El cáncer de pulmón constituye un problema mundial de salud pública. El cáncer de pulmón de célula pequeña es el tipo histológico que presenta peores datos de supervivencia en la actualidad. Identificar factores de riesgo modificables es fundamental para poder incidir en la disminución de la incidencia de una enfermedad altamente letal. El tabaco es la causa principal pero no es el único factor de riesgo modificable del cáncer de pulmón de célula pequeña. Entre otros destaca la exposición al humo ambiental de tabaco, el radón, el humo de motores diésel y otras sustancias carcinogénicas presentes en el ámbito laboral y en el doméstico. El objetivo principal de esta tesis es valorar el impacto de la ocupación como factor de riesgo de cáncer de pulmón de célula pequeña, de manera independiente al consumo de tabaco.

Smoking and exposure to secondhand smoke pose a significant risk to the health of populations. Although this evidence is not new, the commitment of countries to implement laws aimed at controlling consumption and eliminating exposure to secondhand smoke is uneven. Thus, in North America or in Europe, locations like California or Ireland, are pioneers in establishing policies aimed at protecting the population against smoking and secondhand smoke. Identifying measures that have worked would help control this important Public Health problem in other countries that are further behind in tobacco control policies. In Spain, there has been almost 15 years of little political action in legislation oriented to control the tobacco epidemic. If we want to achieve the tobacco endgame, new legislative measures must be implemented. In this paper, we have elucidated tobacco control policies that could be implemented and show how different countries have done so

Objectives Previous studies suggest that there is discordance between actual weight status and body-weight perception. This fact has implications when it comes to designing public health interventions. The aim of this study was to estimate the prevalence of the different categories of weight status and body-weight perception and to analyse their concordance in a representative Spanish population sample. Study design Cross-sectional study. Methods Data were sourced from the 2018 Galician Risk Behaviour Data System, with the target population being all persons aged 16 years and above. We collected data on self-perceived body weight and assessed weight status on the basis of body mass index (BMI). BMI was estimated using self-reported measures of weight and height. To estimate concordance, Cohen's kappa coefficient, both unweighted and weighted with Cicchetti weights, was calculated. Results Data were obtained for 7853 individuals aged 16 years and above, whereas the overall unweighted concordance was 0.393 (95%CI: 0.377–0.409), with an agreement percentage of 61.6%, weighted concordance was 0.503 (0.490–0.517), with an agreement percentage of 86.6%. The highest concordance between self-perceived body weight and weight status was observed in women. By age group, the highest concordance was observed in the youngest group (16–24 years) for the BMI categories of underweight and overweight, and in the 45–64 age group for the category of obesity. Conclusions The results highlight the existence of differences between self-perceived body weight and weight status, according to sex and age

Objectives: This study aims to assess the quality, risk of bias, and conflicts of interest (COIs) of clinical trials conducted on the effects of fortified infant formula. Study Design and Settting: Systematic review including all randomized clinical trials targeting healthy children and using three arms: fortified infant formula; standard formula; and breastfeeding. We performed a descriptive analysis of the studies reviewed, assessed their quality using the “Risk of Bias 2- RoB 2” tool, and identified COIs. Results: A total of 40 studies were included. All showed a high overall risk of bias, with this being especially noteworthy in the “deviations from intention to treat” and “missing outcome data” domains. Of the total included studies, 29 reported conclusions in favor of the fortified formula; 15 studies reported multiple conclusions that were either contradictory or not in line with the results. COIs with industry were identified in 33 studies, and in 17 studies, these conflicts were not declared in the appropriate section. Conclusion: From a methodological perspective, studies on fortified infant formula display low quality, made evident by the high risk of bias. Additionally, there are frequent COIs. These aspects must be considered by health professionals and the population when drawing up recommendations for the use of this product

Background Several studies have estimated the impact of second-hand smoke (SHS) exposure on mortality in the population of different countries. This study aimed to identify and describe studies that have estimated the attributable mortality (AM) associated with SHS exposure in the adult population. Methods A literature search was conducted in MEDLINE, EMBASE, Cochrane Library and CINAHL databases up to January 2023. Studies that estimated the AM associated with SHS exposure in the adult population and used a prevalence-dependent method were included. The main characteristics of the studies and their results were described. Results Fifty-three studies were included. Most of them were conducted in North America (n = 13), Europe (n = 14) and China (n = 6) and included lung cancer (n = 46) or ischaemic heart disease (n = 22) as causes of death. There was considerable variety in the population under study regarding the relationship with tobacco: non-smokers (n = 30); never-smokers (n = 9); both non and never-smokers (n = 2); the whole population (n = 1) and not known (n = 11). The age at which AM was estimated also varied between studies, ranging from 15 to 40 years and older. Conclusions Studies estimating AM associated with SHS exposure are heterogeneous in terms of the causes of death studied, the age at which mortality is attributed, or the population to which mortality referred: consensus should be reached. Despite their importance, studies assessing AM to SHS are infrequent in low- and middle-income countries

