To estimate the prevalence of musculoskeletal disorders (MSK) and rheumatic diseases in an indigenous Wichi population in Argentina.This is a cross-sectional, community-based study using the Community-Oriented Program for the Control of Rheumatic Diseases (COPCORD) methodology in ≥ 18-year-old subjects. Validated surveys were conducted by trained interviewers. Subjects with MSK pain (positive cases) were evaluated by internists and rheumatologists for diagnosis and treatment.A total of 648 interviews were performed (90.4% of the census population). Mean age was 37.5 years (SD 14.8), and 379 (58.5%) were female. The mean years of education was 7.0 (SD 3.7); 552 subjects (85.2%) were covered by the public health care system. A total of 216 (33.3%) subjects had MSK pain in the last 7 days. Rheumatic disease prevalence was as follows: mechanical back pain (19.0%), rheumatic regional pain syndrome (5.2%), osteoarthritis (3.2%), rheumatoid arthritis (RA) (3.2%), inflammatory back pain (1.2%), undifferentiated arthritis (0.3%), Sjögren syndrome (0.15%), and fibromyalgia (0.15%). RA patients included 19 (90.5%) women and 9 (42.9%) with RA family history. One hundred percent were seropositive and 66.7% showed radiologic erosions. The mean of Disease Activity Score [DAS-28 (ESR)] at the time of diagnosis was 5.1 (SD 1.5) and the Health Assessment Questionnaire Disability Index (HAQ-DI) was 0.8 (SD 0.4).RA prevalence was 3.2%, one of the highest reported using the COPCORD methodology in indigenous and non-indigenous peoples in Latin America, with a high percentage of family cases. Pain and functional capacity were the variables allowing patients' early referral to a specialist. Key Points • The RA prevalence was 3.2%, one of the highest reported using COPCORD methodology in indigenous and non-indigenous peoples in Latin America. • The patients with RA had high percentage of familiar history of RA. • The pain and functional capacity were the variables associated with a diagnosis of any rheumatic disease and should be considered for early referral. • The mean of the delay in the diagnosis was 5.8 years. In this community, the lack of the "migration health" phenomenon may be a social determinant that negatively impacts their health.

O objetivo da pesquisa foi compreender a perspectiva de cicloativistas da Região Sul do Brasil na promoção da bicicultura em seus respectivos territórios. Entrevistamos seis cicloativistas tendo como critérios de inclusão a) ser cicloativista militante; b) ocupação de cargo político e c) desenvolvimento de projetos de cidadania tanto na gestão pública e iniciativa privada. A partir da metodologia Fenomenológica-Hermenêutica, emergiram as unidades de significado “organização do movimento cicloativista”, “processos educativos vivenciados na promoção da bicicultura” e “planejamento da política pública” resultando na categoria analítica ‘ciclismo urbano como mobilidade ativa sustentável’. Consideramos que, o fenômeno social ciclismo urbano representa a necessidade de valorizar a convivência na escala humana no trânsito viário com potencial de transformação política no planejamento das cidades.

Current treatment modalities for disseminated cutaneous malignant melanoma (CMM) improve survival; however, relapses are common. A number of receptor tyrosine kinases (RTKs) including EGFR and MET have been reported to be involved in CMM metastasis and in the development of resistance to therapy, targeting the mitogen-activated protein kinase (MAPK pathway). IHC analysis showed that patients with higher MET protein expression had a significantly shorter overall survival. In addition, silencing of MET caused an upregulation of EGFR and p-AKT, which was abrogated by concomitant silencing of MET and EGFR in CMM cells resistant to MAPK-targeting drugs. We therefore explored novel treatment strategies using clinically approved drugs afatinib (ERBB family inhibitor) and crizotinib (MET inhibitor), to simultaneously block MET and ERBB family RTKs. The effects of the combination were assessed in cell culture and spheroid models using established CMM and patient-derived short-term cell lines, and an in vivo xenograft mouse model. The combination had a synergistic effect, promoting cell death, concomitant with a potent downregulation of migratory and invasive capacity independent of their BRAF/NRAS mutational status. Furthermore, the combination attenuated tumor growth rate, as ascertained by the significant reduction of Ki67 expression and induced DNA damage in vivo. Importantly, this combination therapy had minimal therapy-related toxicity in mice. Lastly, the cell cycle G2 checkpoint kinase WEE1 and the RTK IGF1R, non-canonical targets, were altered upon exposure to the combination. Knockdown of WEE1 abrogated the combination-mediated effects on cell migration and proliferation in BRAF mutant BRAF inhibitor-sensitive cells, whereas WEE1 silencing alone inhibited cell migration in NRAS mutant cells. In summary, our results show that afatinib and crizotinib in combination is a promising alternative targeted therapy option for CMM patients, irrespective of BRAF/NRAS mutational status, as well as for cases where resistance has developed towards BRAF inhibitors.

