Purpose: Preterm birth (PTB) is a complex trait with strong genetic background, whose etiology is not fully understood. It was recently suggested that pregnancy duration is affected by fetal genetic variation even more than by the maternal genome. Vitamin D receptor (VDR) is involved in embryonic implantation and fertility. We studied the association between both maternal and neonatal vitamin D receptor (VDR) genetic variation and PTB., Methods: Maternal and fetal (umbilical cord) DNA was isolated from Jewish Israeli idiopathic preterm newborns (24-36 weeks, n = 146) and control term newborns (>37 weeks, n = 229). Maternal and fetal VDR polymorphisms (FokI, ApaI, BsmI, TaqI) were analyzed by restriction fragment length polymorphism analysis. Using SPSS analysis to correlate VDR genotypes with phenotypic variation: pregnancy duration, preterm birth and spontaneous miscarriages, adjusted for gravidity, parity and gender of newborn., Results: Women homozygous to VDR ApaI (AA) genotype had significant twofold increase risk for PTB [OR 1.973, (CI) 1.183-3.289, p = 0.009] compared to heterozygous women. Male newborns had significant (p < 0.05) 1.73-fold increase of PTB. Women with history of previous (≥1) spontaneous miscarriage had a significant increased risk for PTB if their newborn carried either of the VDR BsmI homozygous (BB or bb) genotypes compared to the heterozygous (Bb) genotype [OR 6.857, (CI) 1.273-36.934, p = 0.018 and OR 9.231, (CI) 1.753-48.618, p = 0.008, respectively], or VDR ApaI homozygous (AA or aa) genotype compared to heterozygous (Aa) genotype [OR 4.33, (CI) 1.029-18.257, p = 0.046 and OR 7.2, (CI) 1.34-38.917, p = 0.021, respectively]., Conclusions: We show association between maternal and fetal VDR genotype variants with PTB.

In the absence of amplification methods for proteins, the immune-detection of low-abundance proteins using antibodies is fundamentally limited by binding kinetic rates. Here, we present a new class of surface-based immunoassays in which protein-antibody reaction is accelerated by isotachophoresis (ITP). We demonstrate the use of ITP to preconcentrate and deliver target proteins to a surface decorated with specific antibodies, where effective utilization of the focused sample is achieved by modulating the driving electric field (stop-and-diffuse ITP mode) or applying a counter flow that opposes the ITP motion (counterflow ITP mode). Using enhanced green fluorescent protein (EGFP) as a model protein, we carry out an experimental optimization of the ITP-based immunoassay and demonstrate a 1300-fold improvement in limit of detection compared to a standard immunoassay, in a 6 min protein-antibody reaction. We discuss the design of buffer chemistries for other protein systems and, in concert with experiments, provide full analytical solutions for the two operation modes, elucidating the interplay between reaction, diffusion, and accumulation time scales and enabling the prediction and design of future immunoassays.

Unlabelled: Leptin (LEP) and leptin receptor (LEPR) are suggested to play a role in female reproduction and especially in pregnancy Both LEP and LEPR are synthesized by the pregnant female and embryo. The link between genetic polymorphisms of LEP and LEPR and preterm birth (PTB) is unknown. We studied maternal and neonatal LEP and LEPR genetic polymorphisms and the association with PTB. Blood for DNA analysis was collected from Israeli mothers and from venous umbilical of their respected idiopathic preterm newborns (24-36weeks, n=102) and control term newborns (>37weeks, n=158). Genotypes of maternal and neonatal LEP (rs7799039) and LEPR (rs1137101) polymorphisms were analyzed by restriction fragment length polymorphism analysis. Genotype-phenotype association was assayed using SPSS program. We found a significant independent increased risk of PTB for women and neonates bearing the homozygous AA form of LEP genotype; where women carrying AA LEP genotype had 2.53 fold ([CI] 1.367-4.685, p=0.03) and 2.38 fold ([CI] 1.150-4.915, p=0.019) increased risk for PTB compared to AG and GG genotypes, respectively. Neonates carrying the LEP AA genotype had a significant 2.8 fold increased risk for PTB compared to the AG genotype (CI11.040-7.577, p=0.042). Maternal LEPR polymorphism was significantly associated with severe PTB; where women carrying the AA and AG genotypes had a significant 4.32 and 4.76 fold increased risk for severe PTB compared to women carrying the GG genotype (CI=1.090-17.112 and 1.332-17.027, respectively p=0.035)., In Conclusion: maternal and neonatal LEP and LEPR polymorphisms are significantly associated with increased risk for PTB., (Copyright © 2016 Elsevier B.V. Assessing testamentary capacity in the terminal phase of an illness or at a person's deathbed is fraught with challenges for both doctors and lawyers. Numerous issues need to be considered when assessing capacity for a will. These issues are exacerbated when such an assessment needs to be undertaken at the bedside of a dying patient. The nature and severity of the illness, effects on cognition of the terminal illness, effects of medication, urgency, psychological and emotional factors, interactions with carers, family and lawyers, and a range of other issues confound and complicate the assessment of capacity. What is the doctor's role in properly assessing capacity in this context and how does this role intersect with the legal issues? Doctors will play an increasing role in assessing testamentary capacity in this setting. The ageing of society, more effective treatment of acute illness and, often, the prolongation of dying are only some of the factors leading to this increasing need. However, despite its importance and increasing prevalence, the literature addressing this challenging practical area is scarce and offers limited guidance. The need for better and more accurate assessments of testamentary and decision-making capacity grows as Australian society ages and incidences of mentally disabling conditions increase. Capacity is a legal determination, but one on which medical opinion is increasingly being sought. The difficulties inherent within capacity assessments are exacerbated by the ad hoc approaches adopted by legal and medical professionals based on individual knowledge and skill, as well as the numerous assessment paradigms that exist. This can negatively affect the quality of assessments, and results in confusion as to the best way to assess capacity. This article begins by assessing the nature of capacity. The most common general assessment models used in Australia are then discussed, as are the practical challenges associated with capacity assessment. The article concludes by suggesting a way forward to satisfactorily assess legal capacity given the significant ramifications of getting it wrong.

