Abstract

BackgroundOne-third of ischemic strokes are “cryptogenic” without clearly identified etiology. Although coronary artery disease (CAD) is the main cause of death after stroke, the interest in CAD screening in patients with cryptogenic stroke is still debated.AimThe aim of the study was to assess the incremental prognostic value of stress cardiovascular magnetic resonance (CMR) beyond traditional risk factors for predicting cardiovascular events in patients with a prior cryptogenic ischemic stroke.Materials and methodsBetween 2008 and 2021, consecutive patients with prior cryptogenic strokes referred for stress CMR were included and followed for the occurrence of major adverse cardiovascular events (MACEs), defined by cardiovascular death or non-fatal myocardial infarction (MI). Univariable and multivariable Cox regressions were performed to determine the prognostic value of unrecognized MI and silent ischemia.ResultsOf 542 patients (55.2% male, mean age 71.4 ± 8.8 years) who completed the follow-up (median 5.9 years), 66 (12.2%) experienced MACE. Silent ischemia and unrecognized MI were detected in 18 and 17% of patients, respectively. Using Kaplan–Meier analysis, silent ischemia and unrecognized MI were associated with the occurrence of MACE [hazard ratio, HR: 8.43 (95% CI: 5.11–13.9); HR: 7.87 (95% CI: 4.80–12.9), respectively, p < 0.001]. In multivariable analysis, silent ischemia and unrecognized MI were independent predictors of MACE [HR: 8.08 (95% CI: 4.21–15.5); HR: 6.65 (95% CI: 3.49–12.7), respectively, p < 0.001]. After adjustment, stress CMR findings showed the best improvement in model discrimination and reclassification above traditional risk factors (C-statistic improvement: 0.13; NRI = 0.428; IDI = 0.048).ConclusionIn patients with prior cryptogenic stroke, stress CMR findings have an incremental prognostic value to predict MACE over traditional risk factors.

Abstract

The impact of sex on pathophysiological processes, clinical presentation, treatment options, as well as outcomes of degenerative aortic stenosis remain poorly understood. Female patients are well represented in transfemoral aortic valve implantation (TAVI) trials and appear to derive favorable outcomes with TAVI. However, higher incidences of major bleeding, vascular complications, and stroke have been reported in women following TAVI. The anatomical characteristics and pathophysiological features of aortic stenosis in women might guide a tailored planning of the percutaneous approach. We highlight whether a sex-based TAVI management strategy might impact on clinical outcomes. This review aimed to evaluate the impact of sex from diagnosis to treatment of degenerative aortic stenosis, discussing the latest evidence on epidemiology, pathophysiology, clinical presentation, therapeutic options, and outcomes. Furthermore, we focused on technical sex-oriented considerations in TAVI including the preprocedural screening, device selection, implantation strategy, and postprocedural management.

Abstract

Aims To assess the safety, feasibility, and prognostic value of stress cardiovascular magnetic resonance (CMR) in patients with pacemaker (PM). Methods and results Between 2008 and 2021, we conducted a bi-centre longitudinal study with all consecutive patients with MR-conditional PM referred for vasodilator stress CMR at 1.5 T in the Institut Cardiovasculaire Paris Sud and Lariboisiere University Hospital. They were followed for the occurrence of major adverse cardiovascular events (MACE) defined as cardiac death or non-fatal myocardial infarction. Cox regression analyses were performed to determine the prognostic value of CMR parameters. The quality of CMR was rated by two observers blinded to clinical details. Of 304 patients who completed the CMR protocol, 273 patients (70% male, mean age 71 ± 9 years) completed the follow-up (median [interquartile range], 7.1 [5.4–7.5] years). Among those, 32 experienced a MACE (11.7%). Stress CMR was well tolerated with no significant change in lead thresholds or pacing parameters. Overall, the image quality was rated good or excellent in 84.9% of segments. Ischaemia and late gadolinium enhancement (LGE) were significantly associated with the occurrence of MACE (hazard ratio, HR: 11.71 [95% CI: 4.60–28.2]; and HR: 5.62 [95% CI: 2.02–16.21], both P < 0.001). After adjustment for traditional risk factors, ischaemia and LGE were independent predictors of MACE (HR: 5.08 [95% CI: 2.58–14.0]; and HR: 2.28 [95% CI: 2.05–3.76]; both P < 0.001). Abstract

