Abstract

Objective: This multicentric prospective phase II study aimed to investigate the feasibility, safety, and efficacy of percutaneous-assisted laparoscopic hysterectomy in terms of perioperative outcomes, feasibility, VAS score, and cosmetic outcomes. Study Design: Between May 2015 and October 2017, 382 patients were considered eligible for minimally invasive percutaneous-assisted laparoscopic hysterectomy using PercuvanceTM, Percutaneous Surgical System - PSS, TELEFLEX ltd. Among them, 80 patients (20.9 %) met the inclusion criteria and were enrolled in the study. The coordinator center was the Division of Gynecologic Oncology, Fondazione Policlinico Universitario A. Gemelli IRCCS, Rome, Italy. Enrolled patients underwent total percutaneous-assisted laparoscopic hysterectomy ± bilateral salpingo-oophorectomy ± nodal assessment. Results: The median age was 52 (range, 32–80) years, and the median body mass index was 25 kg/m2 (range, 16–37). Thirty-five patients (43.8 %) had previous surgery. A median operative time of 82.5 (range, 40–190) minutes and a median estimated blood loss of 50 (50–500) mL were registered. We observed 1 (1.3 %) intraoperative complication. The median time to discharge was 1 (range, 1–5) day. Patients were extremely satisfied with the scar evaluation and postoperative pain control. Five (6.25 %) complications were recorded within 30 days after surgery. Conclusion: Percutaneous-assisted technique for extrafascial hysterectomy achieved excellent results in terms of feasibility, safety, and efficacy, even in complex cases and advanced surgical procedures. Therefore, the technique appears to balance the limitations and advantages of minimal surgical invasiveness and standard approach efficacy.

Abstract

Objective: Overactive bladder (OAB) symptoms are frequently associated with pelvic organ prolapse (POP) and both postoperative improvement and de novo onset of OAB symptoms have been described. The aim of the study is to identify risk factors for persistent, de novo and overall postoperative OAB after POP repair. Study design: This was a retrospective study including patients who underwent primary POP surgery. Medical interview, urogenital examination and urodynamics were performed preoperatively; patients were examined one and six months after surgery and then yearly. Results: 518 patients were included. 36.1% of women preoperatively complained of OAB symptoms while detrusor overactivity was found in 20.5%. The rate of persistent and de novo OAB after surgery were respectively 14.1% and 13.5%. Multivariate analysis found age, BMI, preoperative OAB, sling placement and postoperative SUI as independent risk factors for overall OAB after surgery. Moreover, preoperative OAB and postoperative constipations were associated with OAB persistence after surgery. Finally, age, sling placement, postoperative SUI and voiding symptoms were independently associated with de novo OAB. Conclusion: Preoperative OAB symptoms are associated with OAB persistence after POP surgery, while age and sling placement correlate with de novo OAB. Abstract

Introduction: Early toxic death (≤60 days of diagnosis) in elderly multiple myeloma (MM) patients is attributable to active disease, age and co-morbidities. Rate of early toxic deaths is 10% with conventional chemotherapy mainly due to infection and renal failure. Novel agents have improved MM outcome at the expense of newer toxicity. Methods: We analyzed 1146 individual patient data to assess toxic deaths during induction treatment with first-generation novel agents thalidomide, lenalidomide, bortezomib. Results: During first-line therapy, 119/1146 patients (10%) died for any cause, and 47/1146 (4%) due to toxicity, including 12/1146 (1%) early deaths. The 24-month cumulative incidence was 4.1% without any difference between bortezomib (18/503 patients, 4%) and lenalidomide (29/643patients, 5%; p = 0.31). Toxic deaths occurred in 34/1039 (3%) patients <80 years and 13/107 (12%) patients ≥80 years. Causes were cardiac events (28%), infections (26%) and vascular complications (15%). In a multivariate analysis, older age and unfavorable ISS stage increased the risk of death. Conclusion: First-generation novel agents significantly reduced toxic deaths compared to conventional chemotherapy. One third of deaths during first-line therapy were due to cumulative drug-related toxicities, thus supportive approaches and prevention strategies should be optimized. The higher mortality rate for toxicity in octogenarians confirms the need for a careful frailty assessment.

