52. An update of the evidence on factors that influence the impact of fluoride toothpaste on dental caries in New Zealand.

Abstract

Background: Tooth brushing with fluoride toothpaste is the most common form of caries prevention, and its advantages have been thoroughly documented. New Zealand's use of fluoride guidelines was last updated in 2009. To ensure that the optimal anti-caries benefit of fluoride toothpaste can be promoted to New Zealanders, an evaluation of current research in relation to these guidelines is needed. Aims: To update the evidence on factors that influence the impact of fluoride toothpaste on dental caries and make recommendations that provide the maximum anticaries effect and safety of fluoride toothpaste. Methods: To maximise the benefits and safety of the use of fluoride toothpaste, the evidence available since the publication of New Zealand guidelines was examined. The literature review was carried out using electronic databases and hand-searches of reference lists of articles, reports, and textbooks. Articles used in this review were identified using certain keywords, and the findings organised into common themes based on key modifying factors of the use of fluoride toothpaste. Findings: After an extensive search of database and grey literature, 59 separate studies or policy documents from 91 papers or reports were extracted and critically examined. Thematic analyses revealed eight themes: use of fluoride toothpaste; fluoride toothpaste formulations; fluoride concentration; fluoride toothpaste amount; frequency of use; brushing time; post-brushing rinsing behaviours; and supervised brushing in children. Conclusion: Recent evidence suggests that it may be time for the 2009 New Zealand use of fluoride guidelines to be updated and refined. Recommended fluoride toothpaste concentrations and amounts for those with higher and lower risk for caries at different age ranges are now more specific, together with more detail on brushing duration and post-brushing rinsing. Amending the current New Zealand guidelines may help in optimising fluoride toothpaste protection, and ultimately reduces individuals' and society's oral disease burden. [ABSTRACT FROM AUTHOR]

Abstract

Aims: A prolonged PR interval may adversely affect ventricular filling and therefore cardiac function. AV delay can be corrected using right-ventricular-pacing (RVP) but this induces ventricular dyssynchrony, itself harmful. Therefore, in intermittent heart-block, pacing-avoidance algorithms are often implemented. We tested His-bundle pacing (HBP) as an alternative. Methods: Out-patients with a long PR interval(>200ms) and intermittent need for ventricular pacing were recruited. We measured within patient differences in high-precision haemodynamics between AV-optimized RVP, and HBP, as well as a pacing-avoidance algorithm [Managed Ventricular Pacing (MVP)]. Results We recruited 18 patients. Mean left ventricular ejection fraction was 44.3±9%. Mean intrinsic PR interval was 266±42ms and QRS duration was 123±29ms. Abstract

Aims Rate adaptation of the action potential ensures spatial heterogeneities in conduction across the myocardium are minimized at different heart rates providing a protective mechanism against ventricular fibrillation (VF) and sudden cardiac death (SCD), which can be quantified by the ventricular conduction stability (V-CoS) test previously described. We tested the hypothesis that patients with a history of aborted SCD due to an underlying channelopathy or cardiomyopathy have a reduced capacity to maintain uniform activation following exercise. Methods and results Sixty individuals, with (n = 28) and without (n = 32) previous aborted-SCD event underwent electro-cardiographic imaging recordings following exercise treadmill test. These included 25 Brugada syndrome, 13 hypertrophic cardiomyopathy, 12 idiopathic VF, and 10 healthy controls. Data were inputted into the V-CoS programme to calculate a V-CoS score that indicate the percentage of ventricle that showed no significant change in ventricular activation, with a lower score indicating the development of greater conduction heterogeneity. The SCD group, compared to those without, had a lower median (interquartile range) V-CoS score at peak exertion [92.8% (89.8–96.3%) vs. 97.3% (94.9–99.1%); P Conclusion Data from this pilot study demonstrate the potential use of this technique in risk stratification for the inherited cardiac conditions.

