56. Epistemic Trust Is a Critical Success Factor in Psychosomatic Rehabilitation—Results from a Naturalistic Multi-Center Observational Study.

Abstract

Knowledge about critical success factors underpinning beneficial treatment outcomes in psychosomatic inpatient rehabilitation is scarce. The aim of this study was to evaluate the influence of patients' epistemic stance in relation to the improvement of psychological distress during rehabilitation. In this naturalistic longitudinal observational study, n = 771 patients completed routine assessments for psychological distress (BSI-18), health-related quality of life (HRQOL; WHODAS), and epistemic trust (ETMCQ) before (T1) and after (T2) psychosomatic rehabilitation. Patients were grouped as best, average, and worst responders based on their mean BSI-18 changes during treatment, and their mean change in epistemic trust, mistrust, and credulity was compared using repeated measures analyses of variance (rANOVAs). No associations of performance with sex (p = 0.09), age (p = 0.11), or relationship status (p = 0.58) were found. Best responders reported significantly improved epistemic trust (p = 0.001) and reduced epistemic mistrust (p < 0.001), whereas worst responders reported a significant increase in epistemic mistrust (p < 0.001) and credulity (p < 0.001). Average responders did not change for either epistemic trust (p = 0.11), mistrust (p > 0.99), or credulity (p = 0.96). Our results underscore the role of the epistemic stance in psychosomatic and psychotherapeutic treatments. These results help to better understand what might determine psychosomatic rehabilitation outcomes and indicate the role of epistemic trust as a critical success factor. [ABSTRACT FROM AUTHOR]

Abstract

Psychological factors often play an important role in the course of physical illnesses. There is a high probability of suffering from a mental disorder in cancer patients (Singer et al., 2010). Specifically, cancer patients are significantly more likely to suffer from depression compared to the general population (e. g. Hartung et al., 2017). This could be due to the fact that cancer and depression may be mutually reinforcing (Ahmad et al., 2021). Depending on the type of cancer and its progression, the prevalence and severity of depressive symptoms may vary (e. g. Riedl & Schüssler, 2022; Prasad et al., 2014). In addition, it has been observed in numerous clinical studies that the survival time of patients with depressive symptoms is often shorter than that of patients who do not develop depression. The time of onset of depressive symptoms may also play a role in this regard (e. g. Wang et al., 2020; van der Elst et al., 2021). There could be a difference in the survival times of patients depending on whether a diagnosis of depression was made before the cancer was diagnosed or whether the depressive symptoms only appeared during the progression of the physical disease (e. g. Rieke et al., 2017).

Hintergrund: Untersuchungen der Europäischen Union folgend, sind 33 % aller Frauen ab dem 15. Lebensjahr von häuslicher Gewalt betroffen und wenden sich bei der Suche nach Hilfe vorwiegend an das Gesundheitssystem. Methode: Teilnehmerinnen und Teilnehmer an Sensibilisierungstrainings gegenüber häuslicher Gewalt wurden vor und nach dem Training mit Hilfe eines Fragebogens zu ihrer empfundenen Sicherheit im Umgang mit von Gewalt betroffenen Patientinnen und Patienten befragt. Ergebnisse: Die Evaluation eines Schulungszyklus am Landeskrankenhaus Innsbruck zeigte signifikante Verbesserungen im subjektiven Sicherheitsgefühl im Umgang mit gewaltbetroffenen Patientinnen und Patienten. Fazit für die Praxis: Ein strukturierter Aufbau der Sensibilisierungstrainings mit Basiskursen und Aufbauseminaren, sowie einem Fokus auf Beziehungsaufbau und Umgang mit Emotionen erscheint sinnvoll.