The growing literature dedicated to entrepreneurship has recently highlighted the importance of considering the founded firms? exit not as an ending, negative stage in an entrepreneur?s work-life experience. Instead, the decision to leave one?s venture is a strategic choice undertaken for the firm?s sake or as a founding entrepreneur?s lifestyle choice and leading to various after-exit paths. The thesis contributes to the stream of literature dedicated to the entrepreneurial exit and habitual entrepreneurship fields by (i) exploring the antecedents of the cross-road choice that founding entrepreneurs take right-after an acquisition occurs using a prospect theory approach and by (ii) investigating the characteristics that differentiate habitual founders from lifestyle entrepreneurs, borrowing from human capital theory and building on recent work in the field. From a methodological viewpoint, the thesis? main contributions include (i) the use of a large dataset of 14,881 founders, created on the bases of the Crunchbase database (data.crunchbase.com), (ii) the focus on the very-after acquisition step taken by the founding entrepreneur and investigated in North America, Europe, and Asia, and (iii) the introduction of a relatively new unit of analysis -- habitual founders -- when discussing the characteristics of habitual entrepreneurship. After introducing the thesis? main concepts in Chapter 1, the various contributions are developed in the three following chapters. Chapter 2 offers a descriptive study of the current literature in the entrepreneurial exit field, with particular attention given to the individual voluntary choice of exiting the founded firm. Chapter 3 extends the previous study by focusing on the antecedents of the founder?s intention to leave or remain within the firm right after encountering its acquisition from external actors and investigating whether results hold across world regions. Chapter 4 centers the study on the differences leading entrepreneurs toward a lifestyle versus a multiple-ventures creation entrepreneurial approach. The Appendix offers a step-by-step explanation of the procedures undertaken to create the database; it is thought to be used by other researchers as a guideline toward the creation of a similar database.

Publisher: FedOA Press - Federico II Open Access University Press Series: Miscellaneous Language: Multilanguage Abstract: The Corpus of ancient Occitan is a textual corpus that can be interrogated on the Gatto software, developed by OVI (Opera del Vocabolario Italiano, C.N.R., Florence), and can be read on the Rialto (Computerized repertory of ancient troubadour and Occitan literature). The corpus includes complete editions of chansonniers, lyric texts of dispersed tradition and prose texts, all in both diplomatic and interpretive transcription. With regard to the chansonniers, which represent the largest part of the corpus, there are 2500 lyrical compositions preserved from around 30 manuscripts and about 12000 different attestations. Also included are various practical and religious texts in prose, and some 20 lyrical testimonies transmitted adventitiously. The diplomatic editions represent a faithful transcription that preserves the graphic chain and abbreviations of the manuscript, while avoiding reproducing in text the anomalies of the copy, which are commented on in the notes. In the interpretive editions, abbreviations are dropped and words are divided according to modern usage. The manuscript’s lesson is never changed, but only places of controversial interpretation are indicated in italics and commented on in the notes. In Rialto, the diplomatic editions are in html format, while the interpretive ones are downloadable as autonomous pdf files. Abstract

Aims  The accuracy and prognostic value of stress perfusion cardiac magnetic resonance (CMR) are established in coronary artery disease (CAD) patients. Because myocardial contrast kinetics may be altered after coronary artery bypass graft (CABG), most studies excluded CABG patients. This study aimed to assess the prognostic value of vasodilator stress perfusion CMR in CABG patients. Methods and results  Consecutive CABG patients referred for stress CMR were retrospectively included and followed for the occurrence of major adverse cardiovascular events (MACE) including cardiovascular (CV) death or non-fatal myocardial infarction (MI). Cox regression analyses were performed to determine the prognostic association of inducible ischaemia and late gadolinium enhancement (LGE) by CMR. Of 866 consecutive CABG patients, 852 underwent the stress CMR protocol and 771 (89%) completed the follow-up [median (interquartile range) 4.2 (3.3–6.2) years]. There were 85 MACE (63 CV deaths and 22 non-fatal MI). Using Kaplan–Meier analysis, the presence of inducible ischaemia identified the occurrence of MACE [hazard ratio (HR) 3.52, 95% confidence interval (CI): 2.27–5.48; P  < 0.001] and CV death (HR 2.55, 95% CI: 1.52–4.25; P  < 0.001). In a multivariable stepwise Cox regression including clinical characteristics and CMR indexes, the presence of inducible ischaemia was an independent predictor of a higher incidence of MACE (HR 3.22, 95% CI: 2.06–5.02; P  < 0.001) and CV death (HR 2.15, 95% CI: 1.28–3.62; P  = 0.003), and the same was observed for LGE (both P  = 0.02). Conclusion  Stress CMR has a good discriminative prognostic value in patients after CABG, with a higher incidence of MACE and CV death in patients with inducible ischaemia and/or LGE.
