In the context of the COVID-19 pandemic, this paper provides recommendations for medical eye care during the easing of control measures after lockdown. The guidelines presented are based on a literature review and consensus among all Spanish Ophthalmology Societies regarding protection measures recommended for the ophthalmologic care of patients with or without confirmed COVID-19 in outpatient, inpatient, emergency and surgery settings. We recommend that all measures be adapted to the circumstances and availability of personal protective equipment at each centre and also highlight the need to periodically update recommendations as we may need to readopt more restrictive measures depending on the local epidemiology of the virus. These guidelines are designed to avoid the transmission of SARS-CoV-2 among both patients and healthcare staff as we gradually return to normal medical practice, to prevent postoperative complications and try to reduce possible deficiencies in the diagnosis, treatment and follow-up of the ophthalmic diseases. With this update (5(th)) the Spanish Society of Ophthalmology is placed as one of the major ophthalmology societies providing periodic and systematized recommendations for ophthalmic care during the COVID-19 pandemic.

- Abstract
BACKGROUND: The majority of deaths in the Philippines occur out-of-facility and require a medical certificate of cause of death by Municipal Health Officers (MHOs) for burial. MHOs lack a standardised certification process for out-of-facility deaths and when no medical records are available, certify a high proportion of ill-defined causes of death. We aimed to develop and introduce SmartVA Auto-Analyse, a verbal autopsy (VA) based electronic decision support tool in order to assist the MHOs in certifying out-of-facility deaths. METHOD: We conducted a stakeholder consultation, process mapping and a pre-test to assess feasibility and acceptability of SmartVA Auto-Analyse. MHOs were first asked to conduct an open-ended interview from the family members of the deceased, and if they were not able to arrive at a diagnosis, continue the interview using the standardised SmartVA questionnaire. Auto-Analyse then presented the MHO with the three most likely causes of death. For the pilot, the intervention was scaled-up to 91 municipalities. We performed a mixed-methods evaluation using the cause of death data and group discussions with the MHOs. RESULTS: Of the 5649 deaths registered, Auto-Analyse was used to certify 4586 (81%). For the remaining 19%, doctors believed they could assign a cause of death based on the availability of medical records and the VA open narrative. When used, physicians used the Auto-Analyse diagnosis in 85% of cases to certify the cause of death. Only 13% of the deaths under the intervention had an undetermined cause of death. Group discussions identified two themes: Auto-Analyse standardized the certification of home deaths and assisted the MHOs to improve the quality of death certification. CONCLUSION: Standardized VA combined with physician diagnosis using the SmartVA Auto-Analyse support tool was readily used by MHOs in the Philippines and can improve the quality of death certification of home deaths.

- Abstract
BACKGROUND: Accurate and timely cause of death (COD) data are essential for informed public health policymaking. Medical certification of COD generally provides the majority of COD data in a population and is an essential component of civil registration and vital statistics (CRVS) systems. Accurate completion of the medical certificate of cause of death (MCCOD) should be a relatively straightforward procedure for physicians, but mistakes are common. Here, we present three training strategies implemented in five countries supported by the Bloomberg Philanthropies Data for Health (D4H) Initiative at the University of Melbourne (UoM) and evaluate the impact on the quality of certification. METHODS: The three training strategies evaluated were (1) training of trainers (TOT) in the Philippines, Myanmar, and Sri Lanka; (2) direct training of physicians by the UoM D4H in Papua New Guinea (PNG); and (3) the implementation of an online and basic training strategy in Peru. The evaluation involved an assessment of MCCODs before and after training using an assessment tool developed by the University of Melbourne. RESULTS: The TOT strategy led to reductions in incorrectly completed certificates of between 28% in Sri Lanka and 40% in the Philippines. Following direct training of physicians in PNG, the reduction in incorrectly completed certificates was 30%. In Peru, the reduction in incorrect certificates was 30% after implementation and training on an online system only and 43% after training on both the online system and basic medical certification principles. CONCLUSIONS: The results of this study indicate that a variety of training strategies can produce benefits in the quality of certification, but further improvements are possible. The experiences of D4H suggest several aspects of the strategies that should be further developed to improve outcomes, particularly key stakeholder engagement from early in the intervention and local committees to oversee activities and support a

76. Angiografía-OCT para diagnóstico y respuesta al tratamiento de la neovascularización subretiniana secundaria a telangiectasia macular idiopática tipo 2

- Abstract
La telangiectasia macular idiopática tipo 2 (MacTel 2) es una afección bilateral, lenta y progresiva que afecta a individuos de mediana edad y ancianos. La pérdida de visión es generalmente leve y ocurre en el transcurso de muchos años. El desarrollo de neovascularización subretiniana (NVSR) puede ocurrir tarde en el proceso de la enfermedad y conducir a una pérdida de visión más dramática. Presentamos 2casos de MacTel 2 donde la angiografía por tomografía de coherencia óptica (OCTA) fue fundamental para el diagnóstico de NVSR secundaria. Se utilizó la plataforma comercialmente disponible OCTA Cirrus AngioPlex 5000 (Zeiss, Jena, Alemania). La NVSR fue detectable en ambos casos con OCTA a nivel del plexo capilar profundo y en la capa avascular. La OCTA también nos permitió controlar la progresión de la enfermedad y monitorizar la respuesta a la terapia anti-VEGF.

