Die Einordnung funktionsmedizinischer Befunde in etablierte strukturpathologische Konzepte ist wesentliche Voraussetzung fur die Einordnung der manuellen Medizin als Teilgebiet der medizinischen Wissenschaft. Hierbei stellt die arztliche Differenzialdiagnose unter Beachtung von red und yellow flags den zentralen Aspekt der Befunderhebung. Wesentlich ist die Kenntnis der strukturpathologischen Befunde, deren Bewertung und deren Zusammenhange mit manualmedizinisch behandelbaren Symptomen. Eine entsprechende Beachtung der vorhandenen S3-Leitlinie "NVL unspezifischer Kreuzschmerz" wird besprochen. Die in der letzten Zeit erneut aufgenommene Diskussion zur bildgebenden Diagnostik wird in einem gesonderten Unterkapitel gewurdigt. Es werden diesbezuglich Empfehlungen fur die Veranlassung von bildgebender Diagnostik in der tagliche Praxis gegeben. Fur die Erhebung manualmedizinisch relevanter Palpationsbefunde wird ein System von der globalen uber die regionale Untersuchung hin zur gezielten, segmentalen Untersuchung aufgezeigt und die Bewertung der Befunde dargestellt.

ZusammenfassungZur Dignitätsabklärung hypofunktioneller Schilddrüsenknoten kann eine Szintigraphie mit 99mTc-MIBI erfolgen. Bei Minderspeiche- rung im Pertechnetat- und MIBI-Szintigramm lässt sich ein Schilddrüsenkarzinom relativ sicher ausschließen. Bei vermehrter MIBI-An- reicherung wird eine Operation empfohlen. Die Einstufung isointenser Knoten wird kontrovers gesehen. Untersucht wurde, welche Karzinomprävalenz isointense Knoten zeigen und ob sich durch SPECT-Technik ein diagnostischer Zugewinn ergibt. Patienten, Methodik: Retrospektive Auswertung von MIBI-Szintigraphien in planarer und SPECT- Technik. Die Szintigraphie erfolgte nach Applikation von 510 MBq 99mTc-MIBI eine Stunde p. i. Die Auswertung erfolgte visuell mit Einteilung der Knoten in hypo-, iso- und hy- perintens im Vergleich zum paranodulären Gewebe. Ergebnisse: Bei 83 von 225 Patienten mit MIBI-Szintigraphien erfolgte eine histologische Sicherung (Alter 48,6 ± 12,6 Jahre, 72% weiblich). Es fanden sich 12 papilläre Karzinome (14,5%). Bei planarer Aufnahme wurden 12, 34 bzw. 37 Knoten, in den SPECT- Aufnahmen 16, 21 bzw. 46 Knoten als hypo-, iso- bzw. hyperintens eingestuft. Unter den in planarer Technik als hypo-, iso- bzw. hyperin- tens eingestuften Knoten fanden sich 1, 5 bzw. 6 Karzinome und bei tomographischer Technik kein, 4 bzw. 8 Karzinome. Wurden die iso- und hyperintensen Knoten als malignom- supekt zusammengefasst, ergaben sich für Sensitivität, Spezifität, NPV und PPV bei planarer Aufnahme Werte von 91,7, 15,5, 91,7 bzw. 15,6% und bei tomographischer Aufnahme 100, 22,5, 100 bzw. 17,9%. Schlussfolgerung: Hypofunktionelle SD-Knoten mit iso- und hyperintenser MIBI-Speicherung zeigen eine vergleichbare Malignomprävalenz. Durch die Untersuchung in SPECT-Technik ergibt sich eine Verbesserung insbesondere von Sensitivi- tät und negativem Vorhersagewert.

Manualmedizinische Verfahren werden in der Sportbetreuung von den Anwendern mehrheitlich als positiv angesehen. Die Einschatzung reicht vom positiven Einfluss bei der Rehabilitation von Sportverletzungen uber die Prophylaxe von Sportverletzungen bis hin zur Moglichkeit der Leistungssteigerung. Grundlage sind neurophysiologische Erklarungsmodelle, die nahelegen, dass funktionelle Storungen zu einer veranderten Gelenkbeweglichkeit und muskularen Koordination fuhren, die durch geanderte Bewegungsablaufe ein hoheres Verletzungsrisiko und eine verminderte Leistungsfahigkeit zu Folge haben. Als definitiv nachgewiesen kann der positive Einfluss in der Behandlung von Sportverletzungen angesehen werden. Bezuglich der Verletzungsprophylaxe und der Leistungssteigerung liegen bisher nur wenige, haufig gegensatzliche Daten vor. Insofern ist eine sichere Aussage zum Effekt der manuellen Medizin in diesem Zusammenhang bisher nicht moglich. Um das tatsachliche Potenzial manualmedizinischer Verfahren zu verifizieren, sind zukunftig weitere Studien zu diesen Fragestellungen notwendig.

