60. Is prophylactic formal fusion with implant revision necessary in non-ambulatory children with spinal muscular atrophy and growing rods who are no longer lengthened?

Abstract

Study design: Single center, retrospective chart review. Objectives: To determine if routine posterior spinal fusion (PSF) is unnecessary in non-ambulatory growing rod graduates with SMA. Summary of background data: Most non-ambulatory children with SMA develop early-onset scoliosis (EOS). Posterior growing rods (GR) have been shown safe and effective in managing spinal deformities in these children. The best management of these children, once graduated from their GR, is currently unknown. In this study, we report the clinical results of managing these children without routine definitive fusion following a course of GR treatment. Methods: A single-center, retrospective chart and radiographic review was performed on children with SMA treated with posterior distraction GR, with a two-year minimum follow-up since final lengthening. Electronic medical records and radiographs were reviewed for demographic variables, Cobb measurements, implant revisions, occult radiographic implant failure, symptomatic failure, and/or conversion to PSF. Result: 12 patients (2 type 1, 9 type 2, 1 type 1/2) met inclusion criteria. Mean age at growing rod insertion was 6.2 years of age (range 4.1–8.2) and age at final lengthening 10.3 years of age (range 9.3–11.9). The mean time between last lengthening and latest clinical or radiographic review was 5.5 (range 2.1–9.0) years. Average mean pre, post, final Cobb angles were 71°, 27° (p< 0.001), 25°. Following final lengthening, only one patient required hardware revision and conversion to definitive fusion in attempts to alleviate chronic hip pain, which was unsuccessful. One additional patient was found to have an occult rod failure that has not required treatment. Conclusion: While limited by sample size, this single-center cohort of non-ambulatory SMA patients with EOS treated with similar constructs suggests that routine, definitive fusion in SMA GR graduates may be unnecessary. Abstract

Background: The World Health Organization (WHO) introduced the International Classification of Functioning, Disability, and Health (ICF) as a scientific method of disability data collection comprised of >1,200 categories describing the spectrum of impairment types (functional, symptoms-based and anatomical) under the bio-psycho-social model with consideration of environmental and personal factors (pf). ICF Core Sets and ICF Checklists are streamlined disease-specific resources for clinical use, service provision, and for use in health economics and health policy. ICF can disclose strengths and weaknesses across multiple patient-reported outcome measures (PROMs) and help consolidate best-fitting question-items from multiple PROMs. Interstitial lung diseases (ILDs), are generally progressive, with restrictive physiology sometimes occurring in the context of multi-organ autoimmunity/inflammatory conditions such as connective tissue diseases (CTDs). In spite of significant associated morbidity and potential disability, ILD has yet to be linked to the ICF.Methods: Each instrument and their question-items within the consensus-recommended core sets for clinical trials in ILD were deconstructed to single concept units, and then linked per updated ICF linkage rules. Inter-linker agreement was established. Three additional subsequently validated measures were also included.Results: One-hundred-eleven ICF categories were identified for ten PROMs and three traditional objective measures that were amenable to ICF linkage. The proportion of agreement ranged from 0.79 (95% CI: 0.62, 0.91) to 0.93 (0.76, 0.99) with the overall proportion of inter-linker agreement being very high 0.86 (0.82, 0.89) for the initial instruments, with 94–100% for the three additional PROMs. Thirty-four new ‘Personal Factors’ emerged to capture disease-specific qualities not elsewhere described in ICF, e.g. ‘pf_embarrassed by cough’ or ‘pf_panic/afraid when can’t get a breath’.Conclusion: This first known effort in ICF linkage of ILD has provided important revelations on the current utility of the ICF in lung disease. Results have indicated areas for meaningful assessment of ICF descriptors for lung impairment. The mapping across PROMs provides insight into possibilities of developing more streamline and precise instrumentation. Finally, familiarity with the ICF in ILD may enable clinicians to experience a smoother transition with the imminent harmonization of ICD and ICF, ICD-11.

67. Family as Faculty Parents: Influence on Teachers' Beliefs about Family Partnerships

Abstract

The authors describe how the participation of Family as Faculty parents in case-based instruction influenced 16 beliefs of 89 special education teacher candidates about the value of partnering with parents. The parents, who all had children with disabilities, participated in a simulated individualized education program meeting that was embedded in a teacher-preparation course to bring authenticity to the experience. Results indicated that using parents as co-instructors strengthened 3 family-friendly beliefs and reduced 4 beliefs that do not support family partnerships. Implications included the importance of educators' directly involving parents in preparing teacher candidates to collaborate positively with families. (Contains 1 note and 4 tables.)

