Abstract

Dual chamber, rate-modulated pacemakers provide the capability of augmenting the heart rate of patients with chronotropic incompetence but also may cause atrial arrhythmias because of high rate, competitive atrial pacing. We studied ten patients with two consecutive 24-hour Holter monitors during which they were alternately programmed to either DDD or DDDR pacing in random order. Maximum heart rates (max HR) were measured at every 15-minute interval during each 24-hour period. DDDR pacing showed rate augmentation, 80 ± 7 overage max HR when compared with DDD pacing, average max HR 76 ± 5. These results were even more striking when waking hours (7 am to 10 pm) were compared: average max HR 86 ± 7 DDDR versus 78 ± 4 average max HR DDD). Several patients showed marked rate argumentation. Seven of ten patients preferred DDDR pacing over DDD pacing. In the entire population DDDR pacing did not result in an increased number of atrial arrhythmias (125 atrial events/24 hour) when compared to DDE) pacing (1.75 atrial events! 24 hour). We conclude that DDDR pacing provides heart rate augmentation during daily life in a clinical population while not resulting in a significant increase in atrial arrhythmias. [ABSTRACT FROM AUTHOR]

Abstract

ObjectivesThis study sought to determine the long-term outcome of alcohol septal ablation (ASA).BackgroundThere are inadequate data on the long-term outcome of ASA for symptomatic hypertrophic obstructive cardiomyopathy (HOCM).MethodsSix hundred and twenty-nine patients were enrolled consecutively (1996 to 2007) and 98.4% (n = 619) underwent ASA with 92% follow-up in 2007. Evaluation included deaths, procedural complications, pacemaker requirement, repeat ASA, and myectomy/valve surgery. Follow-up parameters included angina (Canadian Cardiovascular Society score), dyspnea (New York Heart Association functional class), exercise time, and echocardiographic indices (septal thickness, ejection fraction, resting and provoked gradients).ResultsEthanol (2.6 ± 1.0 ml) was injected into 1.3 ± 0.5 septal arteries, inducing a septal infarct. Complications included death 1% (n = 6), permanent pacemaker requirement 8.2% (n = 52), coronary dissection 1.3% (n = 8), and worsening mitral regurgitation 0.3% (n = 2). The mean follow-up was 4.6 ± 2.5 years (range: 3 months to 10.2 years). During follow-up, New York Heart Association functional class decreased from 2.8 ± 0.6 to 1.2 ± 0.5 (p < 0.001); Canadian Cardiovascular Society angina score decreased from 2.1 ± 0.9 to 1.0 ± 0 (p < 0.001); and exercise time increased from 4.8 ± 3.3 to 8.2 ± 1.0 (p < 0.001) min. The resting and provoked left ventricular outflow tract gradients decreased progressively (p < 0.001) and remained low during follow-up. The septal thickness decreased from 2.1 ± 0.5 cm to 1.0 ± 0.1 cm (p < 0.001) and the ejection fraction decreased from 68 ± 9% to 62 ± 3% (p < 0.001). The survival estimates at 1, 5, and 8 years were 97%, 92%, and 89%, respectively.ConclusionsThe initial benefits of ASA were maintained during follow-up.

95. SinoOrbital Giant Cell Reparative Granuloma

Abstract

An otherwise healthy, 9-year-old boy had gradual onset of a mass deformity in the region of the left medial canthus with resulting superior and lateral globe displacement. Radiographic evaluation demonstrated an osteolytic, expansile lesion involving the bones of the ethmoid and maxillary sinuses. Combined anterior orbitotomy and nasal endoscopic surgical resection revealed additional involvement of the nasolacrimal sac wall. Histopathology was consistent with a giant cell reparative granuloma. The authors know of 15 case reports of sino-orbital giant cell reparative granulomas with ophthalmic manifestations; only 3 of these appeared in the ophthalmology literature. This case appears unique in that extension of the abnormal tissue into the nasolacrimal sac wall was demonstrated.

96. Ganglioglioma of the Optic Nerve

Abstract

We examined a 15-year-old boy who had a ganglioglioma involving the left optic nerve, which caused proptosis, decreased vision, and optic atrophy. Computed tomography demonstrated an intraconal mass along the course of the orbital portion of the optic nerve. The only evidence of systemic neurofibromatosis was axillary freckling. The patient underwent a combined medial and lateral orbitotomy and excision of the tumor. Histologic examination demonstrated a mass composed of neurons and glial cells characteristic of ganglioglioma. After a one-year follow-up period, we have found no evidence of recurrence.

97. Long-term assessment of unipolar and bipolar stimulation and sensing thresholds using a lead configuration programmable pacemaker

Abstract

Acute and long-term pacing thresholds were measured prospectively in 74 patients with a unipolar/bipolar mul-tiprogrammable pacemaker. At implantation, mean current threshold was 0.48 ± 0.16 mA with unipolar mode and 0.55 ± 0.16 mA bipolar mode (p < 0.01). R wave amplitude at implantation was 7.78 ± 2.4 mV with unipolar and 7.67 ± 2.1 mV in bipolar mode (p = NS). During long-term follow-up (mean 9.3 months; range 3 to 24), no clinically significant differences in pacing or sensing thresholds were observed between bipolar and unipolar configurations.

Abstract

• Leiomyosarcoma rarely occurs in the orbit and is seldom encountered as a postirradiation sarcoma in any anatomic location. Three patients with bilateral retinoblastoma who had received radiation therapy are known to have orbital leiomyosarcoma develop in their third decade of life. The clinical and pathological data pertaining to two of these patients are given and discussed herein.

99. Sympathetic Uveitis After Trauma and Vitrectomy

Abstract

• A case is described in which the earliest clinical manifestations of sympathetic uveitis that occurred after trauma and pars plana vitrectomy were confined primarily to the retina, retinal pigment epithelium, and choroid posteriorly, and resembled focal lesions of acute posterior multifocal placoid pigment epitheliopathy. Similar focal lesions were noted on gross examination of the enucleated inciting eye. Histopathological examination revealed that these lesions were caused by focal choroidal granulomas and overlying Dalen-Fuchs nodules. Sympathetic ophthalmia was also noted in four other patients in whom the condition developed after trauma and vitrectomy.