Abstract

We analyzed the main modalities and clinical outcomes of the early discharge outpatient model in autologous stem cell transplantation (EDOM-ASCT) for multiple myeloma in Italy. EDOM-ASCT was employed in 382 patients, for a total of 522 procedures, between 1998 and 2012. Our study showed high homogeneity among centers in terms of inclusion criteria, supportive care, and in hospital readmission criteria. Overall, readmissions during the aplastic phase occurred in 98 of 522 transplantations (18.8%). The major extrahematological complication was neutropenic fever in 161 cases (30.8%), which required readmission in 76 cases. The incidence of severe World Health Organization grade 3 to 4 mucositis was 9.6%. By univariate analysis, fever, mucositis, altered renal function at diagnosis, second transplantation, and transplantation performed late in the course of the disease were significantly correlated with readmission, whereas fever, mucositis, altered renal function, and timing of transplantation remained the only independent predictors by multivariate analysis. Overall, transplantation-related mortality was 1.0%. No center effect was observed in this study (P = .36). The safety and low rate of readmission of the EDOM-ASCT in myeloma trial suggest that this strategy could be extended to other transplantation centers if a stringent patient selection and appropriate management are applied. Abstract

In a randomized, phase 3 study, superior complete/near-complete response (CR/nCR) rates and extended progression-free survival were demonstrated with bortezomib-thalidomide-dexamethasone (VTD) versus thalidomide-dexamethasone (TD) as induction therapy before, and consolidation after, double autologous stem cell transplantation for newly diagnosed myeloma patients (intention-to-treat analysis; VTD, n = 236; TD, n = 238). This per-protocol analysis (VTD, n = 160; TD, n = 161) specifically assessed the efficacy and safety of consolidation with VTD or TD. Before starting consolidation, CR/nCR rates were not significantly different in the VTD (63.1%) and TD arms (54.7%). After consolidation, CR (60.6% vs 46.6%) and CR/nCR (73.1% vs 60.9%) rates were significantly higher for VTD-treated versus TD-treated patients. VTD consolidation significantly increased CR and CR/nCR rates, but TD did not (McNemar test). With a median follow-up of 30.4 months from start of consolidation, 3-year progression-free survival was significantly longer for the VTD group (60% vs 48% for TD). Grade 2 or 3 peripheral neuropathy (8.1% vs 2.4%) was more frequent with VTD (grade 3, 0.6%) versus TD consolidation. The superior efficacy of VTD versus TD as induction was retained despite readministration as consolidation therapy after double autologous transplantation. VTD consolidation therapy significantly contributed to improved clinical outcomes observed for patients randomly assigned to the VTD arm of the study. Abstract

There is an increasing concern over the cyber security of Critical Infrastructures (CI) due to the increasing ability of cyber attackers to cause even catastrophic failures. It is mainly due to the pervasiveness of ICT (Information and Communication Technologies) and to the consequent de isolation of SCADA (Supervision, Control and Data Acquisition) system, which represents the nervous system of most CIs. Cyber attacks could block the connection between SCADA Control Centre and its remote devices or insert fake commands/ measurements in the equipment communications. With reference to an actual case study, constituted by a SCADA system controlling a portion of a medium voltage power grid and a corporate network, we discuss how cyber threats, vulnerabilities and attacks might degrade the functionalities of SCADA and corporate network, which, in turn, might lead to outages of the electrical grid. We represent SCADA and corporate network under malware propagation, Denial of Service and Man In The Middle attacks and predict their consequent performance degradation. Particularly, we use NetLogo to identify possible malware propagation in relation to SCADA & corporate security policies adopted from the utility and NS2 simulator to compute the consequences of the attacks on SCADA and in turn on power grid. Abstract