Objectives The aim of this study was to estimate smoking-attributable mortality (SAM) in the population aged 35 years and over in Brazil's 27 federal units by sex, in 2019. Study design This is an attributable mortality analysis. Methods We applied a method dependent on the prevalence of smoking, based on the population attributable fractions. Data on mortality due to causes causally related to smoking were derived from Brazil's Death Registry, data on prevalence of smoking from a survey conducted in Brazil in 2019, and data on relative risks from five US cohorts. Crude and age-adjusted SAM rates were calculated by sex. Estimates of SAM were calculated by specific causes of death and major mortality groups for each federal unit by sex. Results In 2019, smoking caused 480 deaths per day in Brazil. Although the SAM varied among the federal units, the pattern is not clear, with the greatest difference being between Rio Grande do Sul (crude rate: 248.8/100,000 inhabitants) and Amazonas (106.0/100,000). When the rates were adjusted by age, the greatest differences were observed between Acre (271.1/100,000) and Distrito Federal (131.1/100,000). SAM was higher in males; however, while the main specific cause of SAM in men was ischemic heart disease, in women it was chronic obstructive pulmonary disease. The major mortality group having the greatest impact on SAM across all federal units was the cardiometabolic diseases. Conclusions The variability in the burden of SAM in the different regions of Brazil reaffirms the need for SAM data disaggregated at the geographic level

BACKGROUND // The violence produced in the context of dating is known as dating violence. Currently, it is a widespread problem in adolescence and there is a great lack of knowledge about the beliefs/attitudes that help perpetuate this phenomenon. This study aimed to assess how adolescents perceive dating violence. Additionally, to estimate the observed frequency of exposure of adolescents to different dating violence items, assessing the differences according to sex and educational level. METHODS // A cross-sectional study was carried out in 2022 among high school students coming from Spain (Galician Region) The data was collected by means of an anonymous online questionnaire. A descriptive analysis of the data obtained was conducted. The observed frequency of adolescents’ exposure to different items of dating violence and identification of violence was estimated. Fisher’s exact test was used to compare the proportions according to sex and educational level. RESULTS // 410 students were included. 99% of the women perceived that it is not normal to control their partner’s clothing compared to 88% of the men; in relation to the control of friendships these percentages were 87.6% and 73.1% and with respect to criticism of the partner 54.7% vs 67.9%, respectively in women and men. 46.8% of the students admitted knowing of cases in which they sent several messages a day to find out what their partner was doing. 21.7% referred to be aware of cases who felt fear of the partner. CONCLUSIONS // The perception of dating violence by women is higher. The largest differences between men and women are in the items of the control domain., FUNDAMENTOS // La violencia en la pareja que se produce en adolescentes se conoce con el nombre de violencia en el noviazgo. Actualmente, es un problema generalizado en la adolescencia y existe un gran desconocimiento al respecto de las creencias/actitudes que están ayudando a perpetuar este fenómeno. El objetivo de este estudio fue conocer la percepción de los adolescentes sobre esta violencia y estimar la frecuencia observada de exposición a diferentes ítems de violencia en el noviazgo, valorando las diferencias según sexo y nivel educativo. MÉTODOS // Se realizó en 2022 un estudio transversal en cuatro centros educativos públicos de Galicia (España) que impartían Educación Secundaria Obligatoria y Bachillerato. El muestreo de los centros fue de conveniencia. Se invitó a los estudiantes a contestar un cuestionario anónimo. Se estimó la frecuencia observada de exposición del estudiantado a diferentes ítems de violencia en el noviazgo y de identificación de la violencia. Para comparar las proporciones en función del sexo y del nivel educativo se empleó la prueba exacta de Fisher. RESULTADOS // Se incluyeron 410 estudiantes. El 99% de las mujeres percibieron que no es normal controlar la vestimenta de la pareja frente al 88% de los hombres; en relación con el control de las amistades, estos porcentajes fueron del 87,6% y el 73,1%, en mujeres y hombres, respectivamente. Con respecto a las críticas a la pareja, estos porcentajes fueron del 54,7% frente al 67,9%, respectivamente en mujeres y hombres. El 46,8% del estudiantado admitió conocer casos que enviaban varios mensajes al día para saber qué estaba haciendo la pareja. El 21,7% refirió conocer casos que sentían miedo de su pareja. CONCLUSIONES // La percepción de violencia en el noviazgo es mayor por parte de las mujeres. Las mayores diferencias entre hombres y mujeres se observan en los ítems del dominio control.

