Background: Bacterial peritonitis (BP) in patients with gastrointestinal (GI) cancer has been poorly described, and its prevalence is unknown. Objectives: This study aimed to evaluate in patients with both GI cancer and ascites the prevalence of BP, associated features, mechanisms, prognosis, and the diagnostic performance of neutrophil count in ascites. Design: A retrospective, multicenter, observational study. Methods: All patients with GI cancer and ascites who underwent at least one paracentesis sample analyzed for bacteriology over a 1-year period were included. BP was defined by a positive ascites culture combined with clinical and/or biological signs compatible with infection. Secondary BP was defined as BP related to a direct intra-abdominal infectious source. Results: Five hundred fifty-seven ascites from 208 patients included were analyzed. Twenty-eight patients had at least one episode of BP and the annual prevalence rate of BP was 14%. Among the 28 patients with BP, 19 (65%) patients had proven secondary BP and 17 (59%) patients had multi-microbial BP, mainly due to Enterobacterales . A neutrophil count greater than 110/mm 3 in ascites had negative and positive predictive values of 96% and 39%, respectively, for the diagnosis of BP. The median survival of patients with BP was 10 days (interquartile range 6–40) after the diagnosis. Conclusion: BP is not rare in patients with GI cancer and is associated with a poor short-term prognosis. When a patient with GI cancer is diagnosed with BP, a secondary cause should be sought. Further studies are needed to better define the best management of these patients.

Abstract This study investigates the possibility of Atlantic Meridional Overturning Circulation (AMOC) noise-induced tipping solely driven by internal climate variability without applying external forcing that alter the radiative forcing or the North Atlantic freshwater budget. We address this hypothesis by applying a rare event algorithm to ensemble simulations of present-day climate with an intermediate complexity climate model. The algorithm successfully identifies trajectories leading to abrupt AMOC slowdowns, which are unprecedented in a 2000-year control run. Part of these AMOC weakened states lead to collapsed state without evidence of AMOC recovery on multi-centennial time scales. The temperature and Northern Hemisphere jet stream responses to these internally-induced AMOC slowdowns show strong similarities with those found in externally forced AMOC slowdowns in state-of-the-art climate models. The AMOC slowdown seems to be initially driven by Ekman transport due to westerly wind stress anomalies in the North Atlantic and subsequently sustained by a complete collapse of the oceanic convection in the Labrador Sea. These results demonstrate that transitions to a collapsed AMOC state purely due to internal variability in a model simulation of present-day climate are rare but theoretically possible. Additionally, these results show that rare event algorithms are a tool of valuable and general interest to study tipping points since they introduce the possibility of collecting a large number of tipping events that cannot be sampled using traditional approaches. This opens the possibility of identifying the mechanisms driving tipping events in complex systems in which little a-priori knowledge is available.

Abstract High Salinity Shelf Water (HSSW) formed in the Ross Sea of Antarctica is a precursor to Antarctic Bottom Water (AABW), a water mass that constitutes the bottom limb of the global overturning circulation. HSSW production rates are poorly constrained, as in-situ observations are scarce. Here, we present high-vertical-and-temporal-resolution salinity time series collected in austral winter 2017 from a mooring in Terra Nova Bay (TNB), one of two major sites of HSSW production in the Ross Sea. We calculate an annual-average HSSW production rate of ~0.4 Sv (106 m 3 s −1), which we use to ground truth additional estimates across 2012–2021 made from parametrized net surface heat fluxes. We find sub-seasonal and interannual variability on the order of $$0.1$$ 0.1 $${Sv}$$ S v , with a strong dependence on variability in open-water area that suggests a sensitivity of TNB HSSW production rates to changes in the local wind regime and offshore sea ice pack.

Abstract Background For at least a decade, concerns have been raised about the physical durability of insecticide-treated nets (ITNs) and their ability to remain in good condition for at least three years. To discover if the resistance to damage (RD) of ITNs has improved or not, the RD scores of ITNs sampled in 2013 and 2020 were compared. Methods The RD scores and disaggregated textile performance data for nine ITNs recommended by the WHO pesticide evaluation scheme (WHOPES) measured in 2013 were compared with WHO-prequalified ITNs sampled in 2020. This included assessment of newer ITNs not available in 2013, to determine the extent to which product development has led to performance improvements across all available ITNs in the intervening years. Results The resistance to damage of ITNs has not generally improved from 2013 to 2020, and in some cases performance is worse. The average RD score of comparable ITNs brands decreased from 40 in 2013 to 36 in 2020. We present CAMELS-CH (Catchment Attributes and MEteorology for Large-sample Studies – Switzerland), a large-sample hydro-meteorological data set for hydrologic Switzerland in central Europe. This domain covers 331 basins within Switzerland and neighboring countries. About one-third of the catchments are located in Austria, France, Germany and Italy. As an Alpine country, Switzerland covers a vast diversity of landscapes, including mountainous environments, karstic regions, and several strongly cultivated regions, along with a wide range of hydrological regimes, i.e., catchments that are glacier-, snow- or rain dominated. Similar to existing data sets, CAMELS-CH comprises dynamic hydro-meteorological variables and static catchment attributes. CAMELS-CH (Höge et al., 2023; available at https://doi.org/10.5281/zenodo.7784632) encompasses 40 years of data between 1 January 1981 and 31 December 2020, including daily time series of stream flow and water levels, and of meteorological data such as precipitation and air temperature. It also includes daily snow water equivalent data for each catchment starting from 2 September 1998. Additionally, we provide annual time series of land cover change and glacier evolution per catchment. The static catchment attributes cover location and topography, climate, hydrology, soil, hydrogeology, geology, land use, human impact and glaciers. This Swiss data set complements comparable publicly accessible data sets, providing data from the “water tower of Europe”.