Context: Subtle hypercortisolism is associated with an increased risk of vertebral fracture (VFx). Objective: The objective was to determine the best parameters of cortisol secretion for detecting the VFx risk in patients with adrenal incidentalomas (AI). Design: This was a retrospective (cross-sectional arm) and prospective (longitudinal arm) design. In the cross-sectional arm, we assessed the accuracy of the cortisol secretion indexes in identifying the patients with VFx (prevalent VFx). In the longitudinal arm, we tested the cortisol secretion parameters, which were able to identify the prevalent VFx, for the prediction of the occurrence of a new VFx (incident VFx) in AI patients followed-up for at least 2 years. Setting: Four referral Italian endocrinology units participated in this study. Patients: A total of 444 and 126 AI patients without symptoms of hypercortisolism enrolled in the cross-sectional arm and longitudinal arm, respectively. Main Outcome Measures: Serum cortisol after a 1-mg dexamethasone suppression test (1 mg DST), urinary free cortisol, ACTH, bone mineral density at lumbar spine and femoral neck (by dual-energy X-ray absorptiometry), and the VFx presence (by X-ray). Results: The cortisol levels after 1 mg DST that were greater than 2.0-g/dl (55 nmol/liter) were the best criteria for detecting patients with both prevalent (73.6% sensitivity, 70.5% specificity) and incident VFx (80% sensitivity, 68.8% specificity) and were associated with a 10-fold increased risk of a new VFx (odds ratio,10.27; 95% confidence interval, 3.39-31.12; P <.0001), regardless of age, gender, bone mineral density at lumbar spine, and prevalent VFx. Conclusions: In AI patients without symptoms of overt hypercortisolism, the cortisol levels after 1 mg DST greater than 2.0-g/dl (55 nmol/liter) represent the best criterion for detecting prevalent and incident VFx.

Abstract

This paper presents 3 years of GIS-based monitoring of western corn rootworm (WCR – Diabrotica virgifera virgifera LeConte), a pest which invaded Northern Italy in 1998. The Veneto Region established a regional monitoring network to detect this important quarantine pest and to employ an eradication programme. Data were collected by means of sex pheromone PAL traps and analysed by Indicator Kriging, a geostatistics tool that determines the probability of data values in a given area being greater than a defined threshold value. Geostatistical analysis proved to be effective in mapping the spread of WCR. The temporal sequence of the probability maps was useful in interpreting the expansion of the insect. The detailed description of the pattern of WCR presence in 2006–2008 proves the temporary and local efficacy of the eradication programme carried out up to the 2006 season in Venezia province. An interpretation of the spatial pattern of WCR between the 2006 and 2008 seasons suggests that the pest colonized the eradication area coming from other invasion fronts. The large-scale pattern of WCR dispersion can be accurately described by the spatial approach, thus optimizing the monitoring and subsequent control of this important insect pest of Northern Italy. Other data analyses, based on stochastic interpolations and a demographical approach are in progress. An interesting perspective would be to build predictive simulation models based on climatic and agronomic data taking into account the spatial representation of WCR patterns.

Abstract

Water plays an important role in the native conformation and activity of myrosinase. At the rhizosphere level, water exists in three phases namely, that associated with mucigel, free water in soil solution, and water adsorbed on soil particles. The activity of myrosinase was investigated as a function of soil water status by adding the enzyme to soil either in water solution form or immobilized in the hydrogel of one of the most important root mucigel polysaccharides. At high soil moisture, water and hydrogel-delivered myrosinase activities were comparable whereas in water stressed soil, the activity of myrosinase immobilized on hydrogel was twice as high as that allowed by water delivery. The mucigel-related increase in myrosinase activity could be ascribed to the polarization of water molecules within the hydrogel system, which prevents them from being adsorbed on soil particles and hence available to preserve the enzyme active conformation.
