Abstract

Myotonic dystrophy type-1 (Steinert disease) is an autosomal dominant, progressive multisystem disease in which myotonic crisis can be triggered by several factors including pain, emotional stress, hypothermia, shivering, and mechanical or electrical stimulation. In this report, dexmedetomidine-based general anesthesia, in combination with a thoracic epidural for laparoscopic cholecystectomy in a patient with Steinert disease, is presented. An Aintree intubation catheter with the guidance of a fiberoptic bronchoscope was used for intubation to avoid laryngoscopy. Prolonged anesthetic effects of propofol were reversed, and recovery from anesthesia was accelerated using an intravenous infusion of theophylline. Resumo A Distrofia Miotônica (DM) tipo-1 (Doença de Steinert) é uma doença multissistêmica progressiva autossômica dominante em que a crise miotônica pode ser desencadeada por vários fatores, incluindo dor, estresse emocional, hipotermia, tremores e estímulo mecânico ou elétrico. O presente relato descreve anestesia geral realizada com dexmedetomidina em combinação com peridural torácica para colecistectomia laparoscópica em paciente com Doença de Steinert. Para evitar laringoscopia, a intubação traqueal foi realizada utilizando cateter de intubação Aintree guiado por broncofibroscopia óptica. Os efeitos anestésicos prolongados do propofol foram revertidos e a recuperação anestésica foi acelerada pelo uso de infusão intravenosa de teofilina.

69. The Effect of Sugammadex on Time of Sciatic Block by Perineural Bupivacaine in Rats.

Abstract

We aimed to investigate the effect of sugammadex on the motor, sensory and deep sensory block in the sciatic nerve created by bupivacaine in rats. 18 Sprague-Dawley adult male rats treated with unilateral sciatic nerve block by bupivacaine (0.2 ml) were randomly divided into three groups. Control group (Group C, n = 6, 1.5 mL saline) perineural sugammadex group (Group PNS, n = 6, 16 mg/kg) and intraperitoneal sugammadex group (Group IPS, n = 6, 16 mg/kg) Motor, sensory, and deep sensory functions were evaluated every 10 minutes by a blind researcher. 6 tissue samples each belonging to the sciatic nerve, 1.5 cm in length and 0.2 cm in diameter, were taken from paraffin blocks. Sections of 3-4 micrometers were stained with Hematoxylin + Eosin, Masson Trichrome dyes and examined under a light microscope. There was no statistically significant difference between 3 groups in terms of the time to return to normal motor, sensory and deep sensory function. There was also no significant difference in edema, extracellular matrix, and myelin. Inflammatory cells were seen in all groups, mainly epineurium, epineurium, and perineurium. There are findings of no histological effects or effects on local block of sugammadex in rats undergoing sciatic nerve block. [ABSTRACT FROM AUTHOR]

70. Ultrasound-Guided Oblique Subcostal Transversus Abdominis Plane Block For Patients Undergoing Laparoscopic Hysterectomy: A Prospective Randomized Controlled Study.

Abstract

Objective: Laparoscopic hysterectomy increases patient comfort in terms of postoperative pain. However, many patients struggle with postoperative pain after laparoscopic operations. This study aims to evaluate the postoperative analgesic efficacy of oblique subcostal transversus abdominis plane (OSTAP) block in laparoscopic hysterectomy operations. Methods: 58 female patients aged between 18 and 65, with the American Society of Anesthesiologists (ASA) physical status I-II, were recruited in this randomized controlled study. In the OSTAP group, a block was applied with 0.25% bupivacaine. For postoperative analgesia, 1gr paracetamol and 100mg tramadol were administered intravenously (IV) to all patients. No additional procedure was applied to the control group (Group C). The amount of postoperative analgesic consumption, time to first analgesic requirement and visual analog scale (VAS) scores of the patients were evaluated. Results: The mean 24-hour tramadol consumption of the patients was 58.62±48.22 mg in the OSTAP group, while it was 165.52±93.640 mg in Group C (p<0.001). While the time to the first analgesic requirement was 396.43±202.508 minutes in the OSTAP group, it was 119.79±71.361 minutes in Group C (p<0.001). VAS values at 0, 1, 2, 6, 12 and 24th hours and the number of patients in need of tramadol were found to be significantly lower in the OSTAP group (p<0.05). Conclusion: In our study, it was concluded that OSTAP block application is an effective analgesic method in laparoscopic hysterectomy because of the reduction in 24-hour total tramadol consumption and VAS scores, and the prolongation of the time until the first analgesic requirement was prolonged. Abstract

Pregnant women undergo non-obstetric surgeries as well as cesarean operations. Airway management can be complicated due to physiological changes which occur in the respiratory system of labors. The most common causes of pregnancy-specific hypoxic respiratory failure are eclampsia, preeclampsia, and pulmonary edema that develops secondary to tocolytics. Approximately 10–15% of pregnant women undergo emergency cesarean section. Regional anesthesia is a preferred technique worldwide most commonly, and general anesthesia is applied with rapid sequence induction for the rest of the patients. Difficult Airway Society Master Algorithm for Obstetric Patients is a useful method to manage the airway in labors.
