55. Italian Evaluation and Excellence in REMS (ITAL-EE-REMS): appropriate placement of forensic patients in REMS forensic facilities

Abstract

Abstract Background We set out to assess the appropriateness of current placement of mentally disordered offenders allocated by the courts in Italy to REMS or to forensic community residences. We hypothesised that as in other countries, the match between a standardised assessment and the decision of the court would be imperfect. Methods The DUNDRUM Toolkit was translated into Italian. The translation had good psychometric properties. In order to compare the current level of therapeutic security with a calculated safest current placement, we compared the DUNDRUM-1 triage security assessment of need for therapeutic security prior to treatment, with evidence for progress made in treatment (DUNDRUM-3) and forensic recovery (DUNDRUM-4). The more conservative of these two would be taken as the safe current level of need for therapeutic security. Results The Italian translation of the DUNDRUM Toolkit had good internal consistency and mean scores had a Reliable Change Index less than one unit. 3.7% of those in REMS (medium security) were assessed as needing high security and 38% were ready to move to a less secure place. In low secure places, 56% were assessed as needing a higher level of therapeutic security and 6% could have moved to open non-secure places. Conclusions The Italian translation of the DUNDRUM Toolkit allows an assessment of the current working of the model of care for forensic psychiatry following the reforms of 2015. Most patients are safely placed. A small but important proportion needed high secure places that are not currently available. (3.7% of 604 nationally, 95% Confidence Interval 1.2% to 8.4%, 7 to 50). Abstract

Abstract Background Uncorrected refractive errors and amblyopia are reported as the two main causes of childhood visual impairment and blindness worldwide. Our purpose was to evaluate refractive status, ocular alignment and effective refractive error coverage (eREC) of school-aged children from low-income areas of Sao Paulo city, Brazil. Methods Data from the “Ver na Escola” Project were used for the current study. Children enrolled in the selected schools had an ophthalmic exam including eye alignment assessed by cover test, automatized and subjective dynamic and static refraction. The associations of demographic variables with occurrence and magnitude of refractive errors and eREC were investigated by multiple logistic regressions and multilevel mixed effect models. Results A total of 17,973 children (51.12% females) with mean ± sd age 8.24 ± 3.54 years old examined from July 2018 to July 2019, were included in the study. Most of the participants (73%) showed orthoposition of the visual axis for both distance and near. Heterophoria was found in about 25% of participants (N = 4,498), with 71.7% of them (N= 3,222) classified as exophoria. Less than 2% (N = 232) showed strabismus, most of them (N = 160) esotropia. Overall, 1,370 (7.70%) of participants had myopia and 577 (3.24%) had hyperopia. Abstract

The detection of genetic alterations in patients with acute leukemias is essential for the targeting of more specific and effective therapies. Therefore, the aim of this study was to compare the sensitivity of Nested-PCR and RT-qPCR techniques in the detection of genetic alterations in patients with acute leukemias. This study included samples from 117 patients treated at the Fortaleza General Hospital. All samples were submitted to analysis using the Nested-PCR and the RT-qPCR techniques. Acute Myeloid Leukemia (AML) patients’ samples were submitted to the analysis of the following alterations: FLT3-ITD, RUNX1::RUNX1T1, CBFB::MYH11 and PML::RARA; meanwhile, BCR::ABL1, TCF3::PBX1, KMT2A::AFF1, ETV6::RUNX1, and STIL::TAL1 fusions were investigated in the Acute Lymphoblastic Leukemia (ALL) patients’ samples. Throughout the study, 77 patients were diagnosed with AML and 40 with ALL. Among the 77 AML patients, FLT3-ITD, RUNX1::RUNX1T1, PML::RARA, and CBFB::MYH11 were detected in 4, 7, 10 and 8 patients, respectively. Among the 40 ALL patients, the presence of 23 patients with BCR::ABL1 translocation and 9 patients with TCF3::PBX1 translocation was observed through the RT-qPCR methodology. Overall, the present study demonstrated that the RT-qPCR technique presented a higher sensitivity when compared to the Nested-PCR technique at the time of diagnosis of the acute leukemia samples studied.