Analisa a (im)possibilidade de o Judiciário impedir a candidatura de uma pessoa que esteja sendo processada criminalmente em primeira instância ao cargo de Presidente da República. O estudo aufere relevância desde a análise da realidade político-jurídico-econômica nos governos Dilma (2º mandato) e Temer, em que pode ser constatada a intensifi cação do casuísmo nas decisões proferidas pelo Supremo Tribunal Federal (STF), principalmente quando se verifi cam os argumentos expressos nos votos de alguns ministros, podendo ser constatados contrastes entre as decisões, ainda que os casos sejam semelhantes. Em decorrência desses dados, emergem indagações em relação à possibilidade de o Judiciário, com a aplicação de determinados princípios (mais especificamente da moralidade, da presunção de inocência e da legalidade), impedir a candidatura de uma pessoa à Presidência da República, conquanto não esteja essa hipótese de inelegibilidade disposta no ordenamento jurídico. Assim, este trabalho tem como objetivos estudar as decisões do STF que possam infl uenciar futuros pensamentos dos magistrados, bem como debater o modo como os princípios da moralidade, da legalidade e da presunção de inocência são aplicados a essa situação. Para alcançar tais fi nalidades, far-se-á um levantamento da legislação e de julgados do STF que tratem (in)diretamente da inelegibilidade para, no segundo momento, confrontá-las com os referidos princípios, concluindo que a atuação dos ministros do STF enseja preocupação em virtude da ausência de previsibilidade de suas decisões. Por isso, por mais que os autores entendam não ser possível a ampliação do rol de inelegíveis, senão por meio de lei complementar, as fundamentações expressas pelos membros da referida Corte dão base ao receio de que, com base na imprecisa moralidade, pessoas possam ser impedidas de se candidatar a um cargo público.

