51. TRANSMISSÃO DE COVID-19 INTRA-HOSPITALAR

Abstract

Introdução: O ambiente hospitalar é um cenário de risco para transmissão da covid19, com possibilidade de surtos e comprometimento de pacientes mais suscetíveis a evolução para doença grave. Por isso, medidas como check list de sintomas respiratórios, rastreio com testes laboratoriais, precaução respiratória e de contato para casos suspeitos e confirmados são empregadas para evitar esta transmissão. Objetivo: Este estudo buscou avaliar a transmissão da covid19 no ambiente hospitalar onde tais medidas de controle de infecção são utilizadas. Métodos: Entre março e agosto de 2021, foi avaliada uma coorte de indivíduos que foram expostos a paciente confirmado com COVID19 durante sua internação. Após contato, os indivíduos expostos foram submetidos à precaução respiratória e contato, e acompanhados por 14 dias. O rastreamento da transmissão envolveu a avaliação diária dos sintomas e, ou, achados típicos em tomografia de tórax. Aqueles identificados com tais alterações foram submetidos a swab nasofaríngeo para RT-PCR. Resultados: Foram identificados 73 pacientes confirmados para covid19 durante sua hospitalização, previamente sem este diagnóstico, sendo a maioria em enfermarias (56,2%), seguido do pronto socorro (35,6%) e UTI (8,2%). Foram 197 pacientes expostos a estes casos, dos quais 61 (31%) desenvolveram sintomas sugestivos de síndrome gripal nos 14 dias consecutivos à exposição e 18 (9,1%) confirmaram covid19. Entre estes últimos, 15 (12,2%) foram expostos a caso fonte sintomático respiratório enquanto que 3 (4,0%) decorreu de contato com assintomáticos positivos (p = 0,07). Não foi observada transmissão da covid19 em UTI. Pacientes expostos a casos positivos sintomáticos respiratórios no pronto socorro tiveram maior risco de aquisição de covid19 do que aqueles cujo contato ocorreu em ambiente de enfermaria [OR = 3,78 (1,15 - 12,39), p = 0,02]. Conclusão: Embora o hospital adote medidas para evitar a transmissão da covid19 intra hospitalar, tais como check list de sintomas respiratórios e pesquisa por RT-PCR em saliva para todas as admissões, ocorre um escape de casos que podem transmitir a doença para os demais hospitalizados. Esta transmissão foi maior em paciente sintomáticos respiratórios e aqueles acamados no pronto socorro, onde o distanciamento entre leitos é menor.

Abstract

Amine modification through nucleophilic attack of the amine functionality is a very common chemical transformation. Under biorelevant conditions using acidic-to-neutral pH buffer, however, the nucleophilic reaction of alkyl amines (pKa ≈ 10) is not facile due to the generation of ammonium ions lacking nucleophilicity. Here, we disclose a unique molecular transformation system, catalysis driven by amyloid-substrate complex (CASL), that promotes amine modifications in acidic buffer. Ammonium ions attached to molecules with amyloid-binding capability were activated through deprotonation due to the close proximity to the amyloid catalyst formed by Ac-Asn-Phe-Gly-Ala-Ile-Leu-NH2 (NL6), derived from islet amyloid polypeptide (IAPP). Under the CASL conditions, alkyl amines underwent various modifications, i.e., acylation, arylation, cyclization, and alkylation, in acidic buffer. Crystallographic analysis and chemical modification studies of the amyloid catalysts suggested that the carbonyl oxygen of the Phe-Gly amide bond of NL6 plays a key role in activating the substrate amine by forming a hydrogen bond. Using CASL, selective conversion of substrates possessing equivalently reactive amine functionalities was achieved in catalytic reactions using amyloids. Abstract

