Abstract

Background Spetzler-Martin (SM) grade I-II (low-grade) arteriovenous malformations (AVMs) are often considered safe for microsurgery or radiosurgery. The adjunctive use of preoperative embolization to reduce surgical risk in these AVMs remains controversial. Objective To assess the safety of combined treatment of grade l-ll AVMs with preoperative embolization followed by surgical resection or radiosurgery, and determine the long-term functional outcomes. Methods With institutional review board approval, a retrospective analysis was carried out on patients with ruptured and unruptured SM I-II AVMs between 2002 and 2017. Details of the endovascular procedures, including number of arteries supplying the AVM, number of branches embolized, embolic agent(s) used, and complications were studied. Baseline clinical and imaging characteristics were compared. Functional status using the modified Rankin Scale (mRS) before and after endovascular and microsurgical treatments was compared. Results 258 SM I-II AVMs (36% SM I, 64% SM II) were identified in patients with a mean age of 38 ± 17 years. 48% presented with hemorrhage, 21% with seizure, 16% with headache, 10% with no symptoms, and 5% with clinical deficits. 90 patients (68%) in the unruptured group and 74 patients (59%) in the ruptured group underwent presurgical embolization (p = 0.0013). The mean number of arteries supplying the AVM was 1.44 and 1.41 in the unruptured and ruptured groups, respectively (p = 0.75). The mean number of arteries embolized was 2.51 in the unruptured group and 1.82 in the ruptured group (p = 0.003). n-Butyl cyanoacrylate and Onyx were the two most commonly used embolic agents. Four complications were seen in four patients (4/164 patients embolized): two peri-/postprocedural hemorrhage, one dissection, and one infarct. All patients undergoing surgery had a complete cure on postoperative angiography. Patients were followed up for a mean of 55 months. Good long-term outcomes (mRS score ≤ 2) were seen in 92.5% of patients with unruptured AVMs and 88.0% of those with ruptured AVMs. Permanent neurological morbidity occurred in 1.2%. Conclusions Curative treatment of SM I-II AVMs can be performed using endovascular embolization with microsurgical resection or radiosurgery in selected cases, with very low morbidity and high cure rates. Compared with other published series, these outcomes suggest that preoperative embolization is a safe and effective adjunct to definitive surgical treatment. Abstract

Patients with malignant glioma, a tumor affecting the glia cells that form the supporting tissue of the nervous system, have a very poor prognosis. In addition, the available chemotherapeutic agents may produce serious toxicity in the brain and retina. For these reasons, considerable attention has been paid to biological response modifiers, a loosely related class of pharmacological compounds that affect cell growth and act upon the immune system. Among these compounds is beta-interferon, which has been shown to have some antitumor activity in both tissue culture experiments and in patients. Although beta-interferon causes side effects such as fatigue, fever, chills, and myalgia when administered systemically, it is well tolerated when injected directly into the tumor itself. In order to determine patient tolerance to beta-serine-interferon, doses from 5 to 180 million units were administered to 20 patients with recurrent glioma. The interferon was administered through an Ommaya brand reservoir which was implanted with its catheter tip in the center of the tumor cavity after surgical removal of the bulk of glioma. All 20 patients have died, but 12 lived long enough for evaluation. Of those 12, 3 had their disease stabilized for 5 to 18 months. There was no correlation between the interferon dose and survival. Since all patients also received radiotherapy and six had received chemotherapy, few conclusions can be drawn about the efficacy of interferon. It can be stated, however, that even the maximum dose used in this study was well tolerated by the patients. The Ommaya reservoir, which functions well when used to administer drugs directly into the cerebrospinal fluid, had a tendency to clog when implanted into a tumor bed. Abstract

BACKGROUND: Studies have shown racial differences in cancer outcomes. We investigate whether survival differences existed in Hispanic Glioblastoma (GBM) patients compared to other ethnicities from our modern radiotherapy series, as no study to date has focused on outcomes in this group after radiation therapy (RT). METHODS: We retrospectively evaluated 428 patients diagnosed with GBM from 1996–2014 at our institution, divided into four groups based on self-report: white, black, Hispanic, and Asian/Indian. The primary outcome was overall survival (OS). We analyzed differences in prognostic factors among the whole cohort compared to the Hispanic cohort alone. RESULTS: Baseline characteristics of the four racial groups were comparable. With a median follow-up of 387 days, no survival differences were seen by Kaplan-Meier analysis. Median OS for Hispanic patients was 355 days versus 450 days for the entire cohort. Factors significant for patient outcomes in the entire cohort differed slightly to those specific to Hispanic patients. Low Karnofsky Performance Status (KPS) was significant on multivariate analysis in the whole population, but not in Hispanic patients. Extent of resection, Recursive partitioning analysis (RPA) class, and RT total dose were significant on multivariate analysis in both the whole population and Hispanic patients. CONCLUSIONS: We found Hispanic GBM patients had no difference in survival compared to other ethnicities in our cohort. Differences exist in factors associated with outcomes on single and multivariate analysis for Hispanic GBM patients compared to the entire cohort. Additional studies focusing on Hispanic patients will aid in more personalized treatment approaches in this group.

Abstract

Prior studies of post-operative stereotactic radiosurgery (SRS) have not distinguished between Adjuvant SRS (ARS) versus Adjuvant SRS to residual/recurrent disease (ARD). In this study, we defined ARS and ARD and investigated local control (LC), overall survival (OS), distant development of brain metastases (DBF), and leptomeningeal disease (LMD). We retrospectively identified BM patients who received surgical resection and SRS for BM from an IRB approved database between Jan 2009-Aug 2015. Patients were stratified into two groups: ARS and ARD. LC was determined by follow-up MRI studies and OS was measured from the date of surgery. LC and OS were assessed using the Kaplan-Meier method. 70 cavities underwent surgical resection of BM and received SRS to the post-operative bed. 41 cavities were classified as ARS and 29 as ARD. There was no significant difference in 12-month LC between the ARS and ARD group (71.4 vs. 80.8%, respectively; p = 0.135) from the time point of SRS. The overall 1-year survival for ARS and ARD was 79.9 and 86.1%, respectively (p = 0.339). Mean time to progression was 6.45 and 8.0 months and median follow-up was 10 and 15 months for ARS and ARD, respectively. 11.8% of ARS patients and 15.4% of ARD patients developed LMD, p = 0.72. 29.4% of ARS and 48.0% of ARD patients developed DBF, p = 0.145. Our findings suggest that observation after surgical resection, with subsequent treatment with SRS after the development of local failure, may not compromise treatment efficacy.