The double-stranded RNA sensor kinase PKR is one of four integrated stress response (ISR) sensor kinases that phosphorylate the α subunit of eukaryotic initiation factor 2 (eIF2α) in response to stress. The current model of PKR activation considers the formation of back-to-back PKR dimers as a prerequisite for signal propagation. Here we show that PKR signaling involves the assembly of dynamic PKR clusters. PKR clustering is driven by ligand binding to PKR's sensor domain and by front-to-front interfaces between PKR's kinase domains. PKR clusters are discrete, heterogeneous, autonomous coalescences that share some protein components with processing bodies. Strikingly, eIF2α is not recruited to PKR clusters, and PKR cluster disruption enhances eIF2α phosphorylation. Together, these results support a model in which PKR clustering may limit encounters between PKR and eIF2α to buffer downstream signaling and prevent the ISR from misfiring.

Ocean surface radiation measurement best practices have been developed as a first step to support the interoperability of radiation measurements across multiple ocean platforms and between land and ocean networks. This document describes the consensus by a working group of radiation measurement experts from land, ocean, and aircraft communities. The scope was limited to broadband shortwave (solar) and longwave (terrestrial infrared) surface irradiance measurements for quantification of the surface radiation budget. Best practices for spectral measurements for biological purposes like photosynthetically active radiation and ocean color are only mentioned briefly to motivate future interactions between the physical surface flux and biological radiation measurement communities. Topics discussed in these best practices include instrument selection, handling of sensors and installation, data quality monitoring, data processing, and calibration. It is recognized that platform and resource limitations may prohibit incorporating all best practices into all measurements and that spatial coverage is also an important motivator for expanding current networks. Thus, one of the key recommendations is to perform interoperability experiments that can help quantify the uncertainty of different practices and lay the groundwork for a multi-tiered global network with a mix of high-accuracy reference stations and lower-cost platforms and practices that can fill in spatial gaps.

BackgroundIndividuals with asthma spend less time engaging in physical activity compared to the general population. Increasing physical activity has become a patient-centered goal for the treatment of treatable traits of individuals with asthma. There are data showing the possible effects of a pulmonary rehabilitation program on physical activity in obese individuals with asthma but not in normal-weight asthmatics. The objective of this feasibility study is to estimate the number of daily steps and time spent on activity in normal-weight individuals with asthma, measured before and after a pulmonary rehabilitation program.MethodsNormal-weight individuals with moderate to severe asthma were evaluated. The individuals measured their daily steps with an accelerometer for 5 days before and after a pulmonary rehabilitation program. The study was registered on ClinicalTrials.gov: NCT05486689.ResultsIn total, 17 participants were enrolled; one dropout and data on the time in activity of two individuals are missing due to a software error during the download. Data from 16 patients were analyzed. The median number of steps/day at baseline was 5,578 (25th, 75th percentiles = 4,874, 9,685) while the median activity time was 214 min (25th, 75th percentiles = 165, 239). After the rehabilitation program, the number of daily steps increased by a median value of 472 (p-value = 0.561) and the time in activity reduced by 17 min (p-value = 0.357). We also found a significant difference in quality of life, muscle strength, and exercise capacity.ConclusionsThe results of this study make it possible to calculate the sample size of future studies whose main outcome is daily steps in normal-weight individuals with asthma. The difficulties encountered in downloading time in activity data do not allow the same for this outcome.Clinical Trial RegistrationClinicalTrials.gov, identifier NCT05486689.

Despite the key role of irrigation in the Earth system, we lack fundamental information regarding the distribution of irrigated fields, irrigation timing and the amount of water utilized. In the past years, the SM_Delta and SM_Inversion approaches have been independently developed to provide estimates of irrigation timing and water amounts based on satellite soil moisture data. The SM_Delta approach retrieves irrigation from variations in soil moisture between an individual pixel and the surrounding rainfed area, while the SM_Inversion approach estimates the total amount of water entering the soil, then irrigation is derived by subtracting precipitation. In this study, we perform a comprehensive assessment of irrigation estimates from the SM_Delta and SM_Inversion algorithms based on Sentinel-1 surface soil moisture retrievals at 1 km resolution. Our analysis focuses on the Ebro basin, an irrigated region in Spain covering 83000 km2, during the period 2017–2019. We assess the ability of the two methods to discriminate irrigated and rainfed pixels, then we quantify the agreement of irrigation timing and water volumes with reference irrigation data. An inter-comparison between estimates from the SM_Delta and SM_Inversion methods is carried out considering both temporal and spatial features, i.e., monthly irrigation peaks and spatial irrigation patterns. Finally, we explore two potential applications of satellite-derived irrigation estimates: attributing irrigation water volumes to specific irrigation systems and to individual crops. We observe that both methods erroneously retrieve irrigation over rainfed pixels, and are therefore not suitable to map irrigated and rainfed fields. However, when auxiliary information on irrigated fields is available, we find a satisfactory agreement between district-scale reference data and satellite-retrieved irrigation, using both the SM_Delta and SM_Inversion approaches (Pearson R equal to 0.67 and 0.71, bias equal to −4.99 and −4.75 mm/15 days, respectively). When aggregated in space or time, the irrigation estimates exhibit coherent temporal dynamics and spatial patterns. For instance, estimates from both SM_Delta and SM_Inversion capture the delayed irrigation that occurred in 2018 due to wetter than usual conditions in spring. However, at the pixel-scale, limited consistency exists between irrigation estimates from the two methods due to different assumptions and parameterizations, e.g., use of constant vs pixel-specific soil water capacity (in the SM_Delta and SM_Inversion, respectively). Overall, the study demonstrates the reliability of irrigation estimates derived from the SM_Delta and SM_Inversion approaches, especially when shifting from small spatial and short temporal scales (pixel level, sub-weekly) to larger and longer scales (district level, seasonal). Hence, satellite-based irrigation estimates could inform water resources managers and basin authorities, as well as serve the modelling community by providing reliable information on the timing and the amounts of water employed at the basin level.