Background/Aim: The albumin-globulin ratio (AGR) is a useful biomarker for predicting postoperative complications and a poor prognosis in patients with various types of cancer and can be evaluated without invasive testing or surgery. In this study, we aimed to evaluate the usefulness of the AGR in predicting the short- and long-term prognoses of patients with gastric cancer who underwent radical resection at our institution. Patients and Methods: This study is a retrospective cohort analysis in which eligible patients were selected from the medical records of patients who underwent radical resection for gastric cancer at Yokohama City University from 2000 to 2020 and their medical records were reviewed. A total of 240 patients with gastric cancer were classified into high-AGR (>1.57) and low-AGR (=1.57) groups and their overall survival (OS), recurrence-free survival (RFS), and postoperative complication rates were compared. Results: Of the total 240 patients, 87 were classified into the high AGR group and 153 were classified into the low AGR group; the incidence of postoperative complications in the two groups did not differ to a statistically significant extent (34.4% vs. 39.2%, p=0.491). The long-term findings showed that the 5-year OS and RFS rates were significantly better in the high AGR group [84.0% vs. 64.8% (p=0.005), 80.0% vs. 61.9% (p=0.015), respectively]. Conclusion: Preoperative low AGR is a risk factor for OS and DFS in patients with gastric cancer who undergo surgery. Abstract

Background/Aim: Gastric cancer is a common cause of cancer death worldwide, especially in East Asia. This study evaluated the impact of preoperative modified Neutrophil-Platelet Score (mNPS) on the survival and recurrence of patients with resectable gastric cancer. Patients and Methods: The study analyzed 168 patients who underwent curative gastrectomy and subsequently received adjuvant treatment for gastric cancer between 2015 and 2021. Univariate and multivariate analyses were performed to identify the risk factors for overall survival (OS) and recurrence-free survival (RFS). Results: Patients were divided into two groups: 76 patients with an mNPS of 0 were classified into the low-mNPS group, whereas 92 patients with an mNPS of =1 were classified into the high-mNPS group. The 3- and 5-year OS rates in the low-mNPS group were 65.6% and 56.2%, respectively, and those in the high-mNPS group were 45.3% and 36.9%, respectively. The difference in OS between the two groups was statistically significant (p=0.007). The 3- and 5-year RFS rates in the low-mNPS group were 45.6% and 38.7%, respectively, whereas those in the highmNPS group were 33.4% and 28.1%, respectively. The difference in RFS between the two groups was statistically significant (p=0.043). A multivariate analysis showed that the mNPS was a significant independent prognostic factor for OS and RFS. Conclusion: mNPS is a potential prognostic marker for patients with gastric cancer who underwent curative gastrectomy. Abstract

Background/Aim: This study aimed to evaluate the clinical impact of the Naples Prognostic Score (NPS) in patients with gastric cancer and to clarify the potential of the NPS as a nutritional and inflammation evaluation system. Patients and Methods: This study included 158 patients who underwent curative treatment for gastric cancer between 2005 and 2020. The prognosis and clinical pathological parameters of the high-NPS (NPS >2) and low-NPS (NPS=0, 1) groups were analyzed. Results: The overall survival (OS) rates at 3 and 5 years were 86.7% and 77.7%, respectively, in the low-NPS group and 55.4% and 47.4%, respectively, in the high-NPS group. There were significant differences in OS between the two groups. Uni- and multivariate analyses demonstrated that the NPS was an independent prognostic factor for OS (HR=2.495, 95%CI=1.240-5.451). In addition, the 3- and 5-year recurrence-free survival (RFS) rates were 82.1% and 76.0%, respectively, in the NPS-low group, and 43.8% and 36.6% in the NPS-high group. Univariate and multivariate analyses demonstrated that the NPS was an independent prognostic factor for RFS (HR=2.739, 95%CI=1.509-4.972). When the first site of recurrence was compared between the low-NPS group and high-NPS group, there were significant differences in peritoneal recurrence (8.7% vs. 34.3%, p=0.001) and hematologic recurrence (5.6% vs. 21.9%, p=0.004). Conclusion: The NPS was a significant prognostic factor in patients with gastric cancer who received curative treatment. 65. Modular synthetic strategy for N/C‐terminal protected amyloidogenic peptides.

Abstract

N/C‐terminal protected amyloidogenic peptides are valuable biomaterials. Optimization of the protective structures at both termini is, however, synthetically laborious because a linear sequence of solid‐phase peptide synthesis protocol (on‐resin peptide assembly/peptide removal from resin/high‐performance liquid chromatography purification) is required for the peptides each time the protective group is modified. In this study, we demonstrate a modular synthetic strategy for the purpose of rapidly deriving the N/C‐terminal structures of amyloidogenic peptides. The precursor sequences that can be easily synthesized due to a non‐amyloidogenic property were stocked as the synthetic intermediates. Condensation of the intermediates with N/C‐terminal units in a liquid phase followed by high‐performance liquid chromatography purification gave the desired peptides P1–P8. The amyloidogenic peptides that have various N/C‐terminal protective structures were therefore synthesized in a labor‐effective manner. This method is suggested to be useful for synthesizing amyloidogenic peptides possessing divergent protective structures at the N/C‐terminus. [ABSTRACT FROM AUTHOR]

