Background This multicenter, double-blind phase II study assessed the antitumor activity and toxicity profile of docetaxel with the antiangiogenic multikinase inhibitor sorafenib or matching placebo as a first-line treatment in patients with metastatic or locally advanced HER2-negative breast cancer. Patients and methods Patients were randomized 1:1 to receive docetaxel 100 mg/m2 on day 1 every 3 weeks in combination with sorafenib 400 mg bid or placebo on days 2–18 of each cycle until tumor progression, or unacceptable toxicity. Sorafenib/placebo could be continued at the investigator's discretion if docetaxel was stopped due to toxicity. Primary endpoint was progression free survival (PFS). Results From October 2008 to December 2013, 102 patients were randomized; 98 patients were evaluable. The trial was prematurely terminated due to slow accrual. Due to increased toxicity the dose of docetaxel was reduced to 75 mg/m2 and an increasing sorafenib dosing schedule was implemented as part of a protocol amendment. The addition of sorafenib to docetaxel did not improve PFS (8.2 vs. 7.3 months for docetaxel/placebo; HR 0.84, log rank p = 0.43), but led to higher rates of early treatment discontinuation. There were no statistically significant differences between sorafenib dosing schedules. Conclusions Addition of sorafenib to taxane-based first-line chemotherapy in patients with metastatic breast cancer failed to improve PFS and resulted in increased toxicity.

Seongnam Atopy Preventive Project for Children's Happiness; The Korean Academy of Pediatric Allergy and Respiratory Disease

A physically active 60-year-old male suffered severe diffuse axonal injury due to a motor vehicle accident. His initial Glasgow Coma Scale score was 3. His magnetic resonance imaging (MRI) showed midbrain, corpus callosum, and pontine involvement with intracerebral hemorrhage and bilateral subdural hygromas. He scored 3 on the extended Glasgow Outcome Scale (GOSE) (lower severe disability) and 18 on the disability rating scale. His 14-day mortality risk was estimated to be 83.6% and risk of unfavorable outcome at 6 months 96.0%. Although he did not have a progressive illness or disease, based on the incurability of his condition, his very low level of functioning, and his previously stated wishes not to prolong his life should it be at the point where it now was, a bioethics committee meeting was held. He was then discharged on home hospice and expired peacefully within 24 hours of arriving home.

Background:Bivalirudin has historically been considered an attractive anticoagulant during percutaneous coronary intervention (PCI) because of reduced bleeding complications reported by early trials. Bivalirudin use during PCIs has been a subject of controversy because of conflicting data and recent findings. Objective: To evaluate the clinical characteristics of patients receiving bivalirudin to determine if an opportunity to improve use exists based on risk of procedure-related bleeding. Methods: This was a single-center, retrospective, observational study (n = 100) of all adult patients who received bivalirudin during cardiac catheterization at St John Hospital and Medical Center from June to August 2013. The risk of bleeding complications associated with PCI was estimated using a clinical risk algorithm developed from the National Cardiovascular Data Registry (NCDR). Results: Treatment with bivalirudin was safe and effective. Of the 100 patients who received bivalirudin, only 34% were identified as having a high risk of procedure-related bleeding according to the NCDR clinical risk algorithm. There was no incidence of stent thrombosis noted and only 1 case of provisional glycoprotein IIb/IIIa inhibitor use. No episodes of Thrombolysis in Myocardial Infarction (TIMI) major bleeding were noted in the study population; however, 1 patient met TIMI minor bleeding criteria. Limitations of this study include small sample size and retrospective nature of the study. Conclusion: Opportunities to establish a more cost-effective use of bivalirudin may exist through implementation of protocols incorporating the NCDR risk assessment model.

Theanalysis shows that the fuel conversion will not lead tosignificant changes in the safety analysis and that thecalculated maximum clad temperatures, minimum criticalheat flux ratios, and minimum onset of flow instabilityratios assure that there is adequate margin to fuel failure.