BackgroundTo date, studies investigating the association between pre-biologic biomarker levels and post-biologic outcomes have been limited to single biomarkers and assessment of biologic efficacy from structured clinical trials.AimTo elucidate the associations of pre-biologic individual biomarker levels or their combinations with pre-to-post biologic changes in asthma outcomes in real-life.MethodsThis was a registry-based, cohort study using data from 23 countries, which shared data with the International Severe Asthma Registry (May 2017-February 2023). The investigated biomarkers (highest pre-biologic levels) were immunoglobulin E (IgE), blood eosinophil count (BEC) and fractional exhaled nitric oxide (FeNO). Pre- to approximately 12-month post-biologic change for each of three asthma outcome domains (i.e. exacerbation rate, symptom control and lung function), and the association of this change with pre-biologic biomarkers was investigated for individual and combined biomarkers.ResultsOverall, 3751 patients initiated biologics and were included in the analysis. No association was found between pre-biologic BEC and pre-to-post biologic change in exacerbation rate for any biologic class. However, higher pre-biologic BEC and FeNO were both associated with greater post-biologic improvement in FEV1 for both anti-IgE and anti-IL5/5R, with a trend for anti-IL4Rα. Mean FEV1 improved by 27-178 mL post-anti-IgE as pre-biologic BEC increased (250 to 1000 cells/µL), and by 43-216 mL and 129-250 mL post-anti-IL5/5R and -anti-IL4Rα, respectively along the same BEC gradient. Corresponding improvements along a FeNO gradient (25-100 ppb) were 41-274 mL, 69-207 mL and 148-224 mL for anti-IgE, anti-IL5/5R, and anti-IL4Rα, respectively. Higher baseline BEC was also associated with lower probability of uncontrolled asthma (OR 0.392; p=0.001) post-biologic for anti-IL5/5R. Pre-biologic IgE was a poor predictor of subsequent pre-to-post-biologic change for all outcomes assessed for all biologics. We present measurements of the electronic absorption spectrum of He-tagged buckminsterfullerene anions, C_{60}^{−}. Using a technique that allows for the efficient tagging of complex molecular anions with He, we achieve atomically resolved action spectra that provide an accurate determination of the gas-phase absorption spectrum in the near-infrared regime. In total we identify 27 absorption bands between 895 and 1057 nm.

ObjectivesApproximately one-third of bariatric surgery patients experience weight regain or suboptimal weight loss within five years post-surgery. Pathological eating styles and psychopathological traits (e.g., emotion dysregulation) are recognized as potential hindrances to sustain weight loss efforts and are implicated in obesity development. A comprehensive understanding of these variables and their interplays is still lacking, despite their potential significance in developing more effective clinical interventions for bariatric patients. We investigate the prevalence of and interactions between pathological eating styles and psychopathological traits in this population.Materials and methods110 bariatric surgery candidates were characterized using the Binge Eating Scale (BES), Hamilton Depression/Anxiety Scales (HAM-D/A), Barratt Impulsiveness Scale (BIS-11), Experiences in Close Relationships (ECR), Difficulties in Emotion Regulation Scale (DERS). We analyzed these variables with multiple logistic regression analyses and network analysis.ResultsPatients with pathological eating styles showed more pronounced anxiety/depressive symptoms and emotion dysregulation. Network analysis revealed strong connections between BES and DERS, with DERS also displaying robust connections with HAM-A/D and ECR scales. DERS and attention impulsivity (BIS-11-A) emerged as the strongest nodes in the network.DiscussionOur findings demonstrate the mediating role of emotion dysregulation between pathological eating styles and psychopathological traits, supporting existing literature on the association between psychopathological traits, insecure attachment styles, and pathological eating behaviors. This research emphasizes the significance of emotion regulation in the complex network of variables contributing to obesity, and its potential impact on bariatric surgery outcomes. Interventions focusing on emotion regulation may thus lead to improved clinical outcomes for bariatric patients.

Time series of the natural isotopic composition (2H, 18O) of precipitation and streamwater can provide important insights into ecohydrological phenomena at the catchment scale. However, multi-year, high-frequency isotope datasets are generally scarce, limiting our ability to study highly dynamic short-term ecohydrological processes. Here we present four years of daily isotope measurements in streamwater and precipitation at the Alp catchment (area 47 km2) in Central Switzerland and two of its tributaries (0.7 km2 and 1.6 km2). This data set reveals short-term responses of streamflow isotopes to precipitation events, which otherwise remain obscured when isotopes are sampled weekly or monthly. The observations span the period June 2015 through May 2019, during which several hydrometeorologic extreme events occurred, including a very dry summer in 2018 and below-average snow accumulation in winter 2016/2017. In addition, we provide daily time series of key hydrometeorological variables that, in combination with the isotope data, can be useful for assessing the robustness of ecohydrological models.

