Abstract

Background: Perinatal asphyxia and resulting hypoxic-ischemic encephalopathy is a major cause of death and long-term disability in term born neonates. Up to 20,000 infants each year are affected by HIE in Europe and even more in regions with lower level of perinatal care. The only established therapy to improve outcome in these infants is therapeutic hypothermia. Allopurinol is a xanthine oxidase inhibitor that reduces the production of oxygen radicals as superoxide, which contributes to secondary energy failure and apoptosis in neurons and glial cells after reperfusion of hypoxic brain tissue and may further improve outcome if administered in addition to therapeutic hypothermia. Methods: This study on the effects of ALlopurinol in addition to hypothermia treatment for hypoxic-ischemic Brain Injury on Neurocognitive Outcome (ALBINO), is a European double-blinded randomized placebo-controlled parallel group multicenter trial (Phase III) to evaluate the effect of postnatal allopurinol administered in addition to standard of care (including therapeutic hypothermia if indicated) on the incidence of death and severe neurodevelopmental impairment at 24 months of age in newborns with perinatal hypoxic-ischemic insult and signs of potentially evolving encephalopathy. Allopurinol or placebo will be given in addition to therapeutic hypothermia (where indicated) to infants with a gestational age ≥ 36 weeks and a birth weight ≥ 2500 g, with severe perinatal asphyxia and potentially evolving encephalopathy. The primary endpoint of this study will be death or severe neurodevelopmental impairment versus survival without severe neurodevelopmental impairment at the age of two years. Abstract

Die adenotonsillare Hyperplasie (ATH; vergroserte Adenoide und Tonsillen) kann zu ausgepragten Atmungsstorungen wie behinderte Nasenatmung, behinderte Mundatmung, habituellem Schnarchen und obstruktiver Schlafapnoe fuhren und sollte dann auch behandelt werden. Bisher wurde meist eine Adenotomie bzw. Adenotonsillektomie als primare Therapie empfohlen. Nun liegen Studien vor, die die Effektivitat von antiinflammatorischen Medikamenten (AIM) zur Behandlung von ATH-bedingten Atmungsstorungen zeigen. Die Darstellung der Evidenz und die Erarbeitung von Empfehlungen fur die Anwendung von AIM zur Behandlung von ATH-bedingten Atmungsstorungen sind Ziele der vorliegenden Arbeit. Zwolf nationale Kinderschlafexperten wurden in ein Delphiverfahren einbezogen und formulierten Indikationen sowie Anwendungsempfehlungen. Der Einsatz von AIM zur Behandlung von ATH-bedingten Atmungsstorungen ist durch randomisierte kontrollierte Studien und systematische Ubersichtsarbeiten ausreichend gerechtfertigt. Es liegen Studien zur Behandlung von Adenoiden mithilfe des nasal-applizierten Beclometasons und Mometasons, zur Behandlung der obstruktiven Schlafapnoe mithilfe des nasal-applizierten Fluticasons und oral-applizierten Montelukasts vor. Der Einsatz von AIM ohne vorherige objektivierende Diagnostik ist allerdings nur unter bestimmten Bedingungen wie ATH-typisches Alter und Ausschluss akuter Atemwegsinfektionen gerechtfertigt. Evidenzbasierte Empfehlungen zu Indikation, Dosierung, Anwendungsdauer und richtiger Applikation von AIM werden ausfuhrlich dargestellt. Antiinflammatorische Medikamente sind eine zusatzliche, einfach durchzufuhrende und effektive therapeutische Option fur die Behandlung von ATH-bedingten Atmungsstorungen. Das vorliegende Positionspapier stellt eine Hilfestellung fur die ambulante Anwendung von AIM durch Kinderarzte dar.

Abstract

In 2013, an estimated 2.8 million newborns died and 2.7 million were stillborn. A much greater number suffer from long term impairment associated with preterm birth, intrauterine growth restriction, congenital anomalies, and perinatal or infectious causes. With the approaching deadline for the achievement of the Millennium Development Goals (MDGs) in 2015, there was a need to set the new research priorities on newborns and stillbirth with a focus not only on survival but also on health, growth and development. We therefore carried out a systematic exercise to set newborn health research priorities for 2013-2025.We used adapted Child Health and Nutrition Research Initiative (CHNRI) methods for this prioritization exercise. We identified and approached the 200 most productive researchers and 400 program experts, and 132 of them submitted research questions online. These were collated into a set of 205 research questions, sent for scoring to the 600 identified experts, and were assessed and scored by 91 experts.Nine out of top ten identified priorities were in the domain of research on improving delivery of known interventions, with simplified neonatal resuscitation program and clinical algorithms and improved skills of community health workers leading the list. The top 10 priorities in the domain of development were led by ideas on improved Kangaroo Mother Care at community level, how to improve the accuracy of diagnosis by community health workers, and perinatal audits. The 10 leading priorities for discovery research focused on stable surfactant with novel modes of administration for preterm babies, ability to diagnose fetal distress and novel tocolytic agents to delay or stop preterm labour.These findings will assist both donors and researchers in supporting and conducting research to close the knowledge gaps for reducing neonatal mortality, morbidity and long term impairment. WHO, SNL and other partners will work to generate interest among key national stakeholders, governments, NGOs, and research institutes in these priorities, while encouraging research funders to support them. We will track research funding, relevant requests for proposals and trial registers to monitor if the priorities identified by this exercise are being addressed.
