Abstract

Objective: This study aimed to report the changes made to ENT placements across the UK in response to the pandemic and their impact on student experience. Methods: A questionnaire eliciting how ENT placements were provided before and after coronavirus disease 2019 was disseminated amongst Student and Foundation Doctors in Otolaryngology representatives. Results: Thirty-eight respondents from 27 medical schools across the UK completed the survey (response rate of 90 per cent). Twenty-nine of the 38 respondents (76 per cent) reported a change in ENT placements in response to the pandemic. Six of the 38 students (16 per cent) remained satisfied with their ENT placements, as compared to 12 students prior to the pandemic (32 per cent). Conclusion: There is considerable variability in how medical schools responded to the pandemic. Most medical schools placed students into smaller groups, with less direct contact in the hospital. These changes resulted in lower student satisfaction. 83. Determinants of enrollment in the NHIS for women in Ghana

Abstract

Background\ud The National Health Insurance Scheme (NHIS) was introduced in 2005 to provide equitable access to healthcare. Furthermore, concessions were made for pregnant women, yet inequities in access continue to exist. This study explores whether dimensions of social exclusion explain why some groups of women are not benefitting from the scheme.\ud \ud Methods\ud Data was collected from 4050 representative households in five districts. Logistic regression is used to examine the factors that determine enrolment of women under the NHIS.\ud \ud Results\ud The study sample consists of a sub-sample of 3,173 women out of whom 58% were insured. The majority (64.9%) of the women were in the reproductive age (15-45 years). The results show that wealth status, age, health status, locality, perception about the quality of care at health facilities and perception of the NHIS, are the key factors that determine enrolment into the scheme.\ud \ud Conclusion\ud With women dominating the informal sector of Ghana’s economy which is often characterised by relatively low incomes, these inequities in access need to be addressed.

Abstract

Background: Chronic lateral ankle instability describes multiple pathologies affecting the tibiotalar or talocalcaneal articulations of the foot. One quarter of cases are caused by isolated or concomitant subtalar instability. The standard anterior drawer test (ADT) is used in routine clinical practice to assess abnormal movement at the tibiotalar joint. However, a positive test does not discriminate whether excessive translation is at the tibiotalar or subtalar joint. We suggest a modification to the standard ADT which separates the ankle joint translation by manually immobilising the tibiotalar joint in order isolate anterior translation at the subtalar joint. In the presence of a positive ADT, the modified anterior drawer test (MADT) would help discriminate if the pathological translation is at the tibiotalar or the subtalar joint or both. Methods: Our clinical study was an observational analysis of a cohort of 12 patients who had presented to an outpatient foot clinic in east of England. In addition, a cadaveric study was used to assess subtalar anterior translation. Results: In 50% of patients who had a clinically positive ADT, the MADT was negative which correlated 100% with Stress Broden radiographic views. Cadaveric analysis was done to show an increase in anterior translation with sequential sectioning of supporting ligaments. We demonstrated an absolute reduction in anterior translation in subtalar instability with the additional posterior force (MADT). Conclusion: The clinical and cadaveric data supports the use of the MADT as a valuable adjunct in the diagnosis and exclusion of subtalar instability as a cause of symptoms in addition to stress radiography.

Abstract

Electrorheological fluids (ERF) are an intriguing class of non-Newtonian industrial fluids. They consist of fine dielectric particles suspended in liquids of low dielectric constants. The objectives of this research were to select a particulate system such that: (1) its density can be varied to match that of the selected liquid, and (2) the dielectric constant of the particles and the liquids should be such that the critical fields needed for asymptotic increase in viscosity are less than or equal to 10 KV/cm. Synthetic Zeolite particles were selected as the solute/suspensions. Octoil oil was selected as the solvent. The results are summarized here.

Abstract

Ferroelectric liquid crystals in a new configuration, termed partially exposed polymer dispersed ferroelectric liquid crystal (PEPDFLC), respond to external pressures and demonstrate pressure-induced electro-optic switching response. When the PEPDFLC thin film is sandwiched between two transparent conducting electrodes, one a glass plate and the other a flexible sheet such as polyvenylidene fluoride, the switching characteristics of the thin film are a function of the pressure applied to the flexible transparent electrode and the bias voltage across the electrodes. Response time measurements reveal a linear dependence of the change in electric field with external pressure.

Report describes preliminary investigation of new monomer/polymer liquid crystal system, thin film of shear-sensitive cholesteric monomer liquid crystal (TI 511) on Xydar (STR800) (or equivalent) liquid crystal polymer substrate. Monomer/polymer liquid crystal films applied to surfaces provide quantitative indications of shear stresses caused by winds blowing along surfaces. Effects of shear stresses reversible in new coating system. System provides quantitative data on flows in wind tunnels.

Abstract

Partially exposed polymer dispersed liquid crystal thin film (10-25 microns) deposited on a flat glass substrate has been used for the first time to measure skin friction. Utilizing the shear-stress-induced director reorientation in the partially exposed liquid-crystal droplets, optical transmission under crossed polarization has been measured as a function of the air flow differential pressure. Direct measurement of the skin friction with a skin friction drag balance, under the same aerodynamic conditions, lets us correlate the skin friction with optical transmission. This provides a unique technique for the direct measurement of skin friction from the transmitted light intensity. The results are in excellent agreement with the model suggested in this paper.

Abstract

Polymer dispersed liquid crystal thin films have been deposited on a glass substrate, utilizing the processes of polymerization and solvent evaporation induced phase separation. Liquid crystal microdroplets trapped on the upper surface of the thin film respond to the shear stress due to air or gas flow on the surface layer. Response to an applied step shear stress input on the surface layer has been measured by measuring the time response of the transmitted light intensity. Initial results on the measurements of the light transmission as a function of the air flow differential pressure indicate that these systems offer features suitable for boundary layer and gas flow sensors.

Abstract

Polymer dispersed liquid crystal thin films have been deposited on glass substrates by the processes of polymerization and solvent evaporation induced phase separation. The electron and the optical polarization microscopies of the films reveal that PDLC microdroplets formed during the process of phase separation near the top surface of the film remain exposed and respond to shear stress due to air or gas flow on the surface. Optical response of the film to an air flow-induced shear stress input on the free surface has been measured. Director orientation in the droplets changes with the applied shear stress leading to time varying transmitted light intensity. Director dynamics of the droplet for an applied step shear stress has been discussed from free energy considerations. Results on the measurement of light transmission as a function of the gas flow parameter unambiguously demonstrate the potential of these systems for use as boundary layer and gas flow sensors.

Abstract

The first experimental observation of a change in the director azimuthal angle due to applied shear stress is reported in a sample configuration involving a liquid-crystal-coated top surface exposed directly to gas flow. The electrooptic response caused by the shear stress is large, fast, and reversible. These findings are relevant to the use of liquid crystals in boundary layer investigations on wind tunnel models.