We present a new method for accurate detection of sample location in peak mode isotachophoresis (ITP). The technique is based on the design of a microchannel with multiple constrictions and on detecting the passage of the ITP interface through these constrictions. We achieve this by monitoring the electric current across the channel, which exhibits sharp decreases as the ITP interface moves more rapidly through the higher current density constrictions. We show that cross-correlation between the electric current signal and a predefined step function is an effective method for detecting changes in the slope of the electric current curve in real-time and is robust to changes in the composition of the buffers. We demonstrate the use of the technique to deliver sample to a designated location in the channel with an accuracy as low as 50 μm. Importantly, the method does not require the use of any optics and thus can be used to monitor the location of unlabeled species for application in a variety of ITP assays.

We present an experimental and analytical study of a novel paper-based analytical device (μPAD) for isotachophoretic sample focusing. Guided by a simple heat transfer model, we further developed wax printing fabrication to enable the creation of shallow channels, which are critical in providing sufficient dissipation of Joule heat, and thus enable the use of high electric fields and short analysis time. This results in a device that is self-contained on a simple piece of filter paper and does not require any specialized enclosures or cooling devices to combat evaporation at high temperatures. Furthermore, we provide an analytical model for isotachophoretic sample accumulation in porous media, introduce a simple figure of merit for evaluating and comparing the efficiency of such devices, and present experimental validation in both paper and glass channels. Using this device we demonstrate the processing of 30 μL of sample achieving 1000-fold increase in peak concentration in 6 min. We believe that this method and device can serve as a guide to the design of low-cost, rapid and highly sensitive paper-based diagnostic platforms.

Balancing the interests of individual autonomy and protection is an escalating challenge confronting an ageing Australian society. One way this is manifested is in the current ad hoc and unsatisfactory way that capacity is assessed in the context of wills, enduring powers of attorney and advance health directives. The absence of nationally accepted assessment guidelines results in terminological and methodological miscommunication and misunderstanding between legal and medical professionals. Expectations between legal and medical professionals can be clarified to provide satisfactory capacity assessments based on the development of a sound assessment paradigm.

Healthcare systems are increasingly adapting to address the issues associated with population ageing. The shift to chronic diseases and a rise in neuroepidemiological conditions, associated with rising life expectancies, means that continued change and accommodation will be required of our health and social support systems. Current social policy environments developed out of early approaches to state-supported health and welfare service provision, most now a century or more old. A feature of these systems has often been a formal separation between them, into silos, that does not and cannot effectively address the issues raised by a growing population of older people. This is especially true in the context of community-based care where the majority of older people currently live and where governments hope to keep more elderly people living into the future. This objective will require a far more sophisticated and responsive approach to the health information environment than is currently the case. One strategy for improving this scenario is the development of augmented and virtual environments that collect and analyse real-time data on which health professionals and support staff can act in a timely manner. In this paper we explore some aspects of a virtualised aged care system and provide some examples of how this would enhance our current strategies for aged care.

Unlabelled: Transthoracic electrical cardioversion (ECV), traditionally using monophasic waveform (MW) shock, has an important role in the treatment of symptomatic atrial flutter (AFI). Biphasic waveform (BW) shock has been demonstrated to be more successful than MW shock for termination of atrial fibrillation, but data about its use for ECV of AFI are limited., Methods and Results: We retrospectively analyzed the records of 53 patients (pts) admitted -to the ER due to symptomatic AFl during the period August 2004 to August 2005: 31 pts received BW shock and 22 pts MW shock. The type of shock waveforms and the initial energy of CV were chosen by the doctor on duty in the ER; the lower energy for ECV was 20 joules, which was increased to 50, 100 and 200 joules if necessary. There were no significant differences between the clinical characteristics of the pts who received BW shock or MW shock. All pts underwent ECV via anterior-laterally positioned hand-held electrode paddles. Successful ECV by BW shock and MW shock was 41% and 42% of the pts, respectively, using 20 joules of energy (p=n.s.); 77% and 80% using 50 joules (p=n.s.); 93% and 90% using 100 joules (p = n.s.); 100% of successful ECV was reached when 200 joules of energy was used, regardless of waveforms type. Median energy for successful ECV was 50 joules in both types of electrical waveforms. No complications were reported., Conclusions: There were no significant differences in the success rates of conversion of atrial flutter to sinus rhythm by BW or MW shock. We recommend 50 joules for starting energy of ECV of AF1 regardless of waveforms type.