Bacterial gene islands add to the genetic repertoire of opportunistic pathogens. Here, we perform comparative analyses of three Pseudomonas aeruginosa strains isolated sequentially over a 3-week period from a patient with ventilator-associated pneumonia (VAP) who received clindamycin and piperacillin-tazobactam as part of their treatment regime. While all three strains appeared to be clonal by standard pulsed-field gel electrophoresis, whole-genome sequencing revealed subtle alterations in the chromosomal organization of the last two strains; specifically, an inversion event within a novel 124-kb gene island (PAGI 12) composed of 137 open reading frames [ORFs]. Predicted ORFs in the island included metabolism and virulence genes. Overexpression of a gene island-borne putative β-lactamase gene was observed following piperacillin-tazobactam exposure and only in those strains that had undergone the inversion event, indicating altered gene regulation following genomic remodeling. Examination of a separate cohort of 76 patients with VAP for integration at this tRNA lys recombination site demonstrated that patients exhibiting evidence of integration at this site had significantly higher 28-day mortality. These findings provide evidence that P. aeruginosa can integrate, rapidly remodel, and express exogenous genes, which likely contributes to its fitness in a clinical setting.

This study investigated the gastroprotective effects of diallyl disulfide (DADS), a secondary organosulfur compound derived from garlic ( Allium sativum L.) on experimental model of ethanol (EtOH)-induced gastric ulcer in rats. The antiulcerogenic activity of DADS was evaluated by gross/histopathological inspection, pro-inflammatory cytokines, and lipid peroxidation with antioxidant enzyme activities in the stomach. DADS (100 mg/kg) was administered by oral gavage 2 h prior to EtOH treatment (5 ml/kg). The animals were killed 1 h after receiving EtOH treatment. Pretreatment with DADS attenuated EtOH-induced gastric mucosal injury, as evidenced by decreased severity of hemorrhagic lesions and gastric ulcer index upon visual inspection. DADS also prevented histopathological alterations and gastric apoptotic changes caused by EtOH. An increase in tumor necrosis factor-α (TNF-α) and inducible nitric oxide synthase was observed in the gastric tissues of EtOH-treated rats that coincided with increased serum TNF-α and interleukin 6 levels. In contrast, DADS effectively suppressed production of pro-inflammatory mediators induced by EtOH. Furthermore, DADS prevented the formation of gastric malondialdehyde and the depletion of reduced glutathione content and restored antioxidant enzyme activities, such as catalase, glutathione peroxidase, and glutathione reductase in the gastric tissues of EtOH-treated rats. These results indicate that DADS prevents gastric mucosal damage induced by acute EtOH administration in rats and that the protective effects of DADS may be due to its potent antioxidant and anti-inflammatory activities.

†‡ § SUMMARY Interspecific reproductive barriers are poorly understood, but are central to the biological species concept. The pre-zygotic barriers between red- and green-fruited species in the tomato clade of the genus Solanum provide a model to better understand these barriers in plants. Compatibility usually follows the SI x SC rule: pollen from self-compatible (SC) red-fruited species is rejected on pistils of the predominantly self-incompatible (SI) green-fruited species, but the reciprocal crosses are compatible. This suggests that the interspecific reproductive barrier may be linked to the intraspecific SI mechanism. However, pollen from the SC red-fruited species is also rejected by SC accessions of green-fruited species that lack S-RNase, a key protein expressed in pistils of SI Solanum species. Thus, multiple mechanisms may contribute to the barrier between red- and green-fruited species. We tested whether an S-RNase-dependent barrier is sufficient for rejection of pollen from red-fruited species by introducing functional S-RNase, HT-A and HT-B genes from SI species into Solanum lycopersicum (cultivated tomato). We found that expressing S-RNase in combination with either HT-A or HT-B in the pistil is sufficient to cause rejection of pollen from all four red-fruited species. Thus, redundant mechanisms must operate side by side to prevent crosses between red- and greenfruited species in the clade, underlining the complexity of interspecific pollination barriers. Our results also have implications for mating system transitions. We suggest that these transitions must occur in a specific sequence, and that the transition from SI to SC also affects interspecific compatibility.