An automated device is designed to measure the density of a liquid material using hydrostatic pressure method. A low cost pressure sensor is calibrated and used to get highly accurate readings. The calibration is done by measuring the pressure values vs. the generated voltage signal. The calibration has been challenging due to the low accuracy of the sensor but proved to be highly effective in applications. The interface is developed using a microcontroller, motor drives, analog to digital converters and sensors. The device is designed to get several readings automatically by changing the positions of the device/liquid column heights to increase the accuracy. Also the device can be programmed to measure the real time density of a liquid continuously. The readings were analyzed and averaged by a software developed in python language. The instruments accuracy was tested against 3 liquid types, water, coconut oil, kerosene oil, and showed a low error (0.007%, 0.001%, and 0.002% respectively) compared to the readings of a standard Pycnometer. The low error percentages confirm the accuracy of the device and the effectiveness of the sensor calibrations.

Men who have sex with men (MSM) are disproportionately impacted by sexually transmitted infections (STIs), including HIV and those preventable through vaccination such as mpox, HPV, HAV, and HBV. A retrospective cohort study was conducted to evaluate the effectiveness of counseling provided during mpox vaccination on the uptake of other recommended vaccines (HPV, HAV, and HBV) and to identify associated factors. Relevant covariates such as nationality, age, HIV status, and use of PrEP were retrieved from electronic medical records. Vaccination status data were retrieved from the regional vaccination registry. Of the 330 participants, 98.8% were males and the mean age was 40.6 years (SD: 11.2). Following consultation, a statistically significant increase for both HPV (from 25.8% to 39.1%) and HAV (from 26.7% to 36.1%) was observed (p < 0.001). The multivariate analysis showed a significant negative association between the uptake of HPV and HBV vaccines and foreign nationality (aOR 0.25 (95%CI 0.08–0.69), p = 0.012; and aOR 0.31 (95%CI 0.11–0.81), p = 0.021). The HBV vaccine uptake was negatively associated with increasing age. Our results suggest that tailored counseling can effectively bridge the gap in vaccine acceptance among vulnerable populations, thereby improving overall public health outcomes.

Since two major earthquakes that hit Central Italy and northeastern Japan in 2009 and 2011 respectively, revisionist plans to make both countries urbanization models and power production and distribution systems more sustainable and resilient have emerged. The governments of both Italy (with the support of the European Union) and Japan have invested considerable resources in establishing model smart communities in disaster-hit areas in LâAquila and Aizuwakamatsu. How has the smart city idea (or ideal) shaped local policies for reconstruction and recovery in disaster-affected areas? Secondly, how have they contributed to informing cooperation at the international level? These questions are relevant in the light of Japan and the European Unionâs pledges to strengthen their bilateral cooperation in smart cities and communities development in the context of the Covid-19 pandemic and war in Ukraine. With the launch of subsequent overarching strategies both the EU and Japan have shown their resolve to promote structural reforms through digitalization and cutting-hedge technology, in the attempt to foster economic recovery while promoting Ê»sustainable economic growthâ. However, such narrative, common to many advanced capitalist societies, appears instrumental to concealing plans to restructure environments and social arrangements while enhancing for-profit capital restructuring and better surveillance.

Abstract Introduction Vaccination strongly reduces the risk of hospitalization and death due to coronavirus disease 2019 (COVID-19). However, the severity of the acute respiratory syndrome coronavirus-2 (SARS-CoV-2) infection and the degree of protection exerted over time by vaccination remains to be fully elucidated among hospitalized comorbid and vulnerable patients with SARS-CoV-2 infection. Methods We report a case series of nine hospitalized vulnerable patients who developed a SARS-CoV-2 infection during a cardiac rehabilitation inpatient program. Results Age ranged from 50 to 81 years. All but one patient had received at least three doses of anti-COVID-19 vaccine more than 4 months before the cardiac event. Indications for cardiac rehabilitation included acute coronary syndromes, congestive heart failure, heart valve surgery, and coronary artery bypass graft. After the confirmed diagnosis of SARS-CoV-2 infection, all patients developed symptoms. Eight patients developed at least one SARS-CoV-2-related complication, including a significant increase in high-sensitivity troponin I levels, new-onset hypoxemia, persistent atrial fibrillation, non-sustained ventricular tachycardia and recurrent sinus arrest, pericardial effusion, and a persistent increase in blood pressure. Conclusion Almost all patients developed complications which, however, did not evolve towards more severe expressions of the disease. These data suggest that even in this new phase of the pandemic, vaccination may exert a potential role to reduce the risk of progression towards more severe disease of SARS-CoV-2 infection in vulnerable patients with cardiovascular comorbidities.

Abstract The ability to form reconstructions beyond line-of-sight view could be transformative in a variety of fields, including search and rescue, autonomous vehicle navigation, and reconnaissance. Most existing active non-line-of-sight (NLOS) imaging methods use data collection steps in which a pulsed laser is directed at several points on a relay surface, one at a time. The prevailing approaches include raster scanning of a rectangular grid on a vertical wall opposite the volume of interest to generate a collection of confocal measurements. These and a recent method that uses a horizontal relay surface are inherently limited by the need for laser scanning. Methods that avoid laser scanning to operate in a snapshot mode are limited to treating the hidden scene of interest as one or two point targets. In this work, based on more complete optical response modeling yet still without multiple illumination positions, we demonstrate accurate reconstructions of foreground objects while also introducing the capability of mapping the stationary scenery behind moving objects. The ability to count, localize, and characterize the sizes of hidden objects, combined with mapping of the stationary hidden scene, could greatly improve indoor situational awareness in a variety of applications.