Die grundlegende Bedeutung der konventionellen Rontgendiagnostik in der manuellen Medizin liegt im pratherapeutischen Ausschluss von Kontraindikationen. Die Basisdiagnostik stellt die Untersuchung in 2 Ebenen (sagittaler und frontaler Strahlengang) des entsprechenden Wirbelsaulenabschnitts dar. An Brustwirbelsaule (BWS) und Lendenwirbelsaule (LWS) weichen die Aufnahmen nicht vom Standardvorgehen ab. An der Halswirbelsaule (HWS) hat sich die Aufnahmetechnik nach Sandberg-Gutmann etabliert. Bei Funktionsaufnahmen der LWS kommt die Technik nach Tilscher-Graf-Hanna zur Anwendung. Um eine sichere Beurteilung der Rontgenaufnahmen zu ermoglichen, ist eine exakte Einstelltechnik notwendig, die insbesondere eine orthograde und vollstandige Darstellung des interessierenden Bereichs gewahrleistet. Besonderheiten ergeben sich bei Sauglingen und Kleinkindern, da noch nicht alle knochernen Strukturen ausgebildet sind und eine hohere Strahlenempfindlichkeit vorliegt. Daher werden im Normalfall lediglich die Kopfgelenke mittels anterior-posteriorer Aufnahme gerontgt. Generell ist die Strahlenexposition, die je nach Wirbelsaulenabschnitt und Alter ein Risiko einer letalen Tumorerkrankung zwischen 10−4 und10−7 beinhaltet, zu beachten.

Der Einsatz der Radiosynoviorthese (RSO) bei aktivierter Arthrose wird kritisch diskutiert, da grosteils schlechtere Ergebnisse im Vergleich zur Rheumatoiden Arthritis (RA) in Studien berichtet werden. Unberucksichtigt blieb hierbei bisher eine mogliche Abhangigkeit des Therapieeffektes vom Stadium der Arthrose. Es wurde daher untersucht, inwieweit die Skelettszintigrafie eine Vorhersage des Therapieerfolges ermoglicht. Bei 67 Kniegelenken wurde der Zusammenhang von Schmerzreduktion durch die RSO bei aktivierter Arthrose quantitativ mit dem Ausmas der Synovialitis in der Weichteilphase und dem Ausmas der knochernen Veranderung in der Skelettphase sowie visuell mit einer begleitenden Retropatellararthrose oder Arthrose des Tibiofibulargelenkes bzw. einer Darstellung der knochernen Lasion bereits in der Weichteilphase untersucht. Es fand sich ein signifikanter Zusammenhang fur die quantitative Beurteilung der knochernen Veranderung bei einer Korrelation von r= − 0,31 sowie fur den visuellen Nachweis (Weich+) bzw. das Fehlen (Weich-) einer Darstellung der knochernen Veranderung bereits in der Weichteilphase. Die Schmerzreduktion in der Gruppe Weich+ betrug 22,4% und in der Gruppe Weich− 79%. Gelenkbezogen ergab sich eine Erfolgsrate von 51% fur Weich+ und von 76% fur Weich−. Die Number-needed-to-treat (NNT) fur eine Schmerzreduktion von ≥50% betrug 3,7 fur Weich+ und 1,5 fur Weich−. Fur die ubrigen Parameter fand sich kein signifikanter Zusammenhang. Es besteht ein Zusammenhang von Therapieerfolg und Ausmas der knochernen Veranderung. Durch die Unterteilung der Patienten in die Gruppen Weich+ und Weich− erlaubt die Skelettszintigrafie eine Vorhersage des Therapieerfolges und in der Gruppe Weich − resultiert eine Erfolgsrate entsprechend der RSO bei RA im Stadium I. Aber auch in der Gruppe Weich + wird mit einer NNT von 3,7 noch eine Erfolgsrate entsprechend einer Kortikoidinjektion bei Gonarthrose erzielt. Eine RSO kann auch bei diesen Patienten, insbesondere bei Kontraindikationen fur eine operative Therapie, zum Einsatz kommen.

