome 3 were seen in all cell lines. Northern blot analysis revealed no expression of the proto-on-cogenes c-fos, c-ros, and c-mos, whereas c-Ki-ras expression was observed in all cell lines. Expression of c-myc was observed in RCC-A, RCC-B, and RCC-D cells, whereas c-raf expression could be detected in RCC-B and RCC-D. Tumour suppressor gene p53 mRNA was observed in the cell line RCC-D.

Abstract

Rheumatoid arthritis is an autoimmune disease involving stimulation of T cells and the production of autoantibodies. In this disease autoantibodies to collagen type II are believed to play a major role in inflammatory events ultimately resulting in joint destruction. However, collagen type II-containing cartilage has been discussed as one of the primary antigens (as evidenced by animal models), despite not being accessible. Evidence is presented here for the involvement of C1q, the collagen-like subunit of the first component of complement, in the pathogenesis of rheumatoid arthritis. The C1q A-chain contains an epitope exhibiting an identical sequence to part of an arthritis modulating epitope from collagen type II. Furthermore, preapplication of a synthetic peptide, representing the epitope on the C1q A-chain, has been shown to delay the onset and reduce the severity of collagen-induced arthritis in a DBA/1 mouse model. Since the complement system, in particular C1q (as part of the first component of the classical pathway), plays a major role in the inflammatory process, we propose that the collagen-like C1q molecule, altered during the inflammatory process, may result in the generation of autoantibodies which also recognize collagen type II, and may thus be considered as a link between the early inflammatory process in the joint and the only later occurring cartilage destruction.

Abstract

The clear, chromophilic, and chromophobe types of human renal cell carcinoma have been defined as distinct morphological entities and can be clearly separated by differences of ultrastructural appearance, cytoskeletal architecture, enzyme synthesis, and cytogenetic aberrations. In this report, the cytomorphological aspects of these tumor types are compared in vitro, showing that essential ultrastructural and cytoskeletal characteristics of each tumor type are expressed even after prolonged in vitro cultivation. The pattern of intermediate filament proteins of each tumor type was preserved in vitro, permitting the separation of exclusively cytokeratin-positive chromophobe tumor cells from clear and chromophilic tumor cells with a co-expression of vimentin and cytokeratins. In vitro, the chromophobe tumor cells continued to exhibit abundant cytoplasmatic microvesicles and sparsely distributed "studded" vesicles, which are known to be characteristic features of this tumor type in vivo. This observation confirmed the structural similarity of the chromophobe cell to the 'intercalated cell' of the cortical collecting duct and provided further evidence for the histogenetic derivation of this tumor subtype from the collecting duct system.