Para metodología de la investigación, se utilizó el enfoque fenomenológicohermenéutico a partir de los generadores de sentido vínculo, abrigo, fijación, reiteración y apoyo en el análisis de los discursos de cinco sujetos que trabajan con mecánica de bicicleta, gerente de agencia bike courier, organizador de eventos deportivos con mountain bike, administrador de empresa bike shop y gestora pública de infraestructura cicloviaria. En la construcción de los resultados, surgieron las unidades de significado: a) valores sociales en la actividad profesional con bicicleta, b) salud ambiental en el compartir vial y c) educación y política pública para la sostenibilidad. No. of bitstreams: 0 Previous issue date: 2018-10-31 The purpose of this dissertation is to analyze the performance of the Judiciary, specifically the Federal Supreme Court, in the current political context. This is due to the fact that the decisions of that court, through the use of hermeneutical subterfuges, are responsible for amending not only the legislation, but also the Constitution, without any modification in the text, thus enabling a function of the power, whose members are not democratically chosen, amends the larger text, as well as limit the scope of certain fundamental rights, arranged as hard clauses, that is, can not be settled, although by a constitutional amendment approved by the quorum required in art. 60, § 2, of the CF / 1988. The research has as an initial hypothesis that members of the Judiciary can not under any circumstances act beyond what the law provides. To prove or reject this hypothesis, it was necessary to discuss the concept of legitimacy, making an approach in the fields of politics, theories of legal norm and judicial decision. Next, it was discussed the need for the Constitution to be modified, verifying the appropriate procedure for this to be accomplished. In the second chapter, we analyzed the fundamentals of the STF's activities: judicialization and judicial activism, demonstrating that these phenomena are inherent to the 1988 Constitution, but must be controlled, in order to avoid that the mechanisms used to protect democracy without reason for its destruction. In the last chapter, there were two decisions of the STF, making it clear that the court acts in a casuistic way, not observing its own precedents, thus weakening the Democratic Rule of Law. In the end, the author of this paper concludes that the constitutional mutation is necessary for any democracy, since the adequacy of the Constitution to the new popular yearnings is essential for there to be legitimacy. The Judiciary can not be seen as the "mouth of the law", having been demonstrated by the hermeneutics that the interpretation made by the magistrate configures in creation. This is why it is essential to differentiate the text from the standard. In addition, mechanisms to control the work of the Judiciary should be built, so that it demonstrates that the STF ministers are the guardians of the Constitution, but not the owners, being the last word for the people, who must act democratically (material). A presente dissertação tem como objetivo analisar a atuação do Judiciário, mais especificamente do Supremo Tribunal Federal, no atual contexto político. Isso se deve à razão de que as decisões do referido tribunal, por meio do uso de subterfúgios hermenêuticos, estão sendo responsáveis pela alteração não só da legislação, mas também da Constituição, sem qualquer modificação no texto, possibilitando, assim, que uma função do poder, cujos membros não são escolhidos democraticamente, emende o texto maior, bem como limite o âmbito de atuação de determinados direitos fundamentais, dispostos como cláusulas pétreas, ou seja, não podem ser dirimidos, ainda que por emenda constitucional aprovada de acordo com o quórum exigido no art. 60, § 2º, da CF/1988. A pesquisa tem como hipótese inicial que os membros do Judiciário não podem, em nenhuma hipótese, atuar além do que a lei prevê. Para comprovar ou rechaçar essa hipótese, foi necessário discutir o conceito de legitimidade, fazendo uma abordagem nos campos da política, das teorias da norma jurídica e da decisão judicial. Em seguida, foi discutida a necessidade de a Constituição ser modificada, verificando qual o adequado procedimento para que isso seja realizado. No segundo capítulo, analisou-se os fundamentos da atuação do STF: judicialização e ativismo judicial, demonstrando que esses fenômenos são inerentes à Constituição de 1988, mas que devem ser controlados, a fim de evitar que os mecanismos utilizados para proteção da democracia sem a razão da sua destruição. No último capítulo, verificou-se duas decisões do STF, ficando evidente que o referido tribunal age de forma casuísta, não observando seus próprios precedentes, fragilizando, portanto, o Estado Democrático de Direito. Ao final, o autor deste trabalho conclui que a mutação constitucional é necessária para qualquer democracia, pois a adequação da Constituição aos novos anseios populares é essencial para que haja legitimidade. O Judiciário não pode ser visto como a ¿boca da lei¿, tendo ficado demonstrado pela hermenêutica que a interpretação feita pelo magistrado configura em criação. Daí o motivo de ser essencial ser diferenciado o texto da norma. Ademais, mecanismos de controle da atuação do Judiciário devem ser construídos, de modo que demonstre que os ministros do STF são os guardiões da Constituição, mas não os donos, ficando a última palavra para o povo, que deve agir democraticamente (material). Improving epithelialization of donor sites of split-thickness skin grafts (STSG) is extremely important in burned patients. We aimed to assess the efficacy of pirfenidone, a drug with anti-inflammatory, antifibrotic, and antioxidant effects, to accelerate wound healing. We hypothesized that pirfenidone accelerates the epithelialization rates in donor sites.We included 28 patients requiring STSGs with donor sites of at least 7.5×10cm. After harvesting, the donor sites were randomly treated with either non-adherent gauze or topical pirfenidone and covered with non-adherent gauze. To assess epithelialization, biopsies were taken at day 7 and 10 on the pirfenidone group, and at day 10 on the control group. Percentage of epithelialization was assessed on the same days through clinical photographs. The pathologists and the clinical observer were blinded to the group and timepoint of the samples.24 patients were included in the study, with a median age of 21(5-73) for control group and 28(9-61) for pirfenidone. The thickness of epithelium was 75.10±60μm at day 10 for the control group; and 98.21±6μm at day 7, and 108±22μm at day 10 for the pirfenidone group (p=0.05). Epithelization rate was 83.58±14.09% at day 10 for the control group; and 98.7±1.8% at day 7, and 99.5±1.6% at day 10 for the pirfenidone group.Pirfenidone is efficient in reducing the healing times when applied in STSG donor sites, at both days 7 and 10.