Indoor radon exposure increases the risk of lung cancer. Radon concentration in workplaces is regulated in EU countries, including Spain, based on a reference level of 300 Bq/m3. The objective of this study is to describe workplace radon exposure in Spain and its influencing factors. To do this, we collected long-term radon measurements with alpha track detectors in 3140 workplaces mainly located in radon prone areas. Radon concentration exceeded 300 Bq/m3 in 1 out of 5 workplaces. Median radon concentration was 107 Bq/m3 in radon prone areas, 28 Bq/m3 off radon prone areas, and 101 Bq/m3 globally for the complete sample. Our results indicate that excessive radon concentrations can be expected in radon prone areas at all floor levels, especially below ground. Floor level, working sector, and location significantly influence radon concentration. The highest radon concentrations were found in the Education & Culture sector, comprising schools, universities, libraries, or cultural centers. These results indicate that radon should no longer be considered a risk for marginal occupations, but a risk everyone has if located in a radon prone area. Immediate action, including radon testing and mitigation, is needed to protect workers in Spain against radon exposure. This is already mandatory since EU regulation for radon has been recently transposed in Spain. Competent authorities should enforce this regulation without further delay, and employers must address their responsibility and communicate with workers about this risk

La mala conducta científica hace referencia a las prácticas realizadas de forma intencionada que socaban los principios éticos de publicación científica. Esta tesis pretende generar conocimiento sobre la mala conducta científica, con el objetivo último de prevenirla y reducirla. Para ello, se ha evaluado cómo se regula la mala conducta científica a nivel europeo y nacional, mediante el estudio de las Oficinas Nacionales de Integridad Científica. Aunque la mayoría de los países europeos cuentan con una oficina, éstas tienen capacidad consultiva y no ejecutiva. España, por su parte, no cuenta con una oficina de estas características. En esta tesis se ha abordado la mala conducta científica desde el punto de vista de otras partes interesadas: las instituciones de los investigadores y las revistas científicas.

Introduction There is still uncertainty about which aspects of cigarette smoking influence the risk of Chronic Obstructive Pulmonary Disease (COPD). The aim of this study was to estimate the COPD risk as related to duration of use, intensity of use, lifetime tobacco consumption, age of smoking initiation and years of abstinence. Methods We conducted an analytical cross-sectional study based on data from the EPISCAN-II study (n = 9092). All participants underwent a face-to-face interview and post-bronchodilator spirometry was performed. COPD was defined as post-bronchodilator FEV1/FVC < 70%. Parametric and nonparametric logistic regression models with generalized additive models were used. Results 8819 persons were included; 858 with COPD and 7961 without COPD. The COPD risk increased with smoking duration up to ≥50 years [OR 3.5 (95% CI: 2.3–5.4)], with smoking intensity up to ≥39 cig/day [OR 10.1 (95% CI: 5.3–18.4)] and with lifetime tobacco consumption up to >29 pack-years [OR 3.8 (95% CI: 3.1–4.8)]. The COPD risk for those who started smoking at 22 or later was 0.9 (95% CI: 0.6–1.4). The risk of COPD decreased with increasing years of cessation. In comparison with both never smokers and current smokers, the lowest risk of COPD was found after 15–25 years of abstinence. Conclusion COPD risk increases with duration, intensity, and lifetime tobacco consumption and decreases importantly with years of abstinence. Age at smoking initiation shows no effect. After 15–25 years of cessation, COPD risk could be equal to that of a never smoker. This work suggests that the time it takes to develop COPD in a smoker is about 30 years

Los ingresos por tromboembolismo pulmonar en España han aumentado en los últimos años y los costes de la hospitalización representan, junto al número de procedimientos diagnósticos y las comorbilidades de estos pacientes, la mayor carga económica de la embolia pulmonar. En este trabajo se ha identificado qué factores influyen en la duración de la estancia hospitalaria por embolia pulmonar, con el fin de poder planificar de manera más adecuada la gestión hospitalaria. Se ha evaluado qué papel juegan en el tromboembolismo venoso los factores de riesgo cardiovascular y finalmente se ha evaluado si la implantación de un Proceso Asistencial Integrado de tromboembolismo pulmonar conlleva una mejora en la calidad asistencial de estos pacientes, evaluada a través de indicadores sencillos y extrapolables a otros entornos hospitalarios.