Rucken- und Nackenschmerzen gehoren zu den am haufigsten angegebenen Beschwerden. Handelt es sich um unspezifische Schmerzen, wird in entsprechenden Leitlinien in den ersten 4 bis 6 Wochen bei fehlender therapeutischer Relevanz keine Bildgebung empfohlen. Finden sich jedoch Zeichen spezifischer Beschwerden infolge ernst zu nehmender Erkrankungen („red flags“), empfiehlt sich eine geeignete bildgebende Diagnostik in Abhangigkeit von der klinischen Verdachtsdiagnose. Das konventionelle Rontgen steht aufgrund der allgemeinen Verfugbarkeit, der leichten Durchfuhrung und der geringen Kosten im Vordergrund, allerdings mit dem Nachteil einer geringen Sensitivitat. Bei einer Traumaanamnese, insbesondere nach frischen Traumata, ist die Computertomographie die Methode der Wahl zur Beurteilung der knochernen Strukturen. Ansonsten ist wegen der besseren Weichteildarstellung die Magnetresonanztomographie vorzuziehen. Besteht der klinische Verdacht auf eine systemische oder multilokulare Erkrankung, stellt die Skelettszintigraphie aufgrund der hohen Sensitivitat und der Ganzkorperdarstellung die gunstigste Untersuchungsmodalitat dar.

Radiosynoviorthesis (RSO) with the nuclides rhenium-186 sulphide (186Re) and erbium-169 citrate (169Er) is an established concept for the treatment of persistent synovitis of medium and small sized joints. The aim of the present studies was to investigate the biological radiation effect based on analysing chromosome aberrations. Patients, methods: Immediately before and 17 to 19 days (186Re) or 45 to 50 (169Er) days after RSO with 186Re or 169Er colloid and subsequent immobilisation of the treated joint, blood samples of a total of 23 patients were collected. The yield of dicentric chromosomes in lymphocytes was determined exclusively in metaphases of the first cell cycle in vitro. In addition, for 186Re the activity leakage was measured three days after RSO by whole-body scintigraphy. Results: No statistically significant increase in the number of dicentric chromosomes (40 and 88 before and 59 and 105 after treatment with 186Re and 169Er, respectively) in a total of 47017 cells analysed from 46 blood samples could be found as a result of RSO. For 186Re an activity leakage of 3.9%±7% with a maximum of 23.4% corresponding to an effective dose of 2.8±4.5 mSv , respectively 13.8 mSv, was determined. Also in the case of the maximum leakage no significant increase of dicentric chromosomes were detected. Conclusions: No significant biological radiation effect can be detected after RSO with 186Re and 169Er, also in cases of high leakage. Therefore, RSO can be classified as a save therapeutic procedure without a relevant radiation risk.

Geological features of NE Greenland suggest large petroleum potential, as well as high uncertainty and risk. The area was the prototype for development of methodology used in the US Geological Survey (USGS) Circum-Arctic Resource Appraisal (CARA), and was the first area evaluated. In collaboration with the Geological Survey of Denmark and Greenland (GEUS), eight ‘assessment units’ (AU) were defined, six of which were probabilistically assessed. The most prospective areas are offshore in the Danmarkshavn Basin. This study supersedes a previous USGS assessment, from which it differs in several important respects: oil estimates are reduced and natural gas estimates are increased to reflect revised understanding of offshore geology. Despite the reduced estimates, the CARA indicates that NE Greenland may be an important future petroleum province.

The US Geological Survey recently assessed the potential for undiscovered conventional petroleum in the Arctic. Using a new map compilation of sedimentary elements, the area north of the Arctic Circle was subdivided into 70 assessment units, 48 of which were quantitatively assessed. The Circum-Arctic Resource Appraisal (CARA) was a geologically based, probabilistic study that relied mainly on burial history analysis and analogue modelling to estimate sizes and numbers of undiscovered oil and gas accumulations. The results of the CARA suggest the Arctic is gas-prone with an estimated 770–2990 trillion cubic feet of undiscovered conventional natural gas, most of which is in Russian territory. On an energy-equivalent basis, the quantity of natural gas is more than three times the quantity of oil and the largest undiscovered gas field is expected to be about 10 times the size of the largest undiscovered oil field. In addition to gas, the gas accumulations may contain an estimated 39 billion barrels of liquids. The South Kara Sea is the most prospective gas assessment unit, but giant gas fields containing more than 6 trillion cubic feet of recoverable gas are possible at a 50% chance in 10 assessment units. Sixty per cent of the estimated undiscovered oil resource is in just six assessment units, of which the Alaska Platform, with 31% of the resource, is the most prospective. Overall, the Arctic is estimated to contain between 44 and 157 billion barrels of recoverable oil. Billion barrel oil fields are possible at a 50% chance in seven assessment units. Undiscovered oil resources could be significant to the Arctic nations, but are probably not sufficient to shift the world oil balance away from the Middle East.