Abstract

Background: Conduction channels have been demonstrated within the postinfarct scar and seem to be co-located with the isthmus of ventricular tachycardia (VT). Mapping the local scar potentials (SPs) that define the conduction channels is often hindered by large far-field electrograms generated by healthy myocardium.Objective: The purpose of this study was to map conduction channel using ripple mapping to categorize SPs temporally and anatomically. We tested the hypothesis that ablation of early SPs would eliminate the latest SPs without direct ablation.Methods: Ripple maps of postinfarct scar were collected using the PentaRay (Biosense Webster) during normal rhythm. Maps were reviewed in reverse, and clusters of SPs were color-coded on the geometry, by timing, into early, intermediate, late, and terminal. Ablation was delivered sequentially from clusters of early SPs, checking for loss of terminal SPs as the endpoint.Results: The protocol was performed in 11 patients. Mean mapping time was 65 ± 23 minutes, and a mean 3050 ± 1839 points was collected. SP timing ranged from 98.1 ± 60.5 ms to 214.8 ± 89.8 ms post QRS peak. Earliest SPs were present at the border, occupying 16.4% of scar, whereas latest SPs occupied 4.8% at the opposing border or core. Analysis took 15 ± 10 minutes to locate channels and identify ablation targets. It was possible to eliminate latest SPs in all patients without direct ablation (mean ablation time 16.3 ± 11.1 minutes). No VT recurrence was recorded (mean follow-up 10.1 ± 7.4 months).Conclusion: Conduction channels can be located using ripple mapping to analyze SPs. 66. Lack of admission biomarkers' clinical utility in outcomes prediction in patients suspected with infection in the emergency department.

Introduction: Initial procalcitonin (PCT) levels may fail in mortality and septic shock prediction and raise cost-effectiveness issues. Since measurement of lactate, C-reactive protein (CRP), white blood cells and neutrophils is common in the emergency department (ED), we compared prediction abilities of these biomarkers to PCT.Methods: From January 1st to December 31st, 2018, an observational, single center, retrospective study was conducted in the adult ED of the Reims University Hospital (France). Endpoints were bacteremia, septic shock, and in-hospital mortality, related to the same ED visit.Results: Over one year, 459 patients suspected with infection were included, of mean age 60.4 years (SD: 22.0), with 50.8% male, and 364 (79.3%) were hospitalized following ED visit. Overall, 45 (9.8%) patients had a bacteremia, 39 (8.5%) a septic shock and 54 (11.8%) died during their hospitalization. PCT and CRP showed the best discrimination for bacteremia, with an area under curve (AUC) of 0.68 for PCT and 0.65 for CRP. PCT and lactate showed similar good discriminative power for septic shock, with an AUC of 0.78 for both, and poor discrimination for in-hospital mortality, with an AUC of 0.62 for PCT and 0.69 for lactate. Systolic blood pressure and pulse oximetry showed similar discrimination for septic shock as PCT or lactate, while they showed higher discrimination for in-hospital mortality than PCT.Conclusion: Usual admission biomarkers lack clinical utility in predicting septic shock or in-hospital mortality. 67. Risk factors for COVID-19 hospitalisation after booster vaccination during the Omicron period: A French nationwide cohort study.

Abstract

In spite of major effectiveness, a residual risk after COVID-19 primary vaccination was identified, in particular, for vulnerable individuals of advanced age or with comorbidities. Less is known about the Omicron period in people protected by a booster dose. We aimed to identify the characteristics associated with severe COVID-19 during the Omicron period in a population that had received a booster dose in France and to compare differences with the previous periods of the pandemic. This study was carried out using the French national COVID-19 vaccination database (VAC-SI) coupled with the National Health Data System (SNDS). Individuals aged 12 years or over who received at least one booster dose were identified. Associations between socio-demographic and clinical characteristics and the risk of COVID-19 hospitalisation occurring at least 14 days after receiving a third dose of vaccine during the period of Omicron predominance, i.e., from 1 January 2022 to 10 November 2022, were assessed using Cox proportional hazard models adjusted for age, sex, time since booster dose and vaccination schedule. Analyses were performed overall and by sub-period of circulation of the strains BA.1, BA.2, and BA.4/BA.5, defined as periods where the main sub-variant accounted for more than 80 % of genotyped samples. In total, 35,640,387 individuals received a booster dose (mean follow-up of 291 days) and 73,989 were hospitalised for COVID-19 during the total period. Older age (aHR 20.5 95 % CI [19.6–21.5] for 90 years of age or older versus 45–54 years of age), being male (aHR 1.52 [1.50–1.55]), and social deprivation (aHR 1.33 [1.30–1.37] for the most deprived areas versus the least deprived) were associated with an increased risk of hospitalisation for COVID-19. Most of the chronic diseases considered were also positively associated with a residual risk, in particular, cystic fibrosis (aHR 9.83 [7.68–12.56]), active lung cancer (aHR 3.26 [3.06–3.47]), chronic dialysis (aHR 3.79 [3.49–4.11]), psychological and neurodegenerative diseases (more markedly than during the periods of circulation of the alpha and delta variants), and organ transplantation. The use of immunosuppressants was also associated with an increased risk (aHR 2.24 [2.14–2.35], including oral corticosteroids aHR (2.58 [2.50–2.67]). Despite an effective booster and a generally less virulent circulating variant, a residual risk of severe COVID-19 still exists in vulnerable populations, especially those with neurological disorders. Abstract