Summary There are numerous nasometric studies to date, including normative nasalance scores for various languages as well as nasometric differences in age, gender, race and region except the Korean language. In this regard, we sought to establish normative nasalance scores for Koreans. We created speech samples based on the everyday use of phonemes in the Korean language which were syntactically simple for children. In addition, we analysed nasometric features based on age and gender and confirmed test–retest reliability. The study included 108 children (54 girls and 54 boys, aged 7–11 years) and 108 adults (54 women and 54 men, aged 18–29 years) with normal articulation, resonance, voice and hearing. Nasometer II 6400 was used to measure the nasalance scores. The subjects read or repeated three speech stimuli, each consisting of 33, 36 and 24 syllables: (1) an oral passage devoid of nasal consonants, (2) an oro-nasal passage and (3) nasal sentences. For each stimulus, mean nasalance scores were obtained and gender or age dependence was analysed, using two-way analyses of variance. The mean nasalance scores for the oral passage, oro-nasal passage and nasal sentences were 11.69% (standard deviation (SD) 3.68), 34.04% (SD 4.88) and 63.72% (SD 6.07), respectively. Female speakers exhibited significantly higher nasalance scores than male speakers on the oro-nasal passage ( p = 0.000) and nasal sentences ( p = 0.004). Children exhibited significantly higher nasalance scores than adults on nasal sentences ( p = 0.000). The nasalance scores in children and females were a little higher. Korean normative data will provide reference information in the evaluation and treatment of resonance problems.

Reactivity insertion accidents have been analyzed for the 20-MW D2O-moderated research reactor (NBSR) at the National Institute of Standards and Technology (NIST). The analysis has been carried out for the present core, which contains highly enriched uranium fuel, and for a proposed equilibrium core with low-enriched uranium fuel. The time-dependent analysis of the primary system is performed with a RELAP5 model that includes the reactor vessel, primary coolant pump, heat exchanger, fuel element geometry, and flow channels for both the 6 inner and 24 outer fuel elements. Postprocessing of the simulation results has been conducted to evaluate minimum critical heat flux (CHF) ratio and minimum onset of flow instability (OFI) ratio using the Sudo-Kaminaga correlations and Saha-Zuber criteria, respectively.Evaluations are carried out for the control rod withdrawal start-up accident and the maximum reactivity insertion accident. This paper models transmission lines (TLs) fabricated over a printed circuit board substrate made of woven fiber fabric that causes resonances in the insertion and return loss. The modeling relies on the fact that these lines can be analyzed as periodically loaded TLs. Here, a method to determine the associated equivalent propagation constant and characteristic impedance is proposed. This allows to accurately reproduce the S-parameters for a TL of arbitrary length and for any periodicity of the fiber weave pattern along the line requiring neither multi-staged equivalent circuits, nor full-wave simulations.

A 5-year-old boy presented with mild autism and attention-deficit hyperactivity disorder (ADHD). Chromosomal microarray demonstrated a 1.7 Mb deletion at Xp22.31, which was consistent with X-linked ichthyosis (XLI). Further exam revealed dry, scaly skin on his abdomen and pretibial areas. Patients with mutations involving solely the STS gene or the recurrent ~2 Mb deletion may present with ADHD, whereas those with larger deletions including the NLGN4 gene can present with both ADHD and autism. However, our patient presented with mild autism in addition to ADHD despite having only the recurrent deletion without loss of NLGN4. Such neurological manifestations of XLI warrant attention as practical targets of clinical management.

Epichlorohydrin (ECH) is an antifertility agent that acts both as an epididymal toxicant and an agent capable of directly affecting sperm motility. This study identified the time course of apoptotic cell death in rat epididymides after ECH treatment. Rats were administrated with a single oral dose of ECH (50 mg/kg). ECH-induced apoptotic changes were evaluated by terminal deoxynucleotidyl transferase dUTP nick end labeling (TUNEL) assay and its related mechanism was confirmed by Western blot analysis and colorimetric assay. The TUNEL assay showed that the number of apoptotic cells increased at 8 h, reached a maximum level at 12 h, and then decreased progressively. The Western blot analysis demonstrated no significant changes in proapoptotic Bcl-2-associated X (Bax) and anti-apoptotic Bcl-2 expression during the time course of the study. However, phospho-p38 mitogen-activated protein kinase (p-p38 MAPK) and phospho-c-Jun amino-terminal kinase (p-JNK) expression increased at 8–24 h. Caspase-3 and caspase-8 activities also increased at 8–48 h and 12–48 h, respectively, in the same manner as p-p38 MAPK and p-JNK expression. These results indicate that ECH induced apoptotic changes in rat epididymides and that the apoptotic cell death may be related more to the MAPK pathway than to the mitochondrial pathway.