Renal cell carcinoma is the most common malignancy of the kidney in adults. In children, however, it only accounts for an estimated 1.8 to 6.3% of all pediatric malignant renal tumors. Papillary renal cell carcinoma is the second most common type of renal cell carcinoma in children. We present the case of a 12-year-old boy with a 2-month history of abdominal pain, unexplained weight loss, and gross hematuria. Computed tomography revealed a horseshoe kidney and a well-defined mass of 4 cm arising from the lower pole of the right kidney. Microscopically the tumor was composed of papillae covered with cells with abundant eosinophilic cytoplasm and high-grade nuclei with prominent nucleoli. Immunohistochemistry was performed; EMA, Vimentin, and AMACR were strongly positive while CK7, CD10, RCC antigen, TFE3, HMB-45, and WT-1 were negative. Currently, 10 months after the surgical procedure, the patient remains clinically and radiologically disease-free.

Destina-se aos serviços de atenção básica, organizados segundo diferentes modelos de atenção, incluindo a Saúde da Família. Contém 50 indicadores sobre oferta e organização do trabalho assistencial e programático e 15 sobre gerenciamento, na forma de questões de múltipla escolha, autorespondidas via web pela equipe local do serviço. Confere a cada resposta valor zero, um ou dois; a média geral atribui ao serviço um grau de qualidade expresso pela distância do melhor padrão correspondente à média dois. Foi construído por processo de consenso interativo, que incluiu metodologias qualitativas, teste-piloto, aplicação em 127 serviços, validação de construto e confiabilidade. Respondido, em 2007, por 598 (92%) dos serviços de 115 municípios paulistas, mostrou bom poder para discriminar níveis de qualidade. Adotado em 2010 como parte de um programa de apoio à Atenção Básica da Secretaria de Estado da Saúde de São Paulo, foi respondido por 95% (2.735) dos serviços de 586 municípios (90,8% do Estado). Os resultados foram encaminhados aos municípios. O QualiAB fornece uma avaliação válida, simples e com a possibilidade de retorno imediato para gerentes e profissionais. Mostrou factibilidade, aceitabilidade, bom poder de discriminação e utilidade para auxiliar a gestão da rede de atenção básica do SUS em São Paulo. A experiência indica aplicabilidade nas redes de atenção básica do Brasil. This paper presents the development, validation and utilization of a methodology for assessing the quality of the primary health care services of Sistema Único de Saúde (SUS - Brazil's National Health System), the Questionnaire for the Assessment of the Quality of Primary Health Care Services - QualiAB. It is addressed to all primary health care services, organized according to different care models, including Family Health. It contains 50 indicators about availability and organization of the care and programmatic work, and 15 about local management, in the form of multiple choice questions self-responded through the Web by the local service team. It attributes to every answer the values zero, one or two; the general average attributes a 'quality degree' to the service, expressed by the distance to the best standard corresponding to the average two. It was built by an interactive consensual process, which included qualitative methodologies, pilot-testing, application to 127 services, construct validation and reliability. It was answered in 2007 by 598 (92%) services of 115 municipalities of the State of São Paulo, showing good sensitivity to differentiate quality levels. It was adopted in 2010 as part of a supporting program to the Primary Health Care of São Paulo's State Health Department, being answered by 95% (2735) of the services of 586 cities (90,8% of the State). The results were sent to the municipalities. QualiAB provides a valid and simple assessment, with immediate feedback to managers and professionals. It has shown feasibility, acceptability, power to differentiate quality levels and utility to help the management of the primary health care network of SUS in São Paulo. This experience might be extended to Brazil as a whole. O ciclismo urbano é um fenômeno recente valorizado como alternativa ao transporte motorizado individual. O objetivo do artigo é compreender os processos educativos do ciclismo urbano, decorrentes da prática social do cicloativismo atuante na cidade de São Paulo. Realizamos quatro entrevistas com pessoas que habitam e se locomovem de bicicleta neste município. A trajetória metodológica seguiu a modalidade fenômeno situado da Fenomenologia. Na construção dos resultados, as unidades de significado: a) luta por reconhecimento no direito de fruir o território em bicicleta; b) solidariedade com outrem no compartilhamento seguro das vias públicas; c) percepção do entorno e d) comunicação do movimento cicloativista resultaram na categoria “Pedalar como direito humano à mobilidade ativa na cidade”. Constatamos que, apesar do processo legítimo de diálogo, as demandas cicloativistas são pouco incorporadas às políticas públicas de mobilidade urbana por falta de representação institucional., Urban bicycling is a recent phenomenon valued as an alternative to individual transportation by motor vehicles. This article looks into the educational processes of urban bicycling that resulted from the social practice of bicycling activism in the city of Sao Paulo. We conducted four interviews with people who live and use bicycles in that city. The methodological trajectory followed Phenomenology’s view on situated phenomena. In the construction of results, the units of meaning a) fight for recognition of the right to enjoy territory by bicycle; b) solidarity with others in safe sharing of public roads; c) perception of the environment; and d) communication of the bicycling activist movement resulted in the category “Pedaling as a human right to active mobility in the city”. We found that, despite the legitimate process of dialogue, the demands of bicycling activism are not highly incorporated into public policies for urban mobility due to lack of institutional representation., El ciclismo urbano es un fenómeno reciente valorado como alternativa al transporte motorizado individual. El objetivo del artículo es comprender los procesos educativos del ciclismo urbano, derivados de la práctica social del cicloactivismo que actúa en la ciudad de São Paulo. Realizamos cuatro entrevistas con personas que habitan y se desplazan en bicicleta en este municipio. La trayectoria metodológica siguió la modalidad fenómeno situado, de la Fenomenología. En la construcción de los resultados, las unidades de significado: a) lucha por reconocimiento en el derecho de disfrutar del territorio en bicicleta; b) solidaridad con otros al compartir de manera segura las vías públicas; c) percepción del entorno y d) comunicación del movimiento cicloactivista, resultaron la categoría "Pedalear como derecho humano a la movilidad activa en la ciudad". Constatamos que, a pesar del proceso legítimo de diálogo, las demandas cicloactivistas son poco incorporadas a las políticas públicas de movilidad urbana por falta de representación institucional.