Background: Adult calcific aortic stenosis is a well-known clinical entity but its pathophysiology and cellular mechanism have yet to be defined., Objectives: To determine whether there is an association between the presence and severity of adult calcific aortic stenosis and Chlamydia pneumoniae seropositivity., Methods: Forty adult patients (23 women, 17 men) were divided into three groups according to echocardiographic aortic valve area: Group A - 7 symptomatic subjects (age 67 +/- 7 years) with normal aortic valve and normal coronary angiogram, Group B - 16 patients (age 73 +/- 6) with moderate ACAS (AVA > 0.8 < or = 1.5 cm2), and Group C - 17 patients (age 76 +/- 7) with severe ACAS (AVA +/- 0.8 cm2). We tested for immunoglobulins M, G and A as retrospective evidence of C. pneumoniae infection using the micro-immunofluorescence method. Past C. pneumoniae infection was defined by IgG titer > 16 < or = 512., Results: No patients in group A showed positive Ig for C. pneumoniae. IgM was not detected in any of the patients with ACAS (groups B and C) while 2 of 17 patients (12%) in group C showed IgA for the pathogen. High titers of IgG were found in 14 of 33 (42%) of the patients with moderate or severe ACAS: 5 of 16 (31%) in group B and 9 of 17 (53%) in group C (P = 0.2). Both groups had the same prevalence of coronary artery disease (66%). AVA was lower in IgG-seropositive patients than in the seronegative group (0.88 +/- 0.3 cm2 vs. 1.22 +/- 0.4 cm2, respectively, P = 0.02)., Conclusions: Past C. pneumoniae infection may be associated with a higher prevalence and greater severity of ACAS.

Introduction: Since the removal of intravenous propafenone from the Israeli market, flecainide is the only intravenous antiarrhythmic class 1C drug available nowadays in Israel., Aim: The study aimed to report our experience of intravenous flecainide administration in the treatment of paroxysmal atrial fibrillation (PAF) in the Emergency Room (ER)., Methods: Patients with AF lasting > 1 hour and <48 hour duration were considered possible candidates for entry into the study. Exclusion criteria were clinical signs of congestive heart failure, acute coronary syndrome, electrolyte imbalances, significant hepatic and renal disease, and any previously documented conduction disturbance. Flecainide was administered as a bolus dose of 2mg/kg in 10 minutes (maximum 150 mg). The patients with a ventricular response > 130 beats/min received intravenous verapamil or metoprolol in order to reduce the ventricular rate. Efficacy was defined as conversion to sinus rhythm (SR) within 120 minutes of starting medication., Results: Twenty three consecutive patients were enrolled in the study. Their mean age was 60 +/- 19 years; the mean ventricular response at admission was 128 +/- 26 beats/min. SR was achieved in 10 patients (43%) after the intravenous bolus of flecainide (10 minutes) and in 17 patients (74%) 120 minutes after the beginning of the therapy., Complications: Hypotension (systolic blood pressure < 90 mmHg) was reported in 1 patient and QRS enlargement was seen in 1 patient., Conclusion: Intravenous flecainide has effective and rapid action in the conversion of PAF and its administration is safe in the treatment of this arrhythmia in the ER.

Background and Aim of the Study: The incidence and clinical significance of immobile and 'frozen' posterior mitral leaflet (FPML) were evaluated in the pathophysiology and immediate outcome of patients with severe pliable mitral stenosis (MS) undergoing percutaneous balloon mitral commissurotomy (PBMC)., Methods: During the past four years, 30 'ideal' patients (mean age 46 +/- 8 years) with Wilkins' score <8, bilateral commissural fusion and absence of commissural calcification underwent peri-procedural echocardiographic analysis. Anterior mitral leaflet (AML) mobility index (MI), chordae tendineae (CT) length, and mitral valve area (MVA) were evaluated., Results: Pre-procedure FPML was noted in 28 patients (93%). All patients achieved MVA > or = 1.5 cm2. Post-procedure MVA in patients with bilateral commissural splitting was 1.9 +/- 0.2 cm2 versus 1.6 +/- 0.1 cm2 in patients with unilateral commissural splitting (p < 0.05). CT lengths directed to the AML and PML were 15 +/- 2 mm and 8 +/- 2 mm, respectively (p < 0.05). MI of the AML before and immediately after PBMC was 0.4 and 0.6, respectively (p < 0.05). None of the patients with FPML showed improved mobility following successful PBMC., Conclusion: FPML may be found in most patients with pliable MS. It is mainly a result of short, rigid and fused CT directed to the PML. A 'single-wing door' or a unicuspid valve may be used as a model for rheumatic pliable MS. It is suggested that pre-procedure leaflet morphology and functional assessment should focus on the AML.

Early and mid-term results of repeat percutaneous balloon mitral commissurotomy (PBMC) were analyzed in 35 patients with symptomatic valvular restenosis: 12 patients (34%) after first successful PBMC and 23 patients (66%) after successful surgical closed mitral commissurotomy. Twenty-one patients had bilateral fused commissures, and 14 patients had unilateral or bilateral split commissures. Mitral valve area gain was significantly greater in the group with fused commissures compared with the group with split commissures (0.6 +/- 0.2 vs 0.3 +/- 0.2 cm(2), respectively, p = 0.04).