We report a multi-resolution search for anisotropies in the arrival directions of cosmic rays detected at the Pierre Auger Observatory with local zenith angles up to $80^\circ$ and energies in excess of 4 EeV ($4 \times 10^{18}$ eV). This search is conducted by measuring the angular power spectrum and performing a needlet wavelet analysis in two independent energy ranges. Both analyses are complementary since the angular power spectrum achieves a better performance in identifying large-scale patterns while the needlet wavelet analysis, considering the parameters used in this work, presents a higher efficiency in detecting smaller-scale anisotropies, potentially providing directional information on any observed anisotropies. No deviation from isotropy is observed on any angular scale in the energy range between 4 and 8 EeV. Above 8 EeV, an indication for a dipole moment is captured; while no other deviation from isotropy is observed for moments beyond the dipole one. The corresponding $p$-values obtained after accounting for searches blindly performed at several angular scales, are $1.3 \times 10^{-5}$ in the case of the angular power spectrum, and $2.5 \times 10^{-3}$ in the case of the needlet analysis. While these results are consistent with previous reports making use of the same data set, they provide extensions of the previous works through the thorough scans of the angular scales., Published version. Simultaneous measurements of air showers with the fluorescence and surface detectors of the Pierre Auger Observatory allow a sensitive search for EeV photon point sources. Several Galactic and extragalactic candidate objects are grouped in classes to reduce the statistical penalty of many trials from that of a blind search and are analyzed for a significant excess above the background expectation. The presented search does not find any evidence for photon emission at candidate sources, and combined p-values for every class are reported. Particle and energy flux upper limits are given for selected candidate sources. These limits significantly constrain predictions of EeV proton emission models from non-transient Galactic and nearby extragalactic sources, as illustrated for the particular case of the Galactic center region., La lista completa de autores que integran el documento puede consultarse en el archivo, Instituto de Física La Plata

We present a combined fit of a simple astrophysical model of UHECR sources to both the energy spectrum and mass composition data measured by the Pierre Auger Observatory. The fit has been performed for energies above 5 · 1018 eV, i.e. the region of the all-particle spectrum above the so-called “ankle” feature. The astrophysical model we adopted consists of identical sources uniformly distributed in a comoving volume, where nuclei are accelerated through a rigidity-dependent mechanism. The fit results suggest sources characterized by relatively low maximum injection energies, hard spectra and heavy chemical composition. A search for ultra-high energy photons with energies above 1 EeV is performed using nine years of data collected by the Pierre Auger Observatory in hybrid operation mode. An unprecedented separation power between photon and hadron primaries is achieved by combining measurements of the longitudinal air-shower development with the particle content at ground measured by the fluorescence and surface detectors, respectively. Only three photon candidates at energies 1-2 EeV are found, which is compatible with the expected hadron-induced background. Upper limits on the integral flux of ultra-high energy photons of 0.027, 0.009, 0.008, 0.008 and 0.007 km-2 sr-1 yr-1 are derived at 95% C.L. for energy thresholds of 1, 2, 3, 5 and 10 EeV. These limits bound the fractions of photons in the all-particle integral flux below 0.1%, 0.15%, 0.33%, 0.85% and 2.7%. For the first time the photon fraction at EeV energies is constrained at the sub-percent level. The improved limits are below the flux of diffuse photons predicted by some astrophysical scenarios for cosmogenic photon production. Cosmic rays are atomic nuclei arriving from outer space that reach the highest energies observed in nature. Clues to their origin come from studying the distribution of their arrival directions. Using 3 × 10 4 cosmic rays with energies above 8 × 10 18 electron volts, recorded with the Pierre Auger Observatory from a total exposure of 76,800 km2 sr year, we determined the existence of anisotropy in arrival directions. The anisotropy, detected at more than a 5.2σ level of significance, can be described by a dipole with an amplitude of 6.5 +1.3 -0.9 percent toward right ascension αd = 100 ± 10 degrees and declination δd = -24 +12 -13 degrees. Arcjet thrusters are soon to be employed for north/south station keeping on commercial communications satellites. A series of tests have been performed to evaluate the possible effects of these thrusters on spacecraft charging and the degradation of thermal control material. During our tests the interaction between arcjet plumes and both charged and uncharged surfaces did not cause any significant material degradation. In addition, firing an arcjet thruster benignly reduced the potential of charged surfaces to near zero.

