Abstract

BackgroundIntravenous iron use in hemodialysis patients has greatly increased over the last decade, despite limited studies on the safety of iron.MethodsWe studied the association of receipt of intravenous iron with hospitalizations in an incident cohort of hemodialysis patients. We examined 9544 patients from Dialysis Clinic, Inc. (DCI). We ascertained intravenous iron use from DCI electronic medical record and USRDS data files, and hospitalizations through Medicare claims. We examined the association between iron exposure accumulated over 1-, 3- or 6-month time windows and incident hospitalizations in the follow-up period using marginal structural models accounting for time-dependent confounders. We performed sensitivity analyses including recurrent events models for multiple hospitalizations and models for combined outcome of hospitalization and death.ResultsThere were 22 347 hospitalizations during a median follow-up of 23 months. Higher cumulative dose of intravenous iron was not associated with all-cause, cardiovascular or infectious hospitalizations [HR 0.97 (95% CI: 0.77-1.22) for all-cause hospitalizations comparing >2100 mg versus 0-900 mg of iron over 6 months]. Findings were similar in models examining the risk of hospitalizations in 1- and 3-month windows [HR 0.88 (95% CI: 0.79-0.99) and HR 0.88 (95% CI: 0.74-1.03), respectively] or the risk of combined outcome of hospitalization and death in the 6-month window [HR 0.98 (95% CI: 0.78-1.23)].ConclusionsHigher cumulative dose of intravenous iron may not be associated with increased risk of hospitalizations in hemodialysis patients. While clinical trials are needed, employing higher iron doses to reduce erythropoiesis-stimulating agents does not appear to increase morbidity in routine clinical care.

57. The Principal's Role in Standards-Based Reform: Linking Accountability to Instructional Improvement.

Abstract

In 1997, the Nevada Legislature enacted the Nevada Education Reform Act to implement standards-based reform in all academic subjects in Nevada's schools. The act required that both teachers and principals be engaged in the process. This report describes a study of secondary principals' and secondary science teachers' perceptions of the impact that the new science standards have had on six areas: (1) instruction; (2) curriculum; (3) assessment; (4) accountability; (5) professional development; and (6) supervision. Attention was focused on the instructional leadership activities of principals and what they did to ensure implementation of Nevada science standards. Data were collected through a questionnaire (analyzed using descriptive statistics and t tests) and telephone interviews (analyzed qualitatively through domain analysis). The sample consisted of 159 science teachers and 56 administrators. The results showed that perceptions of administrators and science teachers on the impact of Nevada science standards differed significantly in all six areas related to instructional leadership. Principals tended to view standards as having greater impact on curriculum and instructional practices than teachers did. Teachers and principals generally perceived that teachers, more than principals, are accountable for student achievement. Also includes a summary of questionnaire results in tabular form. 60. Technology Tools for Data-Driven Decision Making: Promising Professional Development for Rural School Leaders.

Abstract

The Information Environment for School Leadership Preparation (IESLP) is a Web-based instructional system for educational administration preparation programs. The Web site presents a virtual rural school environment: a database of an actual rural school (using pseudonyms) containing information about students, teachers, school board members, test scores, curriculum, policies, and community demographics. It also presents problems that students are to address as school administrators. In an evaluation of the program, students were presented with a scenario concerning parent complaints about low student test scores. Students were organized into teams of four and required to prepare an analysis of student test scores and recommendations for a school improvement plan. Results indicated that after using IESLP, students tended to have a broader perspective on their approaches to decision making in that problems were more apt to be identified in terms of the total organization. Students showed a greater awareness of the use of many different kinds of data, not just peoples' perceptions, as essential sources of information for decision making. Students continued to value participatory decision-making but also indicated the need for people in the organization to possess the necessary information and data to make good decisions. Students reported gains in their professional use of technology in all areas, but especially in the use of spreadsheets and databases. Implications for educational administration preparation and professional development are discussed. (TD)

Abstract

Understanding African-American families' experiences with treatment for kidney failure is necessary for informing the delivery of family-centered care and the design of appropriate interventions. This qualitative study explored treatment-related questions, concerns, and family impacts among African-American family members of patients with pre-kidney failure and kidney failure. Thirty-five family members participated in focus groups stratified by patients' treatment experiences (pre-kidney failure, in-center hemodialysis, peritoneal dialysis, awaiting living-donor kidney transplantation, or post-transplantation). Family members raised questions and concerns about the psychological, lifestyle, and practical aspects of treatment. Similarly, discussions about family impacts emphasized psychosocial effects, lifestyle consequences, and the provision and receipt of support. Efforts to address these questions, concerns, and perceived family impacts through additional research, early and tailored education, and supportive interventions are needed. Abstract