Background: Torsade de pointes is rarely associated with chronic amiodarone treatment, despite the effect of amiodarone on QT interval prolongation., Objective: To identify risk factors and associated conditions that may cause TdP in patients on chronic amiodarone treatment., Methods: We reviewed the data of six consecutive patients on chronic amiodarone treatment who were admitted to the intensive cardiac care unit due to syncope and TdP., Results: The patients' median age was 73.5 years, and five were women. Concomitantly, loratadine was given to two patients and trazodone to one patient. Associated and attributing conditions to the development of TdP were hypokalemia in three patients, drug-induced bradycardia in one and reduced left ventricular function in four., Conclusions: TdP associated with chronic amiodarone treatment may occur when amiodarone is co-administered with drugs that may potentially prolong QT interval. Additional risk factors for amiodarone-associated TdP include female gender, hypokalemia, reduced left ventricular function and bradycardia.

Unlabelled: Transthoracic electrical cardioversion, traditionally monophasic shock waveform, has been a mainstay of the therapy for atrial fibrillation (AF) since its introduction into clinical practice. Recent studies have demonstrated that biphasic shock is more efficient than monophasic shock waveforms for terminating both ventricular fibrillation and AF; however, data on the recommended initial shock energy in conversion of AF by biphasic shocks are limited., Aim: Our study aimed to evaluate the optimal dose of the initial shock energy for conversion of AF to sinus rhythm by transthoracic biphasic shock waveforms in the Emergency Room (ER)., Methods and Results: A total of 144 consecutive patients, who came to the ER because of AF, were our study population. All patients underwent cardioversion via anterior-laterally positioned hand-held electrode paddles. Patients received sequential shocks of 50 J (only the first 40 patients), 100 J, 150 J and 200 J if necessary. There was a significantly greater cumulative conversion success rate with 100 J (70.5%) than 50 J shock energy (55%), p < 0.05; but even greater with 150 J (89%) than 100 J shock energy, p < 0.003; no significant difference was observed between 200 J (94%) and 150 J shock energy, p < 0.58. Nine of 12 patients, whose body weight was less than 70 kg, were successfully converted to sinus rhythm (75%) by 50 J shock 1 energy. After cardioversion there were reports of: a five seconds asystole observed in 1 patient; pulmonary edema in another patient; hypotension was reported in 1 patient and mild erythema in 14 patients (9.7%)., Conclusion: Our findings support that biphasic waveform shock energy of 150 J is advised as a first attempt, but in patients with a body weight less than 70 kg. lower energy shock may be used.

This study examined the utility and accuracy of immediate hand-carried echocardiography in patients presenting to the emergency room with chest pain and a normal or nondiagnostic electrocardiogram. Hand-carried echocardiography was highly concordant (kappa = 0.8) with troponin T tests as well as the discharge diagnosis of acute coronary syndrome, had a 100% sensitivity for the detection of acute coronary syndrome, 93% specificity, and 71% and 100% positive and negative predictive values, respectively.

Atrial fibrillation (AF) is the most common sustained symptomatic arrhythmia encountered in the emergency room (ER). Rapid, safe and effective conversions of AF to sinus rhythm (SR) are important aspects in the treatment of arrhythmia in a busy ER setting. The aim of our study was to investigate the rate of conversion, efficacy and safety of intravenous (i.v.) propafenone administration for the treatment of AF in the ER. Seventy five patients (pts), admitted in the ER with symptomatic AF were randomized to receive propafenone or placebo: 40 pts received propafenone as an i.v. bolus of 2 mg/kg in 15 minutes, followed by maintenance infusion of 0.0067 mg/kg/min until conversion to SR or for up to 90 minutes (Group 1). The matching placebo (an equal volume of saline solution) was administered to 35 pts in the same fashion (Group 2). Direct current (DC) cardioversion was attempted in pts still in AF 15 minutes after infusion administration was terminated. SR was achieved in 24 pts treated with propafenone (60%) versus 10 pts treated with placebo (29%) (p<0.02) and the median conversion time was 15 minutes versus 105 minutes respectively (p<0.001). None of the pts who were in AF for more than 48 hours were converted to SR by infusion. Five patients had mild and transient adverse event during propafenone treatment versus none with placebo (p=ns). Therefore, we conclude that i.v. administration of propafenone in the ER is an effective, rapid and safe therapeutic alternative for pts with symptomatic AF.

We report a case of unsuccessful rescrewing of an atrial lead after its early dislodgment in a 64 years old patient because of entrapped endocardial tissue in the screw-in system.

The aim of this retrospective study was to determine the incidence of symptomatic AF of patients who had undergone coronary artery bypass grafting (CABG) during long-term follow-up. The study population included 305 post-CABG patients who were regularly followed in the outpatient clinic. Paroxysmal AF (PAF) was defined as an episode of symptomatic AF when symptoms were prolonged enough for the patient to request medical care. Perioperative AF occurred in 88 (28.9%) patients. Postdischarge symptomatic PAF occurred in 25 (8.2%) patients with an annual incidence of 2% during a mean follow-up of 48 +/- 30 months. Eighteen (20.4%) patients also experienced perioperative AF with an annual incidence of 5.1%, while only 7 (3.2%) of 217 patients, without perioperative AF, had postdischarge AF (P < 0.0003). During long-term follow-up, postdischarge AF has a low incidence and prophylactic antiarrhythmic therapy is not recommended. The method of follow-up and retrospective analysis may understate PAF and even miss some symptomatic episodes. Perioperative AF is a predictor of symptomatic late PAF recurrences, particularly in patients with reduced left ventricular function.