Introduction The empty pelvis syndrome is a significant source of morbidity following pelvic exenteration surgery. It remains poorly defined with research in this field being heterogeneous and of low quality. Furthermore, there has been minimal engagement with patient representatives following pelvic exenteration with respect to the empty pelvic syndrome. ‘PelvEx—Beating the empty pelvis syndrome’ aims to engage both patient representatives and healthcare professionals to achieve an international consensus on a core outcome set, pathophysiology and mitigation of the empty pelvis syndrome.Methods and analysis A modified-Delphi approach will be followed with a three-stage study design. First, statements will be longlisted using a recent systematic review, healthcare professional event, patient and public engagement, and Delphi piloting. Second, statements will be shortlisted using up to three rounds of online modified Delphi. Third, statements will be confirmed and instruments for measurable statements selected using a virtual patient-representative consensus meeting, and finally a face-to-face healthcare professional consensus meeting.Ethics and dissemination The University of Southampton Faculty of Medicine ethics committee has approved this protocol, which is registered as a study with the Core Outcome Measures in Effectiveness Trials Initiative. Publication of this study will increase the potential for comparative research to further understanding and prevent the empty pelvis syndrome.Trial registration number NCT05683795.

Objectives: Few and small studies previously examined chest CT-scan characteristics of Coxiella burnetii (Cb) community-acquired pneumonia (CAP). Larger studies are needed to guide physicians towards diagnosis of Q fever in case of pneumonia. Methods: We conducted a single-center retrospective observational study between 2013 and 2017. All patients with Cb or Streptococcus pneumoniae (Sp) CAP who had a chest CT-scan on admission at Cayenne Hospital (French Guiana) were included. Chest CT-scan were all analyzed by the same expert radiologist. Results: We included 75 patients with Cb CAP and 36 with Sp CAP. Fifty-nine percent of all patients were men (n = 66) and median age was 52 [IQR = 38–62]. Chest CT-scans of Cb CAP patients revealed 67 alveolar condensations (89 %), 52 ground-glass opacities (69 %), 30 cases of lymphadenopathy(ies) (40 %) and 25 pleural effusions (33 %). Parenchyma lesions caused by Cb were predominantly unilateral (67 %). We found high numbers of alveolar condensations in both Cb and Sp CAP (89 % and 75 %; respectively), but the presence of ground-glass opacities was significantly associated with Cb CAP (69 % versus 30 %; p

A pesar de esto, y dado la necesidad en la Argentina de poder seguir aprovechando la energía eólica en pequeña escala, pero con vinculación a la red eléctrica, y de las diferencias conceptuales con la solar PV, este trabajo, a través de lo hecho en el programa SWTOMP entre UTN FRN, INTI, CIEMAT, y otras instituciones, propone una alternativa de electrónica de potencia, del tipo chopper, (de código abierto o uso libre). Adecuando estos convertidores a la eólica de baja potencia (menor a algunos kW), y en principal para ello desarrollando un novedoso y simple control, que proteja, debido a las rápidas variaciones del recurso eólico, tanto al sistema mecánico por sobre velocidades de giro como al inversor de sobretensiones. Por otra parte, ubique al aerogenerador en un estado general cercano al óptimo según sus especificaciones. Por lo tanto, se muestra el desarrollo tecnológico y los auspiciosos primeros resultados, por medio de las medidas tomadas en el laboratorio de evaluación de aerogeneradores que posee el INTI en CutralCó-Neuquén, Patagonia Argentina, sobre un aerogenerador de 1kW de la firma EOLOCAL.

The Integrated Stress Response (ISR) is an essential homeostatic signaling network that controls the cell’s biosynthetic capacity. Four ISR sensor kinases detect multiple stressors and relay this information to downstream effectors by phosphorylating a common node: the alpha subunit of the eukaryotic initiation factor eIF2. As a result, general protein synthesis is repressed while select transcripts are preferentially translated, thus remodeling the proteome and transcriptome. Mounting evidence supports a view of the ISR as a dynamic signaling network with multiple modulators and feedback regulatory features that vary across cell and tissue types. Here, we discuss updated views on ISR sensor kinase mechanisms, how the subcellular localization of ISR components impacts signaling, and highlight ISR signaling differences across cells and tissues. Finally, we consider crosstalk between the ISR and other signaling pathways as a determinant of cell health.

Abstract Air‐sea interactions are critical to large-scale weather and climate predictions because of the ocean's ability to absorb excess atmospheric heat and carbon and regulate exchanges of momentum, water vapor, and other greenhouse gases. These exchanges are controlled by molecular, turbulent, and wave-driven processes in the atmospheric and oceanic boundary layers. Improved understanding and representation of these processes in models are key for increasing Earth system prediction skill, particularly for subseasonal to decadal time scales. Our understanding and ability to model these processes within this coupled system is presently inadequate due in large part to a lack of data: contemporaneous long-term observations from the top of the marine atmospheric boundary layer (MABL) to the base of the oceanic mixing layer.We propose the concept of “Super Sites” to provide multi-year suites of measurements at specific locations to simultaneously characterize physical and biogeochemical processes within the coupled boundary layers at high spatial and temporal resolution. Measurements will be made from floating platforms, buoys, towers, and autonomous vehicles, utilizing both in-situ and remote sensors. The engineering challenges and level of coordination, integration, and interoperability required to develop these coupled ocean‐atmosphere Super Sites place them in an “Ocean Shot” class.