Abstract

要旨 本研究の目的は，40 代と50 代の中年期の男女を対象にして，理想自己と現実自己の差異と困難な目標への対処との関連を量的と質的の両方のアプローチから検討することであった。まず，研究1 で1200 名を対象としたWeb 調査を実施した。その結果，困難な目標への対処は「粘り強さ」「諦めの早さ」「肯定的解釈」の3 因子構造であることが示された。その中の「肯定的解釈」は現実の自己評価と関連しており，「肯定的解釈」が高いほど，理想自己と現実自己との差異が小さくなることが示された。研究２では，調査協力者の内，理想自己と現実自己の差異が大きいあるいは小さい者10 名を対象に半構造化面接を実施した。その結果，差異の小さい者からの語りから，研究１と一致する結果が見出され，「たとえ失敗してもそれをポジティブに捉える」等の語りのように，困難な目標に対して肯定的解釈をすることの重要さが示唆された。, The present study used quantitative and qualitative approaches and examined correlations in discrepancies between the ideal and actual selves and methods of managing challenging goals. Participants were middle-aged men and women in their 40s and 50s. Study 1 conducted an Internet survey of 1200 participants. The results indicated that coping strategies for challenging goals had a three-factor structure; “persistence,” “giving up speed,” and “positive interpretations.” Moreover, positive interpretations were correlated with actual self-assessment. The discrepancies between the ideal and the actual self decreased with increased positive interpretations. Study 2 conducted semi-structured interviews with 10 participants having large and small discrepancies between their ideal and actual selves. Narratives of participants with a small difference showed consistent results with Study 1, suggesting the significance of making positive interpretations of challenging goals, including not dwelling on things that do not go well.

Com o objetivo de avaliar o desempenho de genótipos de milho-pipoca no Planalto Norte de Santa Catarina, foram realizados experimentos na Epagri/Estação Experimental de Canoinhas, nas safras 2003/04 e 2004/05. Foram avaliados 15 genótipos no primeiro experimento e dez no segundo experimento. Observou-se variação entre os genótipos quanto às características de ciclo, porte, quebra de colmos, produtividade e capacidade de expansão. Os híbridos IAC 112 e IAC 125 apresentaram elevado potencial de produção de grãos e boa capacidade de expansão dos grãos.

Abstract

Management of ureteroenteric anastomotic stricture after urinary diversion remains challenging. Although open surgical repair is the gold standard procedure, less invasive endourological intervention is often preferred. In the event of complete obstruction of anastomosis, combined simultaneous antegrade and retrograde endoscopic treatment is required to achieve through-and-through access. Herein we report a case of complete obstruction of ureteroenteric anastomosis following cystectomy with ileal conduit. The cut-to-the-light method was used with a combination of a percutaneous antegrade flexible ureteroscope and a retrograde flexible cystoscope. A holmium:YAG laser incision was made along the full length of the stricture, and through-and-through access was achieved.

Abstract

We developed a microplate assay method for determining the contents of triacylglycerols (TAGs), phosphatidylcholines (PCs), and free fatty acids (FFAs) in the rice bran of one grain using enzymatic reactions. In this method, enzymes from commercially available kits were used. Optimum reaction conditions were established. It was found that Nonidet P-40 was the optimal among the three surfactants used (Triton X-100, Tween 40, and Nonidet P-40) when lipid was dissolved in a reaction solution. Using this method, it was possible to quantify TAGs, PCs, and FFAs in concentration ranges of 7–150, 5–70, and 8–200 mg L−1, respectively. Furthermore, when the TAG contents in the rice bran were measured, the values closely corresponded to those obtained by extracting from large amounts of rice bran. However, sufficient data on the PC and FFA contents in rice bran are not available for valid comparisons. Although this method can accurately quantify the TAG contents in the rice bran of one grain, the accuracy of the PC and FFA contents has not been verified. Hence, future study is necessary.

