Abstract

Cerebral small-vessel disease (CSVD) is a common disease causing slowly progressive disability and earlier death. In CSVD, the destruction of subcortical networks leads to cognitive impairment, mood disorders, gait instability, and motor deficits. All those weaken the ability to be independent of help. Imaging features of CSVD can be visible on brain scans years before the first clinical symptoms. The imaging features are: cerebral white matter lesions (WML), cerebral microbleeds (CMB), perivascular spaces, brain atrophy, and lacunar infarcts. The aim of this thesis was to investigate the features of cerebral small-vessel disease and their association with long-term outcome in ischaemic stroke patients. The six publications were sub-studies of the Helsinki Stroke Aging Memory (SAM) study. The SAM cohort consists of 486 consecutive patients aged 55 to 85 years with ischaemic stroke who were admitted to Helsinki University Central Hospital. At 3 months, comprehensive clinical, neuropsychological, psychiatric, and radiological data were acquired. The cohort had a follow-up 21 years later using extensive national registers. The findings of the six publications can be summarised as follows: In post-stroke patients, the presence of WMLs was an indicator of ischaemic stroke recurrence up to 5 years after a first-ever ischaemic stroke and indicated a high risk of serious traumatic injuries, especially hip fractures requiring hospital treatment. After ischaemic stroke, patients with severe WMLs spent fewer days at home and became permanently institutionalised earlier, especially within the first 5 years. In post-stroke patients, depression and especially depression-executive dysfunction syndrome were associated with a shorter interval to ischaemic stroke recurrence. Post-stroke dementia predicted the recurrence of ischaemic stroke at the long-term follow-up. Post-stroke dementia was a robust predictor of institutionalisation. Imaging and clinical features of CSVD were associated with unfavourable outcome events, like earlier recurrent stroke, traumatic injuries, and earlier permanent institutionalisation. Aivojen pienten suonten tauti on yleinen tauti, joka aiheuttaa hitaasti etenevää haittaa ja ennenaikaista kuolemaa. Pienten suonten taudissa subkortikaalisten verkkojen rappeutuminen johtaa kognitiivisiin häiriöihin, mielialahäiriöihin, tasapainovaikeuksiin ja motoorisiin puutosoireisiin. Kaikki nämä heikentävät omatoimisuutta. Pienten suonten taudin kuvantamislöydökset voivat näkyä aivokuvantamisessa jo vuosia ennen ensimmäisiä kliinisiä oireita. Kuvantamislöydökset ovat seuraavat: aivojen valkean aineen muutokset, aivojen mikrovuodot, perivaskulaaritilat, aivojen atrofia ja lakunaariset infarktit. Tämä väitöskirjatyön tavoitteena oli, tutkia aivoverenkierron sairauksien ominaisuuksia ja niiden yhteyttä pitkäaikaisennusteeseen potilailla, joilla on ollut iskeeminen aivoinfarkti. Väitöskirjan kuusi osatyöt olivat osa laajaa Helsinki Stroke Aging Memory (SAM)-tutkimusta. SAM-kohortti koostuu 486 perättäisestä 55–85-vuotiaasta Helsingin yliopistolliseen sairaalaan potilaista, joilla oli iskeeminen aivohalvaus. Kolmen kuukauden kohdalla tehtiin kertaalleen kattavia kliinisiä, neuropsykologisia, psykiatrisia ja radiologisia tutkimuksia. Kohorttia seurattiin 21 vuotta käyttäen hoitoilmoitusjärjestelmää, perusterveydenhuollon ja erikoissairaanhoidon potilastietojärjestelmiä, väestörekisterikeskuksen tietoja, ja kaupungin laitoshoitopäätöstietoja. Kuuden osatöiden tiivistetyt tulokset olivat: Potilailla, joilla oli aivohalvaus, valkean aineen muutokset ennustivat iskeemisen aivohalvauksen uusiutumisen jopa viiden vuoden ajan ensimmäisen iskeemisen aivohalvauksen jälkeen ja valkean aineen muutoksiin liittyi suurentunut riski saada sairaalahoitoa vaativia vakavia traumaattisia vammoja, erityisesti lonkkamurtumia. Iskeemisen aivohalvauksen jälkeen potilaat, joilla oli runsaasti valkean aineen muutoksia, viettivät vähemmän päiviä kotona ja joutuivat aikaisemmin pysyvään laitoshoitoon, erityisesti viiden ensimmäisen vuoden aikana. Potilailla, joilla oli aivohalvaus, masennus ja erityisesti syndrooma jossa sekä masennus että heikentynyt toiminnanohjaus masennussyndrooma ennustivat aikaisempaa iskeemisen aivohalvauksen uusiutumista. Aivohalvauksen jälkeinen dementia ennustaa iskeemisen aivohalvauksen toistumista pitkäaikaisessa seurannassa. Aivohalvauksen jälkeinen dementia ennusti pysyvää laitoshoitoa. Pienten suonten taudin kliiniset ja radiologiset ilmentymät ennustivat epäsuotuisia lopputulemia, kuten aikaisempaa aivohalvauksen uusiutumista, traumaattisia vammoja ja aikaisempaa pysyvään laitoshoitoon joutumista.

