En este estudio se aborda la influencia que pueden tener herramientas de gamificación y comunicación en el proceso educativo, más concretamente en la etapa de la Educación Primaria. Se presenta la plataforma y aplicación ClassDojo como una de las más atractivas para ello y con una aportación motivacional bastante relevante en el alumnado. A través de este estudio realizaremos una visión pormenorizada de las diferentes funcionalidades que nos otorga la herramienta para ello. This study addresses the influence that gamification and communication tools may have on the educational process, more precisely in the stage of Primary Education. The ClassDojo platform and application is presented as one of the most attractive for this field and with a very relevant motivational contribution in the students. Through this study we will carry out a a detailed view of the different functionalities that the tool gives us.

Objective: The objective was to determine if dose reduction is non-inferior to full-dose TNFi to maintain low disease activity (LDA) in patients already in remission with TNFi, in axial spondyloarthritis. Methods: Randomized, parallel, non-inferiority, open-label multicentre clinical trial. Patients were eligible if they had axial spondyloarthritis and had been in clinical remission for ≥ 6 months with any available TNFi (adalimumab, etanercept, infliximab, golimumab) at the dose recommended by product labelling. Patients were randomized by automated central allocation to continue the same TNFi dose schedule, or to reduce the dose by roughly half according to the protocol. The main outcome was the proportion of subjects with LDA after 1 year. Serious adverse reactions or infections were recorded. Results: The trial stopped due to end of the funding period, after 126 patients were randomized; 113 patients (84.1% male, mean age (SD) 45.6 (13.0) years) were included in the main per-protocol subset. Non-inferiority was concluded for LDA at 1 year (47/55 (83.8%) patients in the full-dose and 48/58 (81.3%) patients in the reduced-dose arm, adjusted difference (95% CI) - 2.5% (- 16.6% to 11.7%)). Serious adverse reactions or infections were reported in 7/62 patients (11.3%) assigned to full dose and 2/61 patients (3.3%) assigned to reduced dose (p value = 0.164). Conclusion: In patients with ankylosing spondylitis in clinical remission for at least 6 months, dose reduction is non-inferior to full TNF inhibitor doses to maintain LDA after 1 year. Serious adverse events may be less frequent with reduced doses. The aims of this work has been (1) to design and implement a program for learning to play the guitar in the Degree of Early Childhood Education through the use of video tutorials and (2) explore the perceptions of the students and teachers about that program. Its methodological approach is based on the viewing of video tutorials before and after each of the six sessions of which this program consists. The sample of participants of the study was formed by the students of the last year of the Degree in Early Childhood Education of the University of Murcia (N=167) and two teachers who put this program into practice. For the data collection an ad hoc questionnaire was designed for the students and an interview for the teachers. The results revealed that the participants give a fundamental role to the video tutorials in the learning process developed in this program. Likewise, 75% of students verified that their initial expectations were met and they felt competent to accompany songs with the guitar in the Preschool classroom., Los objetivos de este estudio han sido (1) diseñar e implementar un programa para el aprendizaje de la guitarra en el Grado en Educación Infantil a través de videotutoriales y (2) explorar las percepciones del alumnado y profesorado acerca de dicho programa. Su planteamiento metodológico se fundamenta en el visionado de videotutoriales antes y después de cada una de las seis sesiones de las que consta dicho programa El conjunto de participantes del estudio estuvo formado por el alumnado del último curso del Grado en Educación Infantil de la Universidad de Murcia (N=167) y dos profesores que llevaron a la práctica dicho programa. Para la recogida de información se elaboró un cuestionario ad hoc destinado al alumnado y una entrevista para los profesores. Los resultados revelaron que los participantes otorgan un papel fundamental a los videotutoriales en el proceso de aprendizaje desarrollado en este programa. Asimismo, el 75% del alumnado constató sentir cumplidas sus expectativas iniciales y se