Objectives: To demonstrate that a BRASS score≥ 3 at admission of intubated, ventilated and sedated patients is predictive of mortality Methods: we have realized an Observational prospective multicenter study. All Major patients without neurological history, admitted to ICU for a non-neurological cause, sedated and admitted under mechanical ventilation were included. Results: One hundred and ten patients were included, the BRASS score as well as the FOUR and RASS scores were collected. At day 28, patients with a BRASS score ≥ 3 had an excess mortality (OR 3.29 – CI 95% [1.42–7.63], p = 0.005) as well as day 90 (OR 2.65 – CI 95% [1.19–5.88], p = 0.02), without impact on the delirium measured by CAM-ICU (OR 1.8 – CI 95% [0.68–4.77], p = 0.023). After adjustment with SAPS II, FOUR and RASS, difference in mortality was not any more different. It is also noted that patients with BRASS ≥ 3 are more sedated (RASS: −5 [−5 – −5] vs −4 [−5 – −3], p < 0.0001) and more comatose (FOUR: 2 [1–4] vs 6 [4–9], p < 0.0001), and have higher doses of midazolam (10 mg/h [5–15] vs 7.5 mg/h [5–10], p = 0.02) and sufentanil (20 μg/h [15–22.5] vs 10 [10–12.5], p = 0.01). Conclusions: The early alteration of brainstem reflexes measured by the BRASS score was not independently predictable in terms of mortality in the non-neurological ICU patients, admitted under sedation and mechanical ventilation. Abstract

Background and objective: The adolescent stage is critical because of the increase in influences from peers and media and exposure to unhealthy food environments and aggressive marketing. This necessitates the need for dentists, dental and oral health therapists, hygienists, and others to engage with adolescents early for establishing good life-long oral health habits. A review of New Zealand school Year 8 children's and adolescents' dental caries experience shows improvements over recent years for Māori, Pacific, and non-Māori and non-Pacific children. However, large ethnic inequalities persist, with no sign of the gaps closing. The objective of this paper was to examine risk factors associated with dental caries in New Zealand adolescents, identify current strategies to improve caries experience, and make recommendations for further improvement. Methods: Electronic databases and electronic and hand-searches of reference lists of articles, reports, and textbooks used were identified, and findings were organised into common themes. Findings: Seven themes have updated: past caries experience and current caries activity; dietary habits; oral hygiene habits; exposure to fluoride; access to dental services; oral health literacy; and socioeconomic background of adolescents and parents/caregivers. Each of these themes is described in detail. For various reasons, adolescents are susceptible to higher dental caries experience. Conclusion: While dental practitioners and others engage with adolescents early and provide appropriate preventive practices, additional effective strategies proposed are needed to further improve the overall caries experience among New Zealand adolescents and to reduce ethnic inequalities. Abstract

Black beetle, Oryctes rhinoceros L. larvae and adults were inoculated in thelaboratory with three isolates of the fungus, Metarhizium anisopliae (Metsch.) Sor, toscreen their virulence against black beetle. All the isolates caused 100% mortality oflarvae and adults when treated with suspensions of 107 conidia/ml. Comparatively thelocal isolate caused a slow death rate in larvae and adults than the other isolates. Fiftygrams of the maize inoculum per 0.008 m2 of breeding medium were sufficient to cause100% death rate in larvae. In the field, the fungus was capable of disseminating at least10 m and the impregnation boxes (infection foci) were attractive to black beetle. It issuggested that M. anisopliae could be used as an effective bio-control agent in theintegrated management of black beetle.