The US Geological Survey (USGS) recently assessed the potential for undiscovered oil and gas resources of the East Barents Basins and Novaya Zemlya Basins and Admiralty Arch Provinces as part of the USGS Circum-Arctic Resource Appraisal. These two provinces are located NE of Scandinavia and the northwestern Russian Federation, on the Barents Sea Shelf between Novaya Zemlya to the east and the Barents Platform to the west. Three assessment units (AUs) were defined in the East Barents Basins for this study – Kolguyev Terrace Assessment Unit (AU), South Barents Basin and Ludlov Saddle AU, and North Barents Basin AU. A fourth, defined as Novaya Zemlya Basins and Admiralty Arch AU, is coincident with the Novaya Zemlya basins and Admiralty Arch Province. These AUs, all lying north of the Arctic Circle, were assessed for undiscovered, technically recoverable resources resulting in total estimated mean volumes of approximately 7.4 billion barrels of crude oil, 318 trillion cubic feet of natural gas and 1.4 billion barrels of natural gas liquids.

The Siberian Craton consists of crystalline rocks and superimposed Precambrian sedimentary rocks deposited in rift basins. Palaeozoic rocks, mainly carbonates, were deposited along the margins of the craton to form an outwardly younger concentric pattern that underlies an outward-thickening Mesozoic sedimentary section. The north and east margins of the Siberian Craton subsequently became foreland basins created by compressional deformation during collision with other tectonic plates. The Tunguska Basin developed as a Palaeozoic rift/sag basin over Proterozoic rifts. The geological provinces along the north and east margins of the Siberian Craton are immature with respect to exploration, so exploration-history analysis alone cannot be used for assessing undiscovered petroleum resources. Therefore, other areas from around the world having greater petroleum exploration maturity and similar geological characteristics, and which have been previously assessed, were used as analogues to aid in this assessment. The analogues included those of foreland basins and rift/sag basins that were later subjected to compression. The US Geological Survey estimated the mean undiscovered, technically recoverable conventional petroleum resources to be approximately 28 billion barrels of oil equivalent, including approximately 8 billion barrels of crude oil, 103 trillion cubic feet of natural gas and 3 billion barrels of natural gas liquids.

Ublicherweise wird vor einer Manipulationstherapie zum Ausschluss moglicher Kontraindikationen eine Rontgenuntersuchung durchgefuhrt. Hiermit ist das Risiko einer strahleninduzierten letalen Tumorerkrankung verbunden. Je nach Alter der Patienten und untersuchtem Wirbelsaulenabschnitt liegt das Risiko im Bereich zwischen 10−4 und 10−7. Entsprechend fordert die Rontgenverordnung die Stellung einer rechtfertigenden Indikation im Sinne einer Nutzen-Risiko-Analyse. Abschatzungen aus bisherigen Untersuchungen ergaben ein Risiko zwischen 10−2 und 10−8 fur eine durch Rontgenuntersuchung vermeidbare Komplikation einer Manipulation. Diese Studien uber- oder unterschatzten jedoch das tatsachliche Risiko, je nach methodischem Ansatz, deutlich. Eine genaue Nutzen-Risiko-Analyse ist mit den bisherigen Daten daher nicht moglich. Es sind gezielte Studien zur Klarung des Sachverhalts notwendig, da gerichtlich auch ein fehlendes Rontgenbild als Behandlungsfehler gewertet wird, obwohl sich daraus keine Komplikation ergeben hat.

Hypertrophy and inflammation of the synovium with various underlying pathologies - such as rheumatoid arthritis, osteoarthritis, haemophilia and spondyloarthropathy - can be treated successfully by radiosynoviorthesis (RSO). For medium-sized joints (shoulder, elbow, wrist, hip and ankle), the radionuclide of choice is rhenium-186. We review the evidence for the efficacy of this local, relatively non-invasive therapy and evaluate its benefits and risks. We conclude good evidence of rhenium-186 RSO in rheumatoid arthritis and haemophilic arthropathy. In the remaining pathologies, up to now, the therapeutic efficacy has not been confirmed by today's most stringent criteria for clinical studies. The available data support rhenium-186 RSO as a suitable second-line treatment for patients in whom other therapies (including locally injected corticoids) have failed, as long as proper attention is paid to correct administration - including post-treatment immobilization and the co-administration of corticoids.

Purpose To present a method for calculating split renal function solely from routine triphasic helical computed tomography (CT). Subjects and methods We retrospectively included 26 adult patients who received renal scintigraphy and triphasic CT within 4 weeks in the years 2003 and 2004. All scans were performed using a standard abdominal protocol. Split renal function was calculated as relative single-kidney glomerular filtration rate (GFR) using a simplified “two-point Patlak plot” technique. As a reference method, split renal function was determined from renal scintigraphy using the standard technique. Results Linear correlation between the two methods was r = 0.91, split renal function (CT) = 0.0266 + 0.9573 × split renal function (scintigraphy). Conclusion Split renal function can be measured accurately by minimally extended triphasic CT.