Background Oxidative stress, mediated by an imbalance between oxidants and antioxidants, contributes significantly to the pathogenesis of asthma. Objective To evaluate the impact of serum total antioxidant capacity (TAC) on the pulmonary function of Korean asthma patients. Method A total of 104 adult asthma patients enrolled from the COREA (Cohort for Reality and Evolution of Adult Asthma in Korea) programme participated in the study. Baseline clinical parameters at enrolment, and the results of pulmonary function tests at baseline and 1 and 2 years after enrolment were collected. TAC at baseline was measured using a Trolox-equivalent antioxidant capacity assay. Patients were divided into two groups based on TAC levels, and various clinical parameters were compared. Result Serum TAC levels correlated with forced expiratory volume in 1 second (FEV(1)) at baseline (r = 0.22, P = 0.03). The group with higher baseline TAC levels maintained greater mean FEV(1) both 1 and 2 years after enrolment, even after adjusting for sex, age, height, weight, body mass index and smoking status. Conclusion These results suggest an important link between serum TAC levels and pulmonary function, indicating that higher TAC levels may be a biomarker for favourable prognosis in asthma patients.

This article is concerned with a general class of conditionally heteroscedastic time series including possibly nonlinear and asymmetric autoregressive conditional heteroscedastic (ARCH) and generalized ARCH models. A problem of preliminary test of fit (PTF, hereafter) within the broad class under consideration is discussed. It is noted that contrary to usual tests in the literature of conditionally heteroscedastic time series, PTF does not require any specification of the conditional variance in advance. Based on the joint limit distributions of sample autocorrelations, a certain Portmanteau-type statistic for PTF is proposed, and its limit is shown to be a chi-square distribution. In addition, some simulation studies, under various innovations, are reported to support our theoretical results.

Background A study was completed to assess the academic and state-level professional optometry leadership views regarding optometry professionals as surge responders in the event of a catastrophic event. Methods A cross-sectional survey was conducted using a 21-question, self-administered, structured questionnaire. All U.S. optometry school deans and state optometric association presidents were mailed a questionnaire and instructions to return it by mail on completion; 2 repeated mailings were made. Descriptive statistics were produced and differences between deans and association presidents were tested by Fisher exact test. Results The questionnaire response rate was 50% (25 returned/50 sent) for the state association presidents and 65% (11/17) for the deans. There were no statistically significant differences between the leadership groups for any survey questions. All agreed that optometrists have the skills, are ethically obligated to help, and that optometrists should receive additional training for participation in disaster response. There was general agreement that optometrists should provide first-aid, obtain medical histories, triage, maintain infection control, manage a point of distribution, prescribe medications, and counsel the “worried well.” Starting intravenous lines, interpreting radiographs, and suturing were less favorably supported. There was some response variability between the 2 leadership groups regarding potential sources for training. Conclusions The overall opinion of optometry professional leadership is that with additional training, optometrists can and should provide an important reserve pool of catastrophic event responders.

Extending the bifurcating autoregressive (BAR) process (cf. Cowan and Staudte, 1986) to multi-casting (multi-splitting) data, Hwang and Choi (2009) introduced multi-casting autoregression (MCAR, for short) defined on multi-casting tree structured data. This article is concerned with the case when the MCAR model is partially specified only through conditional mean and variance without directly imposing autoregressive (AR) structure. The resulting class of models will be referred to as P-MCAR (partially specified MCAR). The P-MCAR considerably enlarges the class of multi-casting models including (as special cases) MCAR, random coefficient MCAR, conditionally heteroscedastic multi-casting models and binomial-thinning processes. Moment structures for this broad P-MCAR class are investigated. Least squares (LS) estimation method is discussed and asymptotic relative efficiency (ARE) of the generalized-LS over ordinary-LS is obtained in a closed form. A simulation study is conducted to illustrate results.