Abstract

A survey was carried out at Bandirippuwa Estate, Kirimetiyana Estate and at Isolated Seed Garden, Rajakadaluwa for the pests of coconut inflorescence and developing fruits. The mite, Dolichotetranychus sp. (Tenuipalpidae) which lives beneath the perianth and feeds on the epicarp, causes considerable damage to the developing fruit. Bi-monthly observations on the intensity of infestation of nuts among different forms of coconut indicated a significant difference among forms in their susceptibility to mite damage. Pseudococcus cocotis (Maskell) (Pseudococcidae), Pseudococcus citriculus Green (Pseudococcidae) and Planococcus lilacinus (Cockereli) (Pseudococcidae), when feeding in large numbers on the peduncle, cause button nut shedding and drying up of the inflorescence. The parasitoids and predators of these mealybugs recorded for the first time in Sri Lanka are Promuscidea unfasciativentris Girault (Aphelinidae) Platygaster sp. (Platygastridae), Anagyrus sp. nr. pseudococci (Girault) (Encyrtidae) Coccodiplosis sp. (Cecidomyiidae), Cryptogonus bryanti Kapur (Coccinellidae) and Pseudoscymnus sp. (Coccinellidae). Several species of scale insects namely, Coccus hesperidum (Linnaeus) (Coccidae) Aulacaspis sp. (Diaspididae), Pseudaulacaspis cockerelli (Cooley) (Diaspididae), Aonidiella orientalis (Newstead) (Diaspididae), and Pinnaspis strachani (Cooley) (Diaspididae) are recorded as minor pests of the developing fruits of coconut. Coccophagus silvestrii Compere (Aphelinidae), Scymnus sp. (Coccinellidae), Pseudoscymnus sp. (Coccinellidae) Telsimia ceylonica (Weise) (Coccinellidae), Cryptogonus bryanti Kapur (Coccinellidae) and Cybocephalus sp. (Nitidulidae) are recorded as their parasitoids and predators. Abstract

Monocrotophos injected into the trunk of tall coconut palms (15 to 20 m) at 3.0 and 6.0g a.i. per palm, was translocated and accumulated in the leaves in quantities sufficient to kill the leaf eating caterpillar, Opisina arenosella. Monocrotophos was injected, as an undiluted 60% W S C , into freshly made holes, 15cm deep and 2cm in diameter, made at a point lm above the ground. Only one hole per palm was used for this treatment. Bioassays in the laboratory and assessments of the natural population of caterpillars on the experimental palms indicated that the insecticidal effect persisted in the leaves for about four to six months. This treatment would therefore be adequate to control a succession of larvae hatching out of eggs laid over a period of time. Although the injection technique obviates the risks of spray contamination of the environment, the question of pesticide residues in edible tissues arises and this aspect needs further study. Abstract

This study evaluates the potential of ultrawideband penetrating radar for the measurement of sea ice thickness. Electromagnetic modeling and system simulations were first performed to determine the appropriate radar frequencies needed to simultaneously detect both the top ice surface (snow–ice interface) and to penetrate through the lossy sea ice medium to identify the bottom ice surface (ice–ocean interface). Based on the simulation results, an ultrawideband radar system was built that operated in two modes to capture a broad range of sea ice thickness. The system includes a low-frequency mode that operates from 50–250 MHz for measuring sea ice thickness in the range of 1 to 7 m (both first-year and multiyear ice types) and a high-frequency mode that operates from 300–1300 MHz to capture a thinner range of thickness between 0.3 and 1 m (primarily first-year ice type). Two field tests of the radar were conducted in 2003, the first off Barrow, Alaska, in May and the second off East Antarctica in October. Overall the radar measurements showed a mean difference of 14 cm and standard deviation of 30 cm compared with in situ measurements over first-year ice that ranged from 0.5 to 4 m in thickness. Based on these initial results, we conclude that ultrawideband penetrating radar is feasible for first-year sea ice thickness measurements. We discuss approaches for further system improvements and implementation of such a system on an airborne platform capable of providing regional sea ice thickness measurements for both first-year and multiyear ice from 0.3 to 10 m thick.