sintieron competentes para acompañar canciones con la guitarra en el aula de Educación Infantil. Objective Proprotein convertase subtilisin/kexin type 9 (PCSK9) is a serine protease that regulates cholesterol metabolism and has been linked to cardiovascular (CV) risk. The purpose of the present study was to examine whether PCSK9 levels are related to abnormalities in the lipid profile and the development of atherosclerosis that occurs in patients with axial SpA (axSpA). Methods We performed a cross-sectional study that encompassed 545 individuals; 299 patients with axSpA and 246 statin use–matched controls. PCSK9 and standard lipid profiles were analysed in patients and controls. Carotid intima-media thickness (cIMT) and carotid plaques were assessed in patients. A multivariable analysis, adjusted for standard CV risk factors, was performed to evaluate the influence of PCSK9 on axSpA-related dyslipidaemia and subclinical carotid atherosclerosis. Results Total cholesterol, high-density lipoprotein and low density lipoprotein cholesterol, lipoprotein (a) and apolipoprotein A1 were significantly lower in axSpA patients than controls. PCSK9 serum levels [β coefficient −44 ng/dl (95% CI −60, −27), P = 0.000] were also downregulated in axSpA patients after fully multivariable adjustment. ASDAS-CRP was found to be independently and significantly related to PCSK9 [β coefficient 10 ng/dl (95% CI 1, 18), P = 0.023] after analysing fully adjusted models that took age, sex and the rest of the lipid profile molecules into account. Whereas patients taking prednisone showed higher serum levels of PCSK9 [55 ng/ml (95% CI 24, 8), P = 0.001], those under anti-TNF-α therapies exhibited lower levels [β coefficient −26 ng/ml (95% CI −43, −9], P = 0.003]. Conclusion PCSK9 is downregulated in patients with axSpA. Disease activity is positive and significantly related to PSCK9. Anti-TNF-therapy yields a reduction in PCSK9 serum levels. - Abstract
[Abstract] Objectives: To explore the remission concept in rheumatoid arthritis (RA) and to compare remission definitions and related concepts between rheumatologists and patients with the purpose of identifying similarities and disparities to comprehend the different perspectives of the disease. Methods: This was a qualitative study of discourse and content analysis through focus groups, conducted from February to March 2016. Four focus groups were set up, each one with different interests: rheumatologists involved in basic research (BR), rheumatologists with high specialisation in imaging techniques (IR), clinical rheumatologists (CR), and patients (PA). Results: There is no consensus in a remission definition in RA; differences exist between-groups, rheumatologists and patients value remission differently, and there are discrepancies within the group of rheumatologists. Rheumatologists highlight quantifiable objective parameters, in contrast, patients did not consider objective measures as the best instruments, and they prefer subjective measures of remission. The data confirmed the existence of two sources of knowledge of the disease, technical (physicians) and experiential (patients). These sources of knowledge should concur in order to establish new remission criteria well-adjusted to reality. Conclusions: The lack of consensus between key groups implicated in defining remission and remission criteria suggests a new strategy for its operational definition. Our group proposes that subjects with a balance between experiential and technical knowledge, should be the ones in charge of this assignment.

En esta tesis se muestra el proceso de diseño e implementación de videotutoriales para el aprendizaje de la guitarra en alumnos de Grado de Educación Infantil. Actualmente, diferentes factores temporales, logísticos y motivacionales en el aula dificultan el aprendizaje y la predisposición del alumnado hacia la interpretación instrumental. Es por ello que, con la finalidad de encontrar soluciones que faciliten y motiven el proceso de aprendizaje de este instrumento, encontramos en el videotutorial una herramienta 2.0 de gran utilidad. Los objetivos planteados en esta investigación son los siguientes: • Analizar las posibilidades pedagógicas de los videotutoriales en contextos de Educación Superior. • Diseñar e implementar videotutoriales para el aprendizaje de la guitarra destinados a la formación inicial del profesorado de Educación Infantil. • Conocer la percepción y valoración que los alumnos tienen respecto al aprendizaje instrumental de la guitarra a través del uso de los videotutoriales. En esta