Abstract

The Sorrento Peninsula is characterized by a significant occurrence of examples of vernacular architecture, which exhibit distinctive morphological and constructive features. These have been the subject of numerous studies. However, some buildings have undergone a process of transformation over time, the details of which have yet to be investigated. Architectures that initially held a rural character were enlarged and ennobled, thereby becoming what could be termed as “villas of delights”. However, these clearly manifest their origin based on the permanence of some vernacular features. This paper focuses on the analysis of a case study, Villa Murat, which is exemplary in illustrating this process. This thorough interdisciplinary research combines historical investigation, based mainly on archival documents, with a direct examination of the Villa. This has enabled the retracing of the building’s evolution and of the events that occurred in it. An integrated survey, which employed photogrammetry and laser scanning, enabled the assessment of the current state of conservation. The ultimate objective of this research is to propose conservative interventions which, in conjunction with the suggested new intended use, could ensure the preservation of the Villa.

59. Cultural Differences in Teacher Immediacy and Classroom Communication Apprehension: An Empirical Study between Brazil and the United States

Abstract

Teacher immediacy and classroom communication apprehension have been well researched in the United States. Instructional communication research in Brazil is limited, and several topics still demand investigation among the population of Brazil. This study extends current research on teacher immediacy and classroom communication apprehension to the Brazilian population. We should extend the analysis to the Brazilian context instead of relying on previous investigations from other regions as the basis for cross-cultural theorizing. A total of 221 students participated in the survey. This study reports four significant findings: (1) Brazilian students' perceptions of teacher immediacy are greater than American students' perceptions of teacher immediacy; (2) American students have higher levels of Classroom Communication Apprehension than Brazilian students; (3) Classroom communication apprehension is negatively correlated with American students' perceptions of teacher immediacy; and (4) Classroom communication apprehension is not significantly correlated with Brazilian students' perceptions of teacher immediacy. Highly immediate teachers will have less apprehensive students.

Abstract

Through the lenses of Entrepreneurial Event Theory and the Affective Processing Principle, this study aims to explore the interplay between cognition and emotion in the entrepreneurship education-entrepreneurial intention link, exploring the specific role of fear, conceptualized as a negative, avoidance-oriented, emotion. A moderation- mediation empirical model was tested on a sample of Italian University students. The results of the study discovered significant interactions. Fear decreases all the direct and mediating relations, suggesting that an assessment of the importance of emotions in the students' learning and decision-making processes is a vital issue that entrepreneurship scholars are advised to take into account to foster the formation of intentional behavior. Theoretical advancements and implications for academics, professors, and students will be finally discussed.

Abstract

A search for time-directional coincidences of ultra-high-energy (UHE) photons above 10 EeV with gravitational wave (GW) events from the LIGO/Virgo runs O1 to O3 is conducted with the Pierre Auger Observatory. Due to the distinctive properties of photon interactions and to the background expected from hadronic showers, a subset of the most interesting GW events is selected based on their localization quality and distance. Time periods of 1000 s around and 1 day after the GW events are analyzed. No coincidences are observed. Upper limits on the UHE photon fluence from a GW event are derived that are typically at $\sim$7 MeV cm$^{-2}$ (time period 1000~s) and $\sim$35 MeV cm$^{-2}$ (time period 1 day). Due to the proximity of the binary neutron star merger GW170817, the energy of the source transferred into UHE photons above 40 EeV is constrained to be less than 20% of its total gravitational wave energy. Abstract