Exertional heat stroke is a medical emergency that uncommonly results in severe cardiac dysfunction. The military physician diagnosed a 19-year-old military recruit from an elite unit to have exertional heat stroke. Immediate treatment in the field with rapid ice water cooling and vigorous fluid administration resulted in pulmonary edema. Transthoracic echocardiography on admission to the emergency department revealed moderate reduction in left and right ventricular function. After treatment, within a few days, rapid myocardial recovery was noted and persisted after 6 months of follow-up. Possible mechanisms of cardiac dysfunction in exertional heat stroke and treatment strategies are discussed. It is suggested that intravenous fluid administration to patients with suspected exertional heat stroke should preferably be done with appropriate hemodynamic monitoring and after cardiac dysfunction has been ruled out.

Torsades de pointes (TdP) has not been previously reported with loratadine. A 73-year old woman on chronic treatment with amiodarone for atrial fibrillation received loratadine and presented with syncope and multiple episodes of TdP. We suggest that QT interval should be monitored whenever loratadine is co-administered with drugs that may potentially prolong QT.

Background: Current clinical guidelines restrict catheterization laboratory activity without on-site surgical backup. Recent improvements in technical equipment and pharmacologic adjunctive therapy increase the safety margins of diagnostic and therapeutic cardiac catheterization., Objective: To analyze the reasons for urgent cardiac surgery and mortality in the different phases of our laboratory's activity in the last 11 years, and examine the impact of the new interventional and therapeutic modalities on the current need for on-site cardiac surgical backup., Methods: We retrospectively reviewed the mortality and need for urgent cardiac surgery (up to 12 hours post-catheterization) through five phases of our laboratory's activity: a) diagnostic (years 1989-2000), b) valvuloplasties and other non-coronary interventions (1990-2000), c) percutaneous-only balloon angioplasty (1992-1994), d) coronary stenting (1994-2000), and e) use of IIb/IIIa antagonists and thienopiridine drugs (1996-2000)., Results: Forty-eight patients (0.45%) required urgent cardiac surgery during phase 1, of whom 40 (83%) had acute coronary syndromes with left main coronary artery stenosis or the equivalent, and 8 (17%) had mechanical complications of acute myocardial infarction. Two patients died (0.02%) during diagnostic procedures. In phase 2, eight patients (2.9%) were referred for urgent cardiac surgery due to either cardiac tamponade or severe mitral regurgitation, and two patients (0.7%) died. The combined need for urgent surgery and mortality was significantly lower in phase 4 plus 5 as compared to phase 3 (3% vs. 0.85%, P = 0.006)., Conclusion: In the current era using coronary stents and potent antithrombotic drugs, after gaining experience and crossing the learning curve limits, complex cardiac therapeutic interventions can safely be performed without on-site surgical backup.

An elevated left atrial pressure and high diastolic pressure gradient (DPG) across the mitral valve are the major hemodynamic abnormalities in mitral stenosis (MS). However, a subgroup of patients with severe MS is characterized by low initial DPG. The authors reviewed the clinical, echocardiographic and hemodynamic data as well as procedural results in 180 patients who underwent percutaneous balloon mitral valvuloplasty (PBMV). An initial mean DPG > 10 mmHg was found in 144 patients (80%) (group A) and mean DPG < or = 10 mmHg in 36 patients (20%) (group B). Patients in group A had higher left ventricular ejection fraction (LVEF) than in group B (61 +/- 5% versus 42 +/- 6%, respectively) and higher cardiac index (2.8 +/- 0.4 versus 2.0 +/- 0.3 l/min/m(2) ). In group B 12 patients (33%) had normal LVEF, whereas 24/36 (67%) had reduced LVEF. All the latter had wall motion abnormalities on ventriculography. Unlike group A, intraprocedural echocardiography was essential for monitoring and evaluating immediate results of PBMV in group B. On follow-up of three years, 75% of group A patients and 55% in group B were in functional class I (p < 0.05). PBMV did not significantly improve symptoms in patients in group B who had preprocedure LVEF < or = 35%.

Antiarrhythmic drugs increase pacing threshold, but this is rarely of clinical significance. Administration of flecainide acetate in a 75-year-old woman with an implanted AAIR pacemaker because of sick sinus syndrome caused an abrupt rise of pacing threshold and failure of pacing. Pacing threshold returned to the normal value a few days after flecainide treatment was stopped.

During the performance of 280 consecutive percutaneous balloon mitral valvuloplasty operations, two cases of conduction disturbances were reported (0.7%). These rare complications are transient and due to the mechanical pressure of the balloon on the conduction system.

Total occlusion of the left subclavian vein was diagnosed in a 76-year-old patient, 6 years after implantation of an ICD with VVI pacing backup. Replacement of the ICD included upgrading of the ICD system because of the presence of pacemaker syndrome when the patient was VVI paced. Insertion of an atrial lead through the ipsilateral vein system was made possible by using the supraclavicular approach of the subclavian, enabling puncturing of the left subclavian vein medially to the obstruction.

This paper presents a rare cause of angina pectoris in a 43-year-old woman. Her evaluation revealed a compressed and proximally occluded right coronary artery by a giant, unruptured, right sinus of Valsalva aneurysm. The aneurysm was surgically resected and the sinus was successfully reconstructed. On follow-up the patient is asymptomatic.

Implantation of an implantable cardioverter defibrillator by the transvenous approach was impossible from the left side in a patient with on absence of the left brachiocephalic vein; the left subclavian vein was connected by a large left superior intercostal vein to the accessory hemiazygos vein that joined the azygos vein; then the blood flowed into the superior vena cava. Implantation was successfully attempted using the right-sided venous access.

