This book focuses on improving orthopaedic surgery in low and middle-income countries (LMIC). Over the last 35 years, orthopaedic surgeons have made multiple innovations locally in Pakistan that have never come to the surface. Many surgeons in our world work in a resource-constrained setting and have to use frugal innovations to work their way out. These are the ground realities of working in a LMIC, which will be shared globally through this book.Starting from seeing patients in the clinic and to the operating room, very high standards have been set in Pakistan for others to follow and replicate within their own resources. This book will be a narrative of how these innovations were achieved and the way forward considering the resource constraints that are present in low-middle-income countries compared to Western countries.

Eating disorders can profoundly affect the individual and family unit. Changes in the individual include disturbances in body perception, organ damage, and increased risk factors leading to ill-health in later years. There is thus a fundamental requirement to adequately diagnose, treat and manage those individuals with eating disorders which the American Psychiatric Association have recently categorized (DSM-5) into Anorexia Nervosa, Bulimia Nervosa, Binge Eating Disorder, and Other Specified- and Unspecified-Feeding or Eating Disorders.The aim of this reference work is to describe, in one comprehensive resource, the complex relationships between eating disorders, diet, and nutrition. This book provides an overview of newly gathered material focusing on the opening and closure of The Neuquén Basin. The Neuquén Basin contains the most important hydrocarbon reservoirs in Argentina and therefore is characterized by a profound knowledge of the sedimentation mechanisms and closure times. During the last 10 years a considerable amount of new information has been produced that illustrates a complex evolution that involves more than one synrift stage during its evolution, an aborted sag phase associated with the inception of a first foreland basin in late Early Cretaceous times, two extensional destabilizations in the Late Cretaceous-Paleocene and late Oligocene times and a Neogene magmatic expansion coetaneous to a last mountain building. These processes have produced a polyphasic complex structure that exhumed the rich sedimentary record that characterizes the basin.

There are now compelling human epidemiological and animal experimental data that indicate the risk of developing adult-onset complex diseases and neurological disorders are influenced by persistent epigenetic adaptations in response to prenatal and early postnatal exposures to environmental factors. Epigenetics refers to heritable changes in gene function that occur without a change in the sequence of the DNA. The main components of the epigenetic code are DNA methylation, histone modifications, and non-coding RNAs. The epigenetic programs are established as stem cell differentiate during embryogenesis, and they are normally faithfully reproduced during mitosis. Moreover, they can also be maintained during meiosis, resulting in epigenetic transgenerational disease inheritance, and also potentially introducing phenotypic variation that is selected for in the evolution of new species. Palaeobiogeography is a complex subject which processes information provided by both Biology and Earth Sciences. It is conceptually and philosophically equivalent to neobiogeography. Nevertheless, its methods are somewhat different, since it is limited by the incompleteness of the fossil record. On the other hand, it has direct access to the time dimension, a key ingredient of organic evolution. Mesozoic benthonic mollusks, and especially bivalves, have a great potential for palaeobiogeographical analysis due to their commonly good preservation, abundance, diversity and high dispersion potential at the larval stage. From a merely descriptive point of view, the analysis of their distribution shows latitudinal gradients and distributional patterns, both at regional and global scales, which are the basis for the recognition of biochoremas or palaeobiogeographical units of different ranks. Moving forward towards a causal palaeobiogeography, these organisms also provide interesting insight into particular biogeographical questions, such as bipolarity and its origin. The evolution in time of the recognized biochoremas can be discussed in relation to palaeoclimas and extinction events. Finally, some of the results obtained from the analysis of the distribution of past bivalve biotas were even used to propose and discuss the development of marine corridors and argue about the distribution of continents in the past.

This volume deals with the history, aetiology, pathophysiology, symptoms, signs, prognosis, and rational treatment of ascites. During the past decade, our knowledge of the pathophysiology of ascites has increased substantially and more specific therapies are now based on aetiology and pathophysiology. It is the intention of this book to review recent progress in pathophysiology of ascites and therapies based on pathophysiology. Although the different types of ascites have a different aetiology and very different pathophysiology, the development of fluid in the peritoneal cavity is always a bad clinical sign. It has a severe prognosis, which is mainly dependent on the aetiology and progression of the underlying disease. However, among patients with ascites, the prognosis may be very different, mainly owing to the presence of portal venous hypertension, malignancy in the abdominal cavity, and end-stage congestive heart failure. The addition of complications like the hepatorenal syndrome and bacterial peritonitis, whether spontaneous or secondary, adds heavily to the bad prognosis. Since hepatic ascites are by far the most complex with respect to pathophysiology, complications, and treatment, emphasis is put on the description of this entity. Ascites of other aetiologies are mentioned along with hepatic ascites, in particular, if the pathophysiology differs from ascites of hepatic origin.

This book provides a detailed account of the physico-chemical properties and biological functions of the outer membrane vesicles (OMVs) of different pathogenic and non-pathogenic Gram-negative bacteria. It also includes an authentic record of the first systematic study that discovered the mechanism of OMV formation by a pathogen, Vibrio cholerae, and proposed that the process represented a novel secretory activity of bacteria. Evolutionary computation is a class of problem optimization methodology with the inspiration from the natural evolution of species. In nature, the population of a species evolves by means of selection and variation. These two principles of natural evolution form the fundamental of evolutionary - gorithms (EAs). During the past several decades, EAs have been extensively studied by the computer science and arti?cial intelligence communities. As a classofstochasticoptimizationtechniques,EAscanoftenoutperformclassical optimization techniques for di?cult real world problems. Due to the ease of use and robustness, EAs have been applied to a wide variety of optimization problems. Most of these optimization problems ta- led are stationary and deterministic. However, many real-world optimization problems are subjected to dynamic and uncertain environments that are often impossible to avoid in practice. For example, the?tness function is uncertain or noisy as a result of simulation errors, measurement errors or approximation errors. In addition, the design variables or environmental conditions may also perturb or change over time. For these dynamic and uncertain optimization problems, the objective of the EA is no longer to simply locate the global optimum solution, but to continuously track the optimum in dynamic en- ronments, or to?nd a robust solution that operates optimally in the presence of uncertainties. This poses serious challenges to classical optimization te- niques and conventional EAs as well. However, conventional EAs with proper enhancements are still good tools of choice for optimization problems in - namic and uncertain environments.