Abstract

We provide an overview of a project to develop and demonstrate radars for imaging the ice–bed interface, mapping shallow and deep internal layers, and measuring ice thickness. We developed an ultra-wideband very high-frequency (VHF) radar (120 to 300 MHz) for simultaneously measuring ice thickness, mapping internal layers at depth, and imaging the ice–bed interface, and an ultra high-frequency (UHF) ultra-wideband radar (500 to 2000 MHz) for fine-resolution mapping of near-surface internal layers. We used these radars to collect data at Summit Camp, Greenland, during July 2005, and the West Antarctic Ice Sheet (WAIS) deep drill core site during January 2006. These results provide us with an unprecedented view of basal characteristics and internal layers. We also developed an outreach program, a communications system, and rovers to facilitate geophysical measurements in the future.

Abstract

An accurate knowledge of snow thickness and its variability over sea ice is crucial in determining the overall polar heat and freshwater budget, which influences the global climate. Recently, algorithms have been developed to extract snow thicknesses from satellite passive microwave data. However, validation of these data over the large footprint of the passive microwave sensor has been a challenge. The only method used thus far has been with meter sticks during ship cruises. To address this problem, we developed an ultrawideband frequency-modulated continuous-wave radar to measure the snow thickness over sea ice. We synthesized a very linear chirp signal by using a phase-locked loop with a digitally generated chirp signal as a reference to obtain a fine-range resolution. The radar operates over the frequency range from 2-8 GHz. We made snow-thickness measurements over the Antarctic sea ice by operating the radar from a sled in September and October 2003. We performed radar measurements over 11 stations with varying snow thicknesses between 4 and 85 cm. We observed an excellent agreement between radar estimates of snow thickness with physical measurements, achieving a correlation coefficient of 0.95 and a vertical resolution of about 3 cm. Comparison of simulated radar waveforms using a simple transmission line model with the measurements confirms our expectations that echoes from snow-covered sea ice are dominated by reflections from air-snow and snow-ice interfaces.

Abstract

Scaphoid fractures account for 90% of carpal fractures and occur predominantly in young men. The use of immediate surgical fixation to manage this type of fracture has increased, despite insufficient evidence of improved outcomes over non-surgical management. The SWIFFT trial compared the clinical effectiveness of surgical fixation with cast immobilisation and early fixation of fractures that fail to unite in adults with scaphoid waist fractures displaced by 2 mm or less.

His-bundle pacing (HBP) has emerged as an alternative to conventional ventricular pacing because of its ability to deliver physiological ventricular activation. Pacing at the His bundle produces different electrocardiographic (ECG) responses: selective His-bundle pacing (S-HBP), non-selective His bundle pacing (NS-HBP), and myocardium-only capture (MOC). These 3 capture types must be distinguished from each other, which can be challenging and time-consuming even for experts.

99. Prevalence of geophagy and knowledge about its health effects among native Sub-Saharan Africa, Caribbean and South America healthy adults living in France

Abstract

Geophagy is widespread among women from Sub-Saharan Africa, South America and the Caribbean and may persist in western countries. This practice may be associated with adverse effects such as anaemia, constipation or intestinal occlusion. We aimed to determine the prevalence of geophagy and the level of knowledge about its health effects among healthy adults originating from these countries and attending a travel medicine and international vaccination consultation in France. Among 101 travellers enrolled in the study, 83 (82.1%) were born in Sub-Saharan Africa and 13 (12.8%) in South America or the Caribbean. The mean duration of residence in France was 15.6 ± 10.4 years. Previous or current geophagy was present in 42 travellers [previous geophagy in 31 (30.7%) and current consumption in 11 (10.9%)]; 38 (90.5%) were women. The rate of awareness of harmful effects of geophagy as the risk of iron-deficient anaemia (18.8%) and soil-transmitted intestinal parasitic infections (11.9%) was low overall. Women with previous or current geophagy more often had history of iron therapy compared to those who never consumed, both during pregnancy (50.0 versus 14.3%; p= 0.0009) and outside pregnancy (47.4 versus 2.8%; p< 0.0001). Despite a long period of residence in France, geophagy was still a current practice among 10.9% of Sub-Saharan, South American and Caribbean travellers, who are poorly informed of its harmful effects. Therefore, specific information tailored to Sub-Saharan, South American and Caribbean about the risks of geophagy should be implemented in western countries.