Abstract

Background and Purpose— Selective serotonin-reuptake inhibitors (SSRIs) impair platelet function and have been linked to a higher risk of spontaneous intracerebral hemorrhage—an association that may be augmented by oral anticoagulants (OAC). We aimed to assess whether preadmission treatment with SSRIs in patients with acute ischemic stroke is associated with post-thrombolysis symptomatic intracerebral hemorrhage (sICH) and functional outcome. Methods— A multicenter retrospective analysis was conducted in prospective registries of patients treated by thrombolysis within 4.5 hours of stroke onset. The association between preadmission treatment with SSRIs and sICH (ECASS II definition [European Cooperative Acute Stroke Study]) or unfavorable 3-month outcome (modified Rankin Scale >2) was assessed by logistic regression, taking into account potential interaction with concomitant use of antithrombotics. Results— Six thousand two hundred forty-two patients were included (mean age, 70.1±14.0 years; median National Institutes of Health Stroke Scale, 9 [5–16]). Preadmission treatment with SSRIs was present in 4.3% (n=266) of patients. Overall, SICH rate was 3.9% (95% confidence interval [CI], 3.5%–4.4%; n=244), and SSRI use was not significantly associated with sICH in unadjusted (odds ratio [OR], 1.28; 95% CI, 0.72–2.27) or adjusted (OR, 1.30; 95% CI, 0.71–2.40) analysis. However, there was a significant interaction of concomitant use of OACs (international normalized ratio P =0.01). SICH was significantly more frequent in patients taking both OAC and SSRI (23.1%; 95% CI, 8.2%–50.3%) than in patients taking OAC but not SSRI (adjusted OR, 9.04; 95% CI, 1.95–41.89). Preadmission use of SSRI was associated with unfavorable 3-month outcome (unadjusted OR, 1.90; 95% CI, 1.48–2.46; adjusted OR, 1.59; 95% CI, 1.15–2.19). Conclusions— Preadmission treatment with SSRIs was not significantly associated with an increased risk of post-thrombolysis sICH in this cohort study. However, subgroup analysis suggested an increased risk of sICH in patients taking both SSRI and OAC. Preadmission treatment with SSRIs was associated with unfavorable outcome, which may reflect the prognostic significance of prestroke depression.

Abstract

Objective: To investigate whether poststroke dementia diagnosed after ischemic stroke is associated with earlier permanent institutionalization. Methods: We included 410 consecutive patients aged 55 to 85 years with ischemic stroke admitted to the Helsinki University Central Hospital (The SAM cohort) with a 21-year follow-up. Hospitalization and nursing home admissions were reviewed from national registers. Dementia was diagnosed using the Diagnostic and Statistical Manual of Mental Disorders 3rd edition (DSM-III) criteria. Kaplan-Meier plots log-rank and binary logistic regression (odds ratio, OR) and Cox multivariable proportional hazards model were used to study the association of poststroke dementia status and time of permanent institutionalization. HR and OR with their 95% confidence intervals (CI) are reported. Results: Poststroke dementia was associated with more frequent (OR 2.05; CI 1.29-3.24) permanent institutionalization (Mantel-Haenszel), and earlier permanent institutionalization (5.6 versus 13.6 years; log-rank p Conclusions: Poststroke dementia at 3 month after stroke is associated with earlier permanent institutionalization compared with stroke patient without poststroke dementia.