Hybrid hydroclimatic forecasting systems employ data-driven (statistical or machine learning) methods to harness and integrate a broad variety of predictions from dynamical, physics-based models – such as numerical weather prediction, climate, land, hydrology, and Earth system models – into a final prediction product. They are recognized as a promising way of enhancing the prediction skill of meteorological and hydroclimatic variables and events, including rainfall, temperature, streamflow, floods, droughts, tropical cyclones, or atmospheric rivers. Hybrid forecasting methods are now receiving growing attention due to advances in weather and climate prediction systems at subseasonal to decadal scales, a better appreciation of the strengths of AI, and expanding access to computational resources and methods. Such systems are attractive because they may avoid the need to run a computationally expensive offline land model, can minimize the effect of biases that exist within dynamical outputs, benefit from the strengths of machine learning, and can learn from large datasets, while combining different sources of predictability with varying time horizons. Here we review recent developments in hybrid hydroclimatic forecasting and outline key challenges and opportunities for further research. These include obtaining physically explainable results, assimilating human influences from novel data sources, integrating new ensemble techniques to improve predictive skill, creating seamless prediction schemes that merge short to long lead times, incorporating initial land surface and ocean/ice conditions, acknowledging spatial variability in landscape and atmospheric forcing, and increasing the operational uptake of hybrid prediction schemes.

Almost a third of bariatric surgery patients present suboptimal weight loss or important weight regain in the first five postoperative years. While the reasons underlying this are not fully understood, it is known that pathological eating styles (such as emotional or binge eating) can thwart efforts to maintain weight loss. However, detailed characterization and understanding of these eating styles have yet to be achieved. In particular, research on gender differences in pathological eating styles and psychiatric symptoms before bariatric surgery is lacking. To characterize gender differences in eating styles and their association with clinical symptoms, we prospectively enrolled 110 bariatric surgery candidates, collecting eating styles and clinical scores. Women displayed a higher frequency of emotional eating as compared to men (x2 = 9.07, p = 0.003), while men showed a higher frequency of quantitative eating behavioral style (x2 = 4.58, p = 0.044). Binge eating style was associated with higher Difficulties in Emotion Regulation Scale (DERS), Hamilton Depression Scale (HAM-D), and Hamilton Anxiety Scale (HAM-A) scores (p < 0.05). Emotional eating style was associated with higher HAM-D and HAM-A scores (p < 0.05). The present findings highlight the importance of understanding the role of gender differences in emotion regulation processes involved in the development and maintenance of pathological eating styles in bariatric surgery candidates. This paves the way to gender- and symptoms-specific interventions on eating behaviors to improve surgery long-term outcomes.

Background: Research shows that the longer nurses care for terminally ill patients, the greater they experience moral distress. The same applies to nursing students. This study aims to analyze episodes of moral distress experienced by nursing students during end-of-life care of onco-hematologic patients in hospital settings. Methods: This study was conducted in the interpretative paradigm using a hermeneutic phenomenological approach and data were analyzed following the principles of the Interpretative Phenomenological Analysis. Results: Seventeen participants were included in the study. The research team identified eight themes: causes of moral distress; factors that worsen or influence the experience of moral distress; feelings and emotions in morally distressing events; morally distressing events and consultation; strategies to cope with moral distress; recovering from morally distressing events; end-of-life accompaniment; internship clinical training, and nursing curriculum. Conclusions: Moral distress is often related to poor communication or lack of communication between health care professionals and patients or relatives and to the inability to satisfy patients’ last needs and wants. Further studies are necessary to examine the quantitative dimension of moral distress in nursing students. Students frequently experience moral distress in the onco-hematological setting.

ImportanceThe reopening of colleges and universities in the US during the coronavirus disease 2019 (COVID-19) pandemic is a significant public health challenge. The development of accessible and practical approaches for severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) detection in the college population is paramount for deploying recurrent surveillance testing as an essential strategy for virus detection, containment, and mitigation.ObjectiveTo determine the prevalence of SARS-CoV-2 in asymptomatic participants in a university community by using CREST (Cas13-based, rugged, equitable, scalable testing), a CRISPR-based test developed for accessible and large-scale viral screening.Design, setting, and participantsFor this cohort study, a total of 1808 asymptomatic participants were screened for SARS-CoV-2 using a CRISPR-based assay and a point-of-reference reverse transcriptase-quantitative polymerase chain reaction (RT-qPCR) test. Viral prevalence in self-collected oropharyngeal swab samples collected from May 28 to June 11, 2020, and from June 23 to July 2, 2020, was evaluated.ExposuresTesting for SARS-CoV-2.Main outcomes and measuresSARS-CoV-2 status, viral load, and demographic information of the study participants were collected.ResultsAmong the 1808 participants (mean [SD] age, 27.3 [11.0] years; 955 [52.8%] female), 732 underwent testing from May to early June (mean [SD] age, 28.4 [11.7] years; 392 [53.6%] female). All test results in this cohort were negative. In contrast, 1076 participants underwent testing from late June to early July (mean [SD] age, 26.6 [10.5] years; 563 [52.3%] female), with 9 positive results by RT-qPCR. Eight of these positive samples were detected by the CRISPR-based assay and confirmed by Clinical Laboratory Improvement Amendments-certified diagnostic testing. The mean (SD) age of the positive cases was 21.7 (3.3) years; all 8 individuals self-identified as students. These metrics showed that a CRISPR-based assay was effective at capturing positive SARS-CoV-2 cases in this student population. Notably, the viral loads detected in these asymptomatic cases resemble those seen in clinical samples, highlighting the potential of covert viral transmission. The shift in viral prevalence coincided with the relaxation of stay-at-home measures.Conclusions and relevanceThese findings reveal a shift in SARS-CoV-2 prevalence in a young and asymptomatic population and uncover the leading edge of a local outbreak that coincided with rising case counts in the surrounding county and the state of California. The concordance between CRISPR-based and RT-qPCR testing suggests that CRISPR-based assays are reliable and offer alternative options for surveillance testing and detection of SARS-CoV-2 outbreaks, as is required to resume operations in higher-education institutions in the US and abroad.