The Mini Mental Status Exam (MMSE) instrument has been commonly used in the Radiation Therapy Oncology Group (RTOG) to assess mental status in brain cancer patients. Evaluating patient factors in relation to patterns of incomplete MMSE assessments can provide insight into predictors of missingness and optimal MMSE collection schedules in brain cancer clinical trials. This study examined eight RTOG brain cancer trials with ten treatment arms and 1,957 eligible patients. Patient data compliance patterns were categorized as: (1) evaluated at all time points (Complete), (2) not evaluated from a given time point or any subsequent time points but evaluated at all the previous time points (Monotone drop-out), (3) not evaluated at any time point (All missing), and (4) all other patterns (Mixed). Patient characteristics and reasons for missingness were summarized and compared among the missing pattern groups. Baseline MMSE scores and change scores after radiation therapy (RT) were compared between these groups, adjusting for differences in other characteristics. There were significant differences in frequency of missing patterns by age, treatment type, education, and Zubrod performance status (ZPS; P < 0.001). Ninety-two percent of patients were evaluated at least once: seven percent of patients were complete pattern, 49% were Monotone pattern, and 36% were mixed pattern. Patients who received RT only regimens were evaluated at a higher rate than patients who received RT + other treatments (49-64% vs. 27-45%). Institutional error and request to not be contacted were the most frequent known reasons for missing data, but most often, reasons for missing MMSE was unspecified. Pseudomonas aeruginosa is an opportunistic pathogen that can, like other bacterial species, exist in antimicrobial resistant sessile biofilms and as free-swimming, planktonic cells. Specific virulence factors are typically associated with each lifestyle and several two component response regulators have been shown to reciprocally regulate transition between biofilm-associated chronic, and free-swimming acute infections. Quorum sensing (QS) signal molecules belonging to the las and rhl systems are known to regulate virulence gene expression by P. aeruginosa. However the impact of a recently described family of novel quorum sensing signals produced by the Pseudomonas Quinolone Signal (PQS) biosynthetic pathway, on the transition between these modes of infection is less clear. Using clonal isolates from a patient developing ventilator-associated pneumonia, we demonstrated that clinical observations were mirrored by an in vitro temporal shift in isolate phenotype from a non-secreting, to a Type III cytotoxin secreting (TTSS) phenotype and further, that this phenotypic change was PQS-dependent. While intracellular type III cytotoxin levels were unaffected by PQS concentration, cytotoxin secretion was dependent on this signal molecule. Elevated PQS concentrations were associated with inhibition of cytotoxin secretion coincident with expression of virulence factors such as elastase and pyoverdin. In contrast, low concentrations or the inability to biosynthesize PQS resulted in a reversal of this phenotype. These data suggest that expression of specific P. aeruginosa virulence factors appears to be reciprocally regulated and that an additional level of PQS-dependent post-translational control, specifically governing type III cytotoxin secretion, exists in this species.

Hyperbolic metamaterial (HMM) can enhance the radiative recombination rate of a near-field coupled emitter due to its large photonic density of states (PDOS). Thus far, this enhancement has only been demonstrated qualitatively on account of the difficulties to achieve an equal emitter–HMM distance as well as the same polarization for all emitters. Here, we report on the deterministic coupling of epitaxially grown quantum dots (QDs) with HMM, for which a quantitative investigation of the rate enhancement is possible. Advantages of epitaxial QDs over other emitters to investigate HMM coupling include a precise QD–HMM distance, the same polarization direction, and single-exponential decay of photoluminescence. In order to isolate metal-related effects, we have fixed the thickness of the silver (Ag) layer and have varied only that of the germanium (Ge) layer in the HMM to investigate the effect of the PDOS, which depends on the dispersion relation, which in turn depends on the thickness ratio. The recombination rate enhancement, as measured by lifetime reduction, varied from 2.2 to 4.2, depending on the thickness ratio of Ag and Ge in the HMM. These findings match well with simulation results, clearly supporting the role of HMM in the rate enhancement. The coupling of high-quality epitaxial QDs with HMM, demonstrated here for the first time, to the best of our knowledge, may bring about diverse future applications.

The cryogenic system for the KSTAR superconducting (SC) magnets has been commissioned. It consists of a cold box, distribution boxes (DB) and cryogenic transfer lines. The cold box and DB #1 provide 600 g/s of supercritical helium to cool the SC magnets, their SC bus-lines, and the magnet support structures. It also provides 17.5 g/s of liquid helium to the current leads and supplies cold helium flow to the thermal shields. The main duties of the DB #2 are the relative distribution of the cryogenic helium among the cooling channels of each KSTAR cold component and the emergency release of over-pressurized helium during abnormal events such as quenches of the SC magnets. After individual commissioning, the system was integrated and cooled down with the KSTAR device. In this paper, the construction and commissioning results of the KSTAR cryogenic system will be introduced. In addition, we will present the cool-down results of the KSTAR device.