68. Updating our understanding of health-related quality of life issues in children with cancer: a systematic review of patient-reported outcome measures and qualitative studies

Abstract

Background: health-related quality of life (HRQOL) is a key concept in pediatric oncology. This systematic review aims to update the conceptual HRQOL model by Anthony et al. (Qual Life Res 23(3):771–789, 2014), covering physical, emotional, social and general HRQOL aspects, and to present a comprehensive overview of age- and disease-specific HRQOL issues in children with cancer. Methods: Medline, PsychINFO, the Cochrane Database for Systematic Reviews (CDSR), and the COSMIN database were searched (up to 31.12.2020) for publications using patient-reported outcome measures (PROMs) and qualitative studies in children with cancer (8–14-year) or their parents. Items and quotations were extracted and mapped onto the conceptual model for HRQOL in children with cancer mentioned above. Results: of 2038 identified studies, 221 were included for data extraction. We identified 96 PROMS with 2641 items and extracted 798 quotations from 45 qualitative studies. Most items and quotations (94.8%) could be mapped onto the conceptual model. However, some adaptations were made and the model was complemented by (sub)domains for ‘treatment burden’, ‘treatment involvement’, and ‘financial issues’. Physical and psychological aspects were more frequently covered than social issues. Discussion: this review provides a comprehensive overview of HRQOL issues for children with cancer. Our findings mostly support the HRQOL model by Anthony et al. (Qual Life Res 23(3):771–789, 2014), but some adaptations are suggested. This review may be considered a starting point for a refinement of our understanding of HRQOL in children with cancer. Further qualitative research will help to evaluate the comprehensiveness of the HRQOL model and the relevance of the issues it encompasses.

Abstract

Introduction: The oncogenic human papillomaviruses (HPV) types 16 and 18 contribute to more than 73% cases of all HPV-related cancers and commonly affect the anogenital and head and neck region, with rapidly rising incidence rates of HPV-related oropharyngeal squamous cell carcinomas (OPSCC). HPV vaccination has the potential to decrease the burden of HPV-related disease, but vaccination rates remain low in many countries. We investigated the level of awareness of HPV, and HPV-OPSCC in particular, in a representative sample of the German population. Materials and Methods: As part of an online, population-based survey, an electronic questionnaire was administered to a representative sample of 1,095 adult individuals with a specific emphasis on awareness of HPV, transmission, and indicator symptoms of oropharyngeal cancer. Statistical analysis of levels of awareness and relation of these to age, gender, and socioeconomic background were conducted using the IBM SPSS Statistics Version 25.0. Results: 699/1,095 (63.8%) subjects had never heard of HPV. Of the subjects with awareness for HPV, 210 knew that HPV could be transmitted during sex (58.3%) and 138 recognized HPV as a risk factor for OPSCC (14.2%), unrelated to gender (p = 0.357), educational status (p = 0.581), or family status (p = 0.719). 416 subjects knew that a preventive vaccine against HPV existed (44.9%). Women were significantly more aware of HPV (34.2% vs. 22.8%, p < 0.001) and the vaccination (56.4% vs. 32.7%, p < 0.001) as were men. Younger individuals (age group 25-34) were significantly more aware of HPV (p < 0.001), likely as they were offered and/or had received the HPV vaccination. There was no regional variation of HPV awareness within the German state (p = 0.051). Conclusion: Here we demonstrate a significant lack of awareness of HPV and HPV vaccination in a representative sample of the German population. Abstract

Volume 15(2018), Issue 1, Background: Virtual reality has been increasingly used to support established psychological interventions, including relaxation techniques. Only limited knowledge about the occurrence and severity of adverse events (AE) (e.g. cybersickness) in relaxing virtual environments is available. The aim of the study was to assess the frequency of AE in virtual environments and factors associated with these. Methods: A sample of 30 healthy participants was included in the study. The participants completed questionnaires on susceptibility of motion sickness, use of and attitudes towards modern technology prior to the exposition to the virtual environment. They then took part in three short virtual scenarios (no movement of the avatar, steady non-autonomous movement, and autonomous movement) using head-mounted displays and rated the occurrence and severity of AE after each scenario. Results: The participants reported high incidence rates of different AEs (40–70%), but only in the scenario with autonomous movement. In the scenarios with no or only limited control over movement approximately 30% reported slight symptoms of dizziness, and 3–7% reported slight nausea. Nevertheless, the occurrence of AEs resulted in reduced relaxation and mood. Gender, age, and usage of computers and gaming consoles had no influence on the incidence or severity of AEs. Discussion: Our results show that virtual reality is a safe technology to be used in clinical psychology, if certain parameters are being minded. Future studies should routinely assess and report AEs in a structured way, to enable more in–depth insights regarding influential factors and potential prevention strategies.