An electronic nose, designed to replicate human olfaction, captures distinctive ‘fingerprint’ data from mixed gases or odors. Comprising a gas sensing system and an information processing unit, electronic noses have evolved significantly since their inception in the 1980s. They have transitioned from bulky, costly, and energy-intensive devices to today’s streamlined, economical models with minimal power requirements. This paper presents a comprehensive and systematic review of the electronic nose technology domain, with a special focus on advancements over the last five years. It highlights emerging applications, innovative methodologies, and potential future directions that have not been extensively covered in previous reviews. The review explores the application of electronic noses across diverse fields such as food analysis, environmental monitoring, and medical diagnostics, including new domains like veterinary pathology and pest detection. This work aims to underline the adaptability of electronic noses and contribute to their continued development and application in various industries, thereby addressing gaps in current literature and suggesting avenues for future research.

Arterial hypertension is the most frequent cardiovascular risk factor all over the world, and it is one of the leading drivers of the risk of cardiovascular events and death. It is a complex trait influenced by heritable and environmental factors. To date, the World Health Organization estimates that 1.28 billion adults aged 30–79 years worldwide have arterial hypertension (defined by European guidelines as office systolic blood pressure ≥ 140 mmHg or office diastolic blood pressure ≥ 90 mmHg), and 7.1 million die from this disease. The molecular genetic basis of primary arterial hypertension is the subject of intense research and has recently yielded remarkable progress. In this review, we will discuss the genetics of arterial hypertension. Recent studies have identified over 900 independent loci associated with blood pressure regulation across the genome. Comprehending these mechanisms not only could shed light on the pathogenesis of the disease but also hold the potential for assessing the risk of developing arterial hypertension in the future. In addition, these findings may pave the way for novel drug development and personalized therapeutic strategies.

Light and active mobility, as well as multimodal mobility, could significantly contribute to decarbonization. Air quality is a key parameter to monitor the environment in terms of health and leisure benefits. In a possible scenario, wearables and recharge stations could supply information about a distributed monitoring system of air quality. The availability of low-power, smart, low-cost, compact embedded systems, such as Arduino Nicla Sense ME, based on BME688 by Bosch, Reutlingen, Germany, and powered by suitable software tools, can provide the hardware to be easily integrated into wearables as well as in solar-powered EVSE (Electric Vehicle Supply Equipment) for scooters and e-bikes. In this way, each e-vehicle, bike, or EVSE can contribute to a distributed monitoring network providing real-time information about micro-climate and pollution. This work experimentally investigates the capability of the BME688 environmental sensor to provide useful and detailed information about air quality. Initial experimental results from measurements in non-controlled and controlled environments show that BME688 is suited to detect the human-perceived air quality. CO2 readout can also be significant for other gas (e.g., CO), while IAQ (Index for Air Quality, from 0 to 500) is heavily affected by relative humidity, and its significance below 250 is quite low for an outdoor uncontrolled environment.

Solid oxide fuel cells (SOFCs) have emerged as promising candidates for efficient and environmentally friendly energy conversion technologies. Their high energy conversion efficiency and fuel flexibility make them particularly attractive for various applications, ranging from stationary power generation to portable electronic devices. Recently, research has focused on utilizing nanostructured materials to enhance the performance of SOFCs. This comprehensive review summarizes the latest advancements in the design, fabrication, and characterization of nanostructured materials integrated in SOFC. The review begins by elucidating the fundamental principles underlying SOFC operation, emphasizing the critical role of electrode materials, electrolytes, and interfacial interactions in overall cell performance, and the importance of nanostructured materials in addressing key challenges. It provides an in-depth analysis of various types of nanostructures, highlighting their roles in improving the electrochemical performance, stability, and durability of SOFCs. Furthermore, this review delves into the fabrication techniques that enable precise control over nanostructure morphology, composition, and architecture. The influence of nanoscale effects on ionic and electronic transport within the electrolyte and electrodes is thoroughly explored, shedding light on the mechanisms behind enhanced performance. By providing a comprehensive overview of the current state of research on nanostructured materials for SOFCs, this review aims to guide researchers, engineers, and policymakers toward the development of high-performance, cost-effective, and sustainable energy conversion systems.