Paroxysmal atrial fibrillation (AF), a frequent cause of repeated hospitalization, is effectively treated with propafenone. The time to conversion to sinus rhythm is a consideration when managing AF in the emergency room. We investigated the conversion rates of paroxysmal AF by 3 different oral propafenone (P) regimens, in terms of time to conversion. 188 patients with onset of AF within 48 hours were treated with propafenone (P): 48 received 600 mg as a first oral dose followed after 8 hrs by 150 mg (Group A); 82 received 300 mg as a first dose, followed by the same dose 3 and again 8 hrs later (B); 58 received 150 mg every 3 hrs, up to a total dose of 600 mg (C). P was stopped when sinus rhythm was achieved. Rates of conversion to sinus rhythm after 3 hrs in the 3 groups were: 46%, 41% and 26% respectively; after 8 hrs: 77%, 78% and 70%; and after 12 hrs: 81%, 84% and 76%. Treatment was discontinued in 8. There was excessive QRS widening (> 25% of the basal value) in 1 in group A, 1 in group B and 2 in group C; wide-QRS tachycardia occurred in 4 in group B. In Group A there was a higher rate of early successful conversion, with a lower incidence of side-effects than with the other regimens.

A 37-year-old woman complained of fatigue and dizziness because of intermittent sinus arrest and asystole up to 5.2 seconds. She was 3 months into her pregnancy and a dual chamber permanent pacemaker was implanted by transesophageal echocardiographic guidance.

Background and Aims: Nitrate therapy improves hemodynamics in patients with heart failure, but the chronic effects of oral nitrates on exercise performance and clinical status have not been well studied., Methods: Oral isosorbide-5-mononitrate (ISMN) (50 mg once daily) or placebo was administered to 136 patients (NYHA Class 2-3) treated for heart failure, all receiving captopril and most also furosemide. Endpoints were treadmill exercise time at 12 weeks by modified Naughton protocol (primary), with an additional 12-week follow-up period. Secondary endpoints included left ventricular dimensions, ejection fraction, cardiothoracic ratio, functional class, quality of life, hospitalizations and plasma norepinephrine and atrial natriuretic peptide in a four-center substudy., Results: Intention-to-treat analysis showed that mean change in treadmill exercise duration tended to be greater in patients receiving ISMN than placebo (treatment difference +42 s, 95% CI -5, +90 s at 12 weeks and +21 s, 95% CI -25, +74 s after 24 weeks) (NS). Treatment difference was greater in the prespecified subgroup with ejection fraction 31-40% (+55 s, 95% CI -11, +136 s at 12 weeks and +65 s, 95% CI +3, +147 s) (p = 0.035) at 24 weeks. No deleterious effects (i.e. hypotension) were observed with ISMN, although headache was reported in 19% of the active treatment group (p = 0.0001)., Conclusions: ISMN added to captopril increased treadmill exercise time in patients with heart failure and a lesser reduction in baseline ejection fraction, although for the group as a whole, the increase in treadmill time was not significant.

Lead insulation material and implant route have a major impact on lead reliability and durability. We compare the incidence of lead insulation failure resulting from both the venous approach and insulation type. Two hundred ninety consecutive leads were followed for a mean period of 57 +/- 30 months; leads with < 1 year follow-up were excluded. There were 116 Silicone Rubber insulated leads and 174 with polyurethane (151 Pellethane 80A and 23 Pellethane 55D) insulation; 279 leads were bipolar and 11 unipolar; 274 leads were implanted in the ventricle and 66 in the atrium. The venous route was the subclavian vein for 170 leads (58%) and the cephalic vein for 120 leads (42%). Insulation failure was diagnosed when a single sign of oversensing, undersensing, failure to capture, early pulse battery depletion, and lead impedance < 250 omega was present. Measurement of lead impedance was performed intraoperatively at implantation and during lead revision or pulse generator replacement. Lead failure caused by conductor coil fracture was not considered. There were 13 lead insulation failures, all among leads with polyurethane insulation (12 Pellethane 80A and 1 Pellethane 55D). Eleven failures (10%) occurred when the subclavian vein and 2 (3%) when the cephalic vein approach was used. The cumulative survival rate of polyurethane and silicone rubber insulated leads was 88.7% and 100%, respectively (P = 0.02); the cumulative survival rate of polyurethane insulated leads was 83.2% when the subclavian vein and 95.1% when the cephalic vein were used (P = 0.03). The mean time to polyurethane lead failure when the subclavian vein approach was used was 54 +/- 17 months and when the cephalic route was 73 +/- 4 months (P < 0.02). By multivariate analysis, the route of entry was found to be a significant variable related to polyurethane insulated lead failure (P < 0.05). At lead revision failure to capture was present in 7, oversensing in 4, and undersensing in 2 instances; impedance was < 250 omega in all cases. Pellethane 80A insulated leads are prone to insulation failure, but more when the subclavian vein is used, rather than the cephalic vein.

A 78-year-old patient complained of syncope, near syncope, and dizziness when eating; complete atrioventricular block, with ventricular asystole for 3.6 seconds, was recorded on continuous electrocardiographic monitoring. Left and right carotid sinus massage produces sinus arrest and ventricular asystole lasting 6.4 and 4.8 seconds, respectively. These phenomena were prevented by atropine administration. The symptoms were completely relieved by permanent pacing, but the patient died 6 months later because of large cell undifferentiated carcinoma of the lower third of the esophagus.

