Purpose: Many areas of medicine have shown bias towards the publication of studies with positive results. To estimate publication bias in the anesthesia literature, we reviewed all abstracts presented at the American Society of Anesthesiologists (ASA) annual meetings over a four-year period and compared study results (positive vs negative) with publication outcomes.Methods: This review included all abstracts from the 2001-2004 ASA annual meetings performed as randomized-controlled trials in humans. We scored their outcome results as positive or negative and assessed abstract quality using a 13-point scoring system. We then performed a systematic literature search to identify any subsequent publication of the studies and calculated the relative risk (RR) for journal publication by comparing positive vs negative studies.Results: Of 5,918 abstracts reviewed, 1,052 met inclusion criteria, and 564 (53.6%) of the abstracts proceeded to publication. The RR for abstracts with positive results proceeding to journal publication was 1.42 (95% confidence interval, 1.22 to 1.66; P < 0.001). This result did not change significantly after adjusting for study size and abstract quality score during logistic regression modelling. There was no significant difference in the abstract quality score between positive vs negative studies or between abstracts proceeding vs not proceeding to publication.Conclusions: Approximately half of the ASA annual meeting abstracts proceed to publication. After adjustment for study quality and size, abstracts with positive results were more likely to proceed to journal publication than those with negative results, suggesting publication bias in the anesthesia literature. BACKGROUND: Inclusion of nitrous oxide in the gas mixture has been implicated in postoperative nausea and vomiting (PONV) in numerous studies. However, these studies have not examined whether duration of exposure was a significant covariate. This distinction might affect the future place of nitrous oxide in clinical practice. METHODS: PubMed listed journals reporting trials in which patients randomized to a nitrous oxide or nitrous oxide-free anesthetic for surgery were included, where the incidence of PONV within the first 24 postoperative hours and mean duration of anesthesia was reported. Meta-regression of the log risk ratio for PONV with nitrous oxide (lnRR PONVN2O) versus duration was performed. RESULTS: Twenty-nine studies in 27 articles met the inclusion criteria, randomizing 10,317 patients. There was a significant relationship between lnRR PONVN2O and duration (r = 0.51, P = 0.002). Risk ratio PONV increased 20% per hour of nitrous oxide after 45 min. The number needed to treat to prevent PONV by avoiding nitrous oxide was 128, 23, and 9 where duration was less than 1, 1 to 2, and over 2 h, respectively. The risk ratio for the overall effect of nitrous oxide on PONV was 1.21 (CIs, 1.04-1.40); P = 0.014. CONCLUSIONS: This duration-related effect may be via disturbance of methionine and folate metabolism. No clinically significant effect of nitrous oxide on the risk of PONV exists under an hour of exposure. Nitrous oxide-related PONV should not be seen as an impediment to its use in minor or ambulatory surgery. [ABSTRACT FROM AUTHOR]

Objectives. To re-evaluate the accuracy and precision of a non-invasive method for measurement of cardiac output based on the differential CO2 Fick approach using an automated change in respiratory rate delivered by a ventilator under control by a prototype measurement system. Methods. Twenty-four patients during coronary artery bypass surgery, pre- and postcardiopulmonary bypass were recruited. After routine cannulation including pulmonary artery catheter, relaxant general anesthesia was induced. After hemodynamic and ventilatory stability were achieved, simultaneous paired measurements were made by the differential Fick method and by bolus thermodilution. Measurements were generated by inducing a change in respiratory rate by the ventilator under computer control. In Group 1, this involved an increase in respiratory rate from 8 to 12 breaths/min. In Group 2, this involved a decrease from 12 to 6 breaths/min. Results. Nineteen measurements were made in each Group, 12 pre-CPB and 7 post-CPB. In Group 1 mean bias was -0.06 l/min, with a precision of agreement of 0.87 l/min, r = 0.91. In Group 2 (excluding one outlier) mean bias was -0.07 l/min, with a precision of 1.12 l/min, r = 0.71. Conclusions. Acceptable agreement with thermodilution during surgery was found, particularly where the ventilatory change involved an increase in respiratory rate from a lower baseline. We have investigated the elimination of inert gases in the lung during the elimination of nitrous oxide (N 2 O) using a two-step mathematical model that allows the contribution from net gas volume expansion, which occurs in Step 2, to be separated from other factors. When a second inert gas is used in addition to N 2 O, the effect on that gas appears as an extra volume of the gas eliminated in association with the dilution produced by N 2 O washout in Step 2. We first considered the effect of elimination in a single gas-exchanging unit under steady-state conditions and then extended our analysis to a lung having a log-normal distribution of ventilation and perfusion. A further increase in inert gas elimination was demonstrated with gases of low solubility in the presence of the increased ventilation-perfusion mismatch that is known to occur during anesthesia. These effects are transient because N 2 O elimination depletes the input of that gas from mixed venous blood to the lung, thereby rapidly reducing the magnitude of the diluting action., (© 2023 The Authors. Minimally invasive measurement of cardiac output as a central component of advanced haemodynamic monitoring has been increasingly recognised as a potential means of improving perioperative outcomes in patients undergoing major surgery. Methods based upon pulmonary carbon dioxide elimination are among the oldest techniques in this field, with comparable accuracy and precision to other techniques. Modern adaptations of these techniques suitable for use in the perioperative and critical are environment are based on the differential Fick approach, and include the partial carbon dioxide rebreathing method. The accuracy and precision of this approach to cardiac output measurement has been shown to be similar to other minimally invasive techniques. This paper reviews the underlying principles and evolution of the method, and future directions including recent adaptations designed to deliver continuous breath-by-breath monitoring of cardiac output.

A new method for the measurement of pulmonary gas exchange during inhalational anaesthesia is described which measures fresh gas and exhaust gas flows using carbon dioxide as an extractable marker gas. The theoretical precision of the method was compared by Monte Carlo modelling with other approaches which use marker gas dilution. A system was constructed for automated measurement of uptake of oxygen, nitrous oxide, volatile anaesthetic agent and elimination of carbon dioxide by an anaesthetized patient. The accuracy and precision of the method was tested in vitro on a lung gas exchange simulator, by comparison with simultaneous measurements made using nitrogen as marker gas and the Haldane transformation. Good agreement was obtained for measurement of simulated uptake or elimination of all gases studied over a physiologically realistic range of values. Mean bias for oxygen and nitrous oxide uptake was 0.003 l min(-1), for isoflurane 0.0001 l min(-1) and for carbon dioxide 0.001 l min(-1). Limits of agreement lay within 10% of the mean uptake rate for nitrous oxide, within 5% for oxygen and isoflurane and within 1% for carbon dioxide. The extractable marker gas method allows accurate and continuous measurement of gas exchange in an anaesthetic breathing system with any inspired gas mixture.