The KSTAR components requiring cryogenic helium coolant for superconducting magnet operations are connected to the helium refrigeration system (HRS) through the helium distribution system (HDS), the final helium distribution station. Twenty eight cryogenic valves including 4 quench protecting valves, many sensors such as temperature sensors, pressure transmitters, and flowmeters are mounted on the system. The HDS has to control the 4.5 K supercritical helium (600 g/s) for 30 superconducting coils and their superconducting bus-lines, 55 K helium (270 g/s) pressurized to 18 bars for the thermal shields and the gravity supports, and a maximum 17.5 g/s of liquid helium for 18 current leads, provided from the HRS which has a 4.5 K equivalent cooling power of 9 kW. The helium distribution system consists of a helium distribution box (DB), 5 helium transfer lines, a PLC based helium control system (HCS), etc. The helium control system is connected to the KSTAR supervisory control & interlock system. The detailed status regarding the construction, commissioning during first cool-down, and the instrumentation & control (I&C) system are included in this paper.

This study was conducted to investigate the effects of replacing whole crop corn silage (WCCS) with whole crop rice silage (WCRS) in the total mixed ration (TMR) on nutrient intake, milk yield and its composition in Holstein cows. The Chucheong rice variety (Oryza sativa L. Japonica) and corn (Pioneer 32 P75) were harvested at yellow-ripe stage and were ensiled in round bales and in trench silos, respectively. Two TMR containing either WCCS or WCRS were prepared. These diets were randomly assigned to 16 midlactating Holstein cows (8 cows per treatment) and were fed for 120 days. The first 20 days were used for dietary adaptation and for the next 100 days daily feed intake, milk yield and its composition were recorded. The pH, lactic acid, NDF, ADF, CP, Ca and P contents were similar in WCRS and WCCS. The DM, ash and EE contents of WCRS were greater compared with WCCS. Nutrient (DM, NDF, TDN and CP) intakes were similar in cows fed WCCS- and WCRS-based TMR. Daily and 4% fat corrected milk yield were not affected by the treatments. Milk composition (percent milk fat, protein, lactose and total solids) was similar in cows fed either WCCS- or WCRSbased TMR. The concentration of milk urea N was greater in cows fed WCRS-based TMR than those fed WCCS-based TMR. In conclusion, round-baled WCRS can replace WCCS in the diet of mid- to late-lactating Holsteins without any deleterious effects on feed consumption, milk yield and its composition. The present findings raise the possibility that WCRS can be used as an alternative roughage source in the diets of dairy cows in countries with surplus rice production.

In the field of financial time series, threshold-asymmetric conditional variance models can be used to explain asymmetric volatilities [C.W. Li and W.K. Li, On a double-threshold autoregressive heteroscedastic time series model, J. Appl. Econometrics 11 (1996), pp. 253–274]. In this paper, we consider a broad class of threshold-asymmetric GARCH processes (TAGARCH, hereafter) including standard ARCH and GARCH models as special cases. Since sample autocorrelation function provides a useful information to identify an appropriate time-series model for the data, we derive asymptotic distributions of sample autocorrelations both for original process and for squared process. It is verified that standard errors of sample autocorrelations for TAGARCH models are significantly different from unity for lower lags and they are exponentially converging to unity for higher lags. A consensus meeting of leading Asian hypertension experts was held in January 2007 in Seoul, Korea, to discuss how to address the growing challenge of hypertension management in the region. This report summarises key recommendations from the group, including: raising public awareness about the impact of hypertension; improving physician education and training; increasing early detection, for example through routine blood pressure measurement; and development and adoption of pan-Asian treatment guidelines, which would greatly facilitate research into hypertension and its management. The group conclude that these challenges can only be met through a collaborative effort of government, healthcare professionals, food and healthcare industries, and patients and the public. Food and healthcare industries need to develop healthy foods and support healthy living programmes, while increasing research into antihypertensive medications in Asia. Government officials and policy makers need to be made aware of the value of investing in hypertension awareness, prevention and management programmes.

We have designed a single photon emitter based on a single quantum dot embedded within a single mode parabolic solid immersion lens (pSIL) and a capping low-index pSIL. Numerical simulations predicted that the emitter performance should exhibit a high photon collection efficiency with excellent far-field emission properties, broadband operation, and good tolerance in its geometric (spatial configuration) parameters. Good geometric tolerance in a single-mode pSIL without yielding significant losses in the photon collection efficiency is advantageous for device fabrication. The low-index top pSIL layer provided this structure with a high photon collection efficiency, even in the case of a small numerical aperture (NA). Photon collection efficiencies of 64% and 78% were expected for NA values of 0.41 and 0.5, respectively. In addition to the benefits listed above, our combined pSIL design provided excellent broadband performance in a 100 nm range. (C) 2016 Optical Society of America