investigación, predomina un enfoque descriptivo orientado al diseño e implementación de un Programa de Intervención compuesto de 37 videotutoriales destinados al aprendizaje de la guitarra de 193 alumnos de cuarto curso del Grado de Educación Infantil de la Facultad de Educación (Universidad de Murcia). Para llevar a cabo la evaluación de nuestra investigación se han diseñado dos cuestionarios dirigidos a todos los alumnos participantes, uno inicial y otro final. De igual forma, se ha llevado a cabo una entrevista a los profesores encargados de llevar a cabo este proyecto en sus respectivas aulas. Los resultados revelan una gran aceptación por parte del alumnado de la utilización del videotutorial como herramienta facilitadora del proceso de aprendizaje de la guitarra en Educación Superior. Finalmente, las conclusiones ponen de manifiesto un aumento de la predisposición hacia el aprendizaje de la guitarra en el alumnado de Grado de Educación Infantil., In this thesis the design and implementation processes of video tutorials for learning guitar in the Degree of Kindergarten students are shown. Currently, different temporary, logistical and motivational factors hinder the learning and willingness of students towards instrumental performance. That is why, in order to find solutions that facilitate and encourage the learning process of this instrument, we find the 2.0 video tutorial a very useful tool. The objectives in this research are: • To analyse the educational possibilities of video tutorials in the contexts of Higher Education. • To design and implement video tutorials for learning guitar in the initial training of teachers of Early Childhood Education. • To know the perception and appreciation that students have regarding instrumental guitar learning through the use of video tutorials. In this research there is a predominantly descriptive approach, oriented to the design and implementation of an intervention programme, composed of 37 video tutorials for learning guitar by 193 students in the fourth year of the Degree of Kindergarten, Faculty of Education (University of Murcia). To carry out the evaluation of our research we have designed two questionnaires directed to all the participating students, both at the beginning and at the end, of the project. Similarly, an interview was conducted with the teachers in charge of carrying out this project in their respective classrooms. The results reveal a great acceptance, by the students, of the use of video tutorials as a facilitator tool for the process of learning guitar in Higher Education. Finally, the findings show an increased predisposition towards learning guitar in the Degree of Kindergarten.

- Abstract
To evaluate the impact of comorbidities on physical function in patients with ankylosing spondylitis (AS) and psoriatic arthritis (PsA). This was a cross‐sectional analysis of the baseline visit from the Cardiovascular in Rheumatology study. Multivariate models with physical function as the dependent variable (Bath Ankylosing Spondylitis Functional Index and Health Assessment Questionnaire for AS and PsA, respectively) were performed. Independent variables were a proxy for the Charlson Comorbidity Index (CCIp; range 0–27), sociodemographic data, disease activity (erythrocyte sedimentation rate [ESR] and Bath Ankylosing Spondylitis Disease Activity Index [BASDAI] in AS; Disease Activity Score in 28 joints [DAS28] using the ESR in PsA), disease duration, radiographic damage, and treatments. Results were reported as beta coefficients, 95% confidence intervals (95% CIs), and Pvalues. We included 738 patients with AS and 721 with PsA; 21% of patients had >1 comorbidity. Comorbidity burden (CCIp) was independently associated with worse adjusted physical function in patients with PsA (β = 0.11). Also, female sex (β = 0.14), disease duration (β = 0.01), disease activity (DAS28‐ESR; β = 0.19), and the use of nonsteroidal antiinflammatory drugs (β = 0.09), glucocorticoids (β = 0.11), and biologics (β = 0.15) were associated with worse function in patients with PsA. A higher education level was associated with less disability (β = –0.14). In patients with AS, age (β = 0.03), disease activity (BASDAI; β = 0.81), radiographic damage (β = 0.61), and the use of biologics (β = 0.51) were independently associated with worse function on multivariate analyses, but CCIp was not. The presence of comorbidities in patients with PsA is independently associated with worse physical function. The detection and control of the comorbidities may yield an integral management of the disease.