ATP‐binding cassette sub‐family E member 1 (ABCE1) is recognized as a strongly conserved ribosome recycling factor, indispensable for translation in archaea and eukaryotes, however, its role in plants remains largely unidentified. Arabidopsis thaliana encodes two paralogous ABCE proteins (AtABCE1 and AtABCE2), sharing 81% identity. We previously reported that AtABCE2 functions as a suppressor of RNA silencing and that its gene is ubiquitously expressed. Here we describe the structural requirements of AtABCE2 for its suppressor function.Using agroinfiltration assays, we transiently overexpressed mutated versions of AtABCE2 together with GFP, to induce silencing in GFP transgenic Nicotiana benthamiana leaves. The influence of mutations was analysed at both local and systemic levels by in vivo imaging of GFP, Northern blot analysis of GFP siRNAs and observation of plants under UV light.Mutants of AtABCE2 with impaired ATP binding in either active site I or II failed to suppress GFP RNA silencing. Mutations disrupting ATP hydrolysis influenced the suppression of silencing differently at active site I or II. We also found that the N‐terminal iron–sulphur cluster domain of AtABCE2 is crucial for its suppressor function.Meaningfully, the observed structural requirements of AtABCE2 for RNA silencing suppression were found to be similar to those of archaeal ABCE1 needed for ribosome recycling. AtABCE2 might therefore suppress RNA silencing via supporting the competing RNA degradation mechanisms associated with ribosome recycling. [ABSTRACT FROM AUTHOR]

A professional profile is a set of knowledge and skills. In this paper we propose a methodology to define professional profiles giving a quantitative assessment of skills and knowledge associated with an engineering curriculum. This approach can be used to evaluate the curriculum flexibility, i.e., if the curriculum offers the students diverse and flexible approaches and routes to learning. We describe a mathematical model that allows us to calculate the compliance level accomplished by a student with regard to one or more different profiles. Our proposal has been applied in the Undergraduate Program of Electrical Engineering from Universidad Nacional de Colombia. Un perfil profesional es un conjunto de conocimientos y habilidades. En el presente artículo se propone una metodología para definir perfiles profesionales basada en una evaluación cualitativa de habilidades y conocimientos asociados con un plan de estudios de ingeniería. Esta metodología puede ser usada para evaluar la flexibilidad curricular, es decir, si el plan de estudios ofrece a los estudiantes enfoques y rutas de aprendizaje que sean diversos y flexibles. Aqui se describe un modelo matemático que permite calcular el grado de satisfacción logrado por un estudiante con respecto a uno o varios perfiles. Esta propuesta ha sido aplicada en el programa de pregrado de Ingeniería Eléctrica de la Universidad Nacional de Colombia.

93. Successful Sirolimus Treatment for Recurrent Pericardial Effusion in a Large Cervicomediastinal Provisionally Unclassified Vascular Anomaly: A Case Report

Provisionally unclassified vascular anomalies (PUVA) are a group of diseases with unique characteristics that make them unclassifiable within vascular tumors or malformations. We describe a PUVA as the cause of recurrent pericardial effusion and its response to sirolimus. A 6-year-old girl was referred with a cervicothoracic vascular anomaly, a violaceous, and irregular lesion in the neck and upper chest, diagnosed as “hemangioma”. She had pericardial effusion at the neonatal age that required pericardiocentesis, propranolol, and corticosteroids. She remained stable for 5 years, when she presented with a severe pericardial effusion. A magnetic resonance visualized a diffuse vascular image in the cervical and thoracic region with mediastinal extension. The pathological study showed a vascular proliferation in the dermis and hypodermis with positive staining for Wilms' Tumor 1 Protein (WT1) and negative for Glut-1. Genetic testing found a variant in GNA14, for which the diagnosis of PUVA was established. When a pericardial drain was placed without response, treatment with sirolimus was started with resolution of the effusion. Sixteen months later, the malformation is stable and there has been no recurrence of pericardial effusion. In a significant group of patients, definitive diagnosis is not possible despite pathological and genetic analysis. Mammalian target of rapamycin inhibitors may become a therapeutic option if symptoms are severe enough, with a low rate of reported side effects.

Este estudio constituye un análisis preliminar de la calidad del agua presente en tres tramos del Río Pagüey (Nilo-Cundinamarca) empleando como grupo bioindicador los macroinvertebrados acuáticos. La colecta se realizó el 23 de abril de 2013, en la salida del módulo de Profundización I: Recursos Hidrobiológicos del programa de Ingeniería Ambiental, Universidad de Cundinamarca, Seccional Girardot. Se registró en total las clases Gasterópoda con el orden Basommatophora y la familia Lymnaeidae y los Insectos, con ocho órdenes y 17 familias. EL análisis BMWP´Col determinó para todos los tramos una clase de agua II, con calidad aceptable y ligeramente contaminada. No obstante el análisis de Clasificación de Jaccard evidenció una disminución en términos de la comunidad de Macroinvertebrados acuáticos presentes en el tramo 3, Balneario conocido como el charco de la Mula.