Introdução: Eventos neurológicos em pacientes com endocardite infecciosa (EI) são frequentes e impactam manejo e desfechos. Objetivo: Descrever eventos neurológicos em pacientes com EI e compará-lo com outros casos de EI na coorte. Métodos: Pacientes adultos com EI definitiva de acordo com os critérios de Duke modificados foram incluídos, prospectiva e consecutivamente, de 2006 a 2021. EI com eventos neurológicos (EIEN) foram evento isquêmico cerebral, evento isquêmico com transformação hemorrágica e hemorragia intracraniana, identificados por tomografia computadorizada de crânio realizada sistematicamente na EI esquerda. EIEN foi comparada aos demais pacientes com EI da coorte por teste de proporções. Análise estatística foi realizada com o software Jamovi e R Resultados: Eventos neurológicos ocorreram em 26,1% das EI. Não foi observado diferença entre sexo e idade entre os dois grupos, tampouco em relação ao local de aquisição ou tipo de válvula afetada. A valvopatia reumática (37,8% vs 28.3%, p = 0,046) foi a única predisposição que ocorreu com maior frequência nos EIEN. Não houve diferença na proporção de comorbidades (insuficiência cardíaca, insuficiência renal crônica e diabetes) dentre os grupos. Dentre as complicações, esplenomegalia (30,2% vs 15,3%, p < 0,001), aneurisma micótico (28,8% vs 2,7%, p < 0,001) e eventos vasculares embólicos para outros sítios (83,2% vs 31,3%, p < 0,001) foram os mais prevalentes na EIEN. Dos eventos embólicos, o local mais acometido além do sistema nervoso central (SNC) foi o baço, (60,3% vs 26,3%, p < 0,001). Pacientes transferidos apresentaram com maior frequência eventos embólicos para o SNC (59,5% vs 49,3%, p = 0,044). Pacientes com EIEN foram indicados para cirurgia cardíaca em uma proporção similar ao restante da coorte (82,4% vs 83,7%), no entanto, foram menos frequentemente operados (62,7% vs 79,1%, p < 0,001). A taxa de mortalidade dos pacientes na EIEN foi similar ao restante da coorte (23,4% vs 26,9%). Conclusão: Eventos neurológicos ocorreram em cerca de 1/4 dos pacientes da coorte, sendo mais associado a pacientes transferidos de outros hospitais e com EI por viridans, possivelmente pelo quadro arrastado de EI. O número de eventos embólicos (além dos neurológicos) e a incidência de aneurisma micótico foi maior nos pacientes com EIEN. Por fim, é importante ressaltar que os eventos neurológicos interferem diretamente na realização ou não da cirurgia, no entanto, a taxa de mortalidade foi similar ao restante da coorte.

Introdução: O uso de dispositivos cardíacos eletrônicos implantáveis (DCEI) tem crescido nos últimos anos. Nosso objetivo foi avaliar as características clínicas da endocardite infecciosa (EI) envolvendo dispositivos cardíacos (EIDCEI) em uma série de EI nos últimos 17 anos. Métodos: De janeiro de 2006 a maio de 2023, 502 casos definitivos de EI, pelos critérios de Duke modificados, em adultos, foram diagnosticados em nosso centro, e incluídos consecutiva e prospectivamente. As EIDCEI foram buscadas e descritas. Resultados: EIDCEI ocorreu em 37/502 (7,4%) casos de EI. A idade média ± desvio padrão foi de 54,6±19,0 anos, sendo 64,8% do sexo masculino. Aquisição comunitária de EI ocorreu em 11 (29,7%), nosocomial em 19 (51,3%) e associada a assistência à saúde não nosocomial em 7(18,9%). Na história pregressa, 17 (45,9%) tinham feito cirurgia cardíaca, 18 (48,6%) tinham insuficiência cardíaca congestiva (ICC), 12 (32,4%) tinham doença arterial coronariana, 9 (24,3%) insuficiência renal crônica e 24 (64,8%) hipertensão arterial. Ecocardiograma transesofágico foi realizado em 32/37(86.5%) dos pacientes e acometimento concomitante de valva tricúspide, mitral e aórtica foi encontrado em 7/37(18.9%),513.5%) e 1(2,7%) casos respectivamente. Febre foi observada em 89,2%, novo sopro regurgitante em 16,2%, evento vascular embólico em 27%. Proteína C reativa estava elevada em 30/37(81%) e velocidade de hemossedimentação em 5/22(22,7%). Todos os pacientes colheram hemoculturas, das quais, 73% foram positivas. Os microrganismos causadores foram predominantemente Staphylococcus aureus (35%), Gram negativos não HACEK (10,8%) e fungos (10,8%). As principais complicações foram bacteremia persistente em 7(18.9%), IC aguda em 8 (21,6%), insuficiência renal aguda em 14 (37,8%) e embolização para pulmões em 10/35 (28,5%). A cirurgia foi indicada para 33 (89,2%) pacientes e efetivamente realizada em 30 (81%). A mortalidade intra-hospitalar foi de 15/37 (40,5%). Conclusão: A endocardite infecciosa em DCEI tem apresentado incidência crescente na literatura, em função do maior número de dispositivos inseridos, do envelhecimento da população e das comorbidades, e em nosso centro representou quase 10% de todas as EI nos últimos 17 anos. Os agentes etiológicos podem ser não usuais na EI em geral, como bastonetes Gram negativos e fungos. Trata-se de uma doença grave, com alto índice de complicações e óbitos, que requer a retirada do dispositivo.