[SPA] Este trabajo presenta una experiencia de innovación docente cuyo objetivo principal ha sido diseñar, implementar y evaluar videotutoriales para el aprendizaje de la guitarra en contextos de Educación Superior. Nuestro proyecto se ha desarrollado en la asignatura de Percepción y Expresión Musicales del Grado en Educación Infantil, que se imparte en la Facultad de Educación de la Universidad de Murcia. Los participantes (N=65) suponen 91,5% del total del grupo de clase seleccionado. Los materiales elaborados, además de videotutoriales, incluyen una página web, dossiers, partituras, archivos audio, así como diversos materiales disponibles en Internet. El proyecto se ha puesto en práctica durante el segundo cuatrimestre del curso 2012-2013. Para la evaluación de la experiencia se diseñaron dos cuestionarios, uno inicial y otro final, que recogen información sobre tres ámbitos de estudio: (1) Educación Musical en Educación Infantil; (2) actitudes del alumnado; y (3) valoración del curso y de los videotutoriales. Los resultados revelan un alto grado de satisfacción del alumnado con la experiencia y se observan algunos cambios en la percepción que este tiene sobre la docencia de la música en la etapa de Educación Infantil. [ENG] This paper presents an innovative educational experience whose main objective has been to design, implement and evaluate video tutorials for learning guitar in Higher Education contexts .Our project has been carry out in the university course Musical Perception and Expression within the Degree in Early Childhood Education, taught at the Faculty of Education at the University of Murcia. Participants (N = 65 ) represent 91.5 % of the selected class. The material developed, in addition to video tutorials, include a website, files, sheet music , audio files, as well as various materials available online. The project was implemented during the second semester of academic year 2012-2013. For the assessment of the experience two questionnaires have been elaborated, one initial and a final, which collect information on three áreas of study: (1 ) Music Education in Early Childhood, (2 ) attitudes of students and ( 3) assessment of the course and of the video tutorials. The results reveal a high degree of student satisfaction and some changes are observed in the perception that they have on teaching music in the kindergarten. - Abstract
Objective: Proprotein convertase subtilisin/kexin type 9 (PCSK9) is a serine protease that regulates cholesterol metabolism and has been linked to cardiovascular (CV) risk. The purpose of the present study was to examine whether PCSK9 levels are related to abnormalities in the lipid profile and the development of atherosclerosis that occurs in patients with axial SpA (axSpA)., Methods: We performed a cross-sectional study that encompassed 545 individuals; 299 patients with axSpA and 246 statin use-matched controls. PCSK9 and standard lipid profiles were analysed in patients and controls. Carotid intima-media thickness (cIMT) and carotid plaques were assessed in patients. A multivariable analysis, adjusted for standard CV risk factors, was performed to evaluate the influence of PCSK9 on axSpA-related dyslipidaemia and subclinical carotid atherosclerosis., Results: Total cholesterol, high-density lipoprotein and low density lipoprotein cholesterol, lipoprotein (a) and apolipoprotein A1 were significantly lower in axSpA patients than controls. PCSK9 serum levels [β coefficient -44 ng/dl (95% CI -60, -27), P = 0.000] were also downregulated in axSpA patients after fully multivariable adjustment. ASDAS-CRP was found to be independently and significantly related to PCSK9 [β coefficient 10 ng/dl (95% CI 1, 18), P = 0.023] after analysing fully adjusted models that took age, sex and the rest of the lipid profile molecules into account. Whereas patients taking prednisone showed higher serum levels of PCSK9 [55 ng/ml (95% CI 24, 8), P = 0.001], those under anti-TNF-α therapies exhibited lower levels [β coefficient -26 ng/ml (95% CI -43, -9], P = 0.003]., Conclusion: PCSK9 is downregulated in patients with axSpA. Disease activity is positive and significantly related to PSCK9. Anti-TNF-therapy yields a reduction in PCSK9 serum levels., (© The Author(s) 2020. - Abstract
Objectives: To explore the remission concept in rheumatoid arthritis (RA) and to compare remission definitions and related concepts between rheumatologists and patients with the purpose of identifying similarities and disparities to comprehend the different perspectives of the disease., Methods: This was a qualitative study of discourse and content analysis through focus groups, conducted from February to March 2016. Four focus groups were set up, each one with different interests: rheumatologists involved in basic research (BR), rheumatologists with high specialisation in imaging techniques (IR), clinical rheumatologists (CR), and patients (PA)., Results: There is no consensus in a remission definition in RA; differences exist between-groups, rheumatologists and patients value remission differently, and there are discrepancies within the group of rheumatologists. Rheumatologists highlight quantifiable objective parameters, in contrast, patients did not consider objective measures as the best instruments, and they prefer subjective measures of remission. The data confirmed the existence of two sources of knowledge of the disease, technical (physicians) and experiential (patients). These sources of knowledge should concur in order to establish new remission criteria well-adjusted to reality., Conclusions: The lack of consensus between key groups implicated in defining remission and remission criteria suggests a new strategy for its operational definition. Our group proposes that subjects with a balance between experiential and technical knowledge, should be the ones in charge of this assignment.