BackgroundMany patients with chronic kidney disease (CKD) have difficulty becoming actively engaged in the pursuit of preemptive living donor kidney transplantation.Study designThe Talking About Live Kidney Donation (TALK) Study was a randomized controlled trial of the effectiveness of educational and social worker interventions designed to encourage early discussions and active pursuit of preemptive living donor kidney transplantation in patients with progressive CKD.Setting & participantsWe recruited participants with progressive CKD from academically affiliated nephrology practices in Baltimore, MD.InterventionParticipants randomly received: (1) usual care (routine care with their nephrologists), the (2) TALK education intervention (video and booklet), or the (3) TALK social worker intervention (video and booklet plus patient and family social worker visits).OutcomesWe followed participants for 6 months to assess their self-reported achievement of behaviors reflecting their discussions about and/or pursuit of living donor kidney transplantation (discussions with family, discussions with physicians, initiating recipient evaluation, completing recipient evaluation, and identifying a potential living donor).MeasurementsWe assessed outcomes through a questionnaire at 1-, 3-, and 6-months follow-up.ResultsParticipants receiving usual care with their nephrologists (n = 44), TALK education (n = 43), and the TALK social worker (n = 43) were similar at baseline. TALK Study interventions improved participants' living donor kidney transplantation discussion and pursuit behaviors, with the social worker leading to greater patient activation (participants' predicted probability of achieving living donor kidney transplantation discussions, evaluations, or donor identification over 6 months): probabilities were 30% (95% CI, 20%-46%), 42% (95% CI, 33%-54%), and 58% (95% CI, 41%-83%), respectively, in the usual care, TALK education, and TALK social worker groups (P = 0.03).LimitationsOur population was well educated and mostly insured, potentially limiting generalizability of our findings.ConclusionsTALK interventions improved discussion and active pursuit of living donor kidney transplantation in patients with progressive CKD and may improve their use of preemptive living donor kidney transplantation.

65. Selecting renal replacement therapies: what do African American and non-African American patients and their families think others should know? A mixed methods study

Abstract Background Little is known regarding the types of information African American and non-African American patients with chronic kidney disease (CKD) and their families need to inform renal replacement therapy (RRT) decisions. Methods In 20 structured group interviews, we elicited views of African American and non-African American patients with CKD and their families about factors that should be addressed in educational materials informing patients’ RRT selection decisions. We asked participants to select factors from a list and obtained their open-ended feedback. Results Ten groups of patients (5 African American, 5 non-African American; total 68 individuals) and ten groups of family members (5 African American, 5 non-African American; total 62 individuals) participated. Patients and families had a range (none to extensive) of experiences with various RRTs. Patients identified morbidity or mortality, autonomy, treatment delivery, and symptoms as important factors to address. Family members identified similar factors but also cited the effects of RRT decisions on patients’ psychological well-being and finances. Views of African American and non-African American participants were largely similar. Conclusions Educational resources addressing the influence of RRT selection on patients’ morbidity and mortality, autonomy, treatment delivery, and symptoms could help patients and their families select RRT options closely aligned with their values. Including information about the influence of RRT selection on patients’ personal relationships and finances could enhance resources’ cultural relevance for African Americans.

66. Comparative effectiveness studies to improve clinical outcomes in end stage renal disease: the DEcIDE patient outcomes in end stage renal disease study

Abstract

Abstract Background Evidence is lacking to inform providers’ and patients’ decisions about many common treatment strategies for patients with end stage renal disease (ESRD). Methods/design The DEcIDE Patient Outcomes in ESRD Study is funded by the United States (US) Agency for Health Care Research and Quality to study the comparative effectiveness of: 1) antihypertensive therapies, 2) early versus later initiation of dialysis, and 3) intravenous iron therapies on clinical outcomes in patients with ESRD. Ongoing studies utilize four existing, nationally representative cohorts of patients with ESRD, including (1) the Choices for Healthy Outcomes in Caring for ESRD study (1041 incident dialysis patients recruited from October 1995 to June 1999 with complete outcome ascertainment through 2009), (2) the Dialysis Clinic Inc (45,124 incident dialysis patients initiating and receiving their care from 2003–2010 with complete outcome ascertainment through 2010), (3) the United States Renal Data System (333,308 incident dialysis patients from 2006–2009 with complete outcome ascertainment through 2010), and (4) the Cleveland Clinic Foundation Chronic Kidney Disease Registry (53,399 patients with chronic kidney disease with outcome ascertainment from 2005 through 2009). We ascertain patient reported outcomes (i.e., health-related quality of life), morbidity, and mortality using clinical and administrative data, and data obtained from national death indices. We use advanced statistical methods (e.g., propensity scoring and marginal structural modeling) to account for potential biases of our study designs. All data are de-identified for analyses. The conduct of studies and dissemination of findings are guided by input from Stakeholders in the ESRD community. Discussion The DEcIDE Patient Outcomes in ESRD Study will provide needed evidence regarding the effectiveness of common treatments employed for dialysis patients. Carefully planned dissemination strategies to the ESRD community will enhance studies’ impact on clinical care and patients’ outcomes.