69. Evidenced Based Practices that Promote Transition to Postsecondary Education: Listening to a Decade of Expert Voices

Abstract

As increasing numbers of students with disabilities access postsecondary education, research studies and literature reviews have investigated the needs of these students who chose to pursue postsecondary education. These articles included studies that (a) asked students with disabilities to identify needs and (b) summarized needs in literature reviews about students with disabilities in postsecondary education. This article summarizes needs and recommendations from college students with disabilities and authors who reviewed related literature from 1995-2006. The summary includes needs in five areas: self-determination, social skills, academic preparation, accommodations, and assistive technology (AT). Each of these areas of need is described and recommendations for practice are discussed. The purpose of this article is to identify a set of evidence-based transition practices that will address these needs and increase the likelihood of success for students who enroll in postsecondary education institutions.

70. A Call for Collaboration: Not Jack of All Trades

Abstract

With the reauthorization of the Individuals with Disabilities Education Act (IDEA 2004) and No Child Left Behind legislation (2001), the call for collaboration is omnipresent. A review of the current landscape makes this imperative obvious. With the mandate for inclusion and the necessity for special education teachers to collaborate with their general education peers to meet the needs of a continually increasing number of diverse learners, the authors identified the need for faculty in teacher education programs to demonstrate collaborative practices, such as planning and implementing instruction, for all preservice teachers. With the concept of shared responsibility in mind and the importance of modeling best practices for University of North Florida (UNF) preservice teachers, the authors developed a model for teaching the Introduction to Exceptionalities course. The course was designed not only to deliver the critical and required contents of the course, but also to demonstrate to preservice teachers exciting and effective ways of working together and sharing the responsibility for teaching. This article describes how the authors modeled collaboration in curriculum and pedagogy at the university level. Additionally, they present the benefits derived from this collaboration for both students and faculty.

73. Increasing Positive Outcomes for African American Males in Special Education with the Use of Guided Notes

Abstract

The use of appropriate interventions is a critical component of educating students, particularly African Americans in special education. The purpose of this study was to examine the effects of guided notes on the academic performance of eight African American boys identified as having emotional or behavioral disorders (EBD) and learning disabilities (LD) in special education. Findings indicate that the use of guided notes could be an effective intervention strategy for improving academic performance and positive outcomes for students with EBD. This study supported earlier findings in which guided notes were used during class instruction. (Contains 2 tables.)

74. What Classroom Teachers Need to Know about IDEA '97

Abstract

In the general classroom setting, informed teachers can deliver necessary and appropriate services to students with special needs, while at the same time work toward successful outcomes for those children, their peers, and their parents. IDEA '97 clearly defines educators' responsibilities with regard to children with disabilities, including teachers in both special education and inclusive general education settings. Many general education teachers are limited in their knowledge of special education law and policies, yet they play an integral role in educating students with special needs. IDEA '97 mandates that students with disabilities be educated with children without disabilities to the maximum extent appropriate in the least restrictive environment. For many children in special education, this is the general education classroom. The teacher's role in that setting is even more significant. Given that the principles of IDEA '97 are clearly defined by law, educators in general and special education can provide the necessary and appropriate services that will most likely lead to successful outcomes not only for children with disabilities, but also their peers, parents, and teachers that are involved in this very important process called special education.

Abstract

Nailfold capillaroscopy (NFC) is a simple noninvasive microscopic technique used to identify characteristic morphological abnormalities in the nailfold capillaries. The presence of this microvasculopathy appears to be of fundamental importance in the pathological processes that underlie the scleroderma spectrum disorders (including dermatomyositis and antisynthetase myositis). This review discusses the different methodologies and techniques in performing NFC and stresses the diagnostic utility achieved with simple 'bedside' techniques utilising the ophthalmoscope, dermatoscope or smart phone. Recent advances in reporting abnormal microvascular patterns and vascular metrics (e.g. capillary density and dropout) are discussed. The aetiopathogenesis of the microvasculopathy is currently unknown but its close association with Raynaud Phenomena and specific autoantibodies together with recent observations from sequential NFC allows speculations on its possible mechanism. Finally, future developments in the use of NFC as a possible biomarker in the management of the scleroderma spectrum disorders are discussed, with a recommendation that NFC becomes more widely available, particularly in rheumatological, immunological and dermatological practice.