Abstract

Introduction Alterations in haemoglobin levels are frequent in stroke patients. The prognostic meaning of anaemia and polyglobulia on outcomes in patients treated with intravenous thrombolysis is ambiguous.Patients and methods In this prospective multicentre, intravenous thrombolysis register-based study, we compared haemoglobin levels on hospital admission with three-month poor outcome (modified Rankin Scale 3–6), mortality and symptomatic intracranial haemorrhage (European Cooperative Acute Stroke Study II-criteria (ECASS-II-criteria)). Haemoglobin level was used as continuous and categorical variable distinguishing anaemia (female: <12 g/dl; male: <13 g/dl) and polyglobulia (female: >15.5 g/dl; male: >17 g/dl). Anaemia was subdivided into mild and moderate/severe (female/male: <11 g/dl). Normal haemoglobin level (female: 12.0–15.5 g/dl, male: 13.0–17.0 g/dl) served as reference group. Unadjusted and adjusted odds ratios with 95% confidence intervals were calculated with logistic regression models.Results Among 6866 intravenous thrombolysis-treated stroke patients, 5448 (79.3%) had normal haemoglobin level, 1232 (17.9%) anaemia – of those 903 (13.2%) had mild and 329 (4.8%) moderate/severe anaemia – and 186 (2.7%) polyglobulia. Anaemia was associated with poor outcome (ORadjusted1.25 (1.05–1.48)) and mortality (ORadjusted1.58 (1.27–1.95)). In anaemia subgroups, both mild and moderate/severe anaemia independently predicted poor outcome (ORadjusted1.29 (1.07–1.55) and 1.48 (1.09–2.02)) and mortality (ORadjusted1.45 (1.15–1.84) and ORadjusted2.00 (1.46–2.75)). Each haemoglobin level decrease by 1 g/dl independently increased the risk of poor outcome (ORadjusted1.07 (1.02–1.11)) and mortality (ORadjusted1.08 (1.02–1.15)). Anaemia was not associated with occurrence of symptomatic intracranial haemorrhage. Polyglobulia did not change any outcome.Discussion The more severe the anaemia, the higher the probability of poor outcome and death. Severe anaemia might be a target for interventions in hyperacute stroke.Conclusion Anaemia on admission, but not polyglobulia, is a strong and independent predictor of poor outcome and mortality in intravenous thrombolysis-treated stroke patients.

Abstract

Background and Aims: Previous observational data indicate that young adults treated with intravenous thrombolysis (IVT) for acute ischemic stroke have more favorable outcomes and less complications when compared to older adults. Given the limited data on this topic, we aimed to provide more evidence on clinical outcomes and safety in such patients, using a large international thrombolysis registry., Methods: In this prospective multicenter study, we used data from the Thrombolysis in Ischemic Stroke Patients (TRISP) registry from 1998 to 2020. Patients who received endovascular treatment (EVT), as only treatment or in addition to IVT, were not included in this cohort. Using multivariable regression models, we compared thrombolysed young patients aged 18-49 years with those aged ⩾50 years with regards to the following outcomes: favorable outcome in stroke survivors (modified Rankin Scale ⩽2), symptomatic intracranial hemorrhage (sICH) according to European Cooperative Acute Stroke Study II (ECASS II) criteria, and three-months all-cause death., Results: Of the 16,651 IVT treated patients, 1346 (8.1%) were 18-49 years. Young adults in TRISP were more often male (59.6% vs 54.0%), had a lower median NIHSS score on admission, 7 (4-13) versus 8 (5-15), and had less cardiovascular risk factors except for smoking (42.0% vs 19.0%) when compared to older patients. Abstract

TIA is a brief and transient ictal symptom due to a local disturbance of cerebral or ocular arterial circulation. Transitory blindness or blurred vision in one eye (amaurosis fugax) is a sudden TIA symptom lasting a few minutes, usually underlied by thromboembolism originating from an atherosclerotic plaque of the internal carotid artery, less frequently an embolus from the aorta or the heart, or localized thrombosis of vessels of the optic nerve or the retina. We describe two amaurosis fugax patients.