A complexidade do processo saúde-doença tem ensejado a proposição de uma diversidade de modelos explicativos. Fazemos uma breve revisão dessas propostas, confrontando três perspectivas: o modelo oriundo da Medicina do século XIX, a lógica da História Natural da Doença e o debate epidemiológico no contexto da Medicina Social latino-americana. Tomando-se como referência teórica a ideia de causalidade circular presente na teoria da auto-organização, propomos que os fatores causais privilegiados em cada um dos modelos explicativos acima não seriam conflitantes. Uma noção-chave para se pensar o processo de autoorganização biopsicossocial é o "efeito baldwiniano", que descreve uma relação dialética ou coevolutiva entre processos naturais e socioculturais.The complexity of the health-disease process has elicited the postulation of a diversity of explanatory models. We make a brief review of the proposals, starting with the biomedical model derived from the 19th century medicine. This model influenced the approach on the natural history of disease, and the debate on epidemiologic models in the context of the Latin-American Social Medicine. Broadening the spectrum of the discussion, we introduce the idea of circular causality, proposed by theories of self-organizing systems. We argue that, in a transdisciplinary perspective, these explanatory models are not conflicting. A key notion to understand these classes of concomitant explanatory models is the "Baldwin Effect", describing a dialectic or coevolutionary relation between nature, social organization and culture.

Discutimos a epistemologia das ciências da vida e das ciências da saúde de Georges Canguilhem, revendo sua crítica à concepção mecanicista da normalidade e da patologia e seu posicionamento frente ao vitalismo. Sugerimos que, enfatizando o conceito de "normatividade da vida", Canguilhem teria apontado para uma superação da oposição entre mecanicismo e vitalismo. Para tal, fazemos uma breve comparação da "normatividade da vida" com o conceito contemporâneo de auto-organização de Michel Debrun, argumentando que a emergência da norma vital se situa num estágio secundário de um processo de (auto-)organização da vida e, portanto, tal normatividade não teria a conotação vitalista, erroneamente atribuída a Canguilhem.We discuss the epistemology of the sciences of life and health elaborated by Georges Canguilhem. First we review his criticism to mechanicist concepts of normality and pathology, and his position regarding vitalism. We suggest that, when emphasizing the concept of "normativity of life", Canguilhem goes beyond the dichotomy of mechanism and vitalism. We make a brief comparison of his concept of "normativity of life" with the contemporary concept of "self-organization" proposed by Michel Debrun, arguing that the emergency of the vital norm occurs on the second stage of the process of life self-organization and therefore such normativity does not have the vitalist connotation erroneously attributed to Canguilhem.

