58. Prediction of total subcutaneous abdominal, intraperitoneal, and retroperitonal adipose tissue masses in men by a single axial magnetic resonance imaging slice

Abstract

To develop a simplified but accurate method for determining the masses of various abdominal adipose tissue compartments, we studied the predictive value of masses of intraperitoneal, retroperitoneal, and subcutaneous abdominal adipose tissue determined on single axial abdominal magnetic resonance imaging (MRI) slices taken at various intervertebral levels from the 12th thoracic to 1st sacral vertebra (identified on a sagittal section) for the respective total masses of each compartment calculated from contiguous 10-mm thick MRI slices covering the entire abdomen in 49 men (26 without diabetes and 23 with non-insulin-dependent diabetes mellitus). The MRI slice at the intervertebral level between the lumbar (L) 2 and 3 vertebrae showed the highest and most consistent predictive value for all three compartments ([R.sup.2] = 0.85 for all). Furthermore, compared with other intervertebral levels, the L2-L3 level had a higher amount of intraperitoneal and retroperitoneal adipose tissue mass. We conclude that determining the masses of various abdominal adipose tissue compartments at the L2-L3 intervertebral level by MRI is an acceptably reliable and accurate method for studying abdominal adiposity in men.

59. Left ventricular dimensions and mass using magnetic resonance imaging in female endurance athletes

Abstract

Few published studies of left ventricular (LV) mass in the female endurance athletes have been performed with M-mode echocardiography, which involves assumptions of LV geometry. Therefore, magnetic resonance imaging, a 3-dimensional techniques, was used to examine LV mass, LV end-diastolic volume and mean wall thickness in female long distance runners (n = 13; mean age 29 years), cyclists (n = 12; mean age 26 years) and cross-country skiers (n = 11; mean age 24 years), and the findings were compared with sedentary control subjects (N = 10; mean age 27 years) matched for height and body weight. The physical characteristics for all subjects included height (mean 166 cm, and body weight (mean 56 kg). The percent body fat (mean 11.7) and maximal oxygen uptake [VO.sub.2]max, mean 63 ml * [kg.sup.-1] * [min.sup.-1) were similar (p >0.05) among all athletic groups, but significantly different from the control group (body fat, mean 22.5%; [VO.sub.2]max, mean 35 ml * [kg.sup.-1] * [min.sup.-1]. LV mass (mean 159 KG), LV end-diastolic volume (mean 122 ml), and mean wall thickness (mean 11.5 min) were also similar among the athletic groups and significant larger than the following control values: LV mass (mean 115 g), LV end-diastolic volume (mean 93 ml) and mean wall thickness (mean 9.8 mm). Ratios of LV mass to lean body weight were similar among all athletic groups, although athletic groups had larger ratios (p 0.05) among all groups. Our findings indicate that LV mass, LV end-diastolic volume and mean wall thickness are greater in endurance athletes than sedentary control subjects and that the increase in LV mass is proportional to the increase in LV end-diastolic volume. Abstract

Magnetic resonance imaging (MRI) is a powerful tool providing high-resolution images of the heart and great vessels without the use of ionizing radiation or contrast agents. MRI systems currently in use at many hospitals can be used effectively in the routine, clinical evaluation of many forms of cardiovascular disease, including great vessel disease, ischemic cardiac disease and congenital cardiac disease. Moreover, quantitative and cine MRI techniques are now available, which provide highly accurate measures of chamber size, wall motion and wall thickening. Recent developments in the areas of myocardial tagging, high-speed imaging and MR assessments of flow and perfusion suggest potential for an increasing role of MRI in the clinical evaluation of the cardiovascular system. (Am J Cardiol 1990;66:41F-4FF), Magnetic resonance imaging (MRI) has characteristics which make it well suited to imaging the cardiovascular system. Not only does MRI work without dangerous radiation or contrast-enhancing agents, but it is also intrinsically sensitive to motion. Magnetic resonance imaging is contraindicated, however, in patients with a cardiac pacemaker, a cochlear implant, or an aneurysm clamp made of a ferromagnetic material. Currently, magnetic resonance imaging serves three major areas in the diagnosis of cardiovascular disease. MRI is useful in the imaging of abnormalities in the great vessels, in some aspects of heart damage due to ischemia, and in the diagnosis of congenital heart abnormalities. Recently new developments in MRI technology have been made which may make the technique even more valuable for cardiology. One of these developments is a procedure known as ''tagging.'' More precisely, spatial modulation of magnetization (SPAMM) can be used to precisely identify a particular point on the heart wall and then follow it during the motion of the heart's contraction. This method gives precise quantitative results on the function of specific segments of the heart's muscular wall. Another improvement in technology involves the speed with which magnetic resonance images can be obtained. The most rapid current technique requires about 30 milliseconds to acquire an image. New methods may increase this speed by a factor of 10,000, albeit at a slight loss of resolution. The last area of recent advances in MRI is the exploration of new contrast compounds. Using tiny magnetic aggregates or magnetic compounds injected into the blood, blood vessels can be visualized on the MRI in a manner directly analogous to present-day angiography. Abstract

EMRs are essential in modern healthcare. At the same time there has been the rapid development of clinical research systems (CRS) which offer specific tools to facilitate research protocol development, execution and management. However, examples of integrating a commercial EMR and CRS are rare. We describe our experience in the analysis, resolution of issues, design, and implementation of EMR/CR integration in a large academic medical center with affiliated hospitals. Our initial results suggest that (1) neither the EMR nor CRS system alone will suffice to facilitate clinical research, (2) there are important benefits to integration of the EMR and CRS systems and (3) this integration can be leveraged across multiple institutions.

This approach is common in other industries; yet, protecting confidential patient information is unique to the healthcare industry. Guaranteed protection of patient confidentiality is a critical step in any inter-organizational strategy. Because of the need to leverage resources, the University of Texas Southwestern Medical Center (UT Southwestern) and its affiliates initiated a cross-institutional effort to address information security and confidentiality issues.

Abstract

Muscle pain frequently occurs after strenuous activity. This study examines two forms of muscle pain: pain which begins during exercise, usually referred to as muscle strain; and pain which begins hours or days after strenuous exercise, commonly called delayed-onset muscle soreness. Although there has been a great deal of research dealing with muscle pain, not much is known about its pathology. This is due in part to the inadequacies of current methods for evaluating (by clinical examinations, blood enzyme tests, and imaging procedures) the precise muscle abnormality and the severity and duration of the muscle damage. Researchers used magnetic resonance (MR) imaging to assess damage to muscle tissue in sedentary subjects and marathon runners who experienced muscle strain and delayed-onset muscle soreness. MR imaging provided information not available at clinical examination. Areas of abnormality were identified in individual muscles. The presence of large collections of blood (focal hematoma) and muscle herniation were ruled out; both conditions require surgical repair. MR images also confirmed the belief that exercise-induced muscle pain does not cause long-term muscle damage. MR images demonstrated abnormalities in muscle tissue beyond the time that was indicated by other methods of evaluation. MR images revealed abnormalities up to three weeks longer than the duration of patients' symptoms. This means that MR images provide more accurate information about muscle recovery. In comparing the two groups of subjects it was observed that muscle abnormalities usually involved smaller areas in the marathon runners than in the sedentary group.