- Abstract
Objectives: To test the reliability of the Berlin MRI scoring method and the effect of a calibration exercise on the score's reliability among untrained readers in MRI examinations of patients with established ankylosing spondylitis (AS)., Methods: Eleven rheumatologists read blinded images of 20 AS patients before and after a two-day workshop on the Berlin MRI scoring method. Reliability (intra- and inter-reader) and concordance with the expert (all measured by intraclass correlation coefficient (ICC)) were compared before and after 2 weeks of the training. Feasibility in terms of time and difficulty was also measured., Results: The mean Berlin score increased from (mean ± standard deviation) 5.04 ± 6.41 before to 6.40±7.08 after the calibration exercise (p<0.01). Inter-reader ICC decreased from 0.83 (95% CI: 0.75-0.93) to 0.78 (95% CI: 0.66-0.90), and intra-reader ICC from 0.89 (95% CI: 0.84-0.94) to 0.87 (95% CI: 0.82-0.92). Agreement with an experienced reader improved after the calibration exercise, with ICC = 0.59 (95% CI 0.45-0.76) before vs. ICC = 0.65 (95% CI 0.50-0.80) after training., Conclusions: The Berlin method is a reliable scoring method for assessment of spinal inflammatory activity by using MRI in patients with AS, even in the hands of inexperienced readers. A calibration exercise can improve feasibility and sensitivity of the scoring method.

Objectives: This study aims to assess the impact of a structured education and home exercise programme in daily practice patients with ankylosing spondylitis., Methods: A total of 756 patients with ankylosing spondylitis (72% males, mean age 45 years) participated in a 6-month prospective multicentre controlled study, 381 of whom were randomised to an education intervention (a 2-hour informative session about the disease and the implementation of a non-supervised physical activity programme at home) and 375 to standard care (controls). Main outcome measures included Bath Ankylosing Spondylitis Disease Activity and Functional Index (BASDAI, BASFI). Secondary outcome measures were 0-10 cm visual analog scale (VAS) for total pain, nocturnal pain and global disease activity and quality of life (ASQoL), knowledge of disease (self-evaluation ordinal scale) and daily exercise (diary card)., Results: At 6 months, the adjusted mean difference between control and educational groups for BASDAI was 0.32, 95% confidence interval (CI) 0.10-0.54, p=0.005, and for BASFI 0.31, 95%CI 0.12-0.51, p=0.002. Significant differences were found also in VAS for total pain, patient´s global assessment and in ASQoL. Patients in the education group increased their knowledge about the disease and its treatments significantly (p<0.001) and practised more regular exercise than controls (p<0.001)., Conclusions: A structured education and home exercise programme for patients with ankylosing spondylitis in daily practice was feasible and helped to increase knowledge and exercise. Although statistically significant, the magnitudes of the clinical benefits in terms of disease activity and physical function were poor.

- Abstract
Background and Objectives: There are different guidelines for secondary prevention of fractures related with osteoporosis. Our aim is to analyse the appliance of such guidelines in a sample of patients with fragility fracture, Patients and Methods: Adult patients older than 50 years attended in the emergency department with a fragility fracture were invited to participate in a study for secondary prevention. Clinical data and densitometry for the FRAX index were recorded. Current guidelines were employed to calculate the number of patients who needed secondary prevention., Results: With the appliance of current guidelines to 380 patients, 54-100% of women and 26-81% of men were candidates for treatment. For hip fractures the percentage was 81-100% and for forearm fractures 36-93%. FRAX index for hip fracture was ≥3% in 35% of patients. The National Institute for Health and Clinical Excellence and the National Osteoporosis Foundation 2009 were the most restrictive guidelines (54% and 57% respectively). On the other hand the National Guideline Clearinghouse (87%) and the National Osteoporosis Guideline Group (93%)., Conclusion: There are high differences in the percentage of patients who need treatment to prevent new fractures according to the guidelines.