Abstract

Abstract Background Few educational resources have been developed to inform patients’ renal replacement therapy (RRT) selection decisions. Patients progressing toward end stage renal disease (ESRD) must decide among multiple treatment options with varying characteristics. Complex information about treatments must be adequately conveyed to patients with different educational backgrounds and informational needs. Decisions about treatment options also require family input, as families often participate in patients’ treatment and support patients’ decisions. We describe the development, design, and preliminary evaluation of an informational, evidence-based, and patient-and family-centered decision aid for patients with ESRD and varying levels of health literacy, health numeracy, and cognitive function. Methods We designed a decision aid comprising a complementary video and informational handbook. We based our development process on data previously obtained from qualitative focus groups and systematic literature reviews. We simultaneously developed the video and handbook in “stages.” For the video, stages included (1) directed interviews with culturally appropriate patients and families and preliminary script development, (2) video production, and (3) screening the video with patients and their families. For the handbook, stages comprised (1) preliminary content design, (2) a mixed-methods pilot study among diverse patients to assess comprehension of handbook material, and (3) screening the handbook with patients and their families. Results The video and handbook both addressed potential benefits and trade-offs of treatment selections. The 50-minute video consisted of demographically diverse patients and their families describing their positive and negative experiences with selecting a treatment option. The video also incorporated health professionals’ testimonials regarding various considerations that might influence patients’ and families’ treatment selections. The handbook was comprised of written words, pictures of patients and health care providers, and diagrams describing the findings and quality of scientific studies comparing treatments. The handbook text was written at a 4th to 6th grade reading level. Pilot study results demonstrated that a majority of patients could understand information presented in the handbook. Patient and families screening the nearly completed video and handbook reviewed the materials favorably. Conclusions This rigorously designed decision aid may help patients and families make informed decisions about their treatment options for RRT that are well aligned with their values.

68. Enduring Expertise of School Psychologists and the Changing Demands of Schools in the United States.

Abstract

This book chapter explores the integration of two basic premises: (1) education in the United States will always be changing, and (2) school psychologists have fundamental expertise that will always be necessary regardless of changes in schools. Some changes in U.S. education are: school reform and organizational changes; special education reform; school health care services; increased diversity in student populations; school safety; financial cutbacks and downsizing; and implementation of state and national standards such as Goals 2000. Areas of expertise in school psychology that transcend the changes in education are found in the mini-series topics in one of the major journals in school psychology, "School Psychology Review." Topics are: (1) assessment; (2) consultation for educators and parents; (3) addressing the needs of diverse student populations; and (4) application of research to educational practices and policy. Eight guidelines to promote flexibility and growth in school psychology so that school psychologists remain indispensable professionals are presented. One table lists selected topics of mini-series and thematic issues in "School Psychology Digest" and "School Psychology Review." (JBJ)

Abstract Background Living related kidney transplantation (LRT) is underutilized, particularly among African Americans. The effectiveness of informational and financial interventions to enhance informed decision-making among African Americans with end stage renal disease (ESRD) and improve rates of LRT is unknown. Methods/design We report the protocol of the Providing Resources to Enhance African American Patients’ Readiness to Make Decisions about Kidney Disease (PREPARED) Study, a two-phase study utilizing qualitative and quantitative research methods to design and test the effectiveness of informational (focused on shared decision-making) and financial interventions to overcome barriers to pursuit of LRT among African American patients and their families. Study Phase I involved the evidence-based development of informational materials as well as a financial intervention to enhance African American patients’ and families’ proficiency in shared decision-making regarding LRT. In Study Phase 2, we are currently conducting a randomized controlled trial in which patients with new-onset ESRD receive 1) usual dialysis care by their nephrologists, 2) the informational intervention (educational video and handbook), or 3) the informational intervention in addition to the option of participating in a live kidney donor financial assistance program. The primary outcome of the randomized controlled trial will include patients’ self-reported rates of consideration of LRT (including family discussions of LRT, patient-physician discussions of LRT, and identification of a LRT donor). Discussion Results from the PREPARED study will provide needed evidence on ways to enhance the decision to pursue LRT among African American patients with ESRD. Abstract

