Abstract

In a comment by A.A. Zvyagin the phase diagram in our Letter [Phys. Rev. Lett. 86, 516 (2001)] was critisized of being incomplete and a new fixed point was suggested. Abstract

We consider a magnetic s=1/2 impurity in the antiferromagnetic spin chain as a function of two coupling parameters: the symmetric coupling of the impurity to two sites in the chain $J_1$ and the coupling between the two sites $J_2$. By using field theory arguments and numerical calculations we can identify all possible fixed points and classify the renormalization flow between them, which leads to a non-trivial phase diagram. Depending on the detailed choice of the two (frustrating) coupling strengths, the stable phases correspond either to a decoupled spin with Curie law behavior or to a non-Fermi liquid fixed point with a logarithmically diverging impurity susceptibility as in the two channel Kondo effect. Abstract

We analyze the spin- and charge-density oscillations near impurities in spin chains and quantum wires. These so-called Friedel oscillations give detailed information about the impurity and also about the interactions in the system. The temperature dependence of these oscillations explicitly shows the renormalization of backscattering and conductivity, which we analyze for a number of different impurity models. We are also able to analyze screening effects in one dimension. Abstract

The phase separation boundary of isotropic t-J ladders is analyzed using density matrix renormalization group techniques. The complete boundary to phase separation as a function of J/t and doping is determined for a chain and for ladders with two, three and four legs. Six-chain ladders have been analyzed at low hole doping. We use a direct approach in which the phase separation boundary is determined by measuring the hole density in the part of the system which contains both electrons and holes. In addition we examine the binding energy of multi-hole clusters. An extrapolation in the number of legs suggests that the lowest J/t for phase separation to occur in the two dimensional t-J model is J/t~1., Comment: 8 pages in revtex format including 13 embedded figures, one reference added

We use the density matrix renormalization group (DMRG) for transfer matrices to numerically calculate impurity corrections to thermodynamic properties. The method is applied to two impurity models in the spin-1/2 chain, namely a weak link in the chain and an external impurity spin. Abstract

A magnetic impurity in the spin-1/2 chain is a simple realization of the two-channel Kondo problem since the field theoretical descriptions in the spin-sector are identical. The correlation functions near the impurity can be calculated. Using a modified version of the numerical transfer matrix DMRG, we are able to accurately determine local properties close to the impurity in the thermodynamic limit. The local susceptibilities (Knight-shifts) show an interesting behavior in a large range around the impurities. We are able to make quantitative experimental predictions which would allow to observe two-channel Kondo physics for the first time directly by doping of spin-1/2 chain compounds., Comment: 2 pages in revtex format including 2 embedded figures (using epsf)

Abstract

We consider a magnetic impurity in the antiferromagnetic spin-1/2 chain which is equivalent to the two-channel Kondo problem in terms of the field theoretical description. Using a modification of the transfer-matrix density matrix renormalization group (DMRG) we are able to determine local and global properties in the thermodynamic limit. The cross-over function for the impurity susceptibility is calculated over a large temperature range, which exhibits universal data-collapse. We are also able to determine the local susceptibilities near the impurity, which show an interesting competition of boundary effects. Abstract

We investigate the density matrix renormalization group (DMRG) discovered by White and show that in the case where the renormalization eventually converges to a fixed point the DMRG ground state can be simply written as a ``matrix product'' form. This ground state can also be rederived through a simple variational ansatz making no reference to the DMRG construction. We also show how to construct the ``matrix product'' states and how to calculate their properties, including the excitation spectrum. Abstract

The density matrix renormalization group (``DMRG'') discovered by White has shown to be a powerful method to understand the properties of many one dimensional quantum systems. In the case where renormalization eventually converges to a fixed point we show that quantum states in the thermodynamic limit with periodic boundary conditions can be simply represented by a special type of product ground state with a natural description of Bloch states of elementary excitations that are spin-1 solitons. We then observe that these states can be rederived through a simple variational ansatz making no reference to a renormalization construction. Abstract