Abstract Eukaryotic mature mRNAs possess a poly adenylate tail (poly(A)), to which multiple molecules of poly(A)-binding protein C1 (PABPC1) bind. PABPC1 regulates translation and mRNA metabolism by binding to regulatory proteins. To understand functional mechanism of the regulatory proteins, it is necessary to reveal how multiple molecules of PABPC1 exist on poly(A). Here, we characterize the structure of the multiple molecules of PABPC1 on poly(A), by using transmission electron microscopy (TEM), chemical cross-linking, and NMR spectroscopy. The TEM images and chemical cross-linking results indicate that multiple PABPC1 molecules form a wormlike structure in the PABPC1-poly(A) complex, in which the PABPC1 molecules are linearly arrayed. NMR and cross-linking analyses indicate that PABPC1 forms a multimer by binding to the neighbouring PABPC1 molecules via interactions between the RNA recognition motif (RRM) 2 in one molecule and the middle portion of the linker region of another molecule. A PABPC1 mutant lacking the interaction site in the linker, which possesses an impaired ability to form the multimer, reduced the in vitro translation activity, suggesting the importance of PABPC1 multimer formation in the translation process. We therefore propose a model of the PABPC1 multimer that provides clues to comprehensively understand the regulation mechanism of mRNA translation.

Abstract

There is minimal evidence to provide robust conclusions on how periodontal disease may be associated with COVID-19 infection. The objective of this research was to evaluate the gingival inflammatory profile of patients with COVID-19 in the ICU of the Western Paraná State University Hospital (HUOP) in Cascavel/PR, through a pilot clinical study. Data such as age, sex, exams and health history were performed by searching the information system - Tasy®. Patients were divided into two groups: Patients without COVID-19 (control group) and patients with COVID-19 (case group) and were evaluated: Plaque Index (IP), Probing Depth (PS), clinical attachment level (NI) and bleeding on probing (SS) and 3 sites were selected to collect the amount of Gingival Crevicular fluid (GCF). It was observed that in both groups the majority of patients were male, with no differences between them for sex and age. Patients in the COVID-19 group had statistically higher values ​​of IP and FCG and lower values ​​of PS and NI. In addition, patients with COVID-19 had an increase in Hemoglobin and a decrease in Lymphocytes and Platelets compared to the control group. As for Hospital Medication, only the variable Corticosteroids was administered with greater statistical frequency in patients with COVID-19 and each NI unit increases the chance of the patient dying by 10.59 times. It is concluded that there was an increase in the periodontal inflammatory profile of patients with COVID-19, however dexamethasone therapy may have led to an inhibition in the development of periodontitis in these patients.

Abstract

The present study evaluated the clinical impact of other metachronous or synchronous primary cancer (OPC) in patients who received curative treatment for esophageal cancer.The present study included 168 patients who underwent curative treatment for esophageal cancer between 2005 and 2018. Prognosis and differences between the OPC status (metachronous/synchronous) and clinic pathological parameters was analyzed.A total of 168 patients were included in this study. Forty patients were diagnosed with metachronous/synchronous OPC. When comparing the clinicopathological factors between the patients with and without OPC, the patients' background and postoperative clinical courses were very similar between the two groups. The 3- and 5-year overall survival rates in patients with esophageal cancer with OPC were 66.0% and 54.5%, respectively, while those in patients without OPC were 50.1% and 41.4%, respectively. There was no statistically significant difference in these rates (p=0.156). The OPC status was not included in the final multivariate analysis model.The OPC status was not found to be a prognostic factor for patients who received curative treatment for esophageal cancer. Therefore, it is not necessary to avoid performing curative treatment for esophageal cancer because of a patient's OPC status.

Abstract

Abstract Objective: to analyze the epidemiological profile of geriatric patients with facial trauma treated at a Maxillofacial Surgery Department in southern Brazil over a period of 10 years. Methods: a retrospective analysis of the medical records of patients aged over 60 years treated for facial trauma in the period from January 2001 to December 2010 was performed. Result: of a total of 1,385 analyzed medical records of patients with facial trauma, 86 (6.2%) belonged to the group aged 60-89 years. The male gender was the most affected and the age group 60-69 years was the most frequently involved. The middle third was the most affected, and the zygomatic bone was the most commonly fractured. Conclusion: special attention should be given to the 60-69 age group, as while such patients present physiological changes inherent to aging, they remain active in society and exposed to risk factors for facial trauma.