Subependymoma is a rare tumor of the central nervous system, representing 0.2 to 0.7% of all intracranial tumors. They are usually found in the fourth ventricle or in the walls of the lateral ventricles, often remaining asymptomatic. Subependymomas occur rarely in the cervical or thoracic spinal cord. Since its first description in the spinal cord by Boykin et al in 1954, only 47 cases have been reported. A 49-year-old man presented with a 4-year-long-lasting neck pain, which radiated to the upper right extremity for 4 years. It was insidious at onset and revealed gradual progression, difficulty in performing fine hand movements, and hyperalgesia. Magnetic resonance imaging revealed irregular hyperintensity of T2 and dilatation of the spinal cord from C4 to T1 with associated edema and an intramedullary lesion with central location. During the surgical event, a transoperative biopsy was performed that revealed loose fibrillary networks and groups of nuclei showing mild pleomorphism and microcystic formations. Histologically, we observed groups of cells with mild nuclear pleomorphism embedded in a dense, thin, glial fibrillar fundus, and microcystic formations. Immunohistochemical staining revealed diffuse positivity for the glial fibrillary acidic protein, and negative for the epithelial membrane antigen. Subependymoma accounts for 8.3% of all ependymal tumors. There are few descriptions of the cytologic features of the subependymoma because this neoplasm is rare and most commonly found incidentally in autopsies. [ABSTRACT FROM AUTHOR]

Emphasizes the importance of legal mechanisms to control the use of water as a way to preserve the springs and establish programs to recover those degraded.

Este estudo objetiva caracterizar a organização dos serviços de AB em 37 municípios do Centro-oeste paulista, como primeira etapa de um projeto de avaliação da qualidade desses serviços. Trata-se de um estudo transversal conduzido mediante questionário estruturado, autorrespondido pelos gerentes e equipes locais, com questões que abordam características institucionais e de organização e gerência do trabalho. Esses questionários foram enviados para 131 UBS, distribuídas em 37 municípios. Obteve-se resposta de 113 unidades (87%) localizadas em 32 municípios (86,4%). do total de unidades, 57 (50%) são UBS tradicionais, 26 (22,8%), Unidades de Saúde da Família, e 31, (27,2%) organizadas segundo formas mistas. A maioria dos serviços (62%, 70/113) não trabalha com área de abrangência delimitada de modo planejado. Os serviços se polarizam entre aqueles que realizam entre 70 e 100% de consultas médicas agendadas (37,6%, 41/109), e aqueles que realizam entre 70 e 100% de não agendadas (39,4%, 43/109). Não possuem conselhos locais organizados 65 unidades (63,7%, 65/102). Os dados coletados permitem discutir as características dos principais programas, procedimentos e ações realizados pelos serviços. Os perfis organizacionais predominantes apontam a presença de deficiências de estrutura e processo em relação às diretrizes do SUS. O desenvolvimento de instrumentos de autoavaliação permite que as equipes se apropriem, de forma crítica, de seu trabalho, e possam elaborar novos arranjos tecnológicos para melhoria da qualidade. The State of São Paulo, Brazil, has a broad network of primary health care services (PHCS) whose organizational profile is heterogeneous and not well known. This study analyzes the organization of PHCS in 37 municipalities located in the central-west region of São Paulo, as the first part of a broader project that evaluates the quality of these services. This is a cross-sectional study conducted through a structured questionnaire answered by the local managers and health teams. The questions addressed institutional characteristics and the organization and management of the work process. The questionnaires were sent to 131 UBS (Primary Health Care Units) distributed across 37 municipalities. There were responses from 113 units (87%) located in 32 municipalities (86.4%). of the overall number of units, 57 (50%) are traditional UBS, 26 (22.8%) are family health units, and 31 (27.2%) are mixed. Most services (62%, 70/113) do not work with catchment areas delimited in a planned way. Services are divided between those who perform 70% to 100% of scheduled medical consultations (37.6%, 41/109) and those who perform 70% to 100% of non-scheduled consultations (39.4%, 43/109). Sixty-five units (63.7%, 65/102) do not have community-based committees. Based on the collected data, it is possible to discuss the main features of the health programs, procedures and activities carried out by PHCS. The predominant organizational profiles reveal deficiencies of structure and process in relation to the guidelines issued by SUS (Brazil's National Health System). The development of self-assessment instruments allows the teams to appropriate their work in a critical way and to carry out new technological arrangements to improve the quality of the care provided. The optimal positioning of wind turbines, even in one dimension, is a problem with no analytical solution. This article describes the application of computational intelligence techniques to solve this problem. A systematic analysis of the optimal positioning of wind turbines on a straight line, on flat terrain, and considering wake effects has been conducted using both simulated annealing and genetic algorithms. Free parameters were the number of wind turbines, the distances between wind turbines and wind turbine hub heights. Climate and terrain characteristics were varied, like incoming wind speed, wind direction, air density, and surface roughness length, producing different patterns of positioning. Analytical functions were used to model wake effects quantifying the reduction in speed after the wind passes through a wind turbine. Conclusions relevant to the placement of wind turbines for several cases are presented.