We conducted focus group meetings of African American and non-African American patients with end-stage renal disease (six groups) and their family members (six groups), stratified by race/ethnicity and treatment. We elicited differences in participants' experiences with shared decision making about initiating renal replacement therapy (RRT; that is, hemodialysis, peritoneal dialysis, or a kidney transplant). Patients were often very sick when initiating RRT, and had little, if any, time to make a decision about what type of RRT to initiate. They also lacked sufficient information about alternative treatment options prior to initiation. Family members played supportive roles and shared in decision making when possible. Reports were similar for African American and non-African American participants. Our findings suggest that a greater emphasis on the improved engagement of patients and their families in shared decision making about RRT initiation is needed for both ethnic/racial minorities and nonminorities.

84. Development of an Administrative Data-Based Frailty Index for Older Adults Receiving Dialysis

Abstract

BACKGROUND: Frailty is present in ≥50% of older adults receiving dialysis. Our objective was to a develop an administrative data–based frailty index and assess the frailty index’s predictive validity for mortality and future hospitalizations. METHODS: We used United States Renal Data System data to establish two cohorts of adults aged ≥65 years, initiating dialysis in 2013 and in 2017. Using the 2013 cohort (development dataset), we applied the deficit accumulation index approach to develop a frailty index. Adjusting for age and sex, we assessed the extent to which the frailty index predicts the hazard of time until death and time until first hospitalization over 12 months. We assessed the Harrell’s C-statistic of the frailty index, a comorbidity index, and jointly. The 2017 cohort was used as a validation dataset. RESULTS: Using the 2013 cohort (n=20,974), we identified 53 deficits for the frailty index across seven domains: disabilities, diseases, equipment, procedures, signs, tests, and unclassified. Among those with ≥1 deficit, the mean (SD) frailty index was 0.30 (0.13), range 0.02–0.72. Over 12 months, 18% (n=3842) died, and 55% (n=11,493) experienced a hospitalization. Adjusted hazard ratios for each 0.1-point increase in frailty index in models of time to death and time to first hospitalization were 1.41 (95% confidence interval, 1.37 to 1.44) and 1.33 (95% confidence interval, 1.31 to 1.35), respectively. For mortality, C-statistics for frailty index, comorbidity index, and both indices were 0.65, 0.65, and 0.66, respectively. For hospitalization, C-statistics for frailty index, comorbidity index, and both indices were 0.61, 0.60, and 0.61, respectively. Data from the 2017 cohort were similar. CONCLUSIONS: We developed a novel frailty index for older adults receiving dialysis. Further studies are needed to improve on this frailty index and validate its use for clinical and research applications.

Abstract

The sequencing and analysis of two close relatives of Bacillus anthracis are reported. AFLP analysis of over 300 isolates of B. cereus, B. thuringiensis and B. anthracis identified two isolates as being very closely related to B. anthracis. One, a B. cereus, BcE33L, was isolated from a zebra carcass in Nambia; the second, a B. thuringiensis, 97-27, was isolated from a necrotic human wound. The B. cereus appears to be the closest anthracis relative sequenced to date. A core genome of over 3,900 genes was compiled for the Bacillus cereus group, including B anthracis. Comparative analysis of these two genomes with other members of the B. cereus group provides insight into the evolutionary relationships among these organisms. Evidence is presented that differential regulation modulates virulence, rather than simple acquisition of virulence factors. These genome sequences provide insight into the molecular mechanisms contributing to the host range and virulence of this group of organisms.

88. Clomazone: Toxicity, Biotransformation, Resistance and Interaction with P450 Inhibitors in Rice (Oryza sativa) and Watergrasses (Echinochloa spp.)