A 35-year-old patient underwent permanent pacemaker implantation because of symptomatic sinus bradycardia. During the procedure, persistent left superior vena cava was found. The ventricular lead crossed the tricuspid valve only after curving the stylet to form a loop in the right atrium (RA); subsequently, the curved stylet was changed to a straight one and the lead was positioned and screwed into the right ventricular apex. The atrial lead positioning was possible when the stylet was slightly curved and the lead could reach the anterior wall of the RA. At 18 months, a follow-up revealed normal pacemaker function and stable lead position.

Purpose: The aim of this study was to examine cell-to-cell metabolite transfer and connexin distribution in the rabbit corneal epithelium, in the stationary state, and during wound healing., Methods: Rabbit corneas were wounded with a surgical tool, producing a 3-mm-wide elongated debridement. Corneas were allowed to heal in vivo for up to 45 hours. Monoclonal antibodies against connexins Cx 26, Cx 32, Cx 43, and Cx 50 were used to stain cryostat sections. Cell-to-cell metabolite transfer capacity was assessed by a modification of the scrape-loading technique using lucifer yellow as the organic ion tracer., Results: The rabbit corneal epithelium contains Cx 43 and Cx 50, localized in the cell's plasma membrane, as shown previously for other species. Cx 26 and Cx 32 are not detectable. Tracer transfer occurred in both basal and suprabasal cell layers. After wounding, the migrating epithelial monolayer lacked Cx 43 and Cx 50. This change was apparent 6 hours after injury and persisted until complete wound closure (approximately 24 hours). The Cx 50 membrane stain was increased elsewhere, in particular in the transition zone between monolayered and multilayered epithelium. Consistent with the expression changes, migrating cells displayed no or minimal cell-to-cell tracer transfer, whereas in the periphery of the wound, tracer transfer was enhanced in comparison to the control specimen., Conclusions: Corneal epithelial healing involves biphasic changes in the expression of connexins and cell-to-cell communications. These alterations may be critical for the optimization of the healing response.

Since March 1990 we performed 100 balloon mitral valvuloplasties (BMV) in 82 females and 18 males (mean age 37 +/- 1 years; range 16 - 81) Initially we used the single shaft, dual or triple balloon system, while during the past 3 years we have been using the Inoue balloon system with step-wise inflation, monitoring with trans-thoracic echo (TTE) for immediate evaluation of mitral valve area (MVA) and/or severity of mitral regurgitation (MR), with 93% technical success. Hemodynamic data before and immediately after the procedure are in table below: [table: see text] C.O. cardiac output, DGR diastolic gradient, LAP left atrial pressure, MVA mitral valve area; all differences significant, p < 0.05. Acute complications included cardiac tamponade in 4/100; severe, acute MR in 2/100; only 5 were referred for urgent surgery. Neither cardiac tamponade nor severe MR were noted in the past 3 years. There was no periprocedural mortality in the past 5 years. Of 93 patients followed for 27 +/- 6 months (range 0-60), 90 (96%) were in NYHA classes I and II, 5 had late MVR, 2 underwent repeated BMV, and there was 1 death 16 months after the procedure. Immediate hemodynamic improvement followed BMV in most patients with pliable mitral stenosis. The Inoue system, with step-wise inflation and monitoring by TTE, proved to be a safe procedure. Symptomatic improvement continues during more than 4 years of follow-up.

Objectives: This study sought to establish the risk ratio for mortality associated with calcium antagonists in a large population of patients with chronic coronary artery disease., Background: Recent reports have suggested that the use of short-acting nifedipine may cause an increase in overall mortality in patients with coronary artery disease and that a similar effect may be produced by other calcium antagonists, in particular those of the dihydropyridine type., Methods: Mortality data were obtained for 11,575 patients screened for the Bezafibrate Infarction Prevention study (5,843 with and 5,732 without calcium antagonists) after a mean follow-up period of 3.2 years., Results: There were 495 deaths (8.5%) in the calcium antagonist group compared with 410 in the control group (7.2%). The age-adjusted risk ratio for mortality was 1.08 (95% confidence interval [CI] 0.95 to 1.24). After adjustment for the differences between the groups in age and gender and the prevalence of previous myocardial infarction, angina pectoris, hypertension, New York Heart Association functional class, peripheral vascular disease, chronic obstructive pulmonary disease, diabetes and current smoking, the adjusted risk ratio declined to 0.97 (95% CI 0.84 to 1.11). After further adjustment for concomitant medication, the risk ratio was estimated at 0.94 (95% CI 0.82 to 1.08)., Conclusions: The current analysis does not support the claim that calcium antagonist therapy in patients with chronic coronary artery disease, whether myocardial infarction survivors or others harbors an increased risk of mortality.