89. Violência simbólica no campo da saúde: relato de um caso de cura espiritual em um espaço terapêutico híbrido

À luz da teoria social de Pierre Bourdieu, saliento a violência simbólica concebida das relações de forças entre agentes e práticas médicas e religiosas no interior de uma instituição filantrópica voltada à assistência à saúde de portadores de deficiências múltiplas. Do trabalho etnográfico, descrevo detalhes do acordo entre administradores religiosos espíritas e profissionais de saúde durante a implementação de um projeto que incluía especificamente assistência espiritual. Um caso de cura aparece como bem simbólico e sobre ele concorreram duas versões explicativas sobre a abrupta recuperação do paciente assistido durante meses na UTI por caquexia: a versão religiosa, que entendeu a reabilitação como cura espiritual, e a versão médica, que compreendeu o restabelecimento do paciente como resultado das atividades e gerência médica. In light of Pierre Bourdieu's theory, I highlight the symbolic violence derived from the relations of forces between medical and religious agents and practices inside a philanthropic institution that provides healthcare for patients suffering from multiple deficiencies. Concerning the ethnographic work, I describe details of the agreement between spiritualistic religious administrators and health professionals during the development of a project that included spiritualistic assistance. A cure case emerges as a symbolic asset and there were two versions to explain the recovery of the patient, who had been under ICU treatment for a long time: the religious version, which understood the rehabilitation as spiritual cure, and the medical version, which understood it as the result of the medical activities and management.

Se analizo el cambio en areas forestales de la region forestal del Carare-Opon mediante imagenes Landsat de 1991 y 2002 para identificar el dinamismo de la zona. La clasificacion supervisada mas consistente fue la calculada con el modelo de decision no parametrico por espacios de caracteristicas y el parametrico por distancia minima espectral. La comparacion raster se realizo por el metodo de deteccion de cambios de ERDAS y por composicion multitemporal, en ambos casos sobre indices normalizados de vegetacion. Sin embargo, la superposicion vectorial de la clasificacion condujo a resultados mas evidentes. Pese a que en el Carare-Opon existe una fuerte presion sobre los bosques por expansion de las fronteras ganaderas y la historica explotacion forestal de la zona, el estudio encontro un dinamismo positivo con un crecimiento de 15% en las areas de bosques en el periodo dado. Con altas proporciones de cambio en solo once anos se reconoce que las alternativas de manejo silvicultura) de la region deberian desarrollarse en horizontes eficientes a corto plazo.

Introduction: the most widely used classification for maxillofacial traumatism is that described by the French physician Rene Le Fort in 1901, which describes experimentally produced patterns of fractures, as a result of which the majority are low-energy traumatisms. At present traffic accidents are the most common mechanism of injury that conditions high-energy facial injuries. The present classification takes into consideration the functional anatomy of the facial skeleton, analyzed by means of the faucial pillars or supports that give stability to the functional units of the face, combined with the findings of multisection computed tomography in transverse planes and coronal multi-planar and three-dimensional (3D) volumetric reconstructions. The sagittal plane can be used only for exploration of displacement of fragments in fractures. The accumulation of knowledge of the functional anatomy of the facial skeleton, the biomechanics of the faucial pillars in relation to the base of the skull, and the most common fracture patterns will lead to more accurate classification and surgical planning in maxillofacial traumatism. Conclusion: radiologists' learning of the classification most commonly used for maxillofacial traumatism will provide sufficient criteria for surgical planning and for more accurate diagnosis in injuries of the facial skeleton.
