54. Ability and Disability in Autism Spectrum Disorder: A Systematic Literature Review Employing the International Classification of Functioning, Disability and Health-Children and Youth Version

Abstract

Objective: This study is the first in a series of four empirical investigations to develop International Classification of Functioning, Disability and Health (ICF) Core Sets for Autism Spectrum Disorder (ASD). The objective was to use a systematic review approach to identify, number, and link functional ability and disability concepts used in the scientific ASD literature to the nomenclature of the ICF-CY (Children and Youth version of the ICF, covering the life span). Methods: Systematic searches on outcome studies of ASD were carried out in Medline/PubMed, PsycINFO, ERIC and Cinahl, and relevant functional ability and disability concepts extracted from the included studies. These concepts were then linked to the ICF-CY by two independent researchers using a standardized linking procedure. New concepts were extracted from the studies until saturation of identified ICF-CY categories was reached. Results: Seventy-one studies were included in the final analysis and 2475 meaningful concepts con tained in these studies were linked to 146 ICF-CY categories. Of these, 99 categories were considered most relevant to ASD (i.e., identified in at least 5% of the studies), of which 63 were related to Activities and Participation, 28 were related to Body functions, and 8 were related to Environmental factors. The five most frequently identified categories were basic interpersonal interactions (51%), emotional functions (49%), complex interpersonal interactions (48%), attention functions (44%), and mental functions of language (44%). Conclusion: The broad variety of ICF-CY categories identified in this study reflects the heterogeneity of functional differences found in ASD—both with respect to disability and exceptionality—and underlines the potential value of the ICF-CY as a framework to capture an individual's functioning in all dimensions of life. Abstract

Given the variability seen in Autism Spectrum Disorder (ASD), accurate quantification of functioning is vital to studying outcome and quality of life in affected individuals. The International Classification of Functioning, Disability and Health (ICF) provides a comprehensive, universally accepted framework for the description of health-related functioning. ICF Core Sets are shortlists of ICF categories that are selected to capture those aspects of functioning that are most relevant when describing a person with a specific condition. In this paper, the authors preview the process for developing ICF Core Sets for ASD, a collaboration with the World Health Organization and the ICF Research Branch. The ICF Children and Youth version (ICF-CY) was derived from the ICF and designed to capture the specific situation of the developing child. As ASD affects individuals throughout the life span, and the ICF-CY includes all ICF categories, the ICF-CY will be used in this project ("ICF(-CY)" from now on). The ICF(-CY) categories to be included in the ICF Core Sets for ASD will be determined at an ICF Core Set Consensus Conference, where evidence from four preparatory studies (a systematic review, an expert survey, a patient and caregiver qualitative study, and a clinical cross-sectional study) will be integrated. Comprehensive and Brief ICF Core Sets for ASD will be developed with the goal of providing useful standards for research and clinical practice and generating a common language for functioning and impairment in ASD in different areas of life and across the life span. 60. How satisfied are families with nursing home care, and why?

Abstract

Families have been a neglected partner in nursing home research. Yet family members' perceptions about quality of care and interactions with staff have far-reaching effects, both for their own well-being and for sustained involvement with relatives living in nursing homes. This poster describes data from interviews with 932 family members and 655 nursing staff from 20 New York state nursing homes. Measures include families' ratings of the facility, nurses, nursing assistants, and administrator; and perceptions of the quality of interactions between staff and residents and the family members themselves. In terms of structural characteristics, older, spousal caregivers of lower SES expressed the highest levels of satisfaction. Family members who reported low-conflict relationships with staff on multiple dimensions also reported higher satisfaction. Relationships between family satisfaction and characteristics of the nursing home, as well as administrator, director of nursing, and director of social work ratings, are also presented.

Abstract

Soybean has been considered a cold intolerant species; based largely upon seed germination and soil emergent evaluations. This study reports a distinct acquisition of cold tolerance, in seedlings, following short acclimation periods. Diversity in cold responses was assessed in eight cultivars of Glycine max and six accessions of G. soja. All varieties of soybean significantly increased in freezing tolerance following acclimation. This study indicates soybean seedlings are indeed capable of sensing cold and acquiring cold tolerance. Germination rates after cold imbibition were negatively correlated with maturity group, but positively correlated with cold acclimation potential in G. soja. Seed fatty acid composition was varied between the species, with Glycine soja accessions containing about 2-times more linolenic acid (18:3) than G. max. Furthermore, high levels of linoleic acid (18:2) in seeds were positively correlated with germination rates following cold imbibition in G. soja only. We suggest that domestication has not impacted the overall ability of soybean to cold acclimate at the seedling stage and that there is little variation within the domesticated species for ability to cold acclimate. Thus, this brief comparative study reduces the enthusiasm for the 'wild' species as an additional source of genetic diversity for cold tolerance. [ABSTRACT FROM AUTHOR]

Abstract

A drug called prednisone improves the strength and ability to move about in patients with Duchenne's muscular dystrophy. The patients tested were all young boys, aged five to 15 years. Improvement began after as little as a month and peaked at three months. They became almost twice as fast at rising from a lying position to a standing position. They could walk short distances about a third again as fast as before. The could climb four stair steps nearly twice as fast and could breathe more deeply. Some boys received a higher dose of the drug and some a lower dose, but the size of the dose did not affect their rate of improvement. However, patients who needed braces or wheelchairs before treatment still needed them following administration of the drug. More of a substance called creatine, produced when muscles grow, was detected in their urine, suggesting that the drug helped their muscles to grow. Adverse side effects include weight gain, excessive hair growth, and an accumulation of fat on the face, neck and trunk.

Abstract

Depression's association with increased morbidity, mortality, service utilization and disability is well established. What remains unclear, however, is the ability of brief-screening instruments to accurately detect depression in older populations. The purpose of this study was to determine the performance of a two question case-finding instrument (Prime-MD) for late-life depression, including major depressive disorder (MDD) and subsyndromal depression. Data was gathered for three populations over age 60: primary care patients (n=125), hospitalized medical patients (n= 150), and nursing home residents (n=85). The average age of subjects was 75fp8 years, 63% were female, 44% widowed, 37% were ADL dependent and 77% were IADL dependent. Scores from the Prime-MD were compared with the DIS-derived DSM-IV diagnoses and the following results were obtained: sensitivity (79%fp50-94); specificity (58%fp55-60); ROC (.72fp.58-.86). A profile of patients misclassified by the Prime-MD will be presented including results from the CIRS-G, psychiatric diagnosis from the Q-DIS, health and functional status.