Multiple sclerosis (MS) is the most common neurological immune-mediated disease leading to disability in young adults. The outcome of the disease is unpredictable, and over time, neurological disabilities accumulate. Interferon beta-1b was the first drug to be approved in the 1990s for relapsing-remitting MS to modulate the course of the disease. Over the past two decades, the treatment landscape has changed tremendously. Currently, more than a dozen drugs representing 1 substances with different mechanisms of action have been approved (interferon beta preparations, glatiramer acetate, fingolimod, siponimod, mitoxantrone, teriflunomide, dimethyl fumarate, cladribine, alemtuzumab, ocrelizumab, and natalizumab). Ocrelizumab was the first medication to be approved for primary progressive MS. The objective of this review is to present the modes of action of these drugs and their effects on the immunopathogenesis of MS. Each agent's clinical development and potential side effects are discussed.

Abstract

Several concerns have been raised about the use of immunodepleting agents including alemtuzumab, cladribine and CD20-depleting antibodies in people with multiple sclerosis (pwMS) during the coronavirus disease (COVID) 2019 pandemic. As the end of the pandemic is not yet in sight, vaccination against severe acute respiratory syndrome coronavirus type 2 (SARS-CoV-2) may be an elegant strategy to overcome the potential hazards associated with initiating and continuing treatment with immune-depleting agents. In this review, we summarize the immunological effects of immune-depleting therapy and underlying considerations for the hitherto existing recommendations that suggest a restricted use of immune-deleting therapies during the pandemic. Moreover, we critically discuss open questions regarding vaccination in general and against SARS-CoV-2 in pwMS.

O Contrato Organizativo de Ação Pública na Saúde foi instituído pelo Decreto Presidencial nº 7.508/2011 com o propósito de servir como um contrato entre entes políticos no âmbito de uma Região de Saúde. Ele se propõe a definir as responsabilidades de cada ente assumidas pelos respectivos chefes de executivo e secretários de saúde, além do Ministério da Saúde. Entretanto, suas características estão ora mais próximas de um convênio ou consórcio, ora mais próximas de um contrato. A força e a necessidade desse novo instrumento são discutidas, tendo em vista a existência de outros instrumentos já previstos para a regionalização cooperativa. Para o presente estudo foi realizada uma revisão bibliográfica sobre o tema e uma análise da legislação relacionada e confecção de uma minuta de contrato proposta para as regiões de saúde.

Abstract

Summary: Malignant carcinoma metastasis to the skeleton is the third most common site of metastasis after lung and liver, usually involving the axial skeleton and the proximal ends of long bones, ribs, and vertebrae. Acrometastasis, metastasis to hands or feet, comprises only 0.1% of all metastases. Here, we present 2 cases of acrometastasis, one from hepatocellular carcinoma and the other from renal cell carcinoma. We describe the presentation, radiographs, surgical treatment, and outcomes for each patient. Patients presented with swelling and pain at the tumor sites and were treated with amputations. The second patient’s disease progressed resulting in death shortly after amputation indicating acrometastasis may be a poor prognostic indicator of survival. We review the literature and discuss the importance of disease process recognition and prognosis.

Abstract

Extracorporeal membrane oxygenation (ECMO) has been widely used in patients with COVID-19, but uncertainty remains about the determinants of in-hospital mortality and data on post-discharge outcomes are scarce. The aims of this study were to investigate the variables associated with in-hospital outcomes in patients who received ECMO during the first wave of COVID-19 and to describe the status of patients 6 months after ECMO initiation.