The Kubo formula is a cornerstone in our understanding of near-equilibrium transport phenomena. While conceptually elegant, the application of Kubo's linear-response theory to interesting problems is hindered by the need for algorithms that are accurate and scalable to large lattice sizes beyond one spatial dimension. Here, we propose a general framework to numerically study large systems, which combines the spectral accuracy of Chebyshev expansions with the efficiency of divide-and-conquer methods. We use the hybrid algorithm to calculate the two-terminal conductance and the bulk conductivity tensor of 2D lattice models with over $10^7$ sites. By efficiently sampling the microscopic information contained in billions of Chebyshev moments, the algorithm is able to accurately resolve the linear-response properties of complex systems in the presence of quenched disorder. Abstract

PURPOSE Worldwide clinical knowledge is expanding rapidly, but physicians have sparse time to review scientific literature. Large language models (eg, Chat Generative Pre-trained Transformer [ChatGPT]), might help summarize and prioritize research articles to review. However, large language models sometimes 'hallucinate' incorrect information. METHODS We evaluated ChatGPT's ability to summarize 140 peer-reviewed abstracts from 14 journals. Physicians rated the quality, accuracy, and bias of the ChatGPT summaries. We also compared human ratings of relevance to various areas of medicine to ChatGPT relevance ratings. RESULTS ChatGPT produced summaries that were 70% shorter (mean abstract length of 2,438 characters decreased to 739 characters). Summaries were nevertheless rated as high quality (median score 90, interquartile range [IQR] 87.0-92.5; scale 0-100), high accuracy (median 92.5, IQR 89.0-95.0), and low bias (median 0, IQR 0-7.5). Serious inaccuracies and hallucinations were uncommon. Classification of the relevance of entire journals to various fields of medicine closely mirrored physician classifications (nonlinear standard error of the regression [SER] 8.6 on a scale of 0-100). However, relevance classification for individual articles was much more modest (SER 22.3). CONCLUSIONS Summaries generated by ChatGPT were 70% shorter than mean abstract length and were characterized by high quality, high accuracy, and low bias. Conversely, ChatGPT had modest ability to classify the relevance of articles to medical specialties. We suggest that ChatGPT can help family physicians accelerate review of the scientific literature and have developed software (pyJournalWatch) to support this application. Life-critical medical decisions should remain based on full, critical, and thoughtful evaluation of the full text of research articles in context with clinical guidelines. Abstract

The combined fit of the measured energy spectrum and shower maximum depth distributions of ultra-high-energy cosmic rays is known to constrain the parameters of astrophysical models with homogeneous source distributions. Studies of the distribution of the cosmic-ray arrival directions show a better agreement with models in which a fraction of the flux is non-isotropic and associated with the nearby radio galaxy Centaurus A or with catalogs such as that of starburst galaxies. Here, we present a novel combination of both analyses by a simultaneous fit of arrival directions, energy spectrum, and composition data measured at the Pierre Auger Observatory. We find that a model containing a flux contribution from the starburst galaxy catalog of around 20% at 40 EeV with a magnetic field blurring of around $20^\circ$ for a rigidity of 10 EV provides a fair simultaneous description of all three observables. The starburst galaxy model is favored with a significance of $4.5\sigma$ (considering experimental systematic effects) compared to a reference model with only homogeneously distributed background sources. By investigating a scenario with Centaurus A as a single source in combination with the homogeneous background, we confirm that this region of the sky provides the dominant contribution to the observed anisotropy signal. Models containing a catalog of jetted active galactic nuclei whose flux scales with the $\gamma$-ray emission are, however, disfavored as they cannot adequately describe the measured arrival directions.

Abstract

In this work, the concept of optical identification (OI) based on physical unclonable functions is introduced for the first time, to our knowledge, in optical communication systems and networks. The OI assigns an optical fingerprint and the corresponding digital representation to each sub-system of the network and estimates its reliability in different measures. We highlight the large potential applications of OI as a physical layer approach for security, identification, authentication, and monitoring purposes. To identify most of the sub-systems of a network, we propose to use the Rayleigh backscattering pattern, which is an optical physical unclonable function and allows to achieve OI with a simple procedure and without additional devices.