In this evolving era of balloon mitral valvotomy (BMV), radiofrequency ablation (RF) of left-sided bypass tracts via catheter, and hemodynamic evaluation of aortic mechanical prostheses, there has been renewed interest in transseptal left heart catheterization (TSLHC). In the 3 years 1990-1994, 122 consecutive patients were referred for TSLHC to our institute (which lacks thoracic surgical facilities). 12 patients were excluded; 10 with a LA mass proven by transesophageal echocardiography (TEE), 1 with a vascularized thrombus in the circumflex coronary system and 1 with congenital interruption of the inferior vena cava with azygous continuity. In the remaining 110 cases TSLHC was performed for interventions in 90 cases (82%) of BMV, and for left-sided catheter radiofrequency (RF) ablation in 3 (3%). For diagnostic purposes it was performed in 17 (15%) cases for hemodynamic evaluation of mechanical aortic valve prostheses. Using the Brockenbrough needle, the adult Mullins sheath system (MSS) and single plane fluoroscopy, 100% technical success was achieved. Needle puncture was not needed in 30 (27%) due to direct crossing with the MSS through a stretched foramen ovale. In 2 we had to perform SVC dye injection for better interatrial septum localization. There were no complications when TSLHC was only used for diagnostic procedures. 1 patient had perforation of the LA due to right lower pulmonary vein laceration following septal dilatation. Following stabilization by immediate pericardiocentesis, the patient was transferred for open heart surgery. There were no great vessel perforations, systemic embolization or periprocedural deaths. TSLHC can be performed quite safely with single-plane fluoroscopy without an onsight surgical team, as with an experienced staff this procedure has very low morbidity and mortality.

We compared the acute hemodynamic effects of captopril and nitrates in 11 patients with severe congestive heart failure and grade IV cardiac disability. Pressures were measured using a Swan-Ganz catheter system; cardiac output and stroke index were measured by thermodilution, and left-ventricular (LV) volumes and ejection fraction were calculated simultaneously with the hemodynamic measurements from radionuclide ventriculography. Measurements were made in each of 4 treatment states: control, sublingual isosorbide dinitrate (ISDN; 5 and 15 mg), oral captopril (50-200 mg daily) and during combined therapy with captopril and ISDN. Captopril produced a fall in mean arterial pressure (p < 0.01) from 81 +/- 14 to 72 +/- 13 mm Hg and a rise in stroke index from 30 +/- 5 to 35 +/- 91/min/m2 (p < 0.05), while LV ejection fraction increased from 18 +/- 5 to 21 +/- 7% (p < 0.05). ISDN reduced mean arterial, pulmonary arterial, right-atrial and wedge pressure. The combination of captopril and ISDN produced a greater fall in mean arterial pressure, a further rise in ejection fraction to 22 +/- 8% (p < 0.05), a fall in systemic (p < 0.05) and pulmonary vascular resistance (p < 0.01) and a rise in cardiac (p < 0.01) and stroke work index (p < 0.01), while the beneficial effects of ISDN on right-atrial, pulmonary arterial and wedge pressure were again achieved. LV contractility, assessed from end-systolic stress-shortening relations, was essentially unaltered or decreased very slightly. The study showed that combined therapy with captopril and nitrates produced acute hemodynamic benefits superior to those achieved by treatment with captopril or nitrates alone.

A 60-year-old patient was admitted for elective replacement of a depleted pulse generator. The pacemaker was implanted 5 years before because of sick sinus syndrome and it was connected to an epicardial lead due to total occlusion of the superior vena cava. The pacing threshold of the epicardial electrode was unacceptably high, so an endocardial lead was inserted through the iliac vein. The lead was connected to a VVIR pacemaker, which was located in the abdomen just lateral to the umbilicus. During a 1-year follow-up period, the patient felt well and pacemaker performance was satisfactory.

A Soviet nuclear pacemaker is described. It is a large VOO bipolar pacemaker, probably nonprogrammable. It has unique electrode connectors. No isotope leak or excessive radiation hazard was detected. Because we had no knowledge of its end-of-life markers and because of the impossibility of assuming long-term safety, the device was removed.

A 26-year-old woman with severe pliable mitral stenosis underwent successful balloon mitral valvuloplasty at the end of the second trimester of pregnancy. The indication for intervention was severe shortness of breath during most daily activities, despite combined beta-blocker and diuretic treatment (FC III, NYHA). After use of a 2 x 19 bifoil balloon there was significant clinical and hemodynamic improvement. Shortness of breath disappeared, the mean mitral valve diastolic gradient decreased from 24 to 7 mmHg and the mitral valve area increased from 0.8 to 1.8 cm. There were no complications of the treatment, and estimated radiation exposure of the fetus was less than 0.2 rad. Subsequent abdominal ultrasound examination revealed normal fetal functioning. 2.5 months after mitral dilatation delivery was normal. Balloon mitral valvuloplasty may safely be used instead of surgery as a palliative procedure for relief of symptoms in pregnant women with severe pliable mitral stenosis.

25 patients with end-stage renal failure were studied by different echocardiographic techniques before and immediately after hemodialysis using M-mode, 2-dimensional and Doppler echocardiography. Preload reduction after dialysis was manifested by a decrease in body weight, in left atrial diameter and in the left ventricular end-diastolic dimension. Doppler flow patterns immediately after dialysis showed reduction in early diastolic velocities across the atrioventricular valves and increase in the flow velocities across the semilunar valves and the peripheral A-V fistula. The flow velocity integral, calculated by Doppler echocardiography, was lower after dialysis. Significant improvement in left ventricular myocardial function after dialysis was correlated with reduction in afterload and increase in myocardial contractility.

Myocarditis may present with a variety of electrocardiographic patterns. However, polymorphous ventricular tachycardia (PVT), potentially a highly malignant ventricular arrhythmia, has not been described in acute myocarditis. As far as we know, this is the first report of this arrhythmia in that condition. It is characterized by unusual resistance to both the common pharmacological and nonpharmacological approaches. This report of a 28-year-old woman demonstrates the role of PVT as a potential cause for sudden death among patients with subclinical myocarditis.