Abstract

Rice (Oryza sativa), early watergrass (Echinochloa oryzoides); EWG; biotypes resistant (R) and susceptible (S) to thiocarbamates and aryloxyphenoxy propionate herbicides), and late watergrass (Echinochloa phyllopogon; LWG; R and S biotypes) were exposed to clomazone for 7 d. Whole-plant change in fresh weight and B-carotene concentrations were measured. For growth, the No Observed Effect Concentrations (NOECs) were 7.9, 0.21, 0.21, 0.46, and 0.46 µM clomazone for rice, EWG (R), EWG (S), LWG (R), and LWG (S), respectively, while the concentrations causing 25% inhibition in response (IC25s) were 5.6, 0.46, 0.42, 0.92 and 0.79 µM clomazone, respectively. For rice, EWG (R), EWG (S), LWG (R), and LWG (S), the B-carotene NOECs were 0.21, < 0.08, < 0.08, 0.08 and 0.46 µM clomazone, respectively and IC25s were 0.42, 0.08, 0.08, 0.33 and 0.54 µM, respectively. At a field-relevant clomazone application rate resistance to clomazone was observed in EWG (R) and LWG (R) as measured by growth and B-carotene, respectively. Due to similar sensitivity between rice and late watergrass, use of clomazone in rice culture may require the use of a safening technique.Rice and EWG (S) were exposed to 14C-labeled clomazone to determine accumulation, biotransformation and mass balance. In terms of extractable 14C, rice contained more total residues than EWG (p < 0.05), but the concentration in EWG was significantly higher (p < 0.01). More metabolized residue was measured in EWG compared to rice (p < 0.01). Both species produced hydroxylated forms of clomazone, B-D-glucoside conjugates, and unidentified polar metabolites. The suspected active metabolite, 5-ketoclomazone, was found at a significantly higher concentration in EWG vs. rice (21 ± 2 vs. 5.7 ± 0.5 pmol g-1, respectively; p < 0.01). Qualitatively and quantitatively, more clomazone metabolism was observed in the relatively sensitive EWG compared to rice.R and S biotypes of EWG and LWG were exposed to clomazone alone and in combination with P450 inhibitors. Growth reduction (GR25, GR50, and GR75) values were lower for S biotypes than for R biotypes (p < 0.05) with exposure to clomazone alone indicating resistance to clomazone in the R biotypes. In combination with clomazone disulfoton increased 20-d growth in LWG (R) by as much as 5-fold, compared to application of clomazone alone. Oxydemeton methyl increased 20-d growth by as much as 6-fold. Disulfoton and oxydemeton methyl reduced !)-carotene levels compared to treatment with clomazone alone. For EWG (S), disulfoton increased 4-d growth by 46% and increased B-carotene levels by 52%. Demeton-S increased B-carotene levels by 48%. At 6800 g ai ha-1 PBO increased growth and B-carotene levels by 28 and 48%, respectively. These results indicate that P450 enzymes serve both to activate and detoxify clomazone.

Abstract

Background Little is known about the impact of COVID-19 on patient, family member, and stakeholder patient-centered outcomes research engagement. Objective To answer the research questions: (1) What is the impact of COVID-19 on the lives of patients with kidney disease and their families? (2) What is the impact of COVID-19 on research engagement for patient and family member research team members who are themselves at very high risk for poor COVID-19 outcomes? and (3) How can we help patients, family members, and stakeholder team members engage in research during COVID-19? Design We conducted virtual semi-structured interviews with patient and family member co-investigators and kidney disease stakeholders from the PREPARE NOW study during November 2020. The interview guide included questions about participants’ experiences with the impact of COVID-19 on research engagement. Participants Seven patient and family member co-investigators and eight kidney disease stakeholders involved in a kidney disease patient-centered outcomes research project participated in the interviews, data analysis, and writing this manuscript. Approach We used a content analysis approach and identified the main themes using an inductive process. Key Results Respondents reported three main ways that COVID-19 has impacted their lives: emotional impact, changing behaviors, and changes in health care delivery. The majority of respondents reported no negative impact of COVID-19 on their ability to engage in this research project. Suggestions for patient-centered outcomes research during COVID-19 and other emergencies include virtual research activities; active engagement; and promoting trust, honesty, transparency, and authenticity. Conclusions COVID-19 has had a significant negative impact on patient, family member, and stakeholder research team members; however, this has not resulted in less research engagement. Abstract

Purpose of Review To summarize evidence of impact of social determinants of health (SDOH) on diabetes risk, morbidity, and mortality and to illustrate this impact in a population context. Recent Findings Key findings from the American Diabetes Association’s scientific review of five SDOH domains (socioeconomic status, neighborhood and physical environment, food environment, health care, social context) are highlighted. Population-based data on Black/African American adults illustrate persisting diabetes disparities and inequities in the SDOH conditions in which this population is born, grows, lives, and ages, with historical contributors. SDOH recommendations from US national committees largely address a health sector response, including health professional education, SDOH measurement, and patient referral to services for social needs. Fewer recommendations address solutions for systemic racism and socioeconomic discrimination as root causes. Summary SDOH are systemic, population-based, cyclical, and intergenerational, requiring extension beyond health care solutions to multi-sector and multi-policy approaches to achieve future population health improvement.
