Abstract

Abstract Background Endoscopic submucosal dissection (ESD) is an advanced technique of therapeutic endoscopy alternative to endoscopic mucosal resection (EMR) for superficial gastrointestinal neoplasms >2 cm. ESD allows for the direct dissection of the submucosa and large lesions can be resected en bloc. ESD is not limited by resection size, increases histologically complete resection rates and may reduce the local recurrence. Nevertheless, the technique is time-consuming, technically demanding and associated with a high complication rate. To reduce the risk of complications, different devices and technical advances have been proposed with conflicting results and, still, ESD en bloc resections of huge lesions are associated with increased complications. Case Presentation We successfully used a combined ESD/EMR technique for huge rectal laterally spreading tumors (LSTs). ESD was used for circumferential resection of 2/3 of the lesion followed by piecemeal resection (2-3 pieces) of the central part of the tumour. In all three patients we obtained the complete dissection of the polyp and the complete histological evaluation in absence of complications and recurrence at 6 months' follow up. Conclusions In the treatment of rectal LSTs, the combined treatment - ESD/EMR resection may be considered a suitable therapeutic option, indicated in selected cases as an alternative to surgery, in which the two techniques are neither reliable nor safe separately. However, to confirm our results, larger trials with longer follow up are required together with improvement of the technique and of the technical devices.

Geriatric patients require a large number of medicines, and in most cases, these products are administered as solid oral solid dosage forms, as they are by far the most common formulations on the market. However, this population tends to suffer difficulties with swallowing. Caregivers in hospital geriatric units routinely compound in solid oral dosage forms for dysphagic patients by crushing the tablets or opening the capsules to facilitate administration. The manipulation of a tablet or a capsule, if not clearly indicated in the product labeling, is an off-label use of the medicine, and must be supported by documented scientific evidence and requires the patient’s informed consent. Compounding of marketed products has been recognized as being responsible for an increased number of adverse events and medical errors. Since extemporaneous compounding is the rule and not the exception in geriatrics departments, the seriousness and scope of issues caused by this daily practice are probably underestimated. In this article, the potential problems associated with the manipulation of authorized solid oral dosage forms are discussed. 80. Cyber-Physical Energy Systems Approach for Engineering Power System State Estimation in Smart Grids

Abstract

This investigation proposes a CPES architecture and model for engineering energy management application for smart grids. In particular, the investigation considers the implementation of the power systems state estimator (PSSE). The CPES architecture has three layers: physical, monitoring and applications. The physical layer consists of the grid and the various components. Since, the grid is usually engineered with various devices from multiple vendors that have different protocols and standards; data aggregation becomes a problem. The second layer of the CPES architecture overcomes this problem by proposing a middleware that aggregates data from the physical layer. The topmost layer is the applications layer, where the energy management system applications are implemented. These applications require the model, topology and information from the grid. This requires combining the physical aspects of the grid with the cyber ones. This investigation uses the common information model to model the grid and information exchanges. Then the model is combined with measurement and optimization models of the application to realize the PSSE. The proposed approach is illustrated on a Norwegian distribution grid in Steinkjer. Our results show that the CPES approach provides an easier way to engineer future smart grid applications