Radon is a carcinogenic compound, and is particularly concerning in the education sector, where children and teachers may be exposed even longer than at home. Thus, this study intended to characterise radon in the indoor air of scholar environments in different provinces/districts of the Euroregion Galicia–Norte de Portugal. With a pioneering approach, this study evaluated the influence of specific factors/characteristics (location, type of management, construction material, season and floor within the building) and quantified their relative contribution to indoor radon levels. Radon was continuously monitored in 416 classrooms from school buildings located in urban and rural sites from different provinces/districts both in the regions of Galicia (A Coruña and Lugo provinces) and Portugal (Porto and Bragança districts), considering rooms for different age groups (from nursery schools to universities). Single and multivariate linear regression models were built considering the radon concentrations as the outcome variable and different room/building characteristics as predictor variables. Mean and median radon concentrations were 332 Bq·m−3 and 181 Bq·m−3, respectively. The radon concentrations observed are a public health concern, as almost 1/3 of the places monitored exceeded the reference limit value of the European legislation (300 Bq·m−3). Moreover, around 50 % of the indoor levels measured could be attributed to room/building characteristics: the building's location and the main construction material, as well as the occupants' age group, the floor within the building and the school's type of management (public/private). This study concluded that radon testing is needed in all school buildings and classrooms without exceptions. Thus, public administrations are urged to dedicate funds for testing, mitigation and public dissemination initiatives in schools. A special protocol for radon sampling in school buildings should also be developed

Introduction and objectives: Exposure to environmental tobacco smoke (ETS) is associated with increased mortality and morbidity. The objective of this study was to estimate the impact of ETS exposure in Spain on mortality in 2020 in the population aged 35 years and over. Methods: A method of estimating attributable mortality (AM) based on the prevalence of ETS exposure was applied. Prevalence data were obtained from a representative study conducted in Spain and the relative risks were derived from a meta-analysis. AM point estimates are presented along with 95% confidence intervals (95% CI), calculated using a bootstrap naive procedure. AM, both overall and by smoking habit, was estimated for each combination of sex, age group, and cause of death (lung cancer and ischemic heart disease). A sensitivity analysis was performed. Results: A total of 747 (95% CI 676–825) deaths were attributable to ETS exposure, of which 279 (95% CI 256–306) were caused by lung cancer, and 468 (95% CI 417–523) by ischemic heart disease. Three quarters (75.1%) of AM occurred in men and 60.9% in non-smokers. When chronic obstructive pulmonary disease and cerebrovascular disease are included, the burden of AM is estimated at 2242 deaths. Conclusions: ETS exposure is associated with 1.5% of all deaths from lung cancer and ischemic heart disease in the population aged 35 and over. These data underline the need for health authorities to focus on reducing exposure to ETS in all settings and environments

Introduction Chronic Obstructive Pulmonary Disease (COPD) is the third cause of death worldwide. While tobacco smoking is a key risk factor, COPD also occurs in never-smokers (NS). However, available evidence on risk factors, clinical characteristics, and natural history of the disease in NS is scarce. Here, we perform a systematic review of the literature to better describe the characteristics of COPD in NS. Methods We searched different databases following the PRISMA guidelines with explicit inclusion and exclusion criteria. A purpose-designed quality scale was applied to the studies included in the analysis. It was not possible to pool the results due to the high heterogeneity of the studies included. Results A total of 17 studies that met the selection criteria were included, albeit only 2 of them studied NS exclusively. The total number of participants in these studies were 57,146 subjects, 25,047 of whom were NS and 2,655 of the latter had NS-COPD. Compared to COPD in smokers, COPD in NS is more frequent in women and older ages, and is associated with a slightly higher prevalence of comorbidities. There are not enough studies to understand if COPD progression and clinical symptoms in NS are different to that of ever-smokers. Conclusions There is a significant knowledge gap on COPD in NS. Given that COPD in NS account for about a third of all COPD patients in the world, particularly in low-middle income countries, and the decrease in tobacco consumption in high income countries, understanding COPD in NS constitutes a public-health priority