87. Multipath QUIC for Access Traffic Steering Switching and Splitting in 5G Advanced

Abstract

The proliferation of modern mobile devices equipped with several wireless interfaces (multi-homed), along with the increasing demand for high-throughput and ultra-reliable connectivity, motivated 3GPP to specify a solution that enables multi-access communication, called access traffic steering, switching, and splitting (ATSSS). Based on this solution, a mobile device can exchange data traffic with the 5G core network by simultaneously using different access networks (e.g., WiFi and 5G-NR). Consequently, data flows can enjoy aggregated bandwidth and also reduced delay and increased reliability. In this article, we briefly present the key concepts and functionality of ATSSS, and we focus on the ATSSS enhancements considered for 5G Advanced (i.e., in 3GPP Rel-18). In particular, we present a new steering functionality that is based on the QUIC protocol and its multipath extensions. We discuss the motivation for this steering functionality, its features and user-plane operation, and we explain how it can be applied to proxy UDP traffic over HTTP.

93. Bruton's Tyrosine Kinase Inhibition in the Treatment of Preclinical Modelsand Multiple Sclerosis

Abstract

Significant progress has been made to understand the immunopathogenesis of multiple sclerosis (MS)over recent years. Successful clinical trials with CD20-depleting monoclonal antibodies have corroboratedthe fundamental role of B cells in the pathogenesis of MS and reinforced the notion that cells of the B cell lineageare an attractive treatment target. Therapeutic inhibition of Bruton's tyrosine kinase (BTK), an enzyme involvedin B cell and myeloid cell activation and function, is regarded as a next-generation approach that aims toattenuate both errant innate and adaptive immune functions. Moreover, brain-penetrant BTK inhibitors may impactcompartmentalized inflammation and neurodegeneration within the central nervous system by targetingbrain-resident B cells and microglia, respectively. Preclinical studies in animal models of MS corroborated animpact of BTK inhibition on meningeal inflammation and cortical demyelination. Notably, BTK inhibition attenuatedthe antigen-presenting capacity of B cells and the generation of encephalitogenic T cells. Evobrutinib,a selective oral BTK inhibitor, has been tested recently in a phase 2 study of patients with relapsing-remittingMS. The study met the primary endpoint of a significantly reduced cumulative number of Gadolinium-enhancinglesions under treatment with evobrutinib compared to placebo treatment. Thus, the results of ongoing phase2 and 3 studies with evobrutinib, fenobrutinib, and tolebrutinib in relapsing-remitting and progressive MS areeagerly awaited. This review article introduces the physiological role of BTK, summarizes the pre-clinical andtrial evidence, and addresses the potential beneficial effects of BTK inhibition in MS.

Abstract

There are few diseases where as much therapeutic progress has been made in recent years as in multiplesclerosis. Nine different drug classes with more than a dozen approved therapies are now available. Similarly,there have been unimaginable advances in understanding neuromyelitis optica (now neuromyelitis opticaspectrum disorder [NMOSD]) over the past 15 years. Building on the knowledge gained, the first therapieshave been approved in recent years. In this review, we aim to present all therapies approved for the treatment ofMS or NMOSD. The different forms of application, different approval criteria and most important side effectswill be presented. This work is intended for physicians who are interested in MS and NMOSD therapies andwant to get a first overview and does not replace the respective guidelines of the regulatory authorities.

Abstract

Background: To assess the incidence rate and prevalence ratio of multiple sclerosis (MS) in Austria. Methods: Hospital discharge diagnosis and MS-specific immunomodulatory treatment prescriptions from public health insurances, covering 98% of Austrian citizens with health insurance were used to extrapolate incidence and prevalence numbers based on the capture-recapture method. Results: A total of 1,392,629 medication prescriptions and 40,956 hospitalizations were extracted from 2 data sources, leading to a total of 13,205 patients. The incidence rate and prevalence ratio of MS in Austria based on the capture-recapture method were 19.5/100,000 person-years (95% CI 14.3-24.7) and 158.9/100,000 (95% CI 141.2-175.9), respectively. Female to male ratio was 1.6 for incidence and 2.2 for prevalence. Conclusions: Incidence rates and prevalence ratios of MS in our study are within the upper range of comparable studies across many European countries as well as the United States